Pregnant women who misuse alcohol or substances often develop obstetric conditions that further complicate their pregnancy. This case study reflects on the maternity care provided for a woman who continued to use amphetamines during her pregnancy; and who was diagnosed with placenta praevia and subsequently suffered a placental abruption. Alcohol and substance misuse in pregnancy is currently escalating, increasing the risk in maternal and neonatal morbidity and mortality. Midwives must be confident in the advice and care they provide in order to reduce the risks caused by substance misuse, and be able to support this with evidence-based care.PurposeThe purpose of this case study is to discuss the obstetric condition involved with placenta praevia with the occurrence of a placental abruption in a woman who uses amphetamines during pregnancy; and the midwifery and obstetric care involved.Interest/relevance/congruencyIt will highlight the importance of evidence-based care in high risk obstetrics.Content(1) Case summary; (2) discussion; (3) risk factors; screening, diagnosis and management; foetal and neonatal monitoring; postnatal management, and trauma informed care.ConclusionIt was shown with planning, understanding, communication, and vigilance, the care of an amphetamine using pregnant woman with a diagnosis of placenta praevia and abruption can be successfully accomplished. The management of the woman discussed in this case study was within the recommendations currently available in the literature.  相似文献

Self-efficacy theory suggests that one’s perceived ability to successfully find facts may motivate political information seeking. A telephone survey of voters in a presidential campaign attempted to further validate the concept of epistemic political efficacy (EPE), or belief one can discover the “truth” in politics, and applied it to modern information-seeking behaviors. This study of North Carolina registered voters (N = 605) demonstrates that EPE adds predictive power beyond commonly used measures such as individual political efficacy for contemporary media use variables like online information seeking and partisan cable viewing. EPE was a stronger positive predictor of online information seeking than individual political efficacy. In addition, viewing partisan cable shows had a stronger relationship with EPE than mainstream TV news viewing, and EPE significantly predicted MSNBC viewing, even after controlling for partisanship. In a word, voters who are high in the belief that political facts or “truths” exist take steps to find and understand them.  相似文献

This paper reflects on research carried out with a group of women receiving intensive family support aimed at addressing the cause of their family’s ‘anti-social behaviour’. The methodological approach to the research was underpinned by the philosophical principles of critical realism. It was also informed by the ethical and political concerns of feminist scholarship. The paper reports on the potential points of tension that arise between feminism and critical realism in empirical research. In particular, attention is centred on the process of trying to marry approaches which stress the central role of participants’ knowledge, particularly those who are ‘labelled’ and whose voices are not readily heard, with the principle that some accounts of ‘reality’ are better than others.  相似文献

We evaluated the facilitative effects of multiple exemplar training (MET) on the establishment of derived tact relations in typically developing children. A multiple-probe design across stimulus sets was implemented to introduce MET. Participants were first taught to conditionally relate dictated names in English to their corresponding objects (listener behavior; A-B relations), followed by tests for derived tacts (B-A relations). If participants failed these tests, MET was implemented whereby tact relations were explicitly taught with novel stimulus sets, followed by test probes with the original training set. MET continued with novel stimuli until participants met criterion for the emergence of derived tact relations or after exposure to three MET sets. Results indicated failed tests for tact relations following direct training in listener relations, and marked improvements in derived tact relations following MET across all participants.  相似文献
