More than half the children in the United States who were victims of maltreatment in 2000 were victims of neglect (USDHHS, 2002). The majority of all child maltreatment related fatalities in 2000 were the result of neglect (USDHHS, 2002), and 27.8% of these fatalities were associated with both physical abuse and neglect (USDHH, 2002). Neglected children are more likely to be younger children (USDHHS, 2000), and the highest rate of child fatality due to maltreatment is between the ages of zero and five (Petit & Curtis, 1997; Gustavsson & Segal, 1994; USDHHS, 1999). The purpose of this article is to present the findings of a systematic review of the research on child neglect from 1990 to 2002. The review critiques method and synthesizes of the current body of knowledge on child neglect.  相似文献

This study of 267 child deaths associated with abuse or neglect in Texas during 1975 through 1977 suggests a number of indicators for identifying potential child fatalities. Families where abuse or neglect is implicated in a child fatality are characterized by small family size, young parents, and under-utilization of community support services. Over three-fourths of the families in the study had never come to the attention of the state's child protective services agency. In addition, when fathers were present in the home, they were as likely to be involved in the abuse or neglect as mothers. Neglect was implicated in the death of a child as often as abuse. The study suggests the need for further examination of child fatality profiles associated with abuse and neglect and increased community outreach efforts to provide support to high-risk families, The study also has implications for social policy decisions, particularly in relation to efforts to resist attempts to exclude neglect from child protective service responsibility.  相似文献

Study of a sample of child abuse/neglect families where a child fatality has occurred suggests several indicators for identifying potential child fatalities. In contrast to families where child abuse is not followed by a child fatality, child abuse cases where a fatality does occur are characterized by abuse of more than one child in the family, a more chaotic family constellation, the mother as perpetrator of the abuse, and high levels of both human and milieu stress. The study also identifies gaps in services to sample families and suggests changes.  相似文献

First Nations (Native American) children are greatly overrepresented in the Canadian child welfare system, and disproportionality in the substantiation of maltreatment contributes to this overrepresentation. This study explores the factors driving disproportionality in the substantiation of maltreatment and, more specifically, neglect. Data from the Canadian Incidence Study of Reported Child Abuse and Neglect (2008) are used in multivariate analyses which examine the relationship between the substantiation of maltreatment/neglect and worker assessments of case, child, household, and caregiver characteristics. These case factors fully explain disproportionality in maltreatment substantiation for First Nations and non-Aboriginal children; the disproportionality reflects underlying differences in the case, child, household and caregiver characteristics identified in First Nations and non-Aboriginal investigations. However, case factors do not fully explain disproportionality in substantiation of neglect-only investigations. Further analysis indicates that the weight that workers assigned to caregiver substance abuse, housing problems, and presence of a lone caregiver when substantiating neglect also differed for First Nations and non-Aboriginal children. Discussion of these findings explores possible explanations for these differences, and links to broader discussions around definitions of neglect and the role of substantiation in child welfare decision making processes.  相似文献

In times past, parents were usually blamed when an infant died suddenly and unexpectedly. A more caring attitude evolved with the adoption of the term ‘cot death’ or ‘sudden infant death syndrome’ (SIDS). Recent research looking at the factors responsible for SIDS has focused on aspects of child care and parental behaviour. Epidemiological studies confirm that several risk factors associated with child maltreatment are also to be found in many families who experience a sudden infant death. Social deprivation and other well-known associated features linked to maltreatment are correlated with sudden death in infancy. It is suggested that both physical abuse (suffocation is the most usual mechanism proposed) and neglect (failure to thrive, failure to recognize and treat illness) are important mechanisms. Infanticide (filicide) has existed widely over past centuries and its continuation in modern times is reflected in the highest murder rates of all age groups in infancy. However, much fatal abuse and neglect remains hidden, as demonstrated from the results of detailed fatality reviews. The evidence linking child abuse and neglect with sudden unexpected death in infancy is reviewed.  相似文献

Previous research has shown an association between child maltreatment (sexual abuse, physical abuse, neglect, or witnessing interparental violence) and adolescent sexual risk behaviors (SRBs). The mechanisms explaining this association are not well understood, but attachment theory could provide further insight into them. This study examined the relationships between child maltreatment and SRBs and investigated anxious and avoidant attachment as mediators. The sample comprised 1,900 sexually active adolescents (13 to 17 years old; 60.8% girls) attending Quebec high schools. The results of path analyses indicated that neglect was associated with a higher number of sexual partners, casual sexual behavior, and being younger at first intercourse. Anxious attachment mediated the relation between neglect and number of sexual partners, whereas avoidant attachment explained the relation between neglect and number of sexual partners, casual sexual behavior, and age at first intercourse (for boys only). Sexual abuse was directly associated with all three SRBs. Neither anxious attachment nor avoidant attachment mediated these associations. Youth with a history of neglect and sexual abuse represent a vulnerable population that is likely to engage in SRBs. Interventions designed to induce a positive change in attachment security may reduce SRBs among victims of neglect.  相似文献

