Medical cannabis physician decries lack of education

“This is all new to them.” That's how David G. Ostrow, M.D., Ph.D., vice president of the American Academy of Cannabinoid Medicine (AACM), describes medical cannabis, which has actually been legal in some states for many years. It was the precursor to recreational cannabis, but it's not the same thing. Patients with real medical problems ask their doctors about it — and their doctors don't know what to say.     

Twenty‐first century dis‐ease? Habitual reflexivity or the reflexive habitus

While certain theorists have suggested that identity is increasingly reflexive, such accounts are arguably problematised by Bourdieu's concept of habitus, which – in pointing to the ‘embeddedness’ of our dispositions and tastes – suggests that identity may be less susceptible to reflexive intervention than theorists such as Giddens have implied. This paper does not dispute this so much as suggest that, for increasing numbers of contemporary individuals, reflexivity itself may have become habitual, and that for those possessing a flexible or reflexive habitus, processes of self‐refashioning may be ‘second nature’ rather than difficult to achieve. The paper concludes by examining some of the wider implications of this argument, in relation not only to identity projects, but also to fashion and consumption, patterns of exclusion, and forms of alienation or estrangement, the latter part of this section suggesting that those displaying a reflexive habitus, whilst at a potential advantage in certain respects, may also face considerable difficulties simply ‘being themselves’. ‘I noticed how people played at being executives while actually holding executive positions. Did I do this myself? You maintain a shifting distance between yourself and your job. There's a self‐conscious space, a sense of formal play that is a sort of arrested panic, and maybe you show it in a forced gesture or a ritual clearing of the throat. Something out of childhood whistles through this space, a sense of games and half‐made selves, but it's not that you’re pretending to be someone else. You’re pretending to be exactly who you are. That's the curious thing.’ ( DeLillo, 1997 : 103)

I have written this paper with the intention of reading it out loud to you. As I was writing this paper, I imagined your skin, that soft outer membrane of your body. I imagined my tongue moving to shape my words, all the while carefully shifting my teeth out of the way. I imagined having these words thump under your skin and tremble your presence to the beat of our hearts. Because this paper was supposed to have you feel me and my people. And by “my people,” I truly mean everyone around me, including you. So, this paper is essentially about us, you and me. You know, I just had a feeling that you might know us, but you do not feel us. I just had a feeling that you have got to feel how we are. … Do you hear me? Because, only then, can we finally, start.

Constructing a Life That Works: Part 2, An Approach to Practice

The 1st article (C. Campbell & M. Ungar, 2004) of this 2‐part exploration of postmodern approaches to family therapy and narrative approaches to career counseling explored the differences between traditional trait and factor counseling models and postmodern approaches using life narratives and social constructionism. In this 2nd article, the authors discuss 7 aspects of their practice as postmodern career counselors that ask clients to (a) know what you want, (b) know what you have, (c) know what you hear, (d) know what constrains you, (e) map your preferred story, (f) grow into your story, and (g) grow out of your story. Several case studies demonstrate the applicability of these theories to practice.     

Spiralling Up or Spinning Out: A Guide for Reflecting on Action Research Practice

This paper offers a theoretical ‘action research spiral’ model to guide reflection on the dual client focus of action research. While theoretically exploring and highlighting the tensions and dilemmas created by this dual client focus, this paper will argue for a greater degree of reflection on action research practice and utilize vignettes from action research cases to illustrate those reflective processes. It is concluded that further discussion and reflection on the process of action research is an important component of social science’s contribution to phronetic knowledge.

I saw the University as helping us to reflect on what we are doing—they are the expert reflectors. This is particularly what I saw as X’s role. Sometimes his interjections go above their heads and his ‘nine words or less’ statements need to have some explanation, and I should feed this back to him. I also see the University as playing a visionary role, helping to show us new things about what is possible. I don’t see the University as helping to pull the team together—that is when it gets confusing. They are observing us, they are looking at us as the rats, and when they see something that they think needs to be addressed, they can feed this back to us—and this is where teaching and formal learning comes in. This is a difficult role for the University. I can see some of the University people just squirming—you can see it in their face that they want to intervene. They know something about what we are doing but are not imparting the knowledge. This can piss people off. They are withholding what they know and not helping. But it can also piss people off if they come in too early and tell us what is going on and what to do and not let us wallow around for a while, and learn. This is what I see as a major problem for the University. As you observe us, at what point do you reflect the learning and feedback, and yet not prostitute the learning or dirty the data. …We are the rats, the factory is your laboratory. Professionals' Accounts for Work‐related Deceptions

Using interview and observational data, this article analyzes how private detectives account for using work‐related deceptions, I place special emphasis on how practitioners' accounts draw on professional affiliations and organizational resources that are less available to individuals in their private lives. These affiliations and resources include economic and demographic characteristics of employers, practitioners, clients and investigative targets, state licensing, the profession's social repute, and asymmetries in specialized knowledge between practitioners and laypersons. The conclusion addresses how accounts for work‐related deceptions benefit professionals through advantaging them over targets, obscuring harmful consequences of work, and helping them and their clients to avoid negative labeling. If you call someone up and say, “Hi, would you mind telling me where your brother is, so that I can put his ass in jail for the next five years,” you won't last too long in the private detective business. —Anonymous private detective     

Is generic buprenorphine film as good as Suboxone?

