This paper discusses a qualitative and quantitative study of the circumstances of 20 Pakistani and Bangladeshi families with one or more severely disabled children living in Birmingham, England. Parents and other adult carers were interviewed using a combination of structured questionnaires and a semi-structured interview schedule focusing on the families' material circumstances, their use of formal services, informal care arrangements, and aspects of the parents' social and psychological well-being. The study suggests that previous national surveys of disabled families may have under-estimated the extent of material disadvantage, while it confirms that health and social care professionals should not assume that Pakistani and Bangladeshi parents have recourse to high levels of extended family support. The combination of disadvantaged circumstances and difficulties in securing access to appropriate services, which are found for the majority of families with a disabled child, was particularly acute for these ethnic minority families, suggesting the additional dimension of institutional racism.  相似文献

This article explores how paid family eldercare in Sweden is reshaped at the intersection of marketisation, accommodation to ethnic diversity and globalisation of international migration. Using a mixed-methods case study approach, the paper examines how implementation of customer choice in publicly funded homecare services to older adults in the city of Stockholm interacts with paid family care. The results show that some private homecare companies employ family caregivers as a business strategy; moreover, that the majority of employed family caregivers are foreign-born women coming primarily from non-European countries. The findings point towards gains but also risks for all parties involved. Though the family caregiver is ensured an income, the employment is generally associated with low wages and weak social security. In addition, employed family carers often lack formal training, which affects documentation procedures and monitoring of the daily care work. It is therefore difficult to evaluate the quality of care services performed by employed family carers. The results indicate a need for policy-makers to reconsider how customer choice in eldercare interacts with paid family care. Otherwise, unintended consequences may result in negative effects for integration as well as social work practice with foreign-born older adults.  相似文献

This article explores the relationship between the child protection systems and the Nordic welfare state in Denmark, Finland and Norway. Despite an ideological focus on prevention and provision of a fair amount of universal services and in-home child protection services for families and children, there is, nevertheless, a relatively high number of children in the child protection system, even children placed out of their home. The outcome of child protection seems to be relatively poor. One of the problems in the present child protection systems is related to questionable service provision within the system. Other problems are about the normative foundation for the system, for example, family orientation and the least intrusive principle. Such principles are closely related to the logic of universal services, and may as such ignore the diversity of needs, rights, risks and ages of children needing protection and related services. Consequently, many principles of the present child protection systems are – and will remain – challenged by a child-centric orientation, in which valuing children's needs, rights and voice in situ are central.  相似文献

This article describes a change in team practice from a model targeted at individual families to a combined model that supplements family therapy with group programs for children. Unacceptably long waiting lists for family counselling services, high levels of stress in clinicians, and consumer dissatisfaction motivated this change. Robust quantitative data and a qualitative analysis of the project confirm that the change has made a positive impact and the new treatment model deserves to be continued. The article concludes that internal team changes are insufficient in a treatment context which requires more profound changes beyond the boundaries of the team.  相似文献

The post-adoption service use and needs of 125 families who had adopted children with developmental disabilities were examined using a structured mail questionnaire. Although satisfaction with services used was generally high, many families did not have access to basic family support services such as respite care, life planning and support groups. Income was the only demographic variable which correlated with service need. Middle income families (mean = $30,000 to $34,999) were more likely to need support groups and babysitting for other children than either lower or higher income groups. Of particular note in this study is the relatively high incidence of transracial adoptions. Thirty-one percent of the sample had adopted children of a different race than the family. The implications of the findings and service recommendations are discussed.  相似文献

This paper offers reflections on the challenges of refocusing child and family social work services towards family support in Romania and the UK. It analyses the major reforms to Romania's child care legislation introduced in 1997, identifying political, economic and social factors which create both opportunities and obstacles for changing policy and practice in that country. Ways in which intercountry adoption undermines the development of family support services are given particular attention. The insights generated by the discussion are doubled back to shed new light on the continuing difficulties of refocusing services from child protection to family support in the UK. The present study examines the relationship between specific areas of family functioning and family engagement in prevention services. A sample of 147 families who initiated youth-centered treatment services from a non-profit agency was selected. Statistical analysis was used to test for relationships between family scores on the Family Assessment Measure (version III, General Scale) and family levels of engagement in services. Results of binary logistic regression analysis indicated that individual and family characteristics are associated with family engagement in services. In addition to demographic variables of age and race, family patterns of denial made a significant individual contribution to the evaluated model. Implications for engaging families in prevention services are discussed and suggestions are made for future research.  相似文献

