Studies of the early-life origins of adult physical functioning and mortality have found that childhood health and socioeconomic context are important predictors, often irrespective of adult experiences. However, these studies have generally assessed functioning and mortality as distinct processes and used cross-sectional prevalence estimates that neglect the interplay of disability incidence, recovery, and mortality. Here, we examine whether early-life disadvantages both shorten lives and increase the number and fraction of years lived with functional impairment. We also examine the degree to which educational attainment mediates and moderates the health consequences of early-life disadvantages. Using the 1998–2008 Health and Retirement Study, we examine these questions for non-Hispanic whites and blacks aged 50–100 years using multistate life tables. Within levels of educational attainment, adults from disadvantaged childhoods lived fewer total and active years, and spent a greater portion of life impaired compared with adults from advantaged childhoods. Higher levels of education did not ameliorate the health consequences of disadvantaged childhoods. However, because education had a larger impact on health than did childhood socioeconomic context, adults from disadvantaged childhoods who achieved high education levels often had total and active life expectancies that were similar to or better than those of adults from advantaged childhoods who achieved low education levels.  相似文献

This study explores how health is associated with socioeconomic status, subjective social status, and perceptions of inequality simultaneously. Two health outcomes (self-reported health and psychological distress) are examined, and the subtlety of their relationships with each of the three dimensions of inequality is probed. Data used come from a nationally representative sample survey conducted in China. Several findings emerge from empirical analyses: (1) Overall, the three dimensions of social inequality are associated with self-reported health and psychological distress net of each other and other control variables; (2) among focal socioeconomic characteristics, income and Chinese Communist Party membership are significantly associated with both health outcomes, education exhibits a robust effect on self-reported health, and rural and migrant statuses are linked to less distress; (3) subjective social status in comparisons with both socially proximal and broad referents is associated with both health outcomes, and the association with downward comparisons is more salient than with upward comparisons; and (4) perceived degree and perceived sources of inequality in society show varying relationships with the two health outcomes. These findings add to our understanding of the multidimensionality and complexity of social inequality in relation to health.  相似文献

In this article, I evaluate the life-course determinants of cognitive functioning among 1,003 women and men aged 50 and older in Ismailia, Egypt. Three questions motivate this analysis: (1) Do older women have poorer cognitive functioning than do older men?; (2) Do cognitive resources accrued in childhood and adulthood have net positive associations with later-life cognitive functioning for women and men?; and (3) To what extent do differences in the amounts and effects of women’s and men’s cognitive resources account for gaps in their cognitive functioning? Compared with men, women have lower Modified-Mini Mental Status Exam (M-MMSE) scores for overall cognitive functioning. Cognitive resources in childhood and adulthood are jointly associated with the M-MMSE score. About 83% of the gender gap in mean M-MMSE scores is attributable to gaps in men’s and women’s attributes across the life course. Gender gaps in childhood cognitive resources—and especially schooling attainment—account for the largest share (18%) of the residual gender gap in cognitive functioning.Preferential investments in the human resources of boys have been common to many resource-poor settings (King and Mason 2001; Lloyd 2005), and the effects of such investments on gender gaps in child health are known (e.g., Hill and Upchurch 1995). Less well known is the extent to which gender gaps in resources that are accrued across the life course account for gender gaps in later-life health, despite known gender differences in the risks of illness, disability, and death. Demographic research on later-life health also has focused on physical conditions, even though dementia and neuropsychiatric disorders account for a large (~3%) and growing share of the disease burden worldwide (World Health Organization [WHO] 2007).This article assesses the determinants of cognitive functioning among older women and men (e.g., those aged 50 and older) in Ismailia, Egypt. It explores whether and to what extent (1) women have poorer cognitive functioning than men, (2) cognitive resources in childhood and adulthood have net positive associations with cognitive functioning for women and men, and (3) differences in the amounts and effects of women’s and men’s cognitive resources account for differences in their cognitive functioning. Egypt is a superb setting in which to conduct this work because of long-standing gender gaps in opportunities across the life course (e.g., Yount 2001; Yount and Sibai forthcoming) and poor knowledge about their potential effects on later-life cognitive functioning.  相似文献

