Two national panels have recommended similar, specific strategies for detecting hypercholesterolemia: selective screening for children aged 2 to 19, and mass screening for individuals aged 20 and over. It is, however, unclear how best to apply these recommendations to a college student population. In order to determine which strategy is more efficient, this study compared mass with selective screening of college students for hypercholesterolemia. In the mass screening strategy, all entering students were asked to have their cholesterol levels measured and to provide other coronary risk factor information. In the selective screening strategy, all sophomores with a family history of heart disease were asked to participate in a risk factor screening program. In this study, mass screening identified more hypercholesterolemic students with less effort per case found than did selective screening. We recommend that college health practitioners consider mass screening programs to identify students who could benefit from coronary risk reduction.  相似文献

Asian American college students are at high risk for hepatitis B virus (HBV). PARTICIPANTS AND METHODS: Vietnamese American students completed a questionnaire assessing HBV knowledge and attitudes. The authors performed statistical analyses to examine the relationship between HBV knowledge and participant characteristics. They also performed logistic regression to identify predictors of vaccination. Participants' mean age was 22.2 years, and almost 50% were born in the United States. RESULTS: Only 29.9% knew that Asian Americans are at high risk for HBV. Participants who had undergone screening or vaccination or had a family member with HBV or liver disease had significantly higher levels of HBV knowledge. Less acculturated students were less likely to have received vaccination. Those with higher levels of knowledge were more likely to have received the vaccine. CONCLUSIONS: HBV educational programs targeting young Asian American individuals are needed, with emphasis on reaching less acculturated individuals. Institutions of higher education are important locales to coordinate HBV efforts for young Asian Americans.  相似文献

Elevated serum cholesterol levels have been shown to be associated with premature atherosclerosis in adolescents and young adults. The National Cholesterol Education Program recommends cholesterol screening for all adults aged 20 years or older, but normative data on the college-age population are limited. At a university where lipid profiles are made available to students in selected health/wellness courses, the authors analyzed and summarized lipid profiles on 1,088 undergraduates. Mean total cholesterol levels were similar for men (165 +/- 33 mg/dL) and women (168 +/- 27 mg/dL). The men, however, had significantly lower high-density lipoprotein (HDL) cholesterol and higher low-density lipoprotein (LDL) cholesterol than the women. One hundred twenty-one students (11.1% of the sample) had elevated serum cholesterol levels (LDL-C > or = 130 mg/dL). Cholesterol screening can be used as an educational tool for college students to reinforce the link between lipid levels and health habits.  相似文献

The author collected data on serum cholesterol, blood pressure, and self-reported health behavior in 226 college students aged 18 to 26 years. Twenty-nine percent had undesirable total cholesterol levels, 10% had high cholesterol, 10% had high systolic blood pressure, and 11% had high diastolic blood pressure. Half or more of the participants consumed a diet high in saturated fats, engaged in binge drinking, had a parental risk for high cholesterol or blood pressure, or reported they experienced elevated stress levels. Men had higher risk-factor levels than women. Findings from a regression analysis revealed that smoking, binge drinking, lack of cardiovascular exercise, and eating a high saturated-fat diet were predictive of undesirable cholesterol levels. Study limitations included self-selection of participants and single measurements of blood pressure and cholesterol. Trained students served as screeners in the program for providing an effective, low-cost screening intervention.  相似文献

Factors contributing to students' compliance with mass vaccination programs during meningococcal outbreaks have not been well described. A 1997 mass vaccination campaign at Michigan State University provided an opportunity to study such factors. Of 34,024 students in the target population, 17,538 (51.5%) were vaccinated in 5 days. Vaccination rates were higher for women (47.9%) than for men (43.1%) and higher for on-campus residents (65.3%) than for off-campus residents (35.6%). For each year of students' age beyond 19, the adjusted odds of vaccination were reduced by 0.82. Adjusted odds ratios for vaccination, with White students as the reference group at 1.0, were 1.33 for Asian American students, 0.97 (not significant) for Hispanic students, 0.82 for African American students, and 0.80 for Native American students. Students from the Colleges of Business, Engineering, Communication, and Natural Science had the highest vaccination rates; those from the College of Arts and Letters had the lowest rates.  相似文献

