Studies document that, on average, children cared for in centers, as compared to homes, have higher cognitive test scores but worse socioemotional and health outcomes. The authors assessed whether the quality of care received explains these associations. They considered multiple domains of child development—cognitive, socioemotional, and health—and examined whether mediation is greater when quality measures are better aligned with outcome domains. Using the Early Childhood Longitudinal Study Birth Cohort, they found that children in centers have better cognitive skills and behavioral regulation than children in homes, but worse social competence and generally equivalent health (N = 1,550). They found little evidence that quality of child care, as measured by standard instruments (e.g., the Early Childhood Environment Rating Scale—Revised), accounts for associations between type of care and child developmental outcomes.  相似文献

Child care is a necessary work support for many American families, but can be prohibitively expensive for those with low incomes. The federal government provides assistance through direct child care subsidies, but only a fraction of eligible families are in receipt. One factor that may limit access to child care assistance is work schedule. Research suggests that mothers with nonstandard work schedules use relative care more and day care centers less than those with standard work schedules. Research also shows that child care subsidies are disproportionately used for day care centers. This suggests that mothers who work nonstandard schedules may be less likely to receive child care assistance, but little empirical work addresses this question directly. Using data from a cohort of urban, unmarried mothers, this study explores the direct and indirect relationship between work schedule and receipt of child care assistance. The findings suggest that nonstandard work schedules reduce the odds of receiving child care assistance; a relationship mediated entirely by less day care center use among nonstandard schedule workers. The results imply that more flexible child care assistance is needed to meet the needs of these workers, possibly provided outside of the direct-subsidy system.  相似文献

This study analyzes the everyday world of center‐based child care and the climate of suspicion that permeates that world. Based on four and a half years of participant observation field research and thirty focused interviews with men and women child care workers, the author examines the existence of ‘micro panics’ which occur in child care centers when deviant labels are attached to caregiving acts or activities. Drawing from traditional Moral Panic Theory, this paper demonstrates how the context of suspicion surrounding center based child care and the ‘micro panics’ that sustain it are generated historically, structurally, and interactionally. These phenomena, in other words, are in part, a historical artifact from the 1980s moral panics concerning day care abuse, an interactional product of the gendering of child care as ‘women's work’, and a phenomenological byproduct of the positioning of paid child care in the everyday lives of workers, children, and parents.  相似文献

Grandparents’ regular care for children while parents work has been mostly studied from the parental perspective. This paper focuses on the grandparents. Using the Australian Bureau of Statistics Time Use Survey 2006 (N = 7672) we investigate regular-caring grandparents’ demographic characteristics, which childcare activities they undertake, and how regular childcare provision relates to their time in other activities, subjective time pressure and satisfaction. Results indicate the correlates and nature of regular care differ by gender. Regular and non-regular-caring grandmothers’ relative time allocation to different childcare tasks barely differs, while regular-caring grandfathers’ care includes a much higher proportion of active care and travel than non-regular-caring grandfathers’. Regular care provision is associated with less leisure than non-regular-caring counterparts for both genders, but with only grandmothers’ housework, personal care and sleep time. Providing regular care doubles the likelihood of grandmothers reporting high subjective time pressure compared to non-regular-caring grandmothers; there is no association between regular care and time pressure for grandfathers. We conclude that in taking on regular care, grandparents echo the gender patterns found among parents, namely that it is women who are disproportionately impacted by meeting family care needs.  相似文献

Drawing on data from the Family Life Project collected in North Carolina and Pennsylvania, this paper examines the relationship between maternal work characteristics and childcare type and quality in rural communities. Research is limited on the childcare experiences of rural families. Rural areas have less access to formal childcare and families often commute long distances for work, restricting childcare options. Employed mothers using childcare were selected (n=441). Logistic and OLS regression was used to examine which characteristics, including workplace support, objective occupational measures, hours, wage, and shift, predicted care type and quality. Results indicated that most families were using informal care. Those with more hazardous work conditions and working night shifts were less likely to use centers. Higher quality care was related to more workplace support, center use, and higher wages. Implications for social policy and practice are discussed.  相似文献

In this paper we examined the determinants of child care choices among families with young children in Spain, by means of a conceptual framework that encompassed need, costs, and availability. Based on data from the Spanish Time Use Survey and Household Budget Survey, our study indicated that, regardless of model specification, the age of the child, the mother’s labor force decisions, and the prices of child care services available were the most important factors that mothers considered when they chose a type of child care. Day care center services substituted for babysitters when the price of babysitters rose; and relative and parent care substituted for day care centers when these services became more expensive. We found no different sensitivity to price changes for working and non-working mothers. From a public policy perspective, child care price subsidies were found to have much stronger effects on child care decisions than child allowances or public provision of school slots.  相似文献

