Abstract

The rapid growth and increasing visibility of ethnic and racial groups has vigorously challenged social work to educate students for ethnic-competent practice in cross-cultural settings. This article examines an approach to teaching effective intervention involving ethnic and racial groups and helping students integrate cultural factors and society's concerns into clinical practice. Pedagogical issues and classroom processes of teaching culturally sensitive and competent interpersonal practice are discussed. Implications are drawn for culturally sensitive education and issues are raised for critical examination.  相似文献

A systematic literature review was conducted to address the immense national gap in knowledge on child sexual abuse (CSA) and ethnic minority communities in Australia, which necessarily borrowed from overseas. One theme explored within the review was that of prevention, and it was found that school-based programs are the most common type of prevention effort. The literature also calls for ‘culturally tailored’ programs to avoid homogenising victims' needs, however in Western multicultural countries like Australia there is a risk that such school-based programs heighten racism for ethnic minority children. Thus, program elements that are culturally sensitive could be incorporated into universal programs instead. Universal programs are also beneficial because they help reach many children regardless of their cultural background, help send the message that all children are equally valued and protected, help create unity and support among diverse victims, and can be further justified by three relatively stable cross-cultural findings: (i) that the prevalence of CSA is high worldwide, (ii) all children require protection irrespective of gender, and (iii) perpetrators are usually known to the victim. To help mobilise their role, service providers could co-deliver school-based programs especially to address institutional CSA within schools. They could also provide training to other health professionals to improve their identification of CSA and confidence to probe; a form of early intervention and therefore ‘secondary prevention’. Overall, this review argues that the importance of family reputation in collectivist cultures needs to be taken into account when designing and evaluating prevention programs so that delays in disclosure and help-seeking due to cultural pressure are not mistaken as evidence for their ineffectiveness. The findings are significant because they help progress the ‘prevention’ field in Australia where literature is essentially absent, particularly in the school and service system arenas.  相似文献

Child focus is a central construct within Bowen family systems theory (Bowen theory). A clinical implication is that mental health treatment focusing on a child may unwittingly reinforce the operation of child-focused processes, which undermine rather than enhance child well-being. The concept of child focus in Bowen theory presents significant implications for professionals working in school settings and in fields such as children's mental health, which are inherently child-focused. Bowen theory is the guiding theoretical framework for School-Based Filial Therapy (SBFT). SBFT is a play therapy intervention that was initially established in remote and outer-regional New South Wales, Australia in response to the low availability of children's mental health services and the significant barriers associated with caregiver engagement in children's mental health treatment. It involves trained school personnel facilitating therapeutic play sessions with children experiencing emotional–behavioural problems. The intervention occurs on school grounds, during school hours, and children's family members do not participate in the intervention. This mixed-methods study examines the impact of children's participation in SBFT upon family functioning. Interviews with caregivers (n = 10) of children who participated in 10 SBFT sessions were analysed using content analysis. Quantitative data were collected using the Differentiation of Self Inventory – Short Form and Visual Analogue Scale – Family Functioning. A Wilcoxon signed rank test was used to analyse the pre- and post-data. Qualitative outcomes indicated changes in the categories of child functioning, caregiver functioning, and extended family functioning, whilst child-focused processes remained dominant but changed in intensity and valence following SBFT.  相似文献

This study examined the influence of parental expectations on the functioning of sexually abused children. Participants included 67 sexually abused youth and 63 of their nonoffending primary caregivers. Parental expectations about how sexual abuse will impact children were predictive of parents' ratings of children's behavior at pretreatment, while parental expectations of children's overall future functioning were not predictive of parents' ratings of children's behavior. Parental expectations about how sexual abuse will impact their children and about their children's overall future functioning were not predictive of parents' ratings of children's behavior at posttreatment. Results highlight the influential role the sexual abuse label has in shaping parental expectations about children's functioning. Recommendations for research and intervention are discussed.  相似文献