With many community field trials or education interventions, the cost-effectiveness analyses are not given a high priority. However, this type of evaluation is important for purposes of future adoption of the intervention. The accurate measurement of costs can best be served by prospective collection of data. This article describes a methodology for collection of cost data that coincides with the intervention implementation. This cost analysis strategy has seven discrete steps. The Minimal Contact Education for Cholesterol Change study is used as an example of the use of this strategy. This intervention provides cholesterol education at six different levels of intensity at four different sectors. The intensity levels vary along a continuum from very little education input to a maximum level of intervention that might be practical in a screening setting. The cost-effectiveness analysis component of the study will identify the incremental cost-effectiveness of each intervention along the continuum.  相似文献

This study was designed to determine the effect of cholesterol screening on college students' knowledge retention and self-reported behavior change. Students (N = 180, age M = 21.4 +/- 4.1 years) enrolled in a self-paced personal fitness course participated in a screening accompanied by two educational handouts and oral information from the technician. Six weeks later, subjects completed a questionnaire that revealed that 76% remembered their exact cholesterol level. Only 36%, however, remembered specifically why their cholesterol level was important, 22% remembered the three National Cholesterol Education Program (NCEP) categories and values, and only 36% could fully identify their correct NCEP follow-up. Chi-square analyses revealed no significant differences between expected and observed frequencies for gender or cholesterol category by cognitive information retention. Analyses did reveal more self-reported dietary and exercise changes for those testing above 200 mg/dL than for those below 200. Although college students may remember their cholesterol level after 6 weeks and may take some action if they are above the desirable level, the knowledge base established in a typical screening situation appears inadequate. Educational efforts accompanying most screenings should be improved, and some attention should be given to young adults who test at the high end of the desirable category.  相似文献

This article investigates influences on academic achievement among Vietnamese American high school students. Theorists have offered a variety of explanations for Asian American academic success, and characteristics of individual families have received particular attention in many of these explanations. Here, it is argued that the academic success of Vietnamese American students may be understood as the product of “social capital,” or tightly integrated sets of associations, within Vietnamese American communities. If this is the case, it is further argued, high levels of scholastic performance among Vietnamese American youth should be proportionate to their involvement with an ethnic community. The article uses data from a specific Vietnamese American community to find whether community involvement by adolescents and their families is in fact associated with academic achievement. Participation in an ethnic church, proportion of friends who are Vietnamese, and attendance at after‐school Vietnamese classes are used as indicators of adolescents’ community involvement. Membership in ethnic community organizations is used as an indicator of parental community involvement. Findings support the contention that the involvement of Vietnamese American adolescents and of their parents in the ethnic community are strong predictors of academic achievement and that the structure of individual families promotes scholastic performance primarily by promoting community involvement.  相似文献

Using a structural model of time allocation between household production and market work, I estimate the shadow wage of household production. I combine data from the American Time Use Survey and the Current Population Survey??s Food Security Supplement to estimate the time cost as well as the total cost of food preparation at home. Using information on whether a household received food stamp benefits, I compare actual food stamp recipients?? time cost to those who do not receive food stamp benefits. Previous literature that also uses American Time Use Survey data suggests that ??typical?? food stamp recipients incur a higher time cost of preparing food at home. I cannot confirm this finding using actual data on food stamp recipients and non-recipients. In fact, the lower shadow wage of household production of food stamp recipients more than offsets the larger amount of time spent preparing food at home, generating lower time cost of food preparation for them.  相似文献