The purpose of this study is to defend the view that education should be evaluated in terms of the capability to achieve valued functionings, rather than mental satisfaction or resources. In keeping with Amartya Sen’s capabilities approach we argue that mental satisfaction provides an inaccurate metric of well-being because of the phenomenon of adaptive preferences. Equally, resources cannot be used as a metric of well-being because of inequalities in the ability to convert income and commodities into valued functionings. Hence, interpreting education as a means to create human capital is also impoverished because it evaluates education solely in terms of the accumulation of resources. In order to provide evidence in support of the human capabilities approach we statistically examine the channels through which educational attainment affects the health functionings implied by life expectancy. Using panel data analysis for 35 developing countries for the years 1990, 1995 and 2000 we compare the health functionings (as indicated by life expectancy) that are achieved by the income growth generated by educational attainment, with the total health functionings that are achieved by educational attainment. We find that educational attainment (as indicated by average years of schooling) has a significant effect on life expectancy independently of its effect by way of income growth. A 1% increase in per capita income increases life expectancy by 0.073954% while a 1% increase in average years of schooling directly increases life expectancy by 0.055324%. Because it shows that income underestimates the health functionings achieved by educational attainment, our empirical findings lend support to the claim that the value of education should be measured in terms of the capability for functioning, rather than resources.  相似文献

Researchers have extensively documented a strong and consistent education gradient for mortality, with more highly educated individuals living longer than those with less education. This study contributes to our understanding of the education–mortality relationship by determining the effects of years of education and degree attainment on mortality, and by including non-degree certification, an important but understudied dimension of educational attainment. We use data from the mortality-linked restricted-use files of the Panel Study of Income Dynamics (PSID) sample (N = 9821) and Cox proportional hazards models to estimate mortality risk among U.S. adults. Results indicate that more advanced degrees and additional years of education are associated with reduced mortality risk in separate models, but when included simultaneously, only degrees remain influential. Among individuals who have earned a high school diploma only, additional years of schooling (beyond 12) and vocational school certification (or similar accreditation) are both independently associated with reduced risks of death. Degrees appear to be most important for increasing longevity; the findings also suggest that any educational experience can be beneficial. Future research in health and mortality should consider including educational measures beyond a single variable for educational attainment.  相似文献

The two-continua model of mental health contends that both psychological distress and psychological well-being make related-yet-distinct contributions to our understanding of human health and its relations with other quality of life outcomes. Using self-reported somatization, depression, and anxiety symptoms as indicators of psychological distress and self-reported life satisfaction as an indicator of psychological well-being, the present study classified participants into one of four mental-health-status groups—mentally healthy, mentally unhealthy, symptomatic yet content, or asymptomatic yet discontent—and investigated between-group differences across three key indicators of college student functioning: academic achievement, interpersonal connectedness, and physical health. Findings provide further validation for the two-continua model of mental health among college students, showing that, when considered in conjunction with clinical symptoms, life satisfaction serves as a distinguishing indicator of college students functioning across academic, social, and physical health domains—as well as a strong predictor of the absence or presence of clinical symptoms and comorbidity. Implications for theory, practice, and future research are discussed.  相似文献

Trends and gaps in educational attainment by race and gender have received much attention in recent years, but reports of these trends have generally focused on traditional-age college students. Little is known about whether and how enrollment in formal schooling among older adults (between 29 and 61 years old) has changed over time. In this article, I draw on Current Population Survey data from 1978 to 2013 to provide the most comprehensive analysis of trends in adults’ formal school enrollment by demographic group to date. Results indicate that adult black women in particular have seen relatively high growth rates in their enrollment. Black women were 85 % more likely to enroll in 2011 and 46 % more likely in 2013 than they were in 1978. Their growing advantage relative to other racial-gender groups owes largely to their increasing educational attainment rates overall, given the relationship between prior schooling and enrollment later in life. Taken together, this article’s findings suggest that adult enrollment is at once equalizing and disequalizing. On the one hand, it has the potential to narrow the gaps between those with some college experience and those with a four-year degree. On the other hand, patterns of adults’ participation in formal education are widening educational gaps between those with and without traditional-age college experience.  相似文献

