The twinning rate has increased dramatically over the last four decades in developed countries. Two main factors account for this increase: delayed childbearing, as older women tend to have twins more frequently than younger ones, and the expansion of medically assisted reproduction (MAR), which carries an increased probability of multiple births. Using civil registration data, we estimate the share of the increase in twinning rates attributable to the rise in the age at childbearing and to MAR. The effect of MAR is estimated to be about three times as important as the effect of delayed childbearing. Negative health outcomes associated with multiple births and the cost of MAR have raised concerns. We find that in one‐quarter of developed countries with the relevant data, the twinning rate reached a plateau around the early 2000s and decreased thereafter. We examine the reasons for this reversal, in particular changes in MAR policies and practices.  相似文献

Background

The prevalence of fear of birth has been estimated between 8–30%, but there is considerable heterogeneity in research design, definitions, measurement tools used and populations. There are some inconclusive findings about the stability of childbirth fear.

Aim

to assess the prevalence and characteristics of women presenting with scores ≥60 on FOBS-The Fear of Birth Scale, in mid and late pregnancy, and to study change in fear of birth and associated factors.

Methods

A prospective longitudinal cohort study of a one-year cohort of 1212 pregnant women from a northern part of Sweden, recruited in mid pregnancy and followed up in late pregnancy. Fear of birth was assessed using FOBS-The fear of birth scale, with the cut off at ≥60.

Findings

The prevalence of fear of birth was 22% in mid pregnancy and 19% in late pregnancy, a statistically significant decrease. Different patterns were found where some women presented with increased fear and some with decreased fear. The women who experienced more fear or less fear later in pregnancy could not be differentiated by background factors.

Conclusions

More research is needed to explore factors important to reduce fear of childbirth and the optimal time to measure it.  相似文献

Background

Childbirth fear is the most common underlying reason for requesting a caesarean section without medical reason. The aim of this randomised controlled study was to investigate birth preferences in women undergoing treatment for childbirth fear, and to investigate birth experience and satisfaction with the allocated treatment.

Methods

Pregnant women classified with childbirth fear (≥60 on the Fear Of Birth Scale) (n = 258) were recruited at one university hospital and two regional hospitals over one year. The participants were randomised (1:1) to intervention (Internet-based Cognitive Behaviour Therapy (ICBT)) (n = 127) or standard care (face-to-face counselling) (n = 131). Data were collected by questionnaires in pregnancy week 20–25 (baseline), week 36 and two months after birth.

Results

Caesarean section preference decreased from 34% to 12% in the ICBT group and from 24% to 20% in the counselling group. Two months after birth, the preference for caesarean increased to 20% in the ICBT group and to 29% in the counselling group, and there was no statistically significant change over time. Women in the ICBT group were less satisfied with the treatment (OR 4.5). The treatment had no impact on or worsened their childbirth fear (OR 5.5). There were no differences between the groups regarding birth experience.

Conclusion

Women’s birth preferences fluctuated over the course of pregnancy and after birth regardless of treatment method. Women felt their fear was reduced and were more satisfied with face-to-face counselling compared to ICBT. A higher percentage were lost to follow-up in ICBT group suggesting a need for further research.  相似文献

We present our design of a digital service supporting social inclusion among immigrant women in an ethnically and socially diverse neighbourhood of a small Finnish town. The aim is to explore experiences and perceptions as well as potential barriers and challenges for using the web-based service. The case study is framed as design research having a strong user-centred design approach. We describe the design process and present the results of a mixed-method evaluation. The results are scrutinized from the perspectives of experience design. The analysis reveals a potential to facilitate social inclusion through the fulfilment of needs related to relatedness, autonomy, competence, pleasure and stimulations as well as popularity. However, some challenges in the interface design and communication of the security and purpose of the site were identified in the study. Significant differences were also found between immigrant women and local people regarding the need for this digital service.  相似文献

To understand children's peer group affiliation, this study examined to what extent children in naturally occurring groups resemble each other on bullying, likeability, and perceived popularity. Participants were fourth‐ to sixth‐grade pupils (N = 461). Peer groups were identified using the social cognitive map procedure. Resemblance on bullying, likeability, and perceived popularity was evaluated by means of variance components models. Resemblance in peer groups was strongest for perceived popularity, followed by bullying and likeability. Moreover, resemblance on bullying could for a large part be attributed to the high‐perceived popularity of the group, and to a lesser extent, to the low likeability of the group. It is concluded that children showing bullying seem to affiliate with each other most of all to attain or maintain their position in a perceived popular peer group. Results stress the importance of considering the functionality of bullying from a group perspective.  相似文献

This piece of writing is a joint initiative by the participants in the Gender, Work and Organization writing workshop organized in Helsinki, Finland, in June 2019. This is a particular form of writing differently. We engage in collective writing and embody what it means to write resistance to established academic practices and conventions together. This is a form of emancipatory initiative where we care for each other as writers and as human beings. There are many author voices and we aim to keep the text open and dialogical. As such, this piece of writing is about suppressed thoughts and feelings that our collective picket line allows us to express. In order to maintain the open‐ended nature of the text, and perhaps also to retain some ‘dirtiness’ that is essential to writing, the article has not been language checked throughout by a native speaker of English.  相似文献
