Governing drug reimbursement policy in Poland: The role of the state,civil society,and the private sector

This article investigates the distribution of power in Poland’s drug reimbursement policy in the early 2000s. We examine competing theoretical expectations suggested by neopluralism, historical institutionalism, corporate domination, and clique theory of the post-communist state, using data from a purposive sample of 109 semi-structured interviews and documentary sources. We have four concrete findings. First, we uncovered rapid growth in budgetary spending on expensive drugs for narrow groups of patients. Second, to achieve these favorable policy outcomes drug companies employed two prevalent methods of lobbying: informal persuasion of key members of local cliques and endorsements expressed by patient organizations acting as seemingly independent “third parties.” Third, medical experts were co-opted by multinational drug companies because they relied on these firms for scientific and financial resources that were crucial for their professional success. Finally, there was one-way social mobility from the state to the pharmaceutical sector, not the “revolving door” pattern familiar from advanced capitalist countries, with deleterious consequences for state capacity. Overall, the data best supported a combination of corporate domination and clique theory: drug reimbursement in Poland was dominated by Western multinationals in collaboration with domestically based cliques.     

The Politics of Seasonal Labour Migration in Switzerland,France and Spain

This article analyses the effects of the politics of seasonal foreign worker admissions on migrant legality in the context of the post‐1945 and the post‐1990 seasonal foreign worker policy in Switzerland, France and Spain respectively. It seeks historical evidence attesting to the ability of seasonal admissions to restrict workers to their non‐resident status. It presents implications for circular migration, the dernier cri in European migration policy aimed to strike a compromise between a perceived post‐crisis demand for the admission of foreign workers and the reluctance to make them prospective citizens.

Policy Implications

• Migration policy stakeholders have shaped foreign worker admissions according to their interests.
• Under some conditions, low‐skilled seasonal foreign worker admissions have contributed to irregular migration, either through workers' overstays or through parallel irregular entries
• There are both differences and similarities between the seasonal admissions advocated under the post‐2007 circular migration schemes, and those of the 1950s–1960s, as well as those of 1990s to early 2000s. The similarities could, under some conditions, trigger the repetition of some of the unexpected outcomes revealed by historical seasonal foreign worker admissions under the post 2007 schemes.

     

Approximation Algorithms for Bounded Facility Location Problems

The bounded k-median problem is to select in an undirected graph G = (V,E) a set S of k vertices such that the distance from any vertex v V to S is at most a given bound d and the average distance from vertices V\S to S is minimized. We present randomized algorithms for several versions of this problem and we prove some inapproximability results. We also study the bounded version of the uncapacitated facility location problem and present extensions of known deterministic algorithms for the unbounded version.     

From words to deeds: Health service reform in Poland

After two years of fundamental political changes in Poland a lot of enthusiasm has faded. The country is on its way towards a market economy but the process of transformation is proving more difficult than expected. The social sector in particular is being hampered. Budgetary cuts undermine any feeling of security. Society seems to be entirely unprepared to accept changes that lead to a reduction of social packages offered by former regimes. Hopes associated with 5the outcome of health care reform were aroused by the length of time taken to conceptualize the main proposals. The process started as early as 1980 and was supposed steadily to yield easier access and higher technology. When it became clear that this was unrealistic because of general economic crises, the pace of health service reform was slowed down. It was a painful process to abandon illusion. Now it is becoming accepted that the reform is to improve the elementary effectiveness and efficiency of health services. Thanks to this breakthrough it has been possible to make and put into practice two major decisions: the regional integration of health services, and the new law on health care institutions.

Le non‐respect des dispositions relatives au salaire minimum en Europe centrale et orientale

L'étude porte sur l'inobservation du salaire minimum dans dix pays d'Europe centrale et orientale dotés de cette institution. Les auteurs estiment la fréquence et la gravité des infractions à partir des données de l'EU‐SILC (2003–2012), en reprenant la méthode de Bhorat, Kanbur et Mayet (2013). Ils tiennent compte dans leurs régressions des caractéristiques individuelles, des caractéristiques de l'emploi et des données macroéconomiques. Les infractions restent relativement peu fréquentes mais touchent d'abord les groupes que les politiques de salaire minimum devraient protéger. Par ailleurs, la hausse du rapport entre salaire minimum et salaire moyen dans le temps s'accompagne d'une hausse de la fréquence des infractions.     

An ‘ordinary man’s’ protest: self-immolation as a radical political message in Eastern Europe today and in the past

The article describes the tradition of self-immolation in Eastern Europe as a form of radical protest during periods of structural crisis. The authors describe cases of self-immolation (Ryszard Siwiec, Jan Palach, Evzen Plocek, Musa Mamut) which took place in the communist period, particularly in the 1960s and 1970s. In the context of Eastern European tradition, they also describe the last case of self-immolation, when Piotr Szcz?sny set himself on fire in Warsaw in October 2017 to protest against the authoritarian rule of the Law and Justice (PiS) party in Poland. After this incident, Piotr Szcz?sny and his memory have become an element of the ‘culture of protest’ in Poland. Quotes from his manifesto began to appear on walls and during anti-government demonstrations. Piotr Szcz?sny has also become a protagonist of protest songs.     

Poland's Contentious Elites Enter the Age of Revolution: Extending Social Movement Concepts

Scholars of social movements commonly call for the field to be broadened in various ways because movements are often intertwined with other forms of conflict and because the causes or consequences of movements may operate differently in different contexts. Important change processes that were unfolding in Poland at the time of the French Revolution provide an instructive case. Although the contemporaneous French Revolution, with its enormous quantity and variety of collective mobilizations has been a touchstone for social movement scholars, the work of Poland's reform parliament and the adoption of Poland's 1791 constitution have gotten much less attention. Poland's reform politics not only provides both instructive parallels to and differences from French revolutionary developments, but were also deeply intertwined with them and embedded with those French events in a larger, European field of contention. Perhaps one of the most intriguing aspects of the Polish reform movement is that it was largely driven by elites, something noted in Karl Marx's bemused praise. Although social movements played very much less of a role in Poland than in France, we try to show here that familiar tools of social movement analysis permit an account of those Polish events as well.
