The article introduces the Vygotskian tradition in the realist theoretical discussion of the structure-agency problem. Archer's concept of internal conversation is discussed in terms of internalization and externalization of conversational dynamics. The article addresses in particular the methodological issue of observing how external events trigger internal use of language, and how these internal dynamics are externalized. The experience of talk is proposed as a conceptual key to the understanding of internal conversations and of the relation between structured activity and agency. The experience of talk is defined with the help of the notions of emotional experience, personal sense and inner speech, as they are conceptualized in activity theory and in particular in the works of L.S. Vygotsky and A.N. Leont'ev. Students' experiences of critical conversational events with teachers are analyzed on the basis of written autobiographical accounts. The experience of talk emerges from the analysis as a form of emotional experience in which the horizon of the individual's subjective view relates to specific circumstances external to the individual. Autobiographical accounts of critical conversations are suggested as a type of data which allows access to the experience of talk. 相似文献

Abstract Oregon's Death with Dignity Act (ODWDA) has been in effect for eight years. The United States Supreme Court recently decided that Oregon's law did not violate the Controlled Substances Act. Other states may consider a law similar to Oregon's through legislative process or ballot measures. Although social work is not mentioned in the law, our profession interfaces with the terminally ill, particularly in hospice. Eighty-seven percent of those who have used the law were enrolled in a hospice program. As a pilot project, this article explores conversations that the authors have had with patients, families, team members and health systems in hospices and oncology settings under Oregon's unique environment. The following four themes emerged from these conversations: (1) mental health, education, choice; (2) team concerns; (3) family issues; and (4) values, ethics, restricted conversations and professional struggles. 相似文献

This article reports on the themes generated by face-to-face interviews conducted with 10 men over the age of 50, currently homeless and residing in an overnight homeless shelter. In-depth interviews were conducted asking these men to tell their stories, talk about their lives in the shelter, and speak about their needs, hopes, and dreams. Based on an analysis of interviews, the following themes emerged and were presented: Talk of Work, Barriers to Housing are Overwhelming, Family Estrangement, and Coping Strategies. Discussion of these themes is presented and practice, administrative and research implications are suggested. 相似文献

Family members of persons with advanced dementia may be asked to make complex treatment decisions without having adequate knowledge regarding the risks and benefits. This 6-month, prospective, randomized trial tested the effect of an intervention consisting of a face-to-face, structured conversation about end-of-life care options with family members of nursing home residents with advanced dementia. A comparison group received only social contact via telephone. Structured conversations between a palliative care team and intervention group family members included goals of care and how best to achieve those goals, and provision of psychosocial support. Psychosocial support was also provided via telephone at three 2-month intervals. Family members participated in three telephone interviews: baseline, 3, and 6 months. Specific advance directives for persons with dementia were extracted from medical records. Results showed that intervention families had higher satisfaction with care than comparison families at the 6-month time point, and they were more likely to have decided on medical options listed in residents' advance directives (Do Not Resuscitate, Intubate, Hospitalize) over time. Study findings reinforce the need for increased education and support for families around issues of end-of-life care decisions for advanced dementia. 相似文献

The current study has two aims: (1) to examine associations between the emotional content of parent–child past event conversations and two aspects of children's self‐concept—moral self and self‐esteem; and (2) to examine the degree to which talk about past events is uniquely associated with self‐concept when compared with talk about ongoing events and situations. Fifty‐one five‐ and six‐year‐old New Zealand children and their parents discussed four emotional past events and two ongoing conflicts. Children's moral self, self‐esteem and language ability were also assessed. When parents referred to a greater number of positive emotions and evaluations, regardless of conversation type, their children had higher self‐esteem. Past event talk also uniquely predicted children's self‐esteem: Parents who used more explanations during conversations regarding past negative emotions, and more explanations and confirmations of past positive emotions, had children with higher self‐esteem. We discuss these results with respect to an autobiographical memory approach to self‐concept development. 相似文献