American League batters have been hit by pitches 10% to 15% more frequently than National League batters since the designated hitter rule was introduced in 1973. This evidence is consistent with the idea that the American League s adoption of the designated hitter rule created a classic moral hazard problem. Because they are not required to appear at the plate, American League pitchers can throw at opposing hitters with greater impunity (i.e., at lower cost) than National League pitchers who must take their turns at bat and, hence, bear more of the costs of their own actions. A post-offer screening program jointly developed by the Occupational and Physical Therapy Departments (University of Illinois at Chicago) was first implemented in March 1998. A total of 712 screens were completed from 3-01-98 through 2-28-01. A quasi-experimental design was utilized to analyze changes in Workers' Compensation costs between the three-year period prior to, and the three-year period following program inception. Additionally, injury rates and mean cost per injury were compared between injuries sustained between 3-98 and 3-01 which were incurred by workers who had passed screening and those that had not been screened. Finally, the tabulated cost per screen and reduction in Workers' Compensation costs allowed derivation of a dollar spent/ dollar saved ratio. Dramatic declines in number of injuries, total costs and mean cost per case occurred in the three-year period following program inception. Mean cost per case also dramatically declined comparing injured workers who had passed screening versus those who had never been screened. Cost savings were over US dollars 18 per dollar spent on the program. While a number of mitigating variables may have accounted for some of the profound effects noted, decrease in average cost per case (indicating possible reduction in severity of injury) was consistent with existing literature.  相似文献

Summary

Most studies of Hmong Americans focus on the cultural adjustment of refugees who arrived in the United States immediately after the Vietnam War. Few studies have examined the cultural adjustment of the children of these refugees, who have been raised primarily in the United States. This study explored whether American-born [ABH] and overseas-born [OBH] Hmong young adults differed in levels, models, and meanings of cultural orientation. Fourteen ABH and 32 OBH college students were asked what “being Hmong” and “being American” meant to them and complete were asked to the General Ethnicity Questionnaire (American and Hmong versions). Both groups reported being more oriented to American culture than Hmong culture. Despite similarities in mean levels of orientation to Hmong and American cultures and in the meanings of “being Hmong” and “being American,” ABH and OBH differed in their underlying models of cultural orientation. For ABH, “being Hmong” and “being Americank” were unrelated constructs, whereas for OBH, they were negatively correlated constructs.  相似文献

Several research articles have documented the disproportionate representation of minorities in school discipline and incarceration settings. In the 2009–2010 school year, research shows nationally 17 percent of African‐American, 8 percent of Native American, and 7 percent Latino American students were suspended at least one time (Losen and Gillespie 2012 ). In addition, studies show African‐American and Latino males represent the largest percentage of inmates in correctional facilities (i.e. jails and prisons) throughout the United States (Sakala 2014 ). While research has documented the relationship between race and the school‐to‐prison pipeline, the relationship between race, intellectual and emotional disabilities, and the school‐to‐prison pipeline has been relatively unexplored. According to Losen and Gillespie ( 2012 ), 25 percent of Black students who were labeled with an intellectual or emotional disability were suspended from school in the 2009–2010 school year compared to 9 percent for White students diagnosed with an intellectual or emotional disability. Furthermore, data show 70 percent of juvenile inmates were diagnosed with learning disabilities and 33 percent were reading below a 4th grade level (Hubner and Wolfson 2001 ). The current review seeks to explore the complex relationship between disability, race, and the school‐to‐prison pipeline to highlight how minority students with special needs are placed on a trajectory towards settings of incarceration.  相似文献

The nature and impact of authority have been central to social theory since antiquity, and most students of politics, culture and organizations have – in one manner or another – visited the topic. Theorists recognize that the exercise of authority is conditioned by the environment, but their work has not always integrated fundamental changes in communication infrastructure, and in particular the diffusion of mass media. With the daily evolution of telecommunications, this is a good historical moment to re-evaluate the notion of authority. My goal is to assess ways to both preserve extraordinarily useful categories from the past, and at the same time, make those typologies more sensitive to contemporary mediated communication. Here I synthesize American research on media and political communication with recent theoretical approaches to authority, expanding current views of coercion and submission.  相似文献

