大健康视角下社区健康管理问题与思考

社区健康管理以健康为中心,目的是改善社区居民健康状况,提高社区人群健康水平。在健康中国、大健康的前提下,文章基于社区健康管理的概念和理论框架,结合现行常见的社区慢性病健康管理模式和全生命周期健康管理模式,分析社区健康管理服务在部门协调能力、人才建设、服务规范、自我健康管理理念等方面的关键问题,提出针对性、全局性和建设性的建议,以期为社区健康管理的发展提供参考。

Issues and thoughts on community health management from the perspective of comprehensive health

Health-centered community health management is committed to improve the health status of community residents and raise the health level of community people.On the premise of Healthy China and comprehensive health,based on the concept and theoretical framework of community health management,combined with the current common community chronic disease health management model and the life cycle health management model,this paper analyzed the key problems of community health management services in the aspects of department coordination ability,talent construction,service standards and self management concepts,and put forward targeted,overall and constructive suggestions,so as to provide reference for the development of community health management.
