紫杉醇脂质体联合顺铂、氟尿嘧啶一线治疗晚期胃癌的临床观察

目的观察紫杉醇脂质体联合顺铂、氟尿嘧啶（PCF方案）一线治疗晚期胃癌的临床疗效和毒副反应。方法选择晚期胃癌患者静点紫杉醇脂质体175mg／m^2，第一天静滴90min；顺铂75mg／m^2，第一天静滴；亚叶酸钙400mg／m^2，第一天静点2h；氟尿嘧啶2．6g／m^2，亚叶酸钙滴注结束后立即持续泵入46h。21天为一个周期，每2个周期按RE—SIST标准评价疗效，所有患者至少接受2周期化疗。结果28例患者中总有效率RR49．9％，完全缓解3．57％，稳定25％，中位进展时间（TTP）6．8个月，中位生存时间10．8个月。常见的不良反应为血液学毒性，恶心呕吐，肌肉酸痛，疲乏，脱发。结论紫杉醇脂质体联合顺铂、氟尿嘧啶（PCF方案）一线治疗晚期胃癌疗效确切，不良反应轻，值得临床推广应用。

Paclitaxel liposome and cisplatin and fluorouracil for first-line treatment of advanced gastric cancer

Objective Observe the paclitaxel liposome and cisplatin-fluorouracil （PCF） program first-line treatment of advanced gastric cancer clinical efficacy and toxicity. Methods Choice in patients with advanced gastric cancer liposome infusion paclitaxel 175mg/m^2,the first day of infusion 90min;cisplatin 75mg/m^2,the first day of infusion;leucovorin 400mg/m^2,the first day of infusion of 2h;FU 2.6g/m^2,Leucovorin immediately after the continuous infusion pump 46h. 21 days for a cycle,every two cycles according to RE-SIST standards for evaluation of efficacy,all patients received at least two cycles of chemotherapy. Results 28 patients in the total effective RR49.9%,complete remission 3.57%,stable 25%,median time to progression （TTP） 6.8 months and median survival time was 10.8 months. Common adverse events were hematological toxicity,nausea,vomiting,muscle aches,fatigue,hair loss. Conclusion The paclitaxel liposome combination with cisplatin and fluorouracil （PCF） program first-line treatment of advanced gastric cancer curative effect,adverse reaction to light,worthy to be popularized.
