Drawing on theoretical accounts of institutional change, this study explored the politics of welfare regime transformation in regard to Turkey's unemployment compensation system. By using the institutionalist approach, the study shows that the process of welfare regime change was one of “institutional layering” of unemployment insurance (UI) over severance pay. Also, the study demonstrates that the economic bureaucracy played a key role in pushing the establishment of UI (state‐centric approach) in contrast to the class‐based organizations that focused their struggles on the severance pay scheme (power‐resource perspective). However, the economic bureaucracy preferred a rudimentary UI design, which prevented UI from undermining the vested interests behind the severance pay scheme. Furthermore, subsequent attempts at the reformation of the severance pay scheme were not successful because the social welfare bureaucracy lacked the capacity to develop a policy alternative to resolve the stalemate between the societal actors. Lastly, the study used the successful severance pay reform experiences of South Korea and Austria to locate the Turkish case within a broader comparative framework.  相似文献

This article examines the social care of older people in six contrasting European countries. Family, institutional and community care are compared, focusing on vulnerability, empowerment and the gatekeeping of resources. The article considers the position of older people in each care system by presenting individual case studies. The six countries include the family-oriented systems of Ireland, Italy and Greece, and the individual-oriented systems of Denmark, Norway and England. To improve the care of older people in any of these welfare cultures, resources need to be developed that work with existing sources of care but extend the rights of older people, at least to assessment and an equitable matching of needs to the care services available. Overall, the different levels of provision of organised social care services are a major aspect of inequality within and between the countries. Whilst there is little prospect for any major policy transfer across national boundaries, there is potential for selective cross-national learning with regard to particular service developments.  相似文献

An estimated 225 million Chinese people have migrated to cities from China's rural areas over the past two decades. These rural‐to‐urban migrant workers have greatly challenged China's welfare system. The pre‐reform welfare system was a duel scheme with an urban–rural distinction in which rural residents were not covered by state‐run welfare programs and had to rely on their families and rural collectives. The development of employment‐based social insurance programs in 1999 made social welfare programs available for rural‐to‐urban migrant workers. Using an anonymous survey conducted in seven cities across China in 2006, we found that social insurance program participation rates were low among rural‐to‐urban migrant workers. Individual factors, including lack of knowledge of welfare programs and of a willingness to participate, and macro‐level factors, including type of employer and industry, are critical in determining migrant workers' participation in welfare programs. 改革以后,中国政府在承担卫生保健职能方面的意愿和能力发生了变化,本文考察了这些变化对城镇卫生保健筹资和服务公平性的影响。文章着重分析了两个相关问题:在多大程度上,卫生保健的筹资具有累进性?在多大程度上,医疗服务的利用是公平的?第一节解释为什么在改革开放期间中国政府承担卫生保健的意愿与能力都有所下降,并探讨这些变化对卫生总费用的结构产生了什么样的影响。接下来的两节分别对中国城市卫生保健筹资和服务利用的不平等程度进行了实证分析。第四节进一步指出,筹资与服务方面的不平等将本来就处于弱势的社会群体置于更加不利的地位。总之,以市场为导向的卫生体制改革不但没有解决医疗费用上涨问题,反而加剧了这一问题;它因此损害了卫生保健筹资的公平性,降低了弱势人群对卫生服务的获取,增加了因病致贫的几率。  相似文献

The aim of the study was to explore stability and change in mental health problems in Norwegian children aged 6–12 years old (n = 70) in long‐term out‐of‐home care. The children's mental health problems were assessed shortly after the placement and 7–8 years later by the caregivers and the teachers on the Revised Rutter Scales. Information on pre‐placement and placement factors were collected from the child welfare workers. At a group level the children's mental health problems had improved significantly over time according to the teachers' reports. According to the caregivers' reports, however, the children's problems were high and stable across time. Analyses aimed at detecting individual changes revealed a great variability in development according to both informants, indicating that treating the placed children as a homogenous group could be misleading. Several pre‐placement and placement variables were associated with the change in the children's mental health problems from the time of placement to the follow‐up time according to both informants' reports. However, all the predictors were accounted for by the strong effect of the children's problem scores when entering care.  相似文献

This article illustrates the emergence of radical local welfare initiatives as a political response to the imperfect national program in decentralization context in Indonesia. In order to gain further understanding of the topic, it is worth reviewing Kulon Progo Regency's experience which recently embarked on removing class stratification at any in‐patient room in all local government‐owned hospitals through “classless hospital policy” initiatives. Using exploratory case study method, this article aims to review the ideational constructions of healthcare decommodification that is displayed on this initiative. It is concluded that the classless hospital policy reflects how social citizenship was organized through the mechanism of idea contestation which originated in the past community's behavior, combined with the vested interest of political regime for then subduing market logics under state power. This circumstance ultimately has provided the groundwork for encouraging innovative welfare outcome.  相似文献