Much of the research on child sexual abuse focuses on negative outcomes. This brief report explores a potentially protective parenting behavior among black South African female caregivers with and without a child sexual abuse history. Using cross-sectional baseline data, we hypothesized that caregiver child sexual abuse history would be positively associated with caregiver–youth sex communication and this relationship would be strongest for girls. Youth whose caregiver experienced child sexual abuse were more likely to report communicating with their caregiver about sex than youth whose caregivers did not experience child sexual abuse; however, this relation did not hold for caregiver reported communication. Child sexual abuse survivors’ ability and decision to discuss sex with their youth has the potential to protect youth from sexual risk and demonstrates resilience among a group rarely acknowledged for positive parenting practices.  相似文献

This paper reports the results from a Danish national survey of child abuse and neglect. Data were obtained by a written questionnaire sent to all home health visitors. All newborn Danish children receive visits by a home health visitor several times during their first year of life. For children in need of special care for social reasons the visits may continue until school age (at 6 or 7 years). The questionnaire included four checklists of signs of abuse and neglect. The home health visitors were asked to record what they had actually observed visiting the children in their homes. Eighty‐three per cent (covering about 80% of all newborns) answered the questionnaire. The objective was to establish a scientific based framework that could be used to guide preventive efforts. Ten per cent of all children under 1 year of age may be characterized as children in need of special care for social reasons, the most frequent single reason being reliance on social assistance, alcohol abuse or violence against the mother. A minimum of 4% of infants are subjected to broadly defined abuse or neglect from one or more of the four categories: physical abuse, physical neglect, emotional abuse and emotional neglect, physical and emotional neglect being the most frequent and physical maltreatment the most rare. The sample for this exploratory qualitative study of Greek perceptions of elder abuse consisted of 43 Greek participants from urban centers and 28 Greeks from a small island village. Participants were asked to provide examples of extreme, moderate, and mild abuse from an adult child toward his/her aging parent. Half of the examples of extreme abuse were various forms of physical aggression. Predominant examples of moderate abuse were psychological abuse and neglect, and at the mild level neglect was the most highly cited category. The most frequently mentioned specific types of elder mistreatment were verbal abuse, psychological neglect, physical neglect, and hitting. There were no gender differences in the kinds of examples that participants gave or the severity they attributed to their examples. Females were more often presented as victims of abuse than males. A negative correlation was found between age and the number of physical abuse examples given overall, and as types of severe abuse.  相似文献

The Strengths and Stressors Tracking Device (SSTD) is a rapid assessment measure of family well-being that assesses the particular strengths and needs of families at intake to help guide case planning and evaluate the effectiveness of treatment. The device assesses families from an ecological perspective in the domains of environmental conditions, social support, caregiver skills, and child well-being, and may be used at multiple points during treatment to assess change. SSTD has high internal consistency in all domains, distinguishes between physical abuse and neglect, and is sensitive to specific changes made by families across the duration of treatment.  相似文献

Children under one year of age are highly vulnerable to child maltreatment, which can lead to serious immediate and long-term physical and mental health consequences. Child welfare workers encounter unique challenges when assessing allegations of maltreatment involving infants. This study identifies correlates of maltreatment substantiation in a sample of 793 infants less than one year of age investigated by child welfare. It is based on data from the 2003 Canadian Incidence Study of Reported Child Abuse and Neglect (CIS-2003), a national study of reports of child maltreatment. The analysis consisted of two stages. First, bivariate analyses were conducted using unadjusted odds ratios and chi-square tests on variables known to be associated with substantiation. In the second stage, a logistic regression was performed to examine these associations in a multivariate model.Multivariate findings suggest that certain vulnerabilities present in the primary caregiver, such as substance abuse and few social supports, as well as the child's positive toxicology at birth are associated with substantiated infant maltreatment. Exposure to domestic violence, which is reported mostly by the police, and physical harm stemming from maltreatment are strongly associated with substantiation, whereas reports of physical abuse and neglect have an inverse relationship. The authors discuss these results and implications for policy and programs.  相似文献

