When the focus of the child treatment is on the therapist being a good object, this can accentuate a possible countertransference difficulty of the therapist becoming the protector of the child from the bad object. This countertransference can often resonate with rescue fantasies in the child. This paper will explore the topic of rescue fantasies in child treatment, while addressing the issue of coinciding fantasies existing unconsciously in both the therapist and child, leading to their enactment. A case of a nine-year old boy is presented which demonstrates how interpretation and resolution of rescue fantasies can lead to a deepening of the treatment.  相似文献

This paper examines the application of selected concepts from classical psychoanalysis, ego psychology, object relations theory, and self psychology to the assessment and treatment of child abuse. Child maltreatment is conceptualized as deriving, in part, from impaired symbiotic and separation-individuation experiences. The role of certain defense mechanisms employed by the formerly abused parent in perpetuating intergenerational cycles of abuse is explored. Disturbances in the development of the nuclear self (as postulated by Kohut) in abusive parents are considered. Fairbairn's model of the inner object world is used to illuminate scapegoating processes. The concept of protectiveness is discussed in relation to the abused child's use of primitive defenses and inhibition of autonomy. Countertransference issues are viewed in relation to the problem of workers' burnout.This paper was written for completion requirement for the degree of M.S.W. at Hunter College School of Social Work. Winnicott's refreshing view of clinical practice includes the unique notion that delinquency is a sign of hope. Several of Winnicott's interpersonal concepts fit together to develop this thought: holding environment, capacity for concern, the use of the object, and hate in the counter transference. In this paper these four concepts are described and the case of a ten-year-old antisocial youngster is used to illustrate Winnicott's thinking and tie some of his illusive ideas into a difficult but familiar kind of practice situation. The therapeutic approach used was a mixture of case management and play therapy. What is different however is the way in which the therapist interpreted the youngster's behavior and stimulated his rich fantasy life.  相似文献

Illustrating a patient's use of the transference as a play-ground... an intermediate region between illness and real life through which the transition from one to the other is made (Freud, 1914), the author presents the case of a man in his late fifties attempting to transcend former male role stereotypes. Using the therapist as a transitional object, this patient experimented, both in therapy and in his social activities, with various patterns in relationships with women, becoming increasingly aware of his dominating benevolence and his concommitant denial of dependency needs. Several new ways for viewing both masochistic and acting-out behaviors are proposed, ways that lead to therapeutic responses tending to convert both to reparative regressions.  相似文献

Sociology has a long history of social theory and ethnographic research in the area of sexuality. Although it has been largely overlooked, this body of work prefigures later advances by social constructionists and queer theorists. This article reviews some of the early theoretical and empirical contributions to the sociology of sexuality, organized into five broad themes: the denaturalization of sexuality and its origins; the historicization of sexuality; the analytic shift from deviants to deviance/margins to center; the destabilization of sexual categories and identities and emphasis on the fluid and diverse meanings of sexual acts; and the theorization of sexuality and gender as performance. This article suggests that sexual stigma may contribute to the marginalization of sexuality studies in sociology.  相似文献

This paper seeks to explore triangular dimensions in individual psychotherapy. In addition to serving as the dyadic object of transference projection and/or as selfobject for a person in psychotherapy, the therapist may be called upon to help the patient work on relationships with certain early, important people or their representations and on the interplay between such relationships and the therapeutic one, that is, in a triangular configuration. Because patients' attempts to introduce others into individual psychotherapy are sometimes experienced by therapists as resistant rather than as integral to the therapeutic process, I propose an empathic reconsideration of such patient efforts to get us to help with the complicated dimensions of human relatedness.  相似文献

This study analyzes the relationships between child care workers and the families of the children they serve. Because paid child care operates in the borderlands of family, many workers develop intimate relationships—both emotional and physical—with the children they care for and their families. Based on three and a half years of participant-observation fieldwork, and in-depth interviews with child care workers, the researcher examines how worker's subjective meanings are shaped through daily interactions, through organizational processes found in child care centers, and by the gendering of child care as women's work. The child care workers in this study saw themselves in family-like relationship with the families they served. This designation as like-moms and pseudo-parents also meant that workers continually engaged in emotional labor—managing the intimacy they experienced as caregivers against the expectations placed on them as workers.  相似文献

This paper explores the role and importance of the relationship in the treatment of a young foster child. Because of parental neglect and loss, the child related in an inauthentic manner in order to engage her parents. Her manner of relating arose from old patterns, which were defensive adaptations to traumatic situations. The authors employ a relational theory of the self to conceptualize the processes from an intrapsychic and interpersonal perspective. The paper emphasizes that all the interactions between child and therapist, both in and out of the traditional therapy office, converged into a multidimensional authentic relationship. The authors define this relationship experience as the life space and believe that the establishment of the life space contributed to progress in the therapy.  相似文献

This meta-analysis presents the nature of the new academic environment using precepts from New Historicism to suggest connections with the New Economy. The marketplace is often the court of last appeal in conflict resolution. This can be as basic as determining a fair price for a goose in a farmers' market to the disposition of pension funds in an international merger. Economist Lawrence Summers provides insights into theory and practice. Game theory leads from John Nash's equilibrium point to modeling in economics. A University of Chicago economist discusses the application of market reforms to Chile. He worked with Chilean economists called The Chicago Boys. The corporate takeover of mental health treatment has dramatically affected mental health treatment. Two essential constituents of psychotherapeutic process, the relationship between patient and therapist and the special communicative process associated with a talking treatment, have been significantly compromised. Therapists and patients are being forced to consider whether in the absence of confidentiality, self determination, and a secure treatment partnership managed mental health care can be called psychotherapy. Drawing on several detailed sessions from the managed treatment of a young woman, the writer concludes that the answer is no.  相似文献