We examined associations of maternal and child emotional discourse and child emotion knowledge with children's behavioral competence. Eighty‐five upper middle‐income, mostly White preschoolers and mothers completed a home‐based bookreading task to assess discourse about emotions. Children's anger perception bias and emotion situation knowledge were assessed in a separate interview. Children's prosocial behavior, relational aggression, and physical aggression were observed during a preschool‐based triadic play task. Mothers' emotion explanations were correlated with children's emotion situation knowledge and relational aggression. Both mothers' and children's emotion explanations predicted prosocial behavior whereas mothers' use of positive emotional themes was negatively associated with children's anger perception bias. Physical aggression was predicted by mothers' emotion comments, children's anger perception bias, and lack of emotion situation knowledge. Maternal emotion socialization variables were less strongly related to children's behavioral competence after accounting for demographics and child emotional competence. Implications of these findings for future research on emotion socialization are discussed. 相似文献

Final conversations (FCs) are defined as the communicative interactions, both verbal and nonverbal, that occur between terminally ill patients and relational partners. In this study, the “Final Conversations Scale” was developed and tested. A total of 152 participants that had engaged in final conversations with individuals that were terminally ill completed the newly developed instrument. Factor analysis produced a five-factor structure, including: messages of spirituality/religion; expressions of love; proactive difficult relationship talk; everyday communication; and talk about illness/death. Participants' perceptions of the relational closeness and difficulty with the deceased significantly influenced the individuals' recalled frequency of FCs messages. Practical and scholarly implications focus on the needs of the family members regarding their communication with terminally ill individuals, as well as directions for future research with the FCs Scale. 相似文献

This study examined the linkage between low-income mothers' conversations about emotions and their children's understanding of emotion. Forty-five low-income preschoolers and their mothers were videotaped while viewing a wordless picture book designed to elicit talk about emotions. Three maternal and child emotional language behaviors were coded from the videotapes: (a) unelaborated comments about emotions; (b) explanations about the causes and consequences of emotions; and (c) empathy-related statements. The children's questions about emotions were also coded. In a separate interview, the preschoolers were administered tasks that assessed emotional expression knowledge, emotional situation knowledge, and emotional role-taking. The results revealed that emotional situation knowledge was positively predicted by mothers' empathy-related statements. Mothers' explanations about the causes and consequences of emotions were uniquely related to emotional role-taking ability. There were very few correlations between the mothers' and children's talk about emotions. Results are discussed in terms of the functional significance of mothers' emotional language for young children's emotional competence. 相似文献

Education about end-of-life care and treatment options, communication between family and health care providers, and having advance directives and medical orders in place are important for older adults with chronic, progressive decline and end-stage disease who spend their last days in the nursing home. This study used retrospective data (6 months before death) of long-stay nursing home decedents (N?=?300) taken from electronic health records to capture the end-of-life experience. Findings showed for almost all decedents, Do Not Resuscitate and Do Not Intubate orders were in place, and just over one-half had Do Not Hospitalize and No Artificial Feeding orders in place. A small proportion had No Artificial Hydration or No Antibiotic orders in place. Overall, there was congruence between documented medical orders and treatment received. Findings showed that use of hospice and discussions about particular life-sustaining treatments each had significant associations with having less aggressive medical orders in place. These results can inform best practice development to promote high quality, person-directed, end-of-life care for nursing home residents. 相似文献

This study investigates processes within marital discussions about end-of-life medical treatment preferences. The associations among marital quality, the perception of having been understood following discussion, and intentions for whom to appoint as a health care surrogate are addressed. Data from 2,969 white married couples in their mid-60s are analyzed. Four-fifths of primary respondents reported that their partners understood their preferences extremely well. Primary respondents' perceptions of high marital quality were associated with feeling extremely well understood. In turn, feeling understood partially mediated the relationship between perceptions of marital quality and the intention to appoint the partner as durable power of attorney for health care (DPAHC). Implications for research on marital communication and quality end-of-life health care are discussed. 相似文献