In China, family care is the dominant form of care for people with mental disorders. Since 2004, the government has been developing a community‐care model that places more responsibility on community organisations and the local governments at the provincial, municipal and county levels for the provision of formal care. As a large number of people with severe mental disorders live in rural China, this case study was conducted in a rural county in order to examine the development of community care. It was found that, although family care remains dominant, families’ need for formal care is increasing. Community services have improved, but their development is constrained by several contextual and micro factors. In this study, it is argued that the community‐care model introduces a process of reconfiguration of the relative responsibility for care among the family, social organisations and the government, but progress depends on further administrative and fiscal reforms.  相似文献

Not only has research over the past decade documented the emotional and behavioural consequences for children who witness domestic violence, but a number of studies have used children as participants thus, giving them an opportunity to describe their experiences in their own words. In policy terms, there has been a growing emphasis on children's rights and the importance and understanding of children's perspectives on their own lives. Consequently, children can no longer be perceived as forgotten victims where domestic violence is concerned. This paper explores practitioners' awareness of the needs of children and young people living with, and fleeing from, domestic violence. The research, conducted in a rural area in Wales, reveals that although the views of practitioners reflect the concerns reported by young people in other studies, there can be barriers to meeting these needs. While policy prescribes engaging with children, at the institutional level, operational priorities and increasing administrative demands can actually reduce opportunities for working directly with children. These demands may hamper the development of multi‐agency practice.  相似文献

However, the steps introduced in response to growing demands for intervention by the authorities in favour of dependent persons have remained poorly documented in the international literature. We analysed narratives of motherhood produced by 13 women who were involved with the welfare system or the justice system during adolescence. The original contribution of our study was its focus on mothers who were turning age 30, so they were not in that period of generalized instability that characterizes the transition from care and into adulthood. This qualitative study was part of a larger study on French‐Canadian adolescents with a history of residential care. Semi‐directed interviews were conducted with 13 women, more than 15 years after their admission to residential care. Our data highlights that motherhood can contribute to the vulnerability of women who were involved with the welfare or the justice system as adolescents. Our results suggest that in order to shed a perceived deviant label and to compensate for the adverse events they experienced, they pursue internalized ideals of “good motherhood” that translate into restrictions and strain. Furthermore, they tend to refrain from allowing people into their lives and asking for help for fear of being judged. Yet, as their children are getting older and exhibiting problem behaviors of their own, the questioning of their parental practices and skills only becomes more intense.  相似文献

The aim of this study was to find out whether elected politicians' attitudes influence policy-making processes in terms of social and health-care resource allocation. A preliminary hypothesis was developed on the basis of previous empirical findings on the attitudes of politicians. If health-care costs decreased during the period 1993–1999, then secondary care costs should have increased and vice versa; if the total amount of resources increased, then secondary care must have been allocated fewer resources. However, the results of this study showed that the actual resource-allocation processes did not reflect the attitudes of local politicians. Expenditure on secondary health care increased during this period, while primary health-care services were allocated fewer resources. Analysing the actual decision-making processes rather than investigating attitudes to priority setting may prove more useful in gaining a better understanding of priority-setting processes and the mechanisms used in different contexts.  相似文献

Cash‐for‐care (CfC) schemes are monetary transfers to people in need of care who can use them to organize their own care arrangements. Mostly introduced in the 1990s, these schemes combine different policy objectives, as they can aim at (implicitly or explicitly) supporting informal caregivers as well as increasing user choice in long‐term care or even foster the formalization of care relations and the creation of care markets. This article explores from a comparative perspective, how CfC schemes, within broader long‐term care policies, envision, frame, and aim to condition informal care, if different models of relationships between CfC and informal care exist and how these have persisted or changed over time and into which directions. Building on the scholarly debate on familialization vs. defamilialization policies, the paper proposes an analytical framework to investigate the trajectories of seven European countries over a period of 20 years. The results show that, far from being simply instruments of supported familialism, CfC schemes have contributed to a turn towards “optional familialism through the market,” according to which families are encouraged to provide family care and are (directly or indirectly) given alternatives through the provision of market care.  相似文献

This article examines parents' involvement in care order decision‐making in four countries at one particular point in the care order process, namely, when the child protection worker discusses with the parents his or her considerations regarding child removal. The countries represent different child welfare systems with Norway and Finland categorized as ‘family service systems’ and the USA as a ‘child protection system’, with England somewhere in between. The focus is on whether the forms and intensity of involvement are different in these four countries and whether the system orientation towards family services or child protection influences practice in the social welfare agencies with parents. Involvement is studied in terms of providing information to parents, collecting information from parents and ensuring inclusion in the decision‐making processes. A vignette method is employed in a survey with 768 responses from child protection workers in four countries. The findings do not show a consistent pattern of difference regarding parental involvement in care order preparations that align with the type of child welfare system in which staff work. The goal in each child welfare system is to include parents, but the precise ways in which it is carried out (or not) vary. Methodological suggestions are given for further studies.  相似文献

