The U.S. health care system is fundamentally changing. The pace of change is swift but will vary, depending on regional market forces and state legislative mandates. This complex change is leading to rapid market consolidation of providers and insurers into organizations called "integrated health care systems." There is, as yet, no proven role model that will guarantee success. The purpose of this article is two-fold: To help individuals who see an expanding role for themselves in integrated health care management decide if they have what it is going to take to be successful. To identify individual management training needs by use of a self-evaluation tool. Some of the needed skills can be enhanced by education and experience. The role of the physician leader is moving beyond traditional medical staff issues. A recent national survey of physician leaders shows a growing need for education on specific technical, leadership, and practical skills. The results reveal the medical leadership skills that physician executives consider important today, and provide a window to the future about the skills that will be important tomorrow. Physicians say they need training now in quality assurance, clinical benchmarking, decision-making, and strategic planning. And when they gaze into the future and see the rapid changes throughout health care, they say they'll need more training in communication, organizational change, effective listening, and systems thinking.  相似文献

Physician unions are in the news. Patient management and patient care decisions are increasingly being taken out of the hands of physicians and put into the hands of "The Suits." To take their case for a return to physician-driven patient care to the people, some physicians are joining unions. Some are even collectively bargaining for salary and other issues that are historically more closely associated with unions. The simple fact is that physician unions exist and the number of physicians joining them is expected to increase. What are the pros and cons of unionization? What motivates physicians to join unions, and what potential negative and positive factors are associated with physician unionization? This article aims to systematise and provide a structure for research into the Future of Work (FoW). We used SciMAT to conduct a science mapping analysis based on co-word bibliographic networks. The Web of Science (WoS) database was used for article retrieval, and a total of 2,286 documents were identified from 1959 to 2019. Our results are counterintuitive, as concerns over satisfaction, leadership values or corporate social responsibility (CSR) appear alongside traditional human resource management (HRM) themes, such as organisational commitment or careers, as well as more current FoW themes, such as the impact of technological change on employment, wage inequality, vulnerable workers, telework or talent management. In addition, we offer a classification of the most prolific FoW research themes and challenges into technological, social, economic and political categories.  相似文献

The only logical way to avoid unnecessary future costs and improve quality is to analyze the past, providing input for the present. The informational management strategies are ready. There is continuous quality improvement, profiling with case-mix adjustment, and other techniques that will help us manage care and caring. But these strategies all rely on the customer being empowered to make truly informed decisions (the ethical principle of autonomy) and for us to advocate for patients (beneficence). Translating relevant data into information is the concentration of medical informatics. Virtually everyone in medical management can attest to the fact that, competitive forces notwithstanding, it is time for us to recognize not only that we are in the information business, but also that this information belongs to a larger community. Although not initiated by the event, it seems appropriate that this article is written on the tenth anniversary of the founding of The Society for Long Range Planning. It is entirely coincidental that Ansoff's book ‘Corporate Strategy’, which has evolved into the standard work on corporate planning, also appeared about a decade ago. This fortuitous coincidence has been further emphasized by the recent publication of another book from the same authority which introduces an entirely new factor into the future prospects for corporate planners. As the popularity of the Internet's World Wide Web exploded in 1994 and 1995, corporations began adopting the browser software called Mosaic (and its derivatives) for their networks. Why? That same software can be used to "surf" the Internet. Since Intranets are easier to maintain and less expensive, they are replacing the more expensive "groupware" applications based on client-server architectures that corporations installed over the past five years. These Intranets are based on widely-available technologies designed for the Internet, not proprietary software designed for a relatively few customers. Organizations with communication networks integrated with their transaction systems and electronic medical records will be more effective in managing health care resources--and more attractive to employers and insurers for managed care contracting.  相似文献

This study found that moral reasoning predicts intention to act in an ethical dilemma, but that this behavior is not modified by gender differences. Fishbein and Ajzen’s Reasoned Action model was used to predict intention to act in ethical dilemmas, while the Kohlberg and Gilligan paradigms of moral reasoning were introduced as exogenous variables. The theory of reasoned action model was found to be a good predictor of intention as hypothesized. Suggestions for further research are included. This revised version was published online in July 2006 with corrections to the Cover Date.  相似文献

