This paper analyses the introduction of long-term care insurance in Germany and provides an overview of some of the main arguments. It focuses on economic aspects of the new scheme, namely its impact on efficiency, distribution and employment as well as government budgets. The new insurance is suboptimal on a number of dimensions but not clearly inferior to an alternative discussed in the literature.  相似文献

Korea was the second Asian country to introduce universal long-term care insurance in an attempt to meet the challenges posed by its ageing population. This article presents an overview of the key issues that were involved in designing and implementing the new Korean system and the lessons that have been learned. Although the government reformed the long-term care system, a number of new challenges have emerged, such as its limited coverage and the unethical behavior of service providers. These problems appear to be closely associated with the scheme's initial design flaws and the government's passive responses to these problems.  相似文献

Being economically well established, the member countries of the Gulf Cooperation Council have unique social insurance schemes. Based on the defined benefit principle, they have succeeded in creating large reserves, which are managed and invested independently by the social insurance institutions. However, the schemes are facing a continuous escalation in their costs, due to their generosity rather than the population ageing observed in industrialized countries. This trend may well support the argument that the schemes in the region can cope with conventional reforms like increasing contribution rates, raising retirement age and restricting early retirement, rather than switching to a defined contribution system.  相似文献

Accident insurance schemes are among the most widespread systems of social security. However, the need for separate such schemes is occasionally questioned in international statements and scholarly contributions. In the context of these kinds of criticism, this article sets out the standard bases for accident insurance. The unity of prevention, rehabilitation and compensation as the common characteristic structure of individual schemes is presented from a comparative legal viewpoint. The central significance of the principle of self-management is also emphasized. With regard to their economic efficiency, which is supported by statistics, the criticism levelled at accident insurance schemes is shown to be unconvincing. Views calling separate schemes into question on the grounds of equity are also reected.  相似文献

Recently, many developed countries have moved toward comprehensive Long-Term Care (LTC) systems. Along with the conventional explanations of these policy changes based on domestic factors, learning from abroad should be considered. We focus on the social insurance model of LTC, particularly how Japan learned from Germany and the Republic of Korea learned from Japan. Some approaches for how to think about policy learning in general as well as cross-border learning are suggested.  相似文献

The article compares quality of care under the traditional "fee–for–service" system with that given by "managed care" providers in the United States. Outcomes have been mixed, with most studies reporting on one hand a decline in the propensity of patients of health maintenance organizations (HMOs) to seek treatment and, on the other, lower patient satisfaction. The quality of care has not deteriorated, however, except in the case of that given to vulnerable patients.  相似文献

Kinship foster care has experienced significant growth within the Spanish child protection system and now carries considerable weight. Over 40% of children in the public care system are formally fostered by relatives, equalling, and in some areas exceeding, the percentage of those in residential homes. This increase is not the result of a pre‐designed global programme or clearly pre‐established objectives in the system itself. Its current weight within the system has made it the focus of attention of researchers, politicians and protection system practitioners alike. The results of research carried out in Spain have revealed more positives than negatives in this type of fostering, giving rise to an in‐depth reflection on care systems, questioning the determinism of theories anchored in the intergenerational repetition of abuse and making room for perspectives more associated with resilience and children's rights. Positive results are related to fewer breakdowns, more stability, permanency beyond the age of 18, children agreeing with their placement, academic results similar to those with non‐relatives and better results after leaving care. The negatives relate to the lack of economic, educational and psychosocial support, which generally goes with this type of fostering. The experience of support programmes for these placements contributes to the debate.  相似文献

The comparative approach offers improved understanding of what is at stake for enterprises, workers and States in developing private pension schemes to fill the gap left by public schemes. The features that distinguish private schemes result from the freedom with which they were designed — by enterprises themselves or through collective bargaining — and the limits imposed by growing state intervention in granting fiscal concessions or stipulating social guarantees. Depending on how these various components are combined, private schemes may offers workers added protection or form a source of insecurity.  相似文献

Like other fields of social policy, the organization of long-term care (LTC) varies temporally and geographically. The present article aims to advance the comparison of LTC systems worldwide by proposing a conceptual framework to analyse variation, putting a special focus on analysing the role of public and private actor types. In a precluding literature review of existing LTC typologies, we find that there are various promising classification approaches, but with an overwhelming concentration on European countries and often constructed in-transparently and superficially. Building on the concept of the care/welfare mix, we develop a multi-dimensional, actor-centred typology of LTC systems. In doing so, we employ the methodological procedure of theoretically constructing a typological attribute space. We argue that three dimensions, that is service provision, financing and regulation, are crucial for differentiating types. Furthermore, we chose an actor-centred approach, asking who bears the main responsibility in each dimension. Five relevant types of corporate actors are distinguished: state, societal actors, private for-profit actors, private individual actors, and global actors. Finally, we present and discuss the resulting attribute space and further illustrated the typology's use by exemplarily classifying three countries.  相似文献