Considering the importance of mother’s support in the adaptation of a sexually abused child, it is relevant to determine if the mothers and children involved in an intergenerational cycle of child sexual victimization differ from dyads in which only the child has been abused. The purpose of this study was to compare motherchild dyads with sexually abused children according to whether the mother had herself been victim of child sexual abuse. The sample included 87 dyads with sexually abused children aged 3–18 years old and their mothers (44 reporting maternal and child abuse), followed by social welfare services of the province of Quebec (Canada). The two groups of mothers were compared on their past family abuse experiences and past family relations, their mental health history, their current psychological distress, their parenting behaviors, and their current levels of family functioning. Children were compared on their adaptation. Multivariate analyses indicated that mothers reporting child sexual abuse were more likely to report more other maltreatments in their childhood and greater prevalence of lifetime history of alcohol abuse disorders, dysthymia, and panic disorder compared with mothers who had not experienced CSA. Compared to children whose mothers had not experienced CSA, those whose mothers had experienced CSA showed higher rates of problems behaviors and were more likely to report having been sexually abused by a trusted person. These results highlight the specific clinical needs for the assessment and treatment for sexually abused children whose mothers experienced child sexual abuse.  相似文献

Currently, African American grandmothers are becoming caregivers to grandchildren whose parents, for various reasons, are not providing them with adequate care. Replacing their adult children, as parents, while continuing to mother them along with caring for grandchildren are essential components of this complex situation. A part of the findings from this qualitative study of the experience of grandmother caregivers indicated that family relations are affected with their grandchildren, the parents of their grandchildren, and their other adult children. Implications for culturally sensitive, family-centered clinical practice and future research are discussed.  相似文献

Abstract

Social connectedness has been promoted as one factor that may facilitate optimal functioning for families experiencing economic adversity. Using data from 218 Australian families, the current study explored the relative importance of social connectedness to the functioning of economically diverse families. Overall, results indicated similar levels of social connectedness and family functioning across low- and high-income groups. However, important differences were identified with regards to the relative contribution of social connectedness to the functioning of families across the economic spectrum. Clinical implications arising from these findings include the potential benefit of exploring social connectedness as a routine part of clinical assessment and ongoing intervention for families experiencing economic vulnerability.  相似文献

ABSTRACT

Childhood trauma can have a lasting effect on personality and daily functioning, leading to dissociative changes in behavior and identity. How these are interpreted and handled usually depends on local culture and beliefs. This article presents the case of a Mauritian woman with a history of childhood abuse who was exorcised to “dispel an evil spirit.” An in-depth interview exploring her experiences and meaning-making was transcribed and subjected to interpretative phenomenological analysis. This article discusses possession as a culturally accepted metaphor for incomprehensible behaviors. It shows that help-seeking pathways are determined by symptom interpretation models that are reinforced by the local environment. It also demonstrates that exorcisms are potentially retraumatizing by the enactment of painful memories and emotions during this ritual. The authors stipulate that exorcism seekers could benefit from additional clinical assessment by professionals experienced in the dissociation field.  相似文献

Previous studies indicate that the proportion of culturally diverse children to White children is increasing in public social service agencies. In addition, culturally diverse children are more likely to receive more intensive and punitive services, are more likely to stay within the system for longer periods of time and are reported more often to Child Protective Services. The purpose of the study was to explore how child welfare practices with Hispanic children are different from those applied with White non-Hispanic children. This study was a retrospective, two year, longitudinal, survival analysis of differential child welfare placement outcomes of White non-Hispanic and Hispanic children/families which had substantiated cases of abuse/neglect (n=1001).Findings demonstrate that although cases reported for abuse/neglect are relatively proportionate between Hispanic and White non-Hispanic children, substantiated cases are more likely to occur with Hispanic children. These children are more likely to be placed out of the home more quickly and for longer periods of time than their White non-Hispanic counterparts. The current study demonstrates the need for increased cultural awareness among Child Welfare professionals, especially in terms of assessment and case decision making, and the need for the development of culturally sensitive training modules for CPS and case management personnel.  相似文献