When Indivior's popular Suboxone film went off patent, Dr. Reddy's was ready with the generic, and there are more companies now providing it as well — including Indivior itself (distributed by Sandoz). The point of generics, from a patient or payer point of view, is that they are less expensive than brand. But there can be problems — with quality, including of active ingredients. Patients frequently complain when their agonist medications are switched — whether it's from one type of methadone to another or, now, from Suboxone film to a generic.

STRATEGIES FOR ENGAGING ADOLESCENT GIRLS AT AN EMERGENCY SHELTER IN A THERAPEUTIC RELATIONSHIP: RECOMMENDATIONS FROM THE GIRLS THEMSELVES

Engaging clients is an extremely important part of the therapeutic process. Although there is a literature on adult engagement, few articles discuss adolescent engagement. Those articles that do discuss adolescent engagement have been conducted from the perspective of adults. The purpose of this study was to explore, from the client's perspective, ways to engage and build a positive therapeutic alliance with adolescent girls. A focus group (N = 5) was conducted with residents of an emergency shelter for adolescent girls in an urban area. Clients were asked three questions: ‘If you could tell a counselor anything, what would you tell her/him?’ .‘What do counselors need to know?’ and ‘How can a counselor get you to talk?’ Seven messages emerged from the clients' responses, which focused on a request to be respected, listened to, and not judged. More specifically, themes included ‘Treat me like I'm on your level’, ‘Tell me a little about yourself’, ‘Ask my permission to take notes’, ‘Pay attention to what I'm saying’, ‘Tell me what you're doing’, ‘Don't tell me what's in my file’, and ‘Don't call me names’. Clients provided concrete ways in which social workers and other counseling professionals could better work with them.

Constructions and reconstructions of self in virtual reality: Playing in the MUDs

There are over 300 multi‐user games based on at least 13 different kinds of software on the international computer network known as the Internet. Here I use the term “MUD “ to refer to all the various kinds. All provide worlds for social interaction in a virtual space, worlds in which you can present yourself as a “character,” in which you can be anonymous, in which you can play a role or roles as close or as far away from your “real self as you choose.

In the MUDS, the projections of self are engaged in a resolutely postmodern context. Authorship is not only displaced from a solitary voice, it is exploded. The self is not only decentered but multiplied without limit. There is an unparalleled opportunity to play with one's identity and to “try out” new ones. MUDS are a new environment for the construction and reconstruction of self.     

Five artists' valuations of the visual

Abstract

This paper comprises interviews with five practising artists: Terry Atkinson, Torie Begg, Rebecca Fortnum, Lubaina Himid, and James Hugonin. The aim is to gauge the ways in which ‘the visual’ features in contemporary fine art practice, not only at the end of a century in which the visual ‐ at least for much of the first half of that century ‐ was often accorded special status, but also, more recently, after Conceptualism and radical egalitarian interventions have challenged the status of and assumptions about the value of the visual. All five artists were asked the same questions: What/who are the formative influences on your work?; What role does the visual play in your work ‐ to what extent do you value the visual?; Can you say something about (i) aesthetic and (ii) pleasure ‐ however you define them?; How does a work evolve?; How do you define quality ‐ what makes you think one of your works is better than another?; What is the spectator's relationship to your work? ‐ what role does the spectator have? The responses bear witness to significantly different ways in which contemporary artists value the visual.

Why They Don't Come Back: A Clinical Perspective on the No-Show Client

Clinical writers love to publish their success stories. Treatment failures get shuffled to the bottom drawer. Perhaps this is why there is so little in the clinical literature about a problem so frequently encountered by the clinical social worker: the patient or client who never shows up or doesn't return. Why don't they keep their appointments? What are the most common dynamics? How much of the problem rests with the client and how much with the clinician or agency? What should be done when your client “no-shows”? When should you do it? This paper will explore these questions in an attempt to deepen the practitioner's understanding and management of this common clinical challenge.     

Taking on the Role of Doctor: A Dramaturgical Analysis of Professionalization*

This paper applies dramaturgical analysis to the study of professional socialization in an innovative medical school. Using concepts from the theater, professionalization is seen as critically involving performances before legitimating audiences. The data were collected by means of participant observation and interviews of a cohort of students as they proceeded through the professionalizing career. Good acting demands that you are convincing in your part. The audience must be willing to take part in a ritual, in which you represent, say, Hamlet. You must be able to sustain this representation convincingly: that is to say, you must consistently satisfy the audience's imagination and never outrage its acceptance of the fact that yours is a convincing, or consistently credible, Hamlet,…or whatever character you are impersonating (Guthrie, 1971:11).