Long-term care facilities for frail elders are usually based upon the medical model, which is focused primarily on the biological functioning of these elders. The medical model allows for little choice on the part of the residents of these facilities. By way of contrast, culture change is a new approach to long-term care. This model of care seeks to meet a wide variety of needs for the elders and aims to expand their choices. This article presents the observations of college students responding to interactions with frail elders and looks at the implications of culture change for young adult college students.  相似文献

Little research has been done on the topic of end-of-life care in long-term care settings to identify important themes regarding end-of-life care structures, processes, and outcomes. This study utilized data gathered in a stratified, random sample of 437 family members of residents who died in 31 nursing homes (NHs) and 199 residential care/ assisted living facilities. Structural components of care including staffing adequacy, training, and consistency as well as facility environment and size were important factors for family members interviewed. "Being there" and manner of care delivery (e.g., staff attitudes/empathy) were major elements in the process of care. These factors were mentioned more than direct care, Hospice, or resident preferences. Family members identified themes of [dying at] home and being comfortable and clean as important outcomes of care. These identified structural components, processes, and outcomes have implications for the role of social workers in these settings despite that social work support is notably absent in these findings.  相似文献

Exploiting administrative records of the Spanish Public Employment Services for the period 2010–2012, we analyse the influence of participation in training and job search assistance programmes on the employment probability of the long-term unemployed. Using the coarsened exact matching method, we find that participation in both programmes exerts a positive influence on the employability of jobseekers, but participation in training seems to be more effective, especially as regards the likelihood of getting jobs of a certain quality. The treatment effect is higher for the long-term unemployed, thus suggesting the convenience of their participation in these active measures.  相似文献

Abstract

Thailand is the second most rapidly ageing of the Association of Southeast Asian Nations (ASEAN) countries. With this rapid pace of population ageing, the demand for long-term care has increased. Due to lower fertility and migration of adult children, the availability of children as a main source of long-term care has been reduced. In order to retain the traditional lifestyle of ageing-in-place, community-based integrated long-term care has been implemented as an alternative approach in Thailand. The development of such community-based care and its challenges can be used as a lesson for other ASEAN countries in confronting ageing societies.  相似文献

This article argues that there might be important efficiency gains for countries to provide generous unemployment insurance benefits. Firstly, generous unemployment benefits might reduce the risks, real or perceived, associated with labour market transitions such as changing employer and/or career. Secondly, such benefits might increase workers' skills levels, both by functioning as a form of insurance for workers' investments in skills that are not easily transferable between employers and by facilitating the accumulation of skills that are developed through experience with different employers. These arguments are supported by empirical data, covering 14 countries, from the Eurobarometer survey.  相似文献

The Chinese social security system has been the subject of numerous publications, which have made policy developments more accessible to researchers and administrators from all countries. However, the steps introduced in response to growing demands for intervention by the authorities in favour of dependent persons have remained poorly documented in the international literature. Despite the growing body of research on ‘hybrid regimes’, few studies address the issue of their domestic legitimacy. Targeting this gap in the literature, this article explores the legitimation strategies of three hybrid regimes around the globe: Russia, Venezuela, and Seychelles. Although these countries differ markedly in almost every aspect that can be thought of, the political systems of all three cases combine formally democratic institutions with authoritarian political dynamics. The qualitative, comparative analysis presented in this article uncovers a number of remarkable similarities between the regimes’ respective legitimization strategies. However, while the strategies for engendering legitimacy are similar across the cases, the content of these strategies is different: the Russian leadership mainly relies on preserving order and nationalism, the Venezuelan regime employs a more populist strategy, and the Seychellois regime uses a more personal and particularistic approach. Our findings not only provide insights into the mechanisms hybrid regimes use to consolidate their authority, but also highlight important differences and similarities between hybrid regimes around the world.  相似文献

Mumbai and Stockholm are worlds apart in terms of public services, infrastructures and standard of living. However, both cities have known common problems of social exclusion and marginalisation related to neo‐liberal globalisation. Social workers are facing similar challenges regarding collective empowerment as a strategy for community work. This comparative study explored how collective empowerment is undertaken by community workers. The research participants were 13 informants from community‐work organisations in the two settings. Semi‐structured interviews were used and were analysed with the help of Atlas‐ti 6.2 (ATLAS.ti Scientific Software Development GmbH D‐10623 Berlin Germany). Social work in Mumbai is in a context of extreme poverty and mainly within the informal sector, whereas in Stockholm most social work is done in relation to a public welfare model. In Stockholm, interventions are aimed towards strengthening social networks, without direct aim at social change. In Mumbai, community workers organise people for collective empowerment to strengthen marginalised groups and achieve social change.  相似文献