Financial capability—financial knowledge, skills, and access to financial services—may help smooth transitions to adulthood for youth and is affected by socialization experiences with parents and other family members. Among a sample of youth and their parents enrolled in the YouthSave experiment in Ghana (n?=?4,065), this study examined whether physical access to financial services and/or visiting banks with parents or other family members was associated with youth’s perceptions and understanding of financial institutions. We found a statistically significant association between visiting the bank with a parent or other family member and youth’s attitudes (β?=?1.45, p?<?0.001) and understanding of financial services (β?=?2.96, p?<?0.001), controlling for other factors like household income and assets. Conversely, physical access (distance, travel time) was not associated with financial services attitudes or understanding. Parents may play an important role in introducing their children to financial services. Practitioners and policy makers should consider ways to include parents in efforts to promote financial capability among youth.  相似文献

This study tests the hypothesis that rural development projects and programs reduce rural-urban migration. The author presents various factors in the social theories of migration, including those relating to origin and destination, intervening obstacles such as distance, and personal factors. 3 economic models of migration are the human capital or cost-benefit approach, the expected income model, and the intersectoral linkage model. Empirical studies of migration indicate that: 1) rural areas with high rates of out-migration tend to have high population densities or high ratios of labor to arable land, 2) distance inhibits migration, 3) rural-urban migration is positively correlated with family income level, and 4) selectivity differences in socioeconomic status between migrants and nonmigrants often are grouped into development packages which might include irrigation, new varieties of seed, subsidized credit, increased extension, and improved marketing arrangements. The migration impacts of some of these efforts are described: 1) land reform usually is expected to slow rural out-migration because it normally increases labor utilization in rural areas, but this is a limited effect, 2) migration effects of the Green Revolution technology are mainly in rural-rural migration, and 3) agricultural mechanization may stimulate rural-urban migration in the long run. Development of rural social services migh have various effects on rural-urban migration. Better rural education, which improves the chances of urban employment, will stimulate rural-urban migration, while successful rural family planning programs will have a negative effect in the long run as there will be reduced population pressure on arable land. Better rural health services might reduce the incentive for rural-urban migration as well. It is suggested that governments reconsider policies which rely on rural development to curb rural-urban migration and alleviate problems of urban poverty and underemployment.  相似文献

This paper argues that family therapy is failing to attend to the contexts in which family mental health services are provided and, therefore, is losing touch with the realities of family services in communities. We present a model for describing the institutional contexts of family mental health treatment in North America, and explore how these contexts influence family treatment. The model proposes that family mental health care can be categorized into three levels, analogous to the levels of the health care delivery system: (a) primary, (b) secondary, and (c) tertiary care. These levels represent systematically different contexts for family treatment; each has unique advantages and limitations. Translating treatment methods across levels can be hazardous because of differences in contexts. We argue that delineating the contextual levels of family mental health care can encourage more fruitful and respectful collaboration among the diverse professional groups working with families.  相似文献

Long overlooked by the law, the area of family violence is one of the most strident examples of the law's potential impotency when unsupported by strong enforcement procedures and serious re-education programs. The law, by itself, is simply a grouping of words and phrases, innocuous in and of themselves. A law must have the backing of the community for which it is passed. If it means a major change in the lifestyle of the people, concurrent services must be made available to facilitate that change. A domestic violence law without necessary support services involving medical, legal, psychological, economic, and child care needs may as well not exist at all. Even though most states have passed some recent legislation involving family violence, much more needs to be done in order to adequately implement those laws. The major thrust of this article seeks the development of comprehensive domestic violence programs which take into consideration all aspects of the problem.  相似文献

In Sweden, care of elderly people is a public responsibility. There are comprehensive public policies and programs providing health care, social services, pensions, and other forms of social insurance. Even so, families are still the major providers of care for older people. In the 1990s, the family was "rediscovered" regarding eldercare in Sweden. New policies and legislative changes were promoted to support family caregivers. The development of services and support for caregivers at the municipal level has been stimulated through the use of national grants. As a result, family caregivers have received more recognition and are now more visible. However, the "Swedish model" of publicly financed services and universal care has difficulty addressing caregivers. Reductions in institutional care and cutbacks in public services have had negative repercussions for caregivers and may explain why research shows that family caregiving is expanding. At the same time, a growing "caregivers movement" is lobbying local and national governments to provide more easily accessible, flexible, and tailored support. In 2009, the Swedish Parliament passed a new law that states: "Municipalities are obliged to offer support to persons caring for people with chronic illnesses, elderly people, or people with functional disabilities." The question is whether the new legislation represents a paradigm shift from a welfare system focused on the individual to a more family-oriented system. If so, what are the driving forces, motives, and consequences of this development for the different stakeholders? This will be the starting point for a policy analysis of current developments in family caregiving of elderly people in Sweden.  相似文献

