Abstract When a social worker incurs professional obligations in two different actions, but cannot do both, this is termed “an ethical conflict concerning expectations in concurrently affected arenas” (Levy, 1976, p. 113). This article addresses an ethical dilemma that is centered in one of the most complex and controversial issues within Jewish life today: the question of “who is a Jew.” A case is presented which depicts the complexity of ethical conflicts which may arise from this unanswered question. The values at issue are explored and the process used to clarify, legitimate, and operationalize a plan of action is articulated. Ethical dilemmas invariably arise in social work practice. Resolutions that are clear, easily applied, internally consistent, and that consider the values of all concerned should help in reaching an ethical solution. 相似文献

老年人社区医养融合养老模式选择意愿受众多因素的影响,这些因素涉及范围广且关系复杂。为厘清各影响因素之间的层次关系和影响程度,本文从身体特征、家庭特征、认知特征和环境特征四个方面构建了影响因素指标体系。首先采用解释性结构模型分析得到由表层直接影响因素、中层间接影响因素和深层根本影响因素组成的3级多层递阶有向图,然后应用层次分析法对各因素指标的影响力进行排序,建立指标综合权重体系。结果表明:表层直接影响因素包括ADL、医疗支出能力、健康自评等8个,中层间接影响因素包括慢性疾病数量、社会关注度、医疗保障水平等8个,深层根本影响因素包括家庭收入水平、政府支持力度和发展阶段3个。其中,家庭收入水平、ADL、医疗保障水平、慢性疾病数量、政府支持力度等是老年人社区医养融合养老模式选择意愿的主要影响因素。 相似文献

This article is a critical review of Crossing Bridges, a Department of Health training pack for those working with 'mentally ill' parents and their children. The author argues, on the basis of both personal and professional experience, that although the pack pays lip-service to the significance of structural factors, the training resources are primarily rooted in an individual and family-focused pathology model of mental illness. Three arguments are put forward for the necessity of a wider framework: the significance of shame and stigma to those experiencing mental distress and their families; the predominance of understandings of mental illness that translate social phenomena into individualised problems, notably genetic explanations and attributions of risk and dangerousness; and the contribution of motherhood to the experiencing of mental distress in women. Initiatives within the mental health survivor movement are highlighted as examples promoting positive images of mental distress, and the implications of the issues raised for social work education are considered. The article concludes that efforts to help the children of 'mentally ill' parents, not to mention the parents themselves, must incorporate wider strategies to promote openness, respect, acceptance and ultimately, social inclusion, of those who experience mental distress. 相似文献

ABSTRACT Based on 5 months of ethnographic fieldwork and 43 in-depth interviews with villagers of T Township, the author uncovers an emerging approach of endogenous multicultural community practice in the cities where the villagers have migrated to work. Cadres and villagers transplant their localistic networks to the new urban contexts while transforming hierarchical and clientelistic personal ties into more horizontal relations of comradeship. Through these redefined ties, they manage to transcend the persisting boundaries of village units, class status, and lineage institutions that segment the rural society, and succeed in mobilizing community resources to fight for urban survival and social justice. Although neither community service nor the social work profession is fully institutionalized in contemporary China, innovative organizing is appearing in grassroots communities in response to sweeping social changes. This article draws academic attention to these community practices and discusses their implications for the future development of strength-based social work and social services in post-socialist China. 相似文献

British local government has recently undergone its most far-reaching reorganization for twenty-five years. The impact of this reorganization was considerable within local authorities but it also substantially affected other organizations with which they worked at a local level. This paper explores the perspectives of voluntary sector organizations involved in social care: a set of actors which, prior to reorganization, had been encouraged by central and local government, through notions of partnership and through community care legislation, to undertake more direct roles in service delivery, consultation and strategic planning. During the consultative period prior to reorganization, many voluntary organizations reported that they had been marginal to the process. Reflecting on the process of reorganization itself, many voluntary organization respondents commented that it had been disruptive and provoked considerable anxiety. Post-reorganization, voluntary organizations felt that rebuilding of relationships was necessary and that the promise of partnership had meant little in practice at a time of potential crisis for local. 相似文献

The community care reforms which followed the 1989 White Paper "Caring for People"were apparently focused on addressing the needs of people requiring long-term care, and on achieving improved outcomes and better quality of life.
The agenda set out by the White Paper was for community care in the next decade and beyond. Half way through this decade, we question the extent to which the objectives of promoting choice and independence for users and carers have been achieved. The paper draws particularly on a programme of monitoring conducted jointly by the Nuffield Institute for Health and the King's Fund, based on national and local focus groups meeting over a two-year period. It proposes a framework for evaluation which consists of four components: the definition of desired outcomes; specification of service systems necessary to deliver such outcomes; promotion of access to services; and the development of supporting operational policies and resource allocation mechanisms.
This framework offers a substantial step beyond much of the monitoring of the community care reforms which has taken place to date. This has assessed progress largely in terms of the establishment of new systems and processes. We conclude that such changes were essential building blocks for delivering better-quality community care services, and in the short term it may have been legitimate to view their establishment as proxies for progress towards delivering user-centred services. However, monitoring and evaluation should now be increasingly oriented towards ensuring that these changes are in fact producing the desired service outputs and urn outcomes.
