Locomotion is an essential component of independence and well-being at old age. Performance deficits in the gait of older adults most often become evident on multisurface and varying terrains. Research results substantiate that falls occur in everyday movement situations that are characterized by instability. A test track, the Multisurface Obstacle Test for Older Adults (MSOT), was developed to diagnose individual performance. The 10-m track consists of different obstacles and varying surfaces, which represent everyday movement situations in a compact way. Twenty-nine untrained, healthy older adults (11 men, 18 women) were tested on three different days at 1-week intervals in a test–retest design by the same conductor. Mean age of the participants was 68.8?±?5.3 years with a mean body mass index of 24.4?±?2.5 kg/m2. The measured outcome variable was the required time (seconds) on the MSOT. The feasibility for the tested sample of untrained older adults was very good. The MSOT was undertaken safely by the participants, and no falls occurred. The range of the mean for time was between 8.12?±?1.53 s and 9.00?±?1.62 s. Regarding intertrial reliability, mean differences (MD) of ?3.39 to ?5.52 % and coefficients of variation (CV) of 2.72 to 4.19 % between the first and second trials and MD of ?0.69 to ?0.85 % and CV of 2.57 to 4.54 % over the three test sessions were observed. The correlation coefficients between the sessions were .92–.98. There were significant differences (p?<?.05) between the first and second trials of each session and between the first and second sessions. The smallest detectable differences (SDD) revealed that a small improvement is enough to detect changes in performance in the MSOT. Selecting tasks from real-life situations of older adults contributes to substantiate practical usability of the MSOT. The measured time on the MSOT showed high relative and absolute reliability in the target group of older adults between 60 and 80 years.  相似文献

There is general consensus that physical activity is important for preserving functional capacities of older adults and positively influencing quality of life. While accelerometry is widely accepted and applied to assess physical activity in studies, several problems with this method remain (e.g., low retest reliability, measurement errors). The aim of this study was to test the intra-instrumental retest reliability of a wrist-worn accelerometer in a 3-day measurement of physical activity in older adults and to compare different estimators. A sample of 123 older adults (76.5?±?5.1?years, 59?% female) wore a uniaxial accelerometer continuously for 1?week. The data were split into two repeated measurement values (week set) of 3?days each. The sum, the 80?C99th quantiles and the 80?C99th trimmed sums were built for each week set. Retest reliability was assessed for each estimator and graphically demonstrated by Bland?CAltman plots. The intraclass correlation of the retest reliability ranged from 0.22 to 0.91. Retest reliability increases when a more robust estimator than the overall sum is used. Therefore, the trimmed sum can be recommended as a conservative estimate of the physical activity level of older adults.  相似文献

Acceptable reliability of normalisation contractions in electromyography (EMG) is paramount for testing conducted over a number of days or if normal laboratory strength testing equipment is unavailable. This study examined the reliability of maximal voluntary isometric contractions (MVIC) and sub-maximal (60%) isometric contractions for use in neck muscle EMG studies. Surface EMG was recorded bilaterally from eight sites around the neck at C4/5 level from five healthy male subjects. Subjects performed MVIC and sub-maximal normalisation contractions using an isokinetic dynamometer (ID) and a portable cable dynamometer with attached strain gauge (PCD) in addition to a MVIC against a manual resistance (MR). Subjects were tested in flexion, extension, left and right lateral bending and were retested by the same tester within a two-week period. Intra class correlation co-efficients (ICC) were calculated for each testing method and contraction direction and a mean ICC was calculated across all contraction directions. All normalisation methods produced excellent within-day reliability (mean ICC >0.80) but only the MVICs using the ID and PCD had acceptable reliability when assessed between-days. This study confirmed the validity of using MVICs elicited using the ID and PCD as reliable reference contractions for the normalisation of neck EMG.  相似文献

Background

Reduced hip muscle strength has been shown to be a major factor related to falls in older persons. However, comprehensive assessment of hip abduction strength in the clinical setting is challenging. The aim of this study was to investigate the feasibility and intra-rater reliability of a quick and simple hip abductor strength test in a functional standing position.

