The term ‘non‐organic failure to thrive’ (FTT) has long been used in relation to young children with faltering growth for which no organic cause is found. In this paper, problems arising from dichotomised organic/non‐organic responses to FTT are explored and reasons for the assumption that non‐organic FTT was always due to emotional abuse or neglect are presented. The case is made that, except in a minority of cases, children fail to grow because they consume insufficient calories. Research studies on reasons for poor intake, from factors related to the child and their parents to the wider family and environment, are summarised. Following a brief reference to literature on outcomes, I argue that centile charts should be routinely completed for all young children. If faltering growth is identified, a multi‐factorial assessment should be undertaken. Such an assessment reduces the risk of interventions being based on outdated assumptions about non‐organic FTT and maximises the potential for successful resolution of poor growth. After briefly summarising the literature on interventions, I conclude with suggestions for improving practice in this field, including working in partnership with families and with health and social care professionals already known to them. From ‘weak’ to ‘strong’ multifunctionality: Conceptualising farm-level multifunctional transitional pathways 总被引:1,自引:0,他引:1
Building on normative conceptualisations of multifunctionality as a decision-making spectrum bounded by productivist and non-productivist action and thought, this paper analyses farm-level multifunctional agricultural transitions. First, the paper suggests that it may be possible to categorise different farm types along the productivist/non-productivist multifunctionality spectrum, and that transitional potential from weak to strong multifunctionality often differs between different categories of farms and types of farm ownership. Second, the paper conceptualises multifunctional transitional processes at farm level over time, and introduces the notions of multifunctional path dependency and decision-making corridors, the latter of which can be understood as ‘bundles’ of decision-making opportunities bounded by productivist and non-productivist action and thought. The analysis suggests that system memory plays an important role in defining the likelihood of multifunctional actions, and argues that transitional ruptures—sudden breaks in transitional pathways—often characterise farm-level transitions. The paper concludes by highlighting the methodological challenges awaiting future researchers of multifunctional agricultural transitions. 相似文献

While abstinence is an option for many, requiring it also can deter patients from seeking treatment, or doom them to failure if they relapse, according to some researchers. A study published June 17 in Alcoholism: Clinical and Experimental Research has found that gray matter brain volume is the same for people who relapse to low risk levels as it is for people who stay abstinent. Furthermore, the study, “Not All Is Lost for Relapsers: Relapsers With Low WHO Risk Drinking Levels and Complete Abstainers Have Comparable Regional Gray Matter Volumes,” found that low‐risk relapsers have better cognitive performance than high‐risk relapsers. 相似文献

Abstract This article deciphers the views of the German thirteenth century preacher Berthold von Regensburg about ‘social nature’ as they are demonstrated in his sermon ‘Of the five talents’. Berthold von Regensburg interprets the Gospel parable (Matthew, 25: 14–30) quite freely, in accordance with the social realities of his own time. The ‘talents’ given by God to the human being are their personalities, social vocations, or offices, life-time, wealth, and love to their neighbours. Such an interpretation of the sacral text in the sermon read in a big South German town seems to be a kind of reflection of the burghers’ mentality. The hypothesis finds its further confirmation in other sermons in which he enumerates the professional groups of that society; this analysis is clearly town-oriented. In an approach, I term ‘age exclusion through closure,’ I describe the exclusion of older people, from the workforce, and from hospital-based continuing care in Britain. Nineteenth century socioeconomic changes—the introduction of workplace technology, social surveys, the New Poor Law, and workhouse isolation, culminated in workforce exclusion on the grounds of chronological age. In the 20th century, the Poor Law ‘therapy’/‘succour’ divide, was recast in the Welfare State ‘health’/‘social care’ divide. Socioeconomic changes—the rising cost of health care, demographic change, the development of geriatric medicine, and economic restructuring, have served to allocatively ration ‘continuing care’ at this interface through the criteria of ‘clinical need.’ To conclude, age has become forged to clinical need, to become a formidable force of exclusion and exploitation perpetuating Poor Law closure between those ‘able’ and ‘unable’ to work, and creating a consequent ‘process of subordination’ within Welfare State capitalism. 相似文献

This article contends that homogenisation of the term ‘self‐interest’– in sociological and economic discourse – has resulted in many misconceptions about what particular doctrines of ‘self‐interest’ were instituted to achieve at certain historical periods and in specific cultural milieux. At its worst, the article argues, this has led to a misunderstanding of the import of particular doctrines of self interest,which are read in terms of general tradition – such as that which views self‐interested conduct as a natural faculty – rather than in terms of the context specific aims of those advocating them. The article attempts to show how, historically, there have been quite significant changes in the characterisation of the ‘self’ deemed to be ‘self‐interested’. In particular, it focuses on the ‘self’ of certain early modern conceptions of self interest, and suggests this creation is best viewed not as a subjectivity transcendentally presupposed by experience, but as one historically cultivated to counter the exigencies of particular circumstances – the disaster of perpetual ‘warre’ in 17th century Europe – and to meet the purposes of a certain way of life – existence in the civitas. 相似文献

Because drama is so important to the television schedules, and because television remains a ubiquitous and pervasive medium, TV drama is a constant cultural presence. Some of its stories are about politics, featuring the work of government, the contestation of elections, party rivalry and negotiation, and so on, with a cast of characters including leaders, advisors, journalists, celebrities and citizens: they echo, refract, replay, model and feed into narratives about real‐world politics in a variety of ways. Dramatic stories of this kind are important for the sake of their potential contribution to what citizens believe ‐ and feel ‐ about politics itself. Dramatised political stories and characters appear in a wide range of genres, from factually based docudramas to situation comedy and soap opera, and have become the focus of international academic attention for a number of scholars in politics departments as well as those working from within media and cultural studies. This article looks at a range of approaches to studying political drama on television, raising questions about generic variety, the ideas and the kinds of analysis that have been applied and the varying assessments that have been put forward. 相似文献

This article focuses on the way in which women entrepreneurs legitimate their place in a gendered economy by reifying a divide between ‘real work’ and ‘not‐real work’. Using ethnographic approaches to follow the everyday lives of several women who own and operate small businesses in the USA, our article documents three gendering practices the women use for ‘becoming real workers:’ embodied, spatial and temporal. The study shows that women entrepreneurs become ‘productive workers’ by recasting reproductive work as non‐productive or not‐real work. At the end, we explore two possible alternative conceptualizations of ‘work’ that could contribute to dissolving this gendered divide. 相似文献

The ‘reactive transnationalism hypothesis’ posits a relationship between discrimination and transnational practice. The concept has generally been studied using quantitative methods, but a qualitative approach augments our understanding of two context‐specific dimensions: the nature of the discrimination involved, and the types of transnational behaviour that might be affected. Drawing on in‐depth interviews with Bangladesh‐origin Muslims in London, Luton and Birmingham, in the UK, we demonstrate how anti‐Asian and anti‐Muslim racism have been conflated with intensified anti‐migrant racism in the context of ‘hostile environment’ immigration policies and the EU referendum (Brexit), producing an amplification of racist discourses associated with purging the body politic of its non‐white bodies. The insecurity generated is altering some people's relationships to Bangladesh, incentivizing investment in land and property ‘back home'. While this represents an example of ‘reactive transnationalism', we argue that ‘protective transnationalism’ might be a more appropriate way of describing the processes at work. 相似文献