The changing policy landscape with regards to marijuana and a growing epidemic related to opioid misuse may affect caseworkers' decisions to substantiate maltreatment involving use of those two substances. This study examined whether caseworkers factor marijuana and opioid use, including whether or not the drug was medically prescribed, into substantiation decisions. In this study, each respondent (n = 467) was given two vignettes (one for neglect and one for physical abuse) with one of five drug use conditions. Logistic regression models showed that any drug use was related to higher odds of substantiating neglect. Only illicit opioid or marijuana use was positively related to substantiating physical abuse. Any substance use may impair a parent's functioning and be interpreted as placing a child in greater risk for potential harm from neglect. For physical abuse, caseworkers may view illicit substance use as participating in a deviant behavior making physical abuse seem more likely.  相似文献

The current study examines the relationship between aspects of social service availability and child maltreatment. Specifically, the study uses negative binomial regression to estimate whether service availability, accessibility, and receipt are associated with physical child abuse and neglect. Relying on Social Cognitive Theory and Stress and Coping Theory, the moderating effect of the service variables was tested on the relationship between parenting stress and maltreatment. The sample is comprised of 1053 parents who completed a survey in July of 2014. The findings suggest that service availability has a potential protective effect against physical abuse and neglect, and that service accessibility has an additional protective effect above and beyond sheer availability of services for child neglect. Further, the availability of services has a significant moderating effect – it decreased the relationship between parent stress and abuse and neglect. Service receipt had a paradoxical effect — although one might expect receiving social services to reduce maltreatment, there was a positive correlation between the two such that parents reporting receipt of a greater number of services tended to have higher levels of abuse and neglect. Further, service receipt moderated the relationship between parenting stress and neglect, such that the relationship was stronger. The study suggests that efforts to increase service availability and accessibility may prevent child maltreatment.  相似文献

This secondary data analysis examined the risk and protective factor(s) associated with physical neglect within a sample of impoverished children. We conducted a multivariate logistic regression analysis to examine the associations among maternal age, child gender, caregiver depression, caregiver history of maltreatment, income-to-needs ratio, number of children in the home, marital status, neighborhood quality, and physical neglect. Social support was explored as a potential moderator. Among this impoverished sample, children whose caregivers had depression were 2.03 times as likely to experience physical neglect as children whose caregivers were not depressed (95% CI 1.25, 3.30; p = 0.004). Children whose caregivers reported experiencing child maltreatment were 1.81 times as likely to experience physical neglect as children whose caregivers did not experience maltreatment as a child (95% CI 1.17, 2.81; p = 0.008). Children who live in higher quality neighborhoods were 0.74 times as likely to experience physical neglect as children who live in lower quality neighborhoods (95% CI 0.57, 0.96; p = 0.03). Child abuse affects people’s ways of thinking, feeling, and observing the world, resulting in dysfunctional beliefs and maladaptive schemas. Thus, consequences of child abuse may persist during adulthood. Therefore, the aim of this study was to analyze the psychological consequences (anxiety, phobic anxiety, depression, and hopelessness) of different types of maltreatment (physical, sexual, and emotional abuse and physical and emotional neglect) and to study the role of early maladaptive schemas in the onset of symptomatology in adult female victims of child abuse. The sample consisted of 75 women referred by associations for treatment of abuse and maltreatment in childhood. Sexual abuse was the type of maltreatment that was most strongly related to most dysfunctional symptomatology, followed by emotional abuse and physical abuse, whereas physical neglect was the least related. Also, early maladaptive schemas were found to correlate with child abuse and dysfunctional symptomatology. Finally, early maladaptive schemas mediated the relationship between sexual abuse and dysfunctional symptomatology when the effect of other types of abuse was controlled. These results may provide important guidance for clinical intervention.  相似文献

This study examined the severity of externalizing symptomology among adopted boys with preadoptive histories of child sexual abuse, physical abuse, neglect/abandonment, or no abuse. The study was based on data collected across a three-year period from parents who adopted children from Florida's child welfare system. The sample consisted of 1,136 adopted boys aged 6 to 18 years. In repeated cross-sectional multivariate analyses, the results revealed that adopted boys with preadoptive child sexual abuse were prone to significantly higher levels and clinically severe externalizing symptomotology as compared to adopted boys without such histories. The findings highlight the need for postadoption services and empirically validated interventions for families adopting boys with preadoptive child sexual abuse.  相似文献
