BackgroundSevere perineal tears sustained during childbirth cause significant distress and morbidity amongst women. The objective of this study was to compare the use of straight scissors for cutting an episiotomy with the use of curved scissors, which are designed to curve away from the anal sphincter.MethodsWe used a single-centre, randomised feasibility trial. The intervention was the use of curved scissors. Women were recruited during a prenatal visit and randomised in the delivery suite, when it became clear that an episiotomy was required. The feasibility outcomes were the proportion of women able to be recruited, randomised and followed up. We also calculated the incidence of obstetric anal sphincter injury when either straight or curved scissors were used to cut an episiotomy. Other outcomes assessed were pain, length of hospital stay, perineal infection and perineal dehiscence.ResultsOf the 155 patients recruited in the prenatal period, only 20 (12.9%) were eventually randomised at birth. The main reasons for the high loss were that women either did not have a vaginal delivery (38, 24.5%), or they did not need an episiotomy (72, 46.5%). Rates of obstetric anal sphincter injury and other outcomes were similar between groups.DiscussionAnal sphincter injury during childbirth remains an important problem. Although the use of curved scissors provides a theoretical solution, we found that the high attrition rate made feasibility of conducting a suitably powered, randomised trial using the current design untenable. Alternative strategies have been suggested to make any future study more viable.  相似文献

Comparisons of best linear unbiased estimators with some other prominent estimators have been carried out over the last 50 years since the ground breaking work of Lloyd [E.H. Lloyd, Least squares estimation of location and scale parameters using order statistics, Biometrika 39 (1952), pp. 88–95]. These comparisons have been made under many different criteria across different parametric families of distributions. A noteworthy one is by Nagaraja [H.N. Nagaraja, Comparison of estimators and predictors from two-parameter exponential distribution, Sankhyā Ser. B 48 (1986), pp. 10–18], who made a comparison of best linear unbiased (BLUE) and best linear invariant (BLIE) estimators in the case of exponential distribution. In this paper, continuing along the same lines by assuming a Type II right censored sample from a scaled-exponential distribution, we first compare BLUE and BLIE of the exponential mean parameter in terms of Pitman closeness (nearness) criterion. We show that the BLUE is always Pitman closer than the BLIE. Next, we introduce the notions of Pitman monotonicity and Pitman consistency, and then establish that both BLUE and BLIE possess these two properties.  相似文献

Outer product of gradients (OPG) achieves dimension reduction via estimating the gradients of the regression function. In this paper, we propose two novel OPG estimators via local rank regression: the rank OPG estimator and the Walsh-average OPG estimator. Both proposals guard against a wide range of error distributions, and are safe alternatives to existing OPG estimators based on local linear regression or local L1 regression. The effectiveness of the new proposals are demonstrated via extensive numerical studies.  相似文献

Abstract

We introduce a new family of distributions using truncated discrete Linnik distribution. This family is a rich family of distributions which includes many important families of distributions such as Marshall–Olkin family of distributions, family of distributions generated through truncated negative binomial distribution, family of distributions generated through truncated discrete Mittag–Leffler distribution etc. Some properties of the new family of distributions are derived. A particular case of the family, a five parameter generalization of Weibull distribution, namely discrete Linnik Weibull distribution is given special attention. This distribution is a generalization of many distributions, such as extended exponentiated Weibull, exponentiated Weibull, Weibull truncated negative binomial, generalized exponential truncated negative binomial, Marshall-Olkin extended Weibull, Marshall–Olkin generalized exponential, exponential truncated negative binomial, Marshall–Olkin exponential and generalized exponential. The shape properties, moments, median, distribution of order statistics, stochastic ordering and stress–strength properties of the new generalized Weibull distribution are derived. The unknown parameters of the distribution are estimated using maximum likelihood method. The discrete Linnik Weibull distribution is fitted to a survival time data set and it is shown that the distribution is more appropriate than other competitive models.  相似文献

This paper considers the maximum and minimum of a pair of log-normal variables with equal mean. It shows that either order statistic has a smaller coefficient of variation than the two original log-normal variables provided the latter are of equal variance. When the variances are unequal, as the variance ratio increases, the minimum (maximum), has a smaller coefficient of variation if the correlation coefficient of the log-normal variables is small (small) and the variances are large (small).  相似文献

This paper presents a new method for the analysis of moral hazard principal–agent problems. The new approach avoids the stringent assumptions on the distribution of outcomes made by the classical first‐order approach and instead only requires the agent's expected utility to be a rational function of the action. This assumption allows for a reformulation of the agent's utility maximization problem as an equivalent system of equations and inequalities. This reformulation in turn transforms the principal's utility maximization problem into a nonlinear program. Under the additional assumptions that the principal's expected utility is a polynomial and the agent's expected utility is rational in the wage, the final nonlinear program can be solved to global optimality. The paper also shows how to first approximate expected utility functions that are not rational by polynomials, so that the polynomial optimization approach can be applied to compute an approximate solution to nonpolynomial problems. Finally, the paper demonstrates that the polynomial optimization approach extends to principal–agent models with multidimensional action sets.  相似文献

区间数排序的可能度计算模型是学术界一直在不断探索的基础性问题之一。区间数刻画着事物属性特征的取值范围,以往学术界都假设在区间内的取值服从均匀分布。本文将均匀分布推广到一般分布,运用概率论的方法,构建了一个新的区间数排序的可能度计算模型,由此修正了以往关于两个区间数全等的定义,提出了区间数形等的概念,同时进一步修正了可能度的自反性条件和区间数的综合排序方法,并将理论应用于多属性决策问题,给出了基本的决策过程,通过实例决策问题的计算,呈现了新理论和新方法的可行性和合理性,具有很好的推广应用价值。  相似文献