Methods

Individuals over 65 years of age were recruited from the geriatric department of a university hospital and an outpatient clinic. Thirty-two older subjects, including 16 fallers (≥1 fall during the last 12 months) and 16 non-fallers were included. Maximum voluntary isometric strength (MVIS) and rate of force generation (RFG) of the hip abductors of the right leg were evaluated in a standing position using a hand-held dynamometer. Two test-sessions were carried out. All hip strength values were normalized to participants’ weight. Reliability was determined using the intra-class correlation coefficient agreement (ICCagreement), the standard error of measurement (SEM) and a Bland and Altman analysis (BA).

Results

All participants completed the strength tests, which took a mean 2.47?±?0.49 min (one limb). Intra-rater reliability was higher for MVIS (0.98[0.95–0.99]) than RFG (ICC?=?0.93[0.87–0.97]) for the entire sample. In the non-fallers, ICC was 0.98[0.95–1.00] (SEM?=?0.08 N.kg??1) for MVIS and 0.88[0.75–0.96] for RFG (SEM?=?1.34 N.kg-1.s-1). In the fallers, ICC was 0.94[0.89–0.98] (SEM?=?0.11 N.kg??1) for MVIS and 0.93[0.84–0.98] (SEM?=?1.12 N.kg??1.s??1) for RFG. The BA plot showed that the MVIS and RFG values did not differ across test-sessions, showing that no learning effect occurred (no systematic effect). The mean differences between test-sessions were larger and the LOA smaller in the fallers than in the non-fallers.

Conclusion

Assessment of hip strength in a standing position is feasible, rapid and reliable. We therefore recommend this position for clinical practice. Future studies should investigate the diagnostic value of hip abductor strength in standing to discriminate between fallers and non-fallers, and to determine if change in strength following a falls prevention program reduces the risk of falls.

To compare amounts of sedentary behavior and physical activity in adults residing in planned group residential settings to those residing in private homes, this study recruited 31 older adults who resided in planned group residential settings (n = 13) and in private homes (n = 18). Daily activities were measured using the SenseWear Armband for 7 days. Estimates of the duration of daily activities performed across sedentary, light, and moderate-to-vigorous intensities were captured. It was found that participants in planned group residential settings were older (age 85.9 ± 3.5 vs. 78.3 ± 7.2; p = 0.001) and spent more time in sedentary behaviors (12.7 ± 1.5 vs. 11.3 ± 1.6; p = 0.02) than participants in private homes. The difference was attenuated slightly after controlling for age and wear time (adjusted difference 1.2 ± 0.6 hours, p = 0.06). Thus, adults residing in planned group residential settings, which provide supportive services, were more sedentary than adults residing in private homes. The environment in which older adults live may contribute to sedentary behavior.  相似文献

A growing body of evidence suggests a role for homocysteine (Hcys) and folate (FA) in erectile function (EF): Hcys appears to impair EF affecting endothelium via several mechanism whereas the role of FA remains to be elucidated, besides decreasing Hcys. To assess correlation between erectile dysfunction (ED) and serum levels of FA, Hcys, and B12, we enrolled 31 patients affected by ED (Group A; age 52.83?±?11.89 years) and 31 healthy adults (Group B; age 49.14?±?13.63 years). Measurement instruments are needed to assess the child’s sexual abuse prevention program. The purpose of the study was to determine the reliability and validity of the WIST (What If Situations Test) for Turkish culture. Participants were children of the 3–6 age group attending pre-school education institutions and the sample size was identified by means of a power analysis. Seventy children were identified as the sample with 0.85 power and 0.05 type I error according to the power analysis. Language validity, content validity, internal validity coefficient (Cronbach alpha coefficient), and test–retest analyses were conducted in terms of validity and reliability in the scope of efforts for adaptation to Turkish culture. Firstly, Kendall W = 0.83 was the score for the expert opinions concerning the content validity of the language validity scale. It was found that the Cronbach alpha coefficients were between 0.68 and 0.90 for the scale sub-dimensions of appropriate and inappropriate recognition, saying, doing, telling, and reporting. The test–retest reliability of the scale was found to be r = 0.89 and the test–retest reliabilities for the sub-dimensions (appropriate recognition, inappropriate recognition, say skills, do skills, tell skills, and reporting skills) were between r = 0.48 and r = 0.92. The test–retest reliability for the Personal Safety Questionnaire (PSQ), as having complimentary items to the WIST, was found to be r = 0.82. The reliability and validity analysis of the ‘What If’ Situations Test (WIST), used to evaluate pre-schoolers’ skills regarding self-protection against sexual abuse, showed that the Test’s adaptation to Turkish culture was reliable and valid.  相似文献

