瑞舒伐他汀对血脂正常高血压患者血管弹性及内皮功能的影响

目的观察瑞舒托伐他汀对血脂正常高血压患者脉搏波传导速度（PWV）、踝臂指数（ABI）及血管内皮功能的影响。方法选择南京医科大学附属淮安第一医院2011年1月-2012年6月心内科门诊及住院的78例血脂正常高血压患者,随机分成瑞舒伐他汀组39例和对照组39例。瑞舒伐他汀组在服用常规降压药物的基础上,每晚睡前服用瑞舒伐他汀10mg,对照组仅服用常规降压药物,共随访6个月。观察治疗前后两组PWV、ABI、血浆一氧化氮、内皮素-1和高敏C反应蛋白（hs-CRP）的变化。结果两组治疗前后收缩压和舒张压的差值相比无统计学意义（P〉0.05）。瑞舒伐他汀组治疗后PWV、内皮素-1、hs-CRP较治疗前分别降低-4.1±0.5,-17.4±2.5,-2.8±0.2,与对照组（-0.6±0.4,-3.3±0.5,-0.2±0.1）相比,差异有统计学意义（P〈0.05）;ABI、一氧化氮较治疗前分别升高0.21±0.04,19.9±3.6,与对照组（0.02±0.01,2.0±0.8）相比,差异有统计学意义（P〈0.05）。对照组治疗后PWV、ABI、内皮素-1、一氧化氮、hs-CRP与治疗前相比,差异无统计学意义（P〉0.05）。结论瑞舒托伐他汀可以通过改善血管内皮细胞功能和抑制炎症来改善血管的弹性,以保护血脂正常高血压患者的血管功能。

Effects of Rosuvastatin on arterial elasticity and endothelial function in essential hypertension patients with normal blood lipid

Objectives To investigate the effect of Rosuvastatin on pulse wave velocity（ PWV） ,ankle-brachial index（ ABI） and endothelial function in essential hypertension patients with normal blood lipid.Methods 78 essential hypertension patients with nor-mal blood lipid in Huaian first hospital affiliated to Nanjing medical university between January 2011 and June 2012 were divided in-to Rosuvastatin group and control group,Rosuvastatin group were given antihypertensive drug and Rosuvastatin 10mg before bedtime while control group were given antihypertensive drug.PWV,ABI,nitric oxide （NO）,endothelin-1（ET-1） and high sensitivity C-re-active protein （ hs-CRP） were measured before and after treatment of 6 months.Results By the end of six months therapy,no sig-nificant difference in blood pressure was observed between two groups.PWV,ET-1 and hs-CRP were significantly decreased in rosuv-astatin group（-4.1±0.5,-17.4±2.5,-2.8±0.2）compared with control group（ -0.6 ±0.4,-3.3 ±0.5,-0.2 ±0.1）（P〈0.05）,ABI and NO were significantly increased in rosuvastatin group（0.21 ±0.04,19.9 ±3.6） compared with control group （0.02 ±0.01,2.0 ±0.8）（P〈0.05）.There were no significant difference in PWV,ABI, ET-1, NO and hs-CRP in control group after treatment as compared with baseline （ P〉0.05 ） .Conclusion Rosuvastatin can exert beneficial effect on arterial elasticity through improving endothelial function and inhibiniting inflamation reaction in essential hypertension patients with normal blood lipid.