This analysis follows earlier research that hypothesized and substantiated that, in a society with strong son preference, its effect on fertility would be conditional on the level of contraceptive use. Present analysis of the prospective fertility experience of 22,819 women of reproductive age during 3.5 years in Matlab, Bangladesh, shows that this effect is higher among mothers with postprimary schooling versus those with primary or no education. The higher effect conforms with the known positive relationship of contraceptive use with maternal schooling. However, this increase when contrasted with the idea that education promotes modern values, including gender equality, suggests that education in Matlab, with its traditional slant, is not resistant to son preference. In a poor, traditional society with low status for women, schooling alone is not enough to motivate women to abandon low esteem for daughters though schooling promotes child survival. This paper analyzes the trend in the effect of education on social class attainment, and uses this information to test hypotheses on the impact of credential inflation on educational decision making in the United States and the Netherlands. After having shown evidence for credential inflation of three educational transitions, it was shown that credential inflation between two generations increased the likelihood of making a transition into tertiary education in the Netherlands, and into high school completion and into 4-year university degrees in the United States. This supports the theory that education functions as a positional good, and if education loses value people need more of it in order to reach the same social class as their parents. Cross-national variation is explained with the theory that education in the United States functions more as a positional good than it does in the Netherlands.  相似文献

This study examined the extent that pulmonary function is related to perceived health status and global quality of life in adults suffering from cystic fibrosis, and the extent that self-efficacy modifies these relationships. Our sample comprised 86 adults (48% female; mean age, 29 years; age range, 18–54 years) with cystic fibrosis, recruited from the Norwegian Competence Centre for Cystic Fibrosis (83% response rate). Both subjective data on psychosocial aspects of living with cystic fibrosis, and objective data on pulmonary function (FEV1%) were assessed. Psychosocial aspects were assessed by a questionnaire that included St George’s Respiratory Questionnaire (perceived health status), the Generalized Self-Efficacy Scale (perceived self-efficacy), and the Quality of Life Scale (global quality of life). We found that self-efficacy is an additional factor explaining health status and global quality of life. Pulmonary function is important for health status, but not for global quality of life, for which perceived self-efficacy and health status are the most significant variables. These results show that self-efficacy is as important as pulmonary function for the perceived health status and global quality of life of patients. These valuable findings should be considered in the treatment and care of adult patients suffering from cystic fibrosis.  相似文献

The low school attainment, early marriage, and low age at first birth of females are major policy concerns in less developed countries. This study jointly estimated the determinants of educational attainment, marriage age, and age at first birth among females aged 12–25 in Madagascar, explicitly accounting for the endogeneities that arose from modelling these related outcomes simultaneously. An additional year of schooling results in a delay to marriage of 1.5?years and marrying 1?year later delays age at first birth by 0.5?years. Parents’ education and wealth also have important effects on schooling, marriage, and age at first birth, with a woman's first birth being delayed by 0.75?years if her mother had 4 additional years of schooling. Overall, our results provide rigorous evidence for the critical role of education—both individual women's own and that of their parents—in delaying the marriage and fertility of young women.  相似文献

Following previous work on inequality in health opportunity, this paper attempts to determine the contribution of circumstances, efforts and lifestyle, and demographic variables to the overall inequality in health in Luxembourg. Health is measured via the answers given to a question on self-assessed health and is considered as an ordinal variable. The educational level of each parent, the financial situation of the family during childhood, the area of birth of the individual, and of his/her parents, and the year of immigration are considered as circumstances. Effort and lifestyle variables are proxied by information on the educational level of the individual, whether he/she smoked and whether he/she had a physical activity on a regular basis. Sex and age are considered as demographic variables. To solve the issue of correlation between circumstances and effort and lifestyle the estimation is implemented in two stages. In the first stage, each effort and lifestyle variable is regressed in a separate equation against the vector of circumstances and demographic variables and, in a second stage, the individual health status is regressed against the vector of circumstances and demographic variables along with the estimated residuals of the effort equations described previously. The respective impacts of the three categories of explanatory variables (circumstances, effort and lifestyle and demographic variables) on health inequality are derived via a Shapley decomposition of the likelihood ratio of the health ordered logit regression. It appears that differences in circumstances and effort and lifestyle explain each around a quarter of this likelihood ratio.  相似文献

