Love's Labors Lost

A theory of marital dissolution was developed utilizing the inductive method of grounded theory construction. Approximately 1,300 propositions from the research literature were organized into 20 dynamic and structural categories, yielding a theory that predicts marital dissolution as a combination of cultural, societal, dyadic, and individual level factors. Actually, the theory, divided into eight subsections, predicts marital dissolution under three configural combinations, two cases of idiosyncratic outcome-dissolution or nondissolution-and marital nondissolution under the three final combinations. The theory is compared with two other contemporary efforts to explain marital dissolution.

Summary

This article focuses on lesbians in later midlife who are in recovery (for at least six months) from chemical dependency and on the ways the traumatic effects of their chemical dependency affect and influence the issues they face, the decisions they make, the actions they take, and, most especially, the relationships they have with both self and others. Although earlier midlife (from 40 to 50) is an important time, we will focus on the later years (from 50 to 60) because they seem to comprise the critical decade, the time when “old selves are lost and new ones come into being” (Stein, 1983, p. 3).     

An Insight Approach to Marital Therapy

The trend in marital therapy has traditionally been away from insight approaches and toward action-oriented methods, such as manipulating the couple system or teaching partners to negotiate. This paper presents a form of insight therapy based upon systems theory and suggests that this orientation has much to offer. The first part describes general categories of relationship problems with which a couples therapist must deal. The second part takes each of these problem types and suggests how a therapist with the present systems-interpretive approach might proceed. The goal is a shared understanding by partners of their marital system.

Charity Care in Crisis: Reclaiming a Lost Professional Ethic

Abstract

The concept of “charity care” is a fundamental principle in the practice of medicine. This paper examines the origins of the construct charity, the expansion of charitable care during the Middle Ages, and its gradual secularization throughout the Renaissance and the Enlightenment. The paper then highlights the transformation of Almshouses, Infirmaries and Dispensaries into charitable institutions of medical science in the nineteenth century with the birth of progressivism during the early twentieth century. A conceptual model is then presented that depicts the erosion and qualitative changes in charity care as a result ofthe corporatization of health care over the course of the last century. The construct of charity care is now in crisis. The medical profession has gradually disassociated from its obligation of charitable care as an ethical precept for the practice of medicine. The paper ends with a challenge to reclaim and renew the importance of charity in the provision of health services.
