At the same time care managers can respond to clients and families with acknowledgment of the spiritual dimension of caregiving. A model for holistic care management that would enhance outcomes for economically vulnerable older adults who receive an array of disjointed services administered through the older Americans Act (OAA) and local Area Agencies on Aging (AAA) is proposed. Fragmented service delivery is typically wasteful and ineffectual, but comprehensive care management that includes an autonomous care manager, a single interagency plan of care and ongoing monitoring that is client-centered may protect those aging in poverty from negative health outcomes.  相似文献

This study investigated the effects of job characteristics on turnover intention and the mediating effects of job satisfaction. A survey of 317 Korean care workers currently providing in-home elderly care service in the city of Seoul and its suburbs was conducted. The results of multiple regressions and an analysis of mediating effects indicated that role ambiguity and workload had significant effects on turnover intention. In addition, job satisfaction had a mediating effect on the influence of role ambiguity and workload on turnover intention. Finally, theoretical and practical implications are discussed.  相似文献

The growth in the older adult population and the demand for better trained geriatric social workers challenge the existing curricula of social work programs. The Geriatric Enrichment Grant funded by the John A. Hartford foundation has provided financial support for curricular reform. Curriculum reform is a challenging undertaking at any level. Each program and educational institution experiences the process of curriculum reform in unique ways. This paper describes one program's experience of curriculum reform for the purpose of infusing geriatric content.  相似文献

The Institute for Geriatric Social Work (IGSW), located at Boston University School of Social Work (BUSSW), formed a collaborative partnership with Elder Services of the Merrimack Valley (ESMV), a large urban Area Agency on Aging (AAA) near Boston to design, implement, and evaluate an innovative model program of agency-based training for practicing social workers. The collaboration facilitated and strengthened the development of a program that benefited the university-based program and the community-based agency. The training program comprises a "blended" model of core and elective online courses combined with face-to-face training. Evaluation of the model program is integral to its design, and includes a randomized control trial to test the effectiveness of the training program in increasing practice competencies.  相似文献

Preparing social workers to effectively practice with the growing older population requires the identification of geriatric competencies for the profession. The John A. Hartford Geriatric Social Work Initiative provided the impetus and direction for a national strategy to improve the quality of preparation of geriatric social workers. The Geriatric Social Work Practicum Partnership Program (PPP) is the project with the Hartford Initiative that emphasizes field education. The Geriatric Social Work Education Consortium (GSWEC), one of the PPP programs, initiated the development of competencies for work with older adults. GSWEC utilized Geriatric Social Work White Papers and the pioneering work of the Council on Social Work Education's (CSWE) Strengthening Aging and Gerontology Education for Social Work's (SAGE-SW) comprehensive competency list as well as conducted focus groups locally to delineate key competencies for field education. The Coordinating Center for the PPP, located at the New York Academy of Medicine, led in collaboratively developing knowledge based skill competencies for geriatric social work across all 6 demonstration sites (11 universities). The competencies adopted across sites include skills in the following five major domains: values and ethics; assessment (individuals and families, aging services, programs and policies); practice and interventions (theory and knowledge in practice, individual and family, aging services, programs and practice) interdisciplinary collaboration; and evaluation and research. The identified competencies have proven effective in evaluating students (n = 190) pre- and post PPP field education. The implications for further development of competency driven education for geriatric social work are discussed.  相似文献

The article compares quality of care under the traditional "fee–for–service" system with that given by "managed care" providers in the United States. Outcomes have been mixed, with most studies reporting on one hand a decline in the propensity of patients of health maintenance organizations (HMOs) to seek treatment and, on the other, lower patient satisfaction. The quality of care has not deteriorated, however, except in the case of that given to vulnerable patients.  相似文献

In what has become quite a turbulent quasi‐market for residential care for children and youth, providers now compete for public contracts. To create visibility and attract customers, many providers use marketing activities to project the desired impression of themselves to maintain or strengthen their market position. In this article, we analyse how companies that provide residential care for children manage the impressions they project on their websites and in advertisements. The results reveal that residential care providers use a range of impression management strategies to enhance their organizational image and to respond to potentially damaging or threatening images. The information providers choose to disclose leaves customers—in this case, the social workers responsible for choosing and purchasing care on behalf of clients—with rather limited opportunities to evaluate the quality of care. This is a significant problem considering other, more reliable, sources of information are difficult to access.  相似文献

This study describes and analyses the types of informal care provided in Sweden and whether it is possible to distinguish different types of carers. Data were collected in a Swedish county in 2000, by means of telephone interviews. The net sample consisted of 2,697 individuals 18–84 years old, and the response rate was 61 per cent. The results showed that there were large differences in the numbers of male and female carers when the data were divided into a typology of care categories based on different caring tasks. Women were much more likely than men to be involved at the 'heavy end' of caring, i.e. providing personal care in combination with a variety of other caring tasks. Men were more likely to provide some kind of practical help for a mother or a neighbour/friend. Even though the Swedish welfare state has been described as universal and characterised by an extensive system of benefits and services intended to cover the entire population, the results here indicate that informal care plays an important role and that the gender role patterns are similar to those observed in other types of welfare state. When discussing support systems it is important for social policy to develop programmes that take into account the support needs of both caregivers and care recipients, and not to see their needs in isolation from the social care system as a whole.  相似文献