A cross-sectional study was conducted to explore the prevalence and severity of health-related complaints perceived by adult males of Hong Kong by using the Hong Kong Traditional Chinese versions of the Aging males’ symptoms (AMS) scale and the 5-dimensional and 3-level European Quality of life (EQ-5D-3L) questionnaire. A total of 825 adult males aged 40 years or above were surveyed, and observed that 80% of the population was living with little-to-mild levels of aging symptoms with mean total scores ranged between 26.02?±?7.91 and 32.99?±?7.91 in different age groups. Such symptoms were correlated with age, especially for the somato-vegetative and sexual symptoms. The most severe AMS symptoms were observed in the oldest age group at 70 years or above, with 76%, 34% and 70% living with moderate-to-severe levels of somato-vegetative, psychological and sexual symptoms, respectively. The result was highly correlated with the EQ-5D-3L questionnaire. Secondly, the Hong Kong Aging males’ symptoms (AMS) scale was shown to have good reliability with test–retest coefficient at 0.79 (ranged 0.66–0.87) and Cronbach’s alpha coefficient at 0.88 (ranged 0.70–0.84). In summary, the population of Hong Kong male adults was commonly living with little-to-mild levels of aging symptoms, whereas their severity was correlated with age.  相似文献

Background

Quick protective reactions such as reaching or stepping are important to avoid a fall or minimize injuries. We developed Kinect-based choice reaching and stepping reaction time tests (Kinect-based CRTs) and evaluated their ability to differentiate between older fallers and non-fallers and the feasibility of administering them at home.

Methods

A total of 94 community-dwelling older people were assessed on the Kinect-based CRTs in the laboratory and were followed-up for falls for 6 months. Additionally, a subgroup (n?=?20) conducted the Kinect-based CRTs at home. Signal processing algorithms were developed to extract features for reaction, movement and the total time from the Kinect skeleton data.

Results

Nineteen participants (20.2 %) reported a fall in the 6 months following the assessment. The reaction time (fallers: 797?±?136 ms, non-fallers: 714?±?89 ms), movement time (fallers: 392?±?50 ms, non-fallers: 358?±?51 ms) and total time (fallers: 1189?±?170 ms, non-fallers: 1072?±?109 ms) of the reaching reaction time test differentiated well between the fallers and non-fallers. The stepping reaction time test did not significantly discriminate between the two groups in the prospective study. The correlations between the laboratory and in-home assessments were 0.689 for the reaching reaction time and 0.860 for stepping reaction time.

Conclusion

The study findings indicate that the Kinect-based CRT tests are feasible to administer in clinical and in-home settings, and thus represents an important step towards the development of sensor-based fall risk self-assessments. With further validation, the assessments may prove useful as a fall risk screen and home-based assessment measures for monitoring changes over time and effects of fall prevention interventions.

The relationship between isometric force control and functional performance is unknown. Submaximal steadiness and accuracy were measured during a constant force-matching task at 50% of maximal isometric voluntary contraction (MVC) of the knee extensors in 19 older women (70-89 years). Other variables included MVC, rate of torque development, and EMG activity. Functional performance was assessed during maximal performance of walking endurance, chair rising, and stair climbing. Isometric steadiness (but not accuracy) was found to independently predict chair-rise time and stair-climbing power and explained more variance in these tasks than any other variable. Walking endurance was related to muscle strength but not steadiness. These results suggest that steadiness is an independent predictor of brief, stressful functional-performance tasks in older women with mild functional impairment. Thus, improving steadiness might help reduce functional limitations or disability in older adults.  相似文献