ABSTRACT

This study aimed to examine the knowledge, attitude, and practice (KAP) of child sexual abuse (CSA) prevention among parents of children with hearing loss. The study involved 127 pairs of parents with deaf and hard of hearing children from three special education schools in Beijing and Hebei province, China. The findings revealed that the parents lacked knowledge about CSA prevention, such as the character of perpetrators and child victims. Parents were supportive about potential CSA prevention education in schools. Most parents told children much about personal safety, but topics on CSA prevention were still insufficient. The study also found that education level was significantly associated with knowledge and attitude. Elder parents knew more and talked more about CSA prevention. Mothers and girls’ parents were more willing to communicate with children about CSA prevention. The conclusion of the study was that parents appeared to have gained a positive attitude, but poor knowledge and practice. Their knowledge and skills of effective communication should be strengthened to promote CSA prevention practice.  相似文献

SUMMARY

This study evaluated whether a brief screening measure of child psychosocial functioning completed by parents could assist drug and alcohol workers to identify which of their client's children needed follow-up support. As part of a broader research and intervention program for 4–13 year old children with substance dependent parents, the Strengths and Difficulties Questionnaire (SDQ) and a broader clinical assessment were completed with the families of 48 children. Validating its usefulness in this population, significant and moderate correlations were found between SDQ scores and workers' assessments of children's social, emotional, and cognitive development and schooling.  相似文献

SummaryLiterature debates whether young persons' participation in social work decisions is tokenistic in nature or it empowers them to shape their world. By and large, young persons deemed at risk remain the targets of intervention. This paper aims to underscore the need to move beyond soliciting young persons' voice merely in order to corroborate information provided by adults and professionals in social work assessment and intervention. Instead, it explicates the philosophical foundation for social workers to treat young persons as knowledgeable agents capable of contributing to professional knowledge.FindingsThrough reinterpretation of a published case study on children of sex worker mothers in India, this paper highlights how young persons are active agents of their own lives even in extreme dire situations. These children of sex worker mothers were capable of advocating for their own rights as well as that of their mothers. ABSTRACT

Allegations of child sexual abuse (CSA) across various contexts have often been denied or ignored by a multitude of people, including those who do not personally know the alleged perpetrators or victims. The lack of belief of children’s CSA reports is problematic, as this may affect the child’s adjustment, the consequences for the alleged perpetrator, and the likelihood of other victims reporting abuse that they experienced. One plausible explanation for low credibility is the variable of social dominance orientation. In the current study, a diverse sample (N = 60) read a hypothetical vignette of a CSA allegation, rated the credibility of the child, and completed the Social Dominance Orientation-7 scale (SDO-7). Results supported that high social dominance orientation predicts low credibility ratings of the child’s CSA allegation. Findings may impact how clinicians and investigators approach the assessment of credibility of CSA allegations, how they appraise the opinions of others about such credibility, and jury selection in the court system.  相似文献

Blood vengeance is a culturally specific phenomenon that can place Bedouin-Arab children at high risk of neglect. This case study examines the psychological and social implications of vengeance on children, the children's coping strategies, and the role of social work. The social work function includes nonauthoritarianism, strategies for forming a positive helping alliance, and various forms of culturally sensitive assessment and intervention. The study therefore yields insight into bridging the emic-etic gap in conceptualizing and responding to child neglect in a non-Western society.  相似文献

