In recent decades the treatment of schizophrenia has focused primarily on pharmacotherapy with an emphasis on respite for family caregivers, support groups, and compliance regimens to sustain and maintain the affected client and family members. The use of family therapy models to intervene effectively with families affected by a diagnosis of schizophrenia is less common in the professional literature. This case study draws on the Mental Research Institute's communication theory and Bowen Family Systems Theory in therapeutic work with a Korean family.  相似文献

Bowen Family Systems Theory is most commonly used to understand and predict family process. It is also applied to other potentially intense relationship systems, especially the workplace. It has been used by workplace consultants and by individuals to understand, and to manage, their own workplace functioning. This paper will draw from several key Bowen concepts to analyse common workplace dilemmas and to suggest responses that may assist the functioning of the individual and the system. The author's own experience as a manager in a child and adolescent mental health programme will be used as the case study to illustrate the theory's application.  相似文献

The purpose of this paper is to identify and examine interpersonal dynamics within the foster family-foster child system which are considered precursors to allegations of abuse against the foster family. Michael has been involved in family therapy since 1975. He is a past president of the Queensland Association for Family Therapists (QAFT), and for fifteen years was an assessor for the ANZJFT. During this time he has been steadily training, and conducting his practice. Mike's involvement at Centacare has made him part of one of the longest‐running reflecting team projects in Brisbane. Child focus is a central construct within Bowen family systems theory (Bowen theory). A clinical implication is that mental health treatment focusing on a child may unwittingly reinforce the operation of child-focused processes, which undermine rather than enhance child well-being. The concept of child focus in Bowen theory presents significant implications for professionals working in school settings and in fields such as children's mental health, which are inherently child-focused. Bowen theory is the guiding theoretical framework for School-Based Filial Therapy (SBFT). SBFT is a play therapy intervention that was initially established in remote and outer-regional New South Wales, Australia in response to the low availability of children's mental health services and the significant barriers associated with caregiver engagement in children's mental health treatment. It involves trained school personnel facilitating therapeutic play sessions with children experiencing emotional–behavioural problems. The intervention occurs on school grounds, during school hours, and children's family members do not participate in the intervention. This mixed-methods study examines the impact of children's participation in SBFT upon family functioning. Interviews with caregivers (n = 10) of children who participated in 10 SBFT sessions were analysed using content analysis. Quantitative data were collected using the Differentiation of Self Inventory – Short Form and Visual Analogue Scale – Family Functioning. A Wilcoxon signed rank test was used to analyse the pre- and post-data. Qualitative outcomes indicated changes in the categories of child functioning, caregiver functioning, and extended family functioning, whilst child-focused processes remained dominant but changed in intensity and valence following SBFT.  相似文献

The concept of self‐soothing originating in the psychodynamic tradition has attracted interest from therapists as a key skill in the managing and regulating of strong affect and emotional discomfort. While a capacity for self‐soothing is implicit in, and a vital prerequisite to, the process of differentiation, Murray Bowen also predicted that the outcome of increased differentiation is improved emotional equilibrium and a capacity for self‐soothing, clearly a recursive process. The attention of Bowen family systems theory to both the relational and intrapsychic aspects of human functioning provides a useful framework through which to explore these aspects of the dynamics of self‐soothing. This article describes some of the key processes involved in developing a self‐soothing capacity within an effort to define a more autonomous self in significant relationships. The author contrasts Family Systems thinking with other theoretical perspectives that speak to the importance of self‐soothing. Finally, the role of the therapist as a facilitator of an environment in which the self‐soothing resources of clients can emerge is considered alongside suggestions and strategies for how a therapist may contribute to a client's own efforts.  相似文献