We attempt to bring clarity to the concept of strategic leadership and guide its development by reviewing and synthesizing the existing management literature on how top managers and board directors influence organizations. We propose a new definition of strategic leadership and offer a framework organized around the essential questions of what strategic leadership is, what strategic leaders do, why they do it, and how they do it. To answer these questions, we organize our review around the eight functions strategic leaders serve, the key attributes of strategic leaders, the theories scholars have used to relate these functions and attributes to outcomes, contextual factors, and the organizational outcomes that strategic leaders affect. We identify how strategic leadership research is concentrated in five streams that rarely interact with each other, and offer suggestions for connecting these streams. Our review provides a big picture of what is known about individuals at the top levels of organizations and highlights the key areas where future investigation is essential.  相似文献

In March 1998 the Law Commission for England and Wales published the outcome of its review of the law on compensation of those who suffer psychiatric illness as a result of the negligence of another person. The report is largely, although not solely, concerned with work-related illness. It also reviews the literature on the incidenc and forms of psychiatric illness. It makes recommendations for legislation. This paper critically summarizes the findings of the Commission and concludes with some comments. It questions the appropriateness of common law litigation for these kinds of injury. It also comments on whether the Commission's recommendations will be implemented by Parliament and the likely impact of the proposed revision of the law upon the incidence of litigation.  相似文献

Research in business and management has recently taken a co-evolutionary turn, driven both by the pace of environmental change, but also by the increasingly complex and interconnected nature of business environments. However few studies have drawn from the theoretical approaches used to study co-evolutionary processes in other scientific domains. In this paper recent advances in these other fields of study are examined with a view towards filling this gap, and providing a much needed theoretical underpinning to recent developments in co-evolutionary research. It is seen that the identification of the unit of evolution is a critical issue in this endeavor, with two broad approaches taken, namely the entity- and practice-based approach. In the former it is assumed that ideas, knowledge and capabilities are bound to individuals, groups and organizations with change over time being determined largely by external selection forces. In the practice-based view, the focus shifts to the process in which ideas, knowledge and capabilities are continually enacted and modified through actions. The paper further explores key theoretical and methodological issues relating to the development of both approaches, making a contribution not only to co-evolutionary research but to broader research in business and management.  相似文献

We show that deterioration in household balance sheets, or the housing net worth channel, played a significant role in the sharp decline in U.S. employment between 2007 and 2009. Counties with a larger decline in housing net worth experience a larger decline in non‐tradable employment. This result is not driven by industry‐specific supply‐side shocks, exposure to the construction sector, policy‐induced business uncertainty, or contemporaneous credit supply tightening. We find little evidence of labor market adjustment in response to the housing net worth shock. There is no significant expansion of the tradable sector in counties with the largest decline in housing net worth. Further, there is little evidence of wage adjustment within or emigration out of the hardest hit counties.  相似文献

In a recent issue of Risk Analysis, the then‐President of the Society for Risk Analysis (SRA), Pamela Williams, has some interesting reflections about the risk analysis field. She states that the ability and desire to tackle difficult problems using a risk analytical approach is what uniquely defines us as professionals in the field of risk analysis. The point of departure for her discussion is interviews with the plenary speakers of the 2014 SRA Annual Meeting, who addressed two divisive topics: hydraulic fracking and marijuana use. She points to several themes that invite contributions from the field of risk analysis, including: Has the full spectrum of potential risks and benefits been identified and weighted, and what are the risk tradeoffs or countervailing risks? Inspired by Williams's reflections, and by analyzing the issues raised in the interviews, this article seeks to clarify what our field is really providing. A main conclusion of the article is that it is essential to acknowledge that professionals in the field of risk analysis merely support the tackling of such problems, and that their genuine competence—that which distinguishes them from other professionals—lies in the risk analytical approach itself.  相似文献