Better childhood nutrition is associated with earlier physical maturation during adolescence and increased schooling attainment. However, as earlier onset of puberty and increased schooling can have opposing effects on fertility, the net effect of improvements in childhood nutrition on a woman’s fertility are uncertain. Using path analysis, we estimate the strength of the pathways between childhood growth and subsequent fertility outcomes in Guatemalan women studied prospectively since birth. Height for age z score at 24 months was positively related to body mass index (BMI kg/m2) and height (cm) in adolescence and to schooling attainment. BMI was negatively associated (−0.23 ± 0.09 years per kg/m2; p < . 05) and schooling was positively associated (0.38 ± 0.06 years per grade; p < .001) with age at first birth. Total associations with the number of children born were positive from BMI (0.07 ± 0.02 per kg/m2; p < .05) and negative from schooling (−0.18 ± 0.02 per grade; p < .01). Height was not related to age at first birth or the number of children born. Taken together, childhood nutrition, as reflected by height at 2 years, was positively associated with delayed age at first birth and fewer children born. If schooling is available for girls, increased growth during childhood will most likely result in a net decrease infertility.  相似文献

Data from the nationally representative 1994 Inter-Censal Demographic Survey are used to examine the association between family size and children s schooling in Vietnam. The data provide information on several education measures for all children over age 10, including children no longer residing in the household. Although a clear inverse bivariate association between family size and children s school attendance and educational attainment is evident, multivariate analysis controlling for urban/rural residence, region, parents’ education, household wealth, and child’s age, reveals that much of this association, especially that predicting educational attainment, is attributable to these other influences. Moreover, much of the effect that remains after statistical adjustment for the other influences is seen mainly at the largest family sizes. We consider the implications of these findings for current population policy in Vietnam and the possible features of the Vietnamese context that might account for the modest association.  相似文献

In this paper it is investigated whether the positive effect of mothers' education on child survival is similar for boys and girls in Matlab, Bangladesh. The study is based on follow-up of 7,913 live births that occurred in the study area during the whole of 1982. The five independent variables included in the analysis are: sex of children, mother's education, mother's age at the time of birth, household economic condition, and health programme block. Hazard analysis shows that the positive effect of mother's education on child survival is different for boys and girls. For boys, a change in mother's education from no schooling to 1–5 years resulted in reducing the predicted risk of death by 45 per cent, while for the girls the reduction came to only seven per cent. Similarly, a change in mother's education from no schooling to six or more years of schooling resulted in a reduction of risk of 70 per cent for boys, while for girls it was only 32 per cent.  相似文献

In this article, we empirically study the role of education attainment on individual body mass index (BMI), eating patterns, and physical activity. We allow for endogeneity of schooling choices for females and males in a mean and quantile instrumental variables framework. We find that completion of lower secondary education has a significant positive impact on reduction of individual BMI, containment of calorie consumption, and promotion of physical activity. Interestingly, these effects are heterogeneous across genders and distributions. In particular, for BMI and calorie expenditure, the effect of education is significant for females and is more pronounced for women with high body mass and low physical activity. On the other hand, the effect of education on eating patterns is significant mainly for males, being more beneficial for men with elevated calorie consumption. We also show that education attainment is likely to foster productive and allocative efficiency of individuals in the context of BMI formation. Given that the literature suggests that education fosters development of cognition, self-control, and a variety of skills and abilities, in our context it is thus likely to promote lifetime preferences and means of individuals, which in turn enable them to achieve better health outcomes. Education also provides exposure to physical education and to school subjects enhancing individual deliberative skills, which are important factors shaping calorie expenditure and intake. Finally, we show that in the presence of strong socioeconomic inequalities in BMI, education is likely to have a pronounced impact on healthy BMI for the disadvantaged groups, represented in our framework by females.  相似文献