Important changes have occurred in the family movement in the United States since it emerged in the mid‐1950s. Psychiatry supported the movement in its early years, but that changed as psychiatry abandoned the biopsychosocial model and adopted the biological paradigm through the 1970s. This development pushed family and other therapies to the periphery of the discipline. Medicine is dominated by the biological paradigm and cause‐and‐effect thinking. The roots of the medical model go back to Pasteur. In contrast, psychiatry in the United States had been dominated through the 20th century by psychoanalytic thinking, which rendered the discipline a step‐child to medicine. This and other factors pushed psychiatry to get biological. The change generated a ‘failure of nerve’ in the family movement, abandoning the role of the family in schizophrenia in the face of psychiatry's assertion that all serious mental illnesses had nothing to do with the family. Other important changes separating the family movement from psychiatry occurred as well. Bowen theory is anchored in biology and evolution, but it is not a biological paradigm. Maintaining solid connections to the scientific disciplines are important for keeping the theory an open system. The recent emergence of systems biology and systems medicine could challenge psychiatry's allegiance to the biological paradigm and bring family systems theory centre stage in the discipline. These new disciplines may also arrive at a systems theory of the individual that can be integrated with family theory, powering a paradigm shift in medicine as a whole.  相似文献

This article, which appears in a special 30‐year anniversary edition of the Australian and New Zealand Journal of Family Therapy, reflects on the legacies of the work of Michael White. It begins by looking back on Michael's time as editor of the Australian Journal of Family Therapy. Of the many themes that were discussed in the Editorials and Letters to the Editor section of this journal in the early 1980s, this article focuses on three in particular to explore the legacies of the work of Michael White. The author describes how Michael has bequeathed not only a profound body of work, but also a distinctive spirit of originating, and ways of working that consider the effects of social issues and that examine the politics of experience. It is the author's hope that this approach to considering Michael White's legacies honours his work and also honours the contributions of the Australian and New Zealand Journal of Family Therapy in its 30th year.  相似文献

Many individuals with generalized anxiety disorder (GAD) do not respond well to currently available treatments. Moreover, treatments are less effective when GAD is accompanied by romantic relationship distress. In order to develop effective treatments for GAD and relationship distress, it is necessary to conduct theory‐based research to identify links common to both GAD and romantic relationship distress. Drawing on Bowen's family systems theory, the roles of family abuse/violence and differentiation in GAD and romantic relationship distress were examined using existing data from the National Comorbidity Survey Replication (= 2,312; 2005). As predicted, family abuse/violence was directly linked to both GAD and romantic relationship distress. Differentiation mediated the relationship between family abuse/violence and GAD, and partially mediated the relationship between family abuse/violence and romantic relationship distress. Findings suggest that current and past relationship processes may help maintain chronic anxiety and that Bowen's theory may be a useful framework for developing couple therapy treatment of GAD and romantic relationship distress.  相似文献

Inconsistent results have been found in prior research on the Bowen Family Systems Theory concept of differentiation of self and its application to individuals, couples, and families of different cultural backgrounds. In this regard, this study examined the impact of differentiation of self on healthy family functioning, family communication, and family satisfaction with 277 participants including South Koreans living in South Korea, South Korean‐born citizens living in the United States, and White Americans living in the United States. Multigroup confirmatory factor analysis identified the measurement invariance of a differentiation scale (DSI‐R) used for the three study groups. An analysis of covariance (ANCOVA) found significant differences between White Americans and South Koreans with regard to the level of differentiation. Results of multigroup structural equation modeling (SEM) analyses found a significant association between differentiation of self and healthy family functioning across the three groups with the American group having significantly higher differentiation than the two South Korean groups.” Implications for clinical practice and future research are discussed.  相似文献

Bowen family systems theory (BFST) identifies differentiation of self as a crucial characteristic that relates to one's individual and relational maturity. Bowen theorizes that an individual's level of differentiation typically remains static over time and that individuals select and pair in relationships with others who have similar levels of differentiation. This study aimed to test the hypotheses of BFST by using components of differentiation of self, emotional reactivity, and emotional cutoff, in dyadic structural equation modeling. Specifically, a longitudinal dyadic confirmatory factor analysis and structural equation longitudinal panel model were two analyses utilized to statistically test the dyadic nonindependence and stability of emotional reactivity and emotional cutoff across time. Results indicated mixed support for Bowen's assumptions regarding similarity (nonindependence) and stability. That is, small levels of nonindependence and significant stability paths across time were found. Theoretical implications and considerations for future research are discussed.  相似文献