The focus of these analyses was to examine the psychometric properties of the Lichtenberg Financial Decision Screening Scale (LFDSS). The purpose of the screen was to evaluate the decisional abilities and vulnerability to exploitation of older adults. Adults aged 60 and over were interviewed by social, legal, financial, or health services professionals who underwent in-person training on the administration and scoring of the scale. Professionals provided a rating of the decision-making abilities of the older adult. The analytic sample included 213 individuals with an average age of 76.9 (SD = 10.1). The majority (57%) were female. Data were analyzed using item response theory (IRT) methodology. The results supported the unidimensionality of the item set. Several IRT models were tested. Ten ordinal and binary items evidenced a slightly higher reliability estimate (0.85) than other versions and better coverage in terms of the range of reliable measurement across the continuum of financial incapacity.  相似文献

Ramp isometric contractions determine peak torque (PT) and neuromuscular activation (NA), and ballistic contractions can be used to evaluate rate of torque development (RTD) and electrical mechanical delay (EMD). The purposes of this study were to assess the number of sessions required to stabilize ramp and ballistic PT and to compare PT and NA between contractions in older adults. Thirty-five older men and women (age 63.7 ± 3.7 yr, body mass 64.3 ± 10.7 kg, height 159.2 ± 6.6 cm) performed 4 sessions of unilateral ramp and ballistic isometric knee extension, 48 hr apart. PT significantly increased (main time effect p < .05) from the first to the third session, with no further improvements thereafter. There was a trend toward higher PT in ballistic than in ramp contractions. No difference between contraction types on EMG values was observed. Therefore, the authors suggest that 3 familiarization sessions be performed to correctly assess PT. In addition, PT, NA, RTD, and EMD can be assessed with ballistic contraction in older adults.  相似文献

Problem gambling rates in older adults have risen dramatically in recent years and require further investigation. Limited available research has suggested that social needs may motivate gambling and hence problem gambling in older adults. Un-partnered older adults may be at greater risk of problem gambling than those with a partner. The current study explored whether loneliness mediated the marital status–problem gambling relationship, and whether gender moderated the mediation model. It was hypothesised that the relationship between being un-partnered and higher levels of loneliness would be stronger for older men than older women. A community sample of Australian men (n = 92) and women (n = 91) gamblers aged from 60 to 90 years (M = 69.75, SD = 7.28) completed the UCLA Loneliness Scale and the Problem Gambling Severity Index. The results supported the moderated mediation model, with loneliness mediating the relationship between marital status and problem gambling for older men but not for older women. It appears that felt loneliness is an important predictor of problem gambling in older adults, and that meeting the social and emotional needs of un-partnered men is important.  相似文献

Background

Several studies have already examined the positive effects of various forms of endurance training in patient groups and in healthy adults up to 60 years old. The aim of this study was to analyse the effects of Nordic Walking (NW) and XCO Walking (XCO) training on endurance capacity in healthy older adults, aged 60 years and older.

Methods

Twenty-three older participants (mean age: 69.9 ± 5.4 years) were randomly assigned to either the NW group or the XCO group. All participants were measured before and after the 12 weeks of endurance training (2 sessions/week) to examine oxygen uptake (VO2) and energy consumption during an outdoor field test. In addition, heart rates were recorded and lactate samples were collected.

Results

NW mainly demonstrated some significant (p < 0.05) decreases in heart rate, lactate concentration at lower to moderate walking speeds, whereas XCO Walking revealed significant (p < 0.05) decreases in lactate concentration and VO2 at low to higher walking speeds.

Conclusions

NW as well as XCO training increase the efficiency of the cardio-vascular system in older subjects. Both training approaches are suitable options for endurance training, which may serve to counteract age- and inactivity-related decreases in cardio-vascular functioning as well as aid in maintaining overall performance in older adults.

Elderly patients with cardiovascular events are characterized by high drug consumptions. Whether high drug consumptions are related to physical activity is not known. In order to examine whether physical activity is related to drug consumption in the elderly, patients older than 65 years (n?=?250) with a recent cardiovascular event were studied. Physical activity was analyzed according to the Physical Activity Scale for the Elderly (PASE) score and related to drug consumption. PASE score was 72.4?±?45.0 and drug consumption was 8.3?±?2.2. Elderly patients with greater comorbidity took more drugs (8.7?±?2.1) and are less active (PASE?=?64.4?±?50.6) than patients with Cumulative Illness Rating Scale severity score higher than 1.8 than those with a score lower than 1.8 (76.3?±?41.4, p?<?0.05, and 8.0?±?2.0, p?=?0.006, respectively). Multivariate analysis correlation confirmed that PASE score is negatively associated with drug consumption (β?=??0.149, p?=?0.031), independently of several variables including comorbidity. Thus, physical activity is inversely related to drug consumption in elderly patients with cardiovascular events. This inverse relationship may be attributable to the high degree of comorbidity observed in elderly patients in whom poor level of physical activity and high drug consumption are predominant.  相似文献