The death of one's therapist can be considered a traumatic event for the patient. The closeness of the relationship between therapist and patient, especially if the relationship is long-term, makes the loss analogous to losing an intimate attachment. This article will explore the complexity of issues involved for the patient who loses a therapist due to sudden death and examines the issues for the inheriting therapist as well. Case examples will be used to highlight some of the more stressful areas of practice in this difficult transition.  相似文献

In treatment, characterologically disturbed patients evoke affective and behavioral responses that are frequently antitherapeutic. Such responses reflect the therapist's involvement in a reciprocal relationship in which the patient attempts to communicate very early pathogenic experiences. In this process the therapist is induced to act in ways that replicate significant aspects of the patient's first reciprocal relationship. Problematic replications may be recognized and modified when therapists carefully monitor their own boundaries between thought, affect, and action. This developmental paradigm is particularly useful in resolving therapeutic impasses that result from the patient's need to use the therapist as a real rather than a symbolic object.  相似文献

In 1939, Margaret Lowenfeld, a child psychiatrist in London, created a therapeutic medium with which children could freely communicate and express themselves. Through the influence of Jungian, Dora Kalff, and the Jungian theories she applied to it, this new technique spread throughout the United States, Europe, and Japan. Termed Sandplay by Kalff, it is recognized as a highly effective and creative modality used mostly in child therapy, but also in Jungian analysis with adults. As a sandplay therapist who specializes in sexual abuse treatment, I have experienced the healing power of sandplay in the treatment of traumatized children, especially those who were sexually abused. The focus of this article is on the inner world expression and process of a boy I call Adam, who was a participant in my doctoral dissertation study on sandplay with sexually abused children. The article describes Adam's twelve-tray sandplay process and the theories behind the assessments that I made, based on Jungian psychology, Kalffian sandplay theory, and previous research.  相似文献

Self-nurturing is a cognitive behavioral treatment approach specifically developed and used in clinical work with borderline clients. The client is taught to recognize and differentiate both the underdeveloped reality ego and developmentally arrested child components of the spilt-ego structure by their affective and behavioral characteristics. The therapist systematically lends ego strengths to the underdeveloped reality ego which is utilized as the good enough parent to re-parent the child ego state. The reality ego is strengthened in the process and the client's maladaptive behaviors dramatically decrease.  相似文献

Summary In this paper, an attempt was made to relate clinical practice to theory as it provides direction in working with transference manifestations presented by a patient entering therapy with an immediate need to overvalue the therapist. The problem was related to the patient's inability to tolerate separation (object loss) because of failure to develop ego precipitates leading to her own positive self-image. The conditions for developing a positive self-image were outlined. Vignettes from the patient's history were offered to illustrate the problem in practice.  相似文献

This article is concerned with after the interview, a strip of time (Goffman 1974, p. 10) between the end of the formal interview and the culmination of leave-taking rituals. Although there is a considerable and growing literature on qualitative interviewing (Arksey and Knight 1999; Kvale 1996; Rubin and Rubin 1995; Weiss 1994), and some corridor talk about the meaning of off the record post-interview comments, this topic has received little attention in the published literature (but see DeSantis 1980 and Wenger 2001). And we think it is an important one, since it illuminates the interviewee's interpretation of the interviewer and interview process, and highlights aspects of the meaning of the topic, and of the interviewer, to the respondent (which is, after all, the endpoint of the qualitative interviewing method). Further, the question of what constitutes after the interview throws into relief the question of what is an interview.  相似文献

This exploratory investigation of mothers' decisions to voluntarily relinquish child custody indicates that (1) mothers tend to make these decisions alone, i.e., without consulting kin or friends, (2) it is mothers who decide against custody rather than fathers who decide for it, and (3) mothers attribute their decisions either to external factors which disrupted their ability to provide a good home, to idiosyncracies of their relationship to their child, or to stable aspects of their own personalities which rendered them generally incapable of good mothering.Revised version of a paper presented at the annual meeting of the American Orthopsychiatric Association, San Francisco, March 1982. Ecofeminists call attention to the associations that have been made between woman and nature, which can operate as a source of both subjugation and resistance, exploitation, and inspiration. This paper expands upon feminist critiques of purity by phrasing these concerns in an ecological feminist perspective. This theoretical exercise of problematizing the ideal of purity sheds light upon the intersections of human and nonhuman oppression. Preservationist work has tended to employ the logic of purity by focusing on protection of the purity of the wild regions of the earth from the polluting forces of humanity. However, such approaches retain the troublesome nature/culture dualism. The author illustrates how attempts to fragment and radically separate people from the environment can prove to be highly dangerous. She connects the theoretical resistance to purity to the important activist work that is being done to expose environmental racism. Finally, she discusses how muddying the waters and resisting the logic of purity can offer a promising approach to pressing problems revolving around the intersections of human and nonhuman oppression.  相似文献