Twenty children in foster care, ages 8 to 15 years, provided advice to children in care, foster parents and child welfare workers about ways to assist service delivery during the transition into foster care. The children discussed the importance of tending to experiences such as foster home expectations, the importance of time and information, the new foster/parent–child relationship, coping with stress, the ability to be engaged in decision‐making, the benefits of foster care and the need to build a trusting and personal relationship between children in care and their caregivers. The importance of listening to children's experiences of the transition into foster care and incorporating their advice into future research, policy and practice will be discussed.  相似文献

Abstract   Following managed care saturation in the US market, and because of limited prospects in Europe, managed care made some headway in Latin America and Southeast Asia. The following paper compares a general concept, "managed care", across countries, and shows that it has gained acceptance in certain territories (e.g. Hong Kong, the Philippines), but not in others. Managed care is defined as processes or techniques used by any entity that delivers, administers and/or assumes risk for health services in order to control or influence the quality, accessibility, utilization, costs and prices, or outcomes of such services provided to a defined population ( American Medical Association, 1999 ). The first part of the paper reviews specific experiences (Indonesia, the Philippines, Malaysia, Hong Kong, Singapore), and is followed by a summary of difficulties that Managed Care encountered in Asia. Recent trends in medicine reflect an attempt to be more patient-centered and while this is progress from the disease- or provider- focused model familiar in healthcare, the experiences and contributions of family as caregivers continue to be overlooked in some settings. The family-centered care model, developed most notably in pediatrics, but emerging in HIV, cancer, and aging, is presented as a resource to increase family involvement at the end of life. In this paper, family-centered care is defined, caregiving trends including support needs of formal and informal caregivers are discussed, and barriers to family-centered services are identified. Reintroducing family into the focus of care at the end of life is the primary goal of this paper. The family- centered model of care offers an appropriate framework for understanding the value of family in end-of-life care and fits well with social work perspectives that understand individuals in the context of their family system and greater environment.  相似文献

The introduction of cash‐for‐care (CfC) schemes in different European countries over the last years has responded to a plurality of strategies aimed at attending the rising demand and increasing costs of the long‐term care needs of an ageing population. The specific system of care provision in each country shaped the response given to those challenges, as well as the room for manoeuvre for policymakers when trying to transform the domain of care into a sphere where markets may play a larger role, partly relieving families, and also the state, from these responsibilities. Policy debates and scholarly analyses largely overlooked the contribution of these schemes to the creation and shaping of employment. This article provides a comparative analysis of how CfC‐based policies entail—alongside the regulation of informal care—a(n implicit or explicit) connection with care employment and may contribute to structuring employment relations in this sector. It looks jointly at the specific features of CfC and at the institutional context—welfare regime—in which they are embedded in order to assess the extent to which these schemes contributed (generally unintendedly) to a transformation of the care employment size and features in seven European countries.  相似文献

Long-term residential care facilities for older adults currently espouse a philosophy of person-centered care (PCC). However, these facilities operate according to a medical model with highly structured administration and regulation. As a result, there is tension between the priorities of the resident and the organization. A qualitative study conducted with 4 social workers and 1 physician employed in residential care facilities demonstrates not only the structural barriers to PCC, but also the importance of meaningful relationships to residents' socio-emotional well-being. The final section explores how organizational barriers might be mitigated through the complementary use of relational care.  相似文献

Scholars and stakeholders alike have proposed the recruiting of migrant personal care workers as part of a strategy to respond to a prospective aged care deficit in Australia's ageing population. In recent years, there has been an increase in care migration to fulfil shortages among Australia's culturally and linguistically diverse and more remotely based populations. However, it is a difficult and resource‐intensive process to recruit migrant personal care workers under the labour agreement stream of the Temporary Skills Shortage visa. This article addresses the lacuna in existing literature dealing with the question of how Australia's migration framework ought to be structured to recruit migrant personal care workers as part of a strategy to prevent, or respond to, an aged care crisis. The article argues that the formation of an aged care industry labour agreement based on geographical and niche subsector labour market need is necessary to ensure the accessibility of the programme to aged care organisations. However, it is also argued that the introduction of a new low‐skill visa would better facilitate the recruitment of migrant personal care workers.  相似文献

Past research on care leavers has, understandably, tended to focus on those who are in their mid‐ to late‐teens or early 20s. This reflects the profound impact of central and local government policy on those young people. It also reflects their prominence in contemporary analyses of most of the indicators of social exclusion among young people in the UK – unemployment, homelessness and lack of educational qualifications among them. However, some issues affecting adults who grew up in care apply across the life course. One such issue is the access that former care adults have to their child care files. Indeed, as we shall see, this issue has particular importance for many older adults (in their 30s and upwards). Policy and practice in this field has changed significantly during the past 20 years and there is a growing awareness of the needs of former care adults in this area. Access to such files can be a significant element in the process of seeking to address identity concerns centring around family and childhood experiences. This paper explores some of these identity concerns and analyses how access to care files both reflects such concerns and attempts to address them.  相似文献

Abstract

Increases in the foster child population, at a time when terminations in foster care placement have decreased along with a decline in the number of non‐related foster parents, has resulted in an increasing use of relative foster care placement. Relative care as an emerging trend in foster care placement, is not without controversy. Its merits, however, should be assessed in terms of its benefits to the children in care, the fiscal and organizational costs, and the problems it creates for the child welfare system. This paper reviews what is known about the characteristics of relative caregivers and the children in their care. Federal and state policies that affect the use of relatives as an alternative placement resource for children are described. The available data regarding the length of time in placement, number of placements, physical, mental health and educational and permanency outcomes for children placed with relatives are discussed. Implications for practice are drawn and areas for further research are suggested.  相似文献