Although many older adults reportedly maintain an active sex life, limited research has focused on the reasons they engage in sex. The present study identified and described sexual motives in the second half of life reported by 47 older adults. Twenty-four men (51%) and 23 women (49%) aged 60–91 (M = 66, SD = 8.1) were interviewed. In-depth interviews were thematically analyzed using the theory of motivational approach-avoidance and personal-interpersonal motivational framework. Five main themes were identified: (a) to maintain overall functioning, (b) to feel young again, (c) to feel attractive and desirable, (d) from lust to love, and (e) from “getting sex” to “giving sex.” Investigating sexual motives in the second half of life can improve the understanding of older adults’ sexual behavior, function, and satisfaction.  相似文献

ABSTRACT

Self-neglect (SN) and frailty in older adults is associated with increased disability and mortality. Despite these commonalities, there have been no studies objectively assessing frailty in older adults who SN. This secondary analysis classified frailty in N = 37 older adults with Adult Protective Services validated SN using the Fried Frailty Phenotype (FFP) of weight loss, weakness, exhaustion, activity level, and walking speed. Overall, 3% were classified as robust, 62% as prefrail, and 35% as frail. Most (72%) were overweight/obese, with clinically significant decreases in activity level (60%) and walking speed (97%). Compared to the original FFP population, older adults who SN exhibit important differences in frailty phenotypes, and finding that the largest percentage of older adults who SN were prefrail may indicate a critical opportunity for intervening in this population to reduce future functional decline and mortality.  相似文献

Carbon dioxide (CO2) concentration variations in Rome in the period January 2009 to December 2010 was monitored. Five representative sites (T sites) in the city centre with different traffic levels and urbanistic characteristics, and four urban parks (P sites) of different sizes were selected. The regression analysis underlines the significant (p?≤?0.05) correlation between traffic levels and CO2 concentration. Considering the daytime CO2 concentration peak among T sites, Cerchi Street and Teatro Marcello Street, located in the city centre and closed by buildings, showed the highest traffic levels (85?±?3 and 63?±?2 cars min?1, respectively) and the highest CO2 concentration (512?±?11 and 488?±?8 ppm, respectively). With regards to the considered P sites, those extending over the largest surface areas (Villa Pamphilj Park and Villa Borghese Park, 180 and 80 ha, respectively) characterized by large tree coverage (81 ha and 60 ha, respectively) had a significant lower CO2 concentration (404?±?9 ppm, mean value) than those extending over a small surface area (Villa Torlonia Park and Villa Celimontana Park, 14 ha and 11 ha, respectively). More efforts should be made to further reduce CO2 concentration in the cities in order to ameliorate ecosystem services related to urban parks.  相似文献

The aim of this study was to determine test-retest reliability of a Functional Capacity Evaluation for patients with non-specific Work Related Upper Limb Disorders (WRULD FCE). The study sample included 33 healthy adults, consisting of 14 males and 19 females with a mean age of 29.2 years. The WRULD FCE consisted of 8 different tests including 26 items measuring repetitive movements, duration, working in awkward positions, forceful movements and static postures. Two FCE sessions were held with a 10-day interval. Means, 95% confidence intervals, one-way random Intraclass Correlation Coefficients (ICCs), 95% limits of agreement and repeated measures were calculated. An ICC between 0.75 and 0.90 was considered as good and an ICC above 0.90 was considered as an excellent reliability. The results showed that 14 of 26 items (54%) had excellent reliability, 9 of 26 items (35%) had good reliability and 3 of 26 items (11%) had moderate reliability based on ICC values. Significant learning effects were present in the Purdue Pegboard Task and in the Complete Minnesota Dexterity Test. It is concluded that the WRULD FCE is reliable in healthy adults.  相似文献