Informal care provided by family and friends is widely recognized as one of the key factors in keeping long-term care financially manageable for individuals as well as for public programs. Sociodemographic trends predict that the demand for formal and informal home care services among the elderly will increase faster than the supply. Programs that allow volunteers to earn credits later redeemable for comparable services when they may be required are beginning to be examined as a way to help fill the need for respite services and other basic home care services. This paper examines key considerations of the service credit concept in the context of existing programs and initiatives designed to encourage its development.  相似文献

"In recent years Asian-Australians, especially those born in Indochina, have become a target of criticism by anti-immigration groups which accuse them of choosing to live only in ?closed societies' and ?ethnic ghettos' in Australian cities.... Evidence presented in the article shows clearly that while immigrant groups may prefer to settle in close proximity of their family and kin for practical and/or emotional reasons, their preference is not ethnically determined.... We also show that affordability, closeness to spouse's work, and neighbourhood services, rather than family and social contacts, are important factors for Asian-born immigrants when they move." This paper, which is authored by members of the Japanese Association of Family Therapy (JAFT), describes the COVID‐19 pandemic in Japan from a family systems perspective. The authors are active members of JAFT and include current and past presidents and officers. We describe the course of the pandemic and the ways in which government policies to mitigate the pandemic have affected Japanese families. Challenges that affect Japanese families include the inability to participate in family and social rituals, prescribed gender roles that specifically affect women, high suicide rates, and prejudice against those who are at risk of spreading the infection. The need to shelter in place has also forced family homes to function as a workplace for parents, classrooms for children, and day care services for frail elders, which has resulted in psychological distress among individuals and conflicts among families. We discuss ways that therapists have worked with Japanese families using online therapy.  相似文献

Many of the Australian family support schemes are income-tested transfers, targeted towards the lower end of the income distribution, whereas the Norwegian approach is to provide subsidized non-parental care services and universal family payments. We contrast these two types of policies and discuss policy changes within these policy types by presenting results from simulations, using microsimulation models developed for Australia and Norway. Labor supply effects and distributional effects are discussed for the hypothetical policy changes of replacing the means-tested family payments of Australia by the Norwegian universal child benefit schedule and vice versa, and of reducing the childcare fees in both countries. The analysis highlights that the case for policy changes is restricted by the economic environment and the role of family policy in the two countries. Whereas there is considerable potential for increased labor supply of Australian mothers, it may have detrimental distributional effects and is likely to be costly. In Norway, mothers already have high labor supply and any adverse distributional effects of further labor supply incentives occur in an economy with low initial income dispersion. However, expenditure on family support is already high and the question is whether this should be further extended.  相似文献

The availability of family can be considered a protective factor for aging well. In this article, we examine to what extent the family situation of older people creates vulnerability with respect to their quality of life. Because not everyone is vulnerable to the same degree, we try to identify the conditions under which older people benefit more from having family resources. Based on the resources perspective, we argue that the impact of family resources on life satisfaction is stronger for older people with fewer resources at both the individual level (material, physical and non-familial social resources) and the country level (welfare state services targeted at older adults). To test our hypotheses we make use of the fourth wave of the European Values Study, and the MULTILINKS Social Policy Indicators database. In general our data offer support for the idea that the presence of intimate family ties (with partner and children) can be considered an important resource for achieving psychological well-being, whereas their absence or loss may act as a constraint. Our vulnerability argument is partly supported by the findings. Partner resources are more important for the life satisfaction of older people with a low education and health problems. Similarly, having children only improves the life-satisfaction of lower educated older adults. However, family resources are not more important for older people with fewer material resources or for older people living in countries with low services levels targeted at older adults.  相似文献

Adult siblings raising their younger siblings is a family configuration that rarely comes to mind; yet, in the United States adult siblings are the third largest relative caregiver group. The experiences of a sample of 77 adult siblings raising 154 younger siblings are described. The findings revealed that adult sibling caregivers may have multiple unmet service needs but they have a relatively high degree of parenting ability, which is increased by the availability of religious-based services, availability of friends and neighbors, and the ability to network with other caregivers. Additionally, adult siblings who are parenting a younger sibling who has special needs are more likely to commit to adopting that sibling. Social work practice strategies that can be used to address the service needs of adult sibling caregivers are provided.  相似文献
