Re-visioning nurses' punitive attitudes within forensic psychiatric and correctional nursing: the significance of ethical sophistication

1. Development is an essential lifelong process if we are to compassionately care for patients in forensic and correctional nursing. 2. Many of us are blocked in our ability to understand and to care; we will need to grow to meet the challenges and to bear the shocks of forensic and correctional nursing. 3. Concepts that correspond to Kegan's levels of consciousness imply a progression from punitive attitudes toward more positive attitudes.     

Mental health and homosexuality

The current study examined some of the factors that may contribute to the low rates of participation of male versus female students in sexual coercion research. Specifically, we tested whether recruitment rates are adversely affected by the topic of the research or the gender of the recruiter. Introductory psychology classes were randomly assigned to a male or female recruiter and to either the memory or sex research condition. Three significant predictors assisted in differentiating students who signed up for the studies from those who did not. Overall, female students were more likely to volunteer to participate. In addition, all students were more likely to sign up for the sex research, especially when they were recruited by a male research assistant. This study confirmed our hypothesis that men are not volunteering to participate at a rate equal to their enrollment. However this problem appears to be a general concern for all researchers who use similar recruitment practices rather than specifically for researchers of sexual coercion.     

The effect of exercise on psychological & physical health outcomes: preliminary results from a Norwegian forensic hospital

People with mental illness are more likely to experience physical health problems and die prematurely than are comparable populations. This study evaluated whether exercise, when offered as part of routine treatment, affects the psychological and physical health of patients in a high-secure forensic unit in Norway. Thirteen patients completed a structured exercise program lasting 8 to 12 weeks. After completion of the program, resting heart rate and systolic blood pressure after treadmill testing were significantly improved. In addition, patients' subjective feelings of well-being and safety improved significantly. This study shows that structured exercise is possible to perform with noticeable improvements and low attrition among patients with complex conditions. Implications for future studies and practice are discussed.     

Mental health considerations in pediatric enuresis

Enuresis is a common condition during the childhood years. Although distressing to both children and parents, enuresis often goes unreported. Because of its common occurrence, psychiatric nurses should become knowledgeable about enuresis and understand how to assess and treat it when appropriate.     

Evaluating correctional health care. The lessons of New York City.

New York City's correctional health care system is a complicated one, with many component parts. Correspondingly, the task of developing an evaluation process to assess this system turned out to be complex as well. The major responsibility for developing such a process and for keeping it in motion lies with the Contract Evaluation Unit of the Department of Health. Our experiences in meeting these challenges, as well as the historical and institutional factors that have influenced our efforts, should be of interest to professional groups facing similar tasks in other localities and to readers curious about how particular health service evaluation approaches take form. This article describes our method, how it emerged, and that it entails, with an eye toward identifying what might usefully be applied to the study of other correctional health care services and settings.     

Recruitment & retention. A successful model in forensic psychiatric nursing

A liberal arts health promotion course.

With a grant from the W. K. Kellogg Foundation, the University of North Carolina at Asheville and the Mountain Area Health Education Center established a campus health promotion program that also trained family practice residents in health promotion skills. The heart of the program was a 3-credit course that emphasized stress management, aerobic conditioning, interpersonal relationship skills, and nutrition. Follow-ups after 2 years revealed that 90% of the students who responded said the course had had some lasting effect on their lives.     

Mental health of the oldest-old

As more individuals live past age 100, it is important for health professionals to better understand characteristics important to the mental health of the oldest-old in our society. Few studies have been conducted on mental health in this group, but current literature suggests these individuals are very resilient with respect to mental health and life satisfaction, despite their health disadvantages. In interviews in a variety of studies, participants talked of the importance of both inner and outer resources for empowerment. Greater understanding of mental health in aging can inform policy makers as they draft new legislation and programs to enhance quality of life for the oldest-old.

To explore the extent to which adult retail stores may contribute to a community's sexual health promotion infrastructure, we collected data from 294 customer service employees of 80 adult retail stores in 61 U.S. cities. Findings indicated that these stores and their employees do possess at least a baseline level of characteristics that indicate they are serving, or have the potential to serve, as sexual health resources in their communities. As researchers and practitioners continue to explore new and effective mechanisms for responding to sexual health issues, they should consider outlets such as adult stores. Enhancing the capacity of these stores to contribute to sexual health may require strategic collaborations between sexual health researchers, sexual health practitioners, and the adult retail industry in order to develop initiatives that are responsive to the unique goals and cultures of each.

Worksite e-mail health promotion trial: Early lessons

In preparation for an electronic mail (e-mail) and web-based health promotion intervention across multiple worksites, secure, regulatory-compliant, user-friendly e-mail and Internet applications were used to recruit potential participants across worksites, to enroll participants, and to collect baseline health assessment data. Specific hardware and software information technology environments were required of the 19 participating worksites. Sequential e-mails introduced the study and invited participation. Twenty-four percent of all employees (1106 of approximately 4600) provided consent. E-mail delivered a web link for the baseline study assessment, and reminder e-mails were sent to prompt completion. Of those who consented, 888 (80%) completed baseline health and behavior data surveys. An HTML-native web survey software was more stable across computing environments. Using e-mail and web assessment, this research recruited, enrolled, and collected data from more than 850 participants. Technical and operational challenges emerged at each step. Solutions and recommendations are discussed. Overall, this experience suggests that the use of e-mail and web software can facilitate recruitment, enrollment, and data acquisition through direct contact with study participants. This experience yields a series of lessons learned for using e-mail and the Internet to support multi-site trials.     

The association between aging inmate housing management models and non-geriatric health services in state correctional institutions

One of the current debates in the literature on aging inmates asks, what is the most efficient housing/health care model for this "special needs" population? State and federal correctional systems place elderly inmates either in specialized segregated housing units away from the general inmate population, consolidate elderly inmates in housing units within the prison, or provide a combination of both models. In general, proponents of the segregated housing model argue that aging inmates will receive specialized medical services in geriatric units. However, proponents of consolidated housing model argue that aging inmates will have the benefit of both geriatric and non-geriatric health services. This paper examines the association between the type of housing management model for aging inmates and the availability of non-geriatric physical and mental health services. Data for the analysis come from the 2000 Bureau of Justice Statistics Census of State and Federal Adult Correctional Facilities. The results suggest that institutions offering consolidated geriatric care provide more mental health services and that these effects are independent of the characteristics of prison facility.     

Mental health assessment of infants in foster care

Infants placed in foster care are at high risk for emotional and behavioral problems. Assessment of their mental health must account for their often-adverse life experiences prior to placement and the involvement of multiple systems that shape their lives in lieu of parents' authority. This article presents practice guidelines for infant mental health evaluations with consideration of legal requirements and the unique issues conferred by foster care.