Nationally, the United States has a higher rate of teen pregnancy than any other industrialized nation. Native American youth have a higher birth rate than the national rate. A full-year healthy relationship program, based on Native American teachings, traditions, and cultural norms, was delivered to all eighth-grade students at a rural tribal school and a new group of eighth graders every year for 5 years, to teach healthy relationships and encourage abstinence. This article summarizes the Discovery Dating curriculum and compares the participants in the healthy relationship program to the comparison group, at the end of 5 years, in regards of the number of pregnancies as well as self-reported sexual behavior. The comparison group comprises students in the same community who attended a public middle school, rather than the tribal middle school, but the same public high school. All students had a similar age, socioeconomic status, culture, and ethnicity. Students who received the healthy relationship program (treatment group) had fewer pregnancies than the students who did not receive the healthy relationship program (comparison group). Of those who reported that they were sexually active, the students who received the healthy relationship program (treatment group) reported higher condom use than the students who did not receive the healthy relationship program (comparison group).  相似文献

This study examines how strength of ethnic identity, multiethnic identity, and other indicators of biculturalism relate to the drug use norms of urban American Indian middle school students. The article distinguishes categories of norms that may affect drug use. Regression analysis of self-reports by 434 American Indian seventh graders attending middle schools in a large southwestern U.S. city indicated that students who had a more intense sense of ethnic pride adhered more strongly to certain antidrug norms than those who did not. Whereas American Indian students with better grades in school held consistently stronger antidrug norms, there were few differences by gender, socioeconomic status, or age. These results have implications in social work practice for better understanding and strengthening the protective aspects of American Indian culture in drug prevention efforts.  相似文献

A sample of 289 Chinese American and 138 White students from a university campus was recruited from social science courses to complete a survey on perceptions of and experiences with dating violence and gender role beliefs. White students were more likely to define dating violence as physical and sexual aggression compared to the Chinese American students. Although the majority of students from both ethnic groups did not agree that dating violence is justified under various circumstances, the Chinese American students were more likely to provide a contextual justification for the use of dating violence. Findings also indicated that 20% of Chinese American students and 31.3% of the White students have experienced some form of physical dating violence since they started dating. For both the Chinese American and White students, those who were more likely to agree that various acts of physical aggression are considered dating violence were less likely to perpetrate physical dating violence in the last 12 months. The sociocultural context of dating violence and implications for practice and research are discussed.  相似文献

The purpose of this project was to evaluate the effectiveness of an initial screening and referral process in reducing the impact of musculosketetal injuries among soldiers attending Health Care Specialist training. Musculoskeletal injury among Army Health Care Specialist students have been reported to be approximately 24% for men and 24-30% for women. The highest student visit rate to the troop medical clinic for musculoskeletal injuries, for men and women, occurs during the first week of training. Anecdotal reports indicate that many students arrive for training with existing injuries or symptoms. This project was designed to assess whether formalized early screening, referral, and intervention could 1) identify arriving students who need musculoskeletal injury-related medical attention, 2) reduce the number of students receiving limited duty status during their 10-week training, 3) decrease the total number of limited duty days for students, and 4) decrease the number of students who cannot graduate due to musculoskeletal injury. Students (N=291) from one company were divided into three groups of 97 students. Three methods were used to screen and refer students for medical intervention: 1) traditional (T), 2) by health care providers (HCP), or 3) by Drill Sergeants (DS). Screening by HCP and DSs involved using a new screening tool to identify and consequently refer students with symptoms to a troop medical clinic (TMC) for early evaluation and intervention. Using the screening tool, HCPs identified 92% of students with injuries, while DSs accurately identified 80%. The screening did not reduce the number of students receiving limited duty status, total limited duty days, or the number of students that could not graduate due to musculoskeletal injury ("holdovers") (p>0.05). The screening tool demonstrated good sensitivity and specificity whether conducted by HCPs or DSs. It failed to demonstrate efficacy in reducing the impact of musculoskeletal injuries among Combat Medic soldiers, as measured by limited duty days and holdovers.  相似文献