Introducing social insurance for long‐term care in Taiwan: Key issues Taiwan will shortly complete its comprehensive social safety net, which includes national health insurance, retirement security, and unemployment insurance, by introducing long‐term care (LTC) insurance – putting it ahead of the many countries that rely on a patchwork of policies to address the need for LTC. The program, to be implemented in 3 to 5 years, will cover all citizens on a primarily social insurance basis. The range of LTC policy options considered is discussed, particularly how to structure the program, how to finance and regulate it, and how to develop its inadequate LTC infrastructure and workforce. Particularly thorny issues include the choice of social insurance, the feasibility of cash benefits, and how to address Taiwan's heavy reliance on foreign workers. Taiwan's increasingly democratic character, along with high levels of public support for the program, creates significant pressure on politicians to deliver on their promises to implement LTC reform. Contact with family for children in care is identified as a right under the United Nations Convention on the Rights of the Child. However, such contact often presents challenges because of the protective concerns that have led to care arrangements being made. The Family Links: Kinship Care and Family Contact research study explored the nature and extent of family contact in kinship care, with a particular focus on the circumstances that create positive contact and foster family relationships. Findings included evidence of a large proportion of parental contact that was negatively affecting children's well‐being, and was sometimes unsafe. By contrast, the frequent contact that children were enjoying with their siblings and wider family was reported to be mostly positive and supportive. Kinship carers described a range of services needed to facilitate more positive parental contact and to enable children to keep contact with significant family members.  相似文献

This paper investigates migrant workers’ endangered pension entitlement in China from a perspective of political economy. The decentralized pension governance structure in China led to limited portability of the urban pension program. The movement of migrant workers challenges this decentralized governance structure, and have also produced clear winner and loser provinces in the pension program. This study argues that the interests of migrant‐receiving provinces, the winners, became a major obstacle in reforms that sought to address the pension portability problem. The migrant workers’ pension issue has in fact contributed to inter‐regional welfare inequality. This resulting inter‐regional inequality makes any central government's plan on centralizing the pension program a challenging task.  相似文献

ABSTRACT

This study aimed to compare specific work related-factors (personal factor: length of work experience; structural factors: personal subjection to violence and fear of being subjected to violence; and psychological factors: professional quality of life—compassion satisfaction, burnout, and compassion fatigue) between welfare social workers (SWs) (n = 200) and health care and community SWs (n = 173). Furthermore, since the issue of turnover among SWs has important implications for the provision and programming of social services, the study also aimed to assess the relationship between these factors and SW intention to leave the profession. For the purpose of this study, online questionnaires were distributed to SWs working at social agencies and services. The findings suggest that the two categories of SWs showed a similarity with regard to the association between the personal and structural factors and intention to leave the profession. However, with regard to the psychological factors, the components of professional quality of life were found to operate differently in each category of SWs. It is suggested that future research examine other work-related factors and explore the intention to leave the profession among other social work specializations and employment sectors.  相似文献

In this article, I review a range of theoretical and practical issues that are relevant to the foster care of teenagers. I argue that topics such as the recent research evidence from neuroscience regarding brain development in adolescence, as well as control, the promotion of autonomy, and resilience, all have major implications for the provision of appropriate care for teenagers. I outline some of the key conclusions reached by researchers on these pertinent topics and then go on to consider the implications from this evidence for the training of foster carers.  相似文献

The Mothers Unit is a reunification and treatment programme in a welfare emergency centre in Israel. The unit is the only one in Israel offering joint residence for mothers at risk of abusing or neglecting their children. The unit provides an alternative to out‐of‐home care for young children suffering from maltreatment in order to enable them and their mothers to return to the community together at the end of the treatment. The current qualitative study examines the lived experiences of the women and their children from the subjective perspective of the women currently or previously involved in the programme and of the professionals working with them. Thematic analysis of 36 in‐person semi‐structured interviews reveals the multiple meanings they attribute to life in the Mothers Unit and its perceived effects on their children and themselves.  相似文献

Since 2014, it has been a legal requirement in England and Wales for child care proceedings to be concluded, apart from “exceptional cases,” within 26 weeks. When this was first proposed, there were concerns that it might lead to poorer decision‐making or to delay being squeezed to either side of the court proceedings, before or afterwards. This paper reports on the messages from a pilot programme to hit the 26‐week target that took place in London in 2012–2013. The study compared the progress of the cases from the pilot year with those the year before, 180 cases in total, involving 256 children. The local authorities involved were able to achieve considerable improvements in timeliness, not just in the proceedings, but for the pre‐ and post‐court processes too; and the quality and fairness of decisions did not seem to be impaired, in terms of the plans for the children and subsequent outcomes over a period of 2 years. “Targets” do not generally find a warm welcome in the social work literature, but this paper shows that when collaboratively implemented, with a measure of flexibility and adequate resources, they can be an effective way of helping to achieve positive change.  相似文献
