福州市新型农村合作医疗制度下就诊行为实证分析

以Grossman模型为理论模型，采用Probit模型、Multi—Logit模型等计量方法对福州市所辖闽清、连江、闽侯等6个县农户参加新型农村合作医疗（简称“新农合”）制度中的就诊行为、就诊单位的选择、就诊的医疗费用负担及影响因素进行实证研究，得出以下结论：（1）农户家庭社会经济特征因素影响农民的就诊选择；文化程度、健康情况、负担系数、收入水平、家庭规模等因素影响较显著，而性别、年龄以及是否党员、是否村干部等因素影响较不显著。（2）到就诊单位的距离影响了农户对就诊单住的选择。（3）“新农合”制度对减轻农民负担有积极影响。建议，应当尽量提升报销比例，降低报销起付线，尽可能简化与农民相关的制度安排；要合理利用合作医疗基金。制定适宜的门诊和住院补偿方案等。

Farmers’ Medical Care Seeking Behaviors in the New Rural Cooperative Medical Care System in Fuzhou:An Empirical Study

Based on Grossman’s theory,this study makes an empirical study on farmers’ medical care seeking behavior,choice of hospital,burden of medical expenses and the influencing factors in several counties of Fuzhou city by applying the quantitative methods in Probit Model and Multi-logit Model.The following conclusions have been made:(1) such socioeconomic factors as level of education,health,burden coefficient,income level and family size have significant influence on the farmers’ medical selection;while factors such as gender,age,and whether the farmer is a CPC member or a village official do not have remarkable effect;(2) the distance to medical institutions has influenced the farmers’ selection of medical institutions;(3) the New Rural Cooperative Medical Care System has a positive impact on reducing the farmers’ burdens.It is suggested that the government raise the medical reimbursement rate,set a lower standard for medical reimbursement,simplify the systems related to farmers,utilize the cooperative medical fund rationally and formulate proper plans for outpatient and inpatient reimbursement.