This article examines whether position in the demographic cycle is an important factor in determining earnings and earnings growth. Earnings equations for white males are estimated by using March Current Population Survey data. Position in the demographic cycle is captured by including both measures of own cohort size and the size of surrounding cohorts in the estimated earnings equations. Position in the demographic cycle matters. Increases in own cohort size lead to flatter earnings profiles, whereas increases in the size of surrounding cohorts are associated with steeper earnings profiles. The net effect is that those who enter the labor market before or after the peak of the demographic cycle start out with lower earnings but experience faster earnings growth. This pattern is uniform across all schooling groups: high school dropouts, high school graduates, those with some college, and college graduates.  相似文献

Contemporary stratification research on developed societies usually views the intergenerational transmission of educational advantage as a one-way effect from parent to child. However, parents’ investment in their offspring’s schooling may yield significant returns for parents themselves in later life. For instance, well-educated offspring have greater knowledge of health and technology to share with their parents and more financial means to provide for them than do their less-educated counterparts. We use data from the 1992–2006 Health and Retirement Study (HRS) to examine whether adult offspring’s educational attainments are associated with parents’ survival in the United States. We show that adult offspring’s educational attainments have independent effects on their parents’ mortality, even after controlling for parents’ own socioeconomic resources. This relationship is more pronounced for deaths that are linked to behavioral factors: most notably, chronic lower respiratory disease and lung cancer. Furthermore, at least part of the association between offspring’s schooling and parents’ survival may be explained by parents’ health behaviors, including smoking and physical activity. These findings suggest that one way to influence the health of the elderly is through their offspring. To harness the full value of schooling for health, then, a family and multigenerational perspective is needed.  相似文献

Do the social and cultural specialists differ from the technocrats and other social classes with respect to their socio-political, cultural and economic preferences and behaviour? If they do, is this attributable to their level and field of education? The social and cultural specialists are assumed to form ?new’ social class and the technocrats ?old’ social class. A typical social class should differentiate itself with respect to moral attitudes, socio-political behaviour and lifestyle forms. To reveal whether these assumed classes are indeed typical classes, we have studied a variety of socio-political, cultural and economic preferences and behaviour of members of these social classes using the Family Survey of the Dutch Population (FSDP) 1992, 1998, 2000 and 2003. To unravel the net differences between the social and cultural specialists and the technocrats, we controlled for the field of study, the level of education and background factors. Taking these control factors into consideration, the social and cultural specialists differentiate themselves substantially from the technocrats in their socio-political and especially economic preferences and behaviour. With respect to their cultural preferences and behaviour, they do not differ significantly from the technocrats.  相似文献

Although Pakistan remains in a pretransitional stage (contraceptive prevalence of only 11.9% among married women in 1992), urban women with post-primary levels of education are spearheading the gradual move toward fertility transition. Data collected in the city of Karachi in 1987 were used to determine whether the inverse association between fertility and female education is attributable to child supply variables, demand factors, or fertility regulation costs. Karachi, with its high concentration of women with secondary educations employed in professional occupations, has a contraceptive prevalence rate of 31%. Among women married for less than 20 years, a 10-year increment in education predicts that a woman will average two-fifths of a child less than other women in the previous 5 years. Regression analysis identified 4 significant intervening variables in the education-fertility relationship: marriage duration, net family income, formal sector employment, and age at first marriage. Education appears to affect fertility because it promotes a later age at marriage and thus reduces life-time exposure to the risk of childbearing, induces women to marry men with higher incomes (a phenomenon that either reduces the cost of fertility regulation or the demand for children), leads women to become employed in the formal sector (leading to a reduction in the demand for children), and has other unspecified effects on women's values or opportunities that are captured by their birth cohort. When these intervening variables are held constant, women's attitude toward family planning loses its impact on fertility, as do women's domestic autonomy and their expectations of self-support in old age. These findings lend support to increased investments in female education in urban Pakistan as a means of limiting the childbearing of married women. Although it is not clear if investment in female education would have the same effect in rural Pakistan, such action is important from a human and economic development perspective.  相似文献