In this study, we evaluated 275 empirical studies from Journal of Marital and Family Therapy and Family Process from 2010 to 2015 on their use of theory, and compared our findings to those of a similar previous analysis (Hawley & Geske, 2000). Overall, theory seems to have become much better incorporated in empirical family therapy research, with only 16.4% of the articles not using theory in either their introductory or discussion sections. Theory appeared better incorporated in the introductory sections than in the discussion sections. Systems theory remained the most commonly used conceptual framework, followed by attachment theory. We discuss areas for improving theory incorporation in family therapy research, and offer suggestions for both family therapy researchers and educators.  相似文献

A Bowen Family Systems therapist employs concepts of triangles and the family projection process to view a child's symptoms as embedded in the broader family patterns. This article will examine the dynamics of two family therapy cases where parents anxiously asked for their children's symptoms to be fixed. These cases will be used to explore the common presentation in child and adolescent mental health, where the parents are concerned for their children but are also keen not to open their own ‘can of worms’. The presenting problem in the first case was violent hostility between adolescent sisters and in the second case was an adolescent's anorexia. Drawing on client feedback, I reflect on the therapy process behind the divergent outcomes. In case one, the parents were willing to address their own troubled relationship and family of origin, while in case two, the parents discontinued therapy when family of origin dynamics began to be explored. The article suggests how the therapist can evoke parents' curiosity about their role in anxious family patterns, without them feeling blamed.  相似文献

Van Eeden-Moorefield, Few-Demo, Benson, Bible, and Lummer (2018) recently reviewed the content of seven family social science journals with respect to theory and methods used in 153 articles featuring LGBT research or reviews of LGBT research. They concluded that less than three percent of 6,832 articles published in those journals between 2000 and 2015 were LGBT-related and that most had not used explicit theory and had used purposive cross-sectional samples consisting mostly of White and middle-class participants. During the tenure of the present editor, between 2010 and 2017, Marriage & Family Review featured 14 research reports, reviews, or commentary on LGBT-related issues out of 250 articles, not counting brief editorials or biographies between 2010 and 2017, an odds ratio of over 2.0 compared to the other “top-tier” journals. Likewise, the Journal of Family Theory & Review published at least 15 articles, mainly reviews or commentaries, on LGBT-related issues between 2009 and 2017. Articles in Marriage & Family Review and Journal of Family Theory & Review were more likely to be reviews of the literature, or feature explicit theory, to be qualitative in methodology, or to involve interviews or surveys, while other characteristics were similar to those in the other seven journals. Thus, even if some mainstream family journals did not publish often on LGBT-related issues, other important family science journals have done so. Furthermore, there are numerous scholarly journals that welcome LGBT-related research and reviews almost exclusively.  相似文献

Abstract

Dr. Paul Denhalter completed his doctoral candidacy research on adoptive family attachment and entitlement. Nineteen parents were interviewed about what elements of adoptive family life contributed to a sense of entitlement. The act of care giving increased entitlement; negative comments from others about adoption tended to decrease it. In his social work practice, Dr. Denhalter counseled adoptive and birth families and advocated education, counseling and support for successful open adoption relationships. His positive perspective on adoption included his father's adoption at age five. Dr. Denhalter was critical of the current trends in adoption that have pathologized adoption for political purposes. He emphasized the results of the Search Institute's study that had positive conclusions about adopted children as well as a blueprint for successful adoptive families to follow.  相似文献

Schools of family therapy have been highly selective in their presentation of the theory/practice nexus. Family therapy's method of teaching (the infamous workshop format) has hampered it's growth as a practice and academic discipline. An inadvertent, unhelpful legacy of Gregory Bateson has been that lesser scholars have aped his capacity to draw on other fields of knowledge without his rigour, or his propriety. Family therapy's cavalier dealings with bodies of knowledge and its reliance on miraculous case studies has resulted in the bypassing of individual suffering. The heroic narrative that has dominated family therapy has precluded other styles of stories for therapists, theorists and clients. Family therapy has been dominated by the myth of the hero, with its accompanying motif of the puer eternus (the eternal youth). Family therapy has been forever reinventing itself, forever the ‘new kid on the block’. This fascination with newness has interfered with family therapy's capacity, at times, to consolidate its genuine value as a therapeutic entity.  相似文献