Self-harm behavior exhibited by adolescents with developmental disabilities can be the most challenging behavior for caregivers and health care professionals to address. Past interventions have taken a behavioral approach and focused on functional analysis to guide assessment and treatment. However, self-harm behavior is becoming more recognized as a means of communication. Therefore, it is important for health care professionals and caregivers to listen to adolescents' attempts to communicate and try to understand the meaning of the behavior. Early assessment is crucial to understanding the meaning of the behavior, and early intervention is necessary to prevent escalation or chronicity. Thorough assessment guides interventions and must be implemented in the context of clients' families and social world, and the broader community. Establishment of trusting relationships among adolescents, their families, and health care professionals is imperative. This article discusses self-harm behavior from a psychosocial perspective related to prevalence, onset, purpose, maintenance, and escalation. ABSTRACT

The number of interventions available for children with autism spectrum disorder (ASD) has expanded greatly in recent years, although relatively little is known about the factors that influence family caregivers as they make treatment decisions for their children. This study involved a statewide survey of parents of young children with ASD to examine the relative weights of the factors that influenced their treatment decisions. Results suggested that caregivers rely on their own intuition for much decision making, although selected professionals are also influential. Implications for professionals working with children with ASD and their families are discussed.  相似文献

Abstract

Traditionally, caregivers of children with severe emotional and behavioral disturbances have been excluded from treatment planning, and service providers have often blamed caregivers for children's mental health problems. More recently, an approach to treatment based on Systems of Care has emphasized the importance of including caregivers in every aspect of treatment. This article describes a family-oriented residential treatment program in eastern North Carolina and reports initial results of a pilot study in which caregiver participation is valued and emphasized. Mothers exhibit a wide range of emotional responses to the discovery that their children may have been sexually abused. The clinical lore suggests that a personal history of child sexual abuse (CSA) may influence maternal reactions to such a discovery. Interestingly, however, while the impact of a history of CSA has been examined in many different populations, the impact of such a history has rarely been studied with respect to nonoffending mothers of CSA victims. The current investigation, therefore, aimed to compare nonoffending mothers with and without a history of CSA with respect to demographic variables, psychosocial functioning, and personal responses to the CSA allegations concerning their children. Nonoffending mothers with a history of CSA exhibited more general symptom distress as measured by the SCL-90-R and reported greater feelings of aloneness in facing the crisis of the CSA allegations. However, maternal history of CSA did not differentiate the groups with respect to demographic variables or maternal responses to the CSA allegations concerning their children. BackgroundLittle evaluation research has been conducted on the effectiveness of services and intervention provided to children in out-of-home care. This study evaluated an innovative Queensland, Australia program employing a collaborative wrap-round model of care in combination with a flexible intervention approach, individually tailored to children and young people in out-of-home care presenting with complex and extreme behavioural and mental health problems.MethodThe sample consisted of 664 children and young people. Two clinician-rated measures, the CGAS and HoNOSCA, were used to assess young people's functioning via a pre-post-treatment design.ResultsResults revealed significant improvements across a range of problems areas: general functioning and adjustment; disruptive, antisocial and aggressive behaviour; overactivity, poor attention and concentration; non-accidental self-injury; problems with scholastic and language skills; non-organic somatic symptoms; emotional symptoms; peer and family relationships; self-care and independence; and school attendance.ConclusionFindings provide good evidence for the effectiveness of the therapeutic intervention program. ABSTRACT

The present study focuses on elderly abuse committed by caregivers in nursing homes. It aimed at a better understanding of neglect and abusive behaviors by considering the working context and the emotional dissonance of these professionals. To achieve this goal, direct effects of emotional dissonance, job demands (workload and emotional demands) and organizational resources (high-quality relationships with the colleagues and the supervisor) on neglect and abusive behaviors were analyzed. Moreover, the moderating role of organizational resources was explored. The study was conducted among 481 nurses and healthcare assistants from different French nursing homes. Overall, results contributed to the literature by pointing out the impact of emotional dissonance and caregivers’ working context on neglect and abusive behaviors. Moreover, results highlighted the moderating effect of high-quality relationships with the colleagues and the supervisor and give rise to potential implications in preventing elderly abuse in nursing homes.  相似文献