In the Netherlands, an increasing number of green care farms are providing day care to community-dwelling elderly people with dementia. Currently, it is unknown whether activities, activity participation, and facility use of elderly people with dementia at green care farms differ from those at regular day care facilities. The authors performed group and individual observations at 11 green care farms and 12 regular day care facilities. Activities of elderly people at green care farms were more frequent, occurred outdoors more often, were of a higher physical intensity, and more often were aimed at individuals than activities at regular day care facilities. Therefore, the green care farms’ environment may be more beneficial for elderly people with dementia than the regular day care facility environment.  相似文献

The Southeastern Coastal Center for Agricultural Health and Safety (SCCAHS) is one of many newly-funded federal research centers, housing five multidisciplinary research projects and seven pilot projects, and serving a multi-state region. In the early stages of such a complex project, with multiple teams separated by geography and disciplines, the evaluation program has been integral in connecting internal and external stakeholders at the center and project levels. We used a developmental evaluation (DE) framework to respond to the complex political environment surrounding agricultural health and safety in the southeast; to engage external stakeholders in guiding the center’s research and outreach trajectories; to support center research teams in a co-creation process to develop logic models and tailored indicators; and to provide timely and feedback within the center to address communications gaps identified by the evaluation program. By using DE principles to shape monitoring and evaluation approaches, our evaluation program has adapted to the dynamic circumstances presented as our center’s progress has been translated from a plan in a grant proposal to implementation.  相似文献

The aim of this study was to explore how a 1‐year peer‐mentoring relationship contributed to change in young women “in care.” Twenty semistructured, one‐to‐one interviews were conducted with mentors (n = 11) and mentees (n = 9) recruited from two different London local authorities. Participants’ accounts were interpreted through a developmental lens to uncover developmental aspects and locus mechanisms through which transformative change took place. Resilience as a healthy outcome was the result of the dual function the mentoring relationship performed. The mentoring relationship was protective against the risks associated with transitioning to independent living and/or adulthood, and promoted internal assets and competencies whereby the mentees’ ability to resist them was enhanced. Establishing a trustworthy connection with a role model promoted developmental domains within mentees.  相似文献

Abstract

Objective: This study reviews an initiative to educate providers on pediatric cancer survivor care and to establish a cancer survivor registry in a college health center. Participants: Participants were University of Georgia (UGA) college health providers. Methods: Providers attended lectures on survivor care and were encouraged to register on Cancer SurvivorLink. Changes in provider familiarity and practice were measured using baseline and follow-up surveys. A survivor registry was created using health entrance forms and medical records abstraction. Results: Twenty-four providers registered on SurvivorLink, and 16 completed both surveys. Familiarity with survivor care (p = .003) and a survivor health care plan (p = .016) increased. Likelihood to deliver survivor care increased (p = .01). UGA follows 95 survivors; 71 diagnosed at < 21 years. Among survivors diagnosed at < 21 years, 91% reported their diagnosis on entrance forms. Conclusions: Through education and optimization of health informatics, college health centers can identify and provide survivor care to this medically vulnerable population.  相似文献

Parents face a trade‐off in the effect of child‐care problems on employment. Whereas large settings may increase problems because of child illness, small group care may relate to provider unavailability. Analyzing the NICHD Study of Early Child Care, we find that child‐care centers and large family day care lead to mothers’ greater work absences because of a sick child, but not to maternal job exits. Greater work absences because of unavailability of small home‐based providers are associated with mothers’ job exits, especially when mothers have low earnings and use nonrelative caregivers. Our findings accentuate the need for improved hygiene practices in child care, expanded personal leave coverage for parents, and greater backup care for sick and well children.  相似文献

In this article, we have proposed that some elements of Cumming and Henry's [1961] disengagement theory are relevant in understanding the social world of the very old. The findings indicate that both physical and social losses make it difficult to maintain an active social network. Almost one-half of the sample intentionally remains socially engaged by making new friends and attending senior centers. The remainder, who are generally older and in poorer health, modify their social world both psychologically and socially. First, these individuals redefine their optimal level of social integration and become content with a narrower, more constricted social world. Second, they are likely to reject those norms which place expectations on them incongruent with their capacities. Third, respondents tend to redefine their time orientation from the future to the present and become content to live day to day. Fourth, as they detach themselves from social involvement, increasing introspection occurs.  相似文献

This paper discusses an example of action research, the evaluation of a nurse-managed, family-oriented health care center serving a low-income, black population in an isolated section of a highly industrialized suburban community. The evaluation was the result of many necessary compromises, yet other than lack of cost effectiveness, it brought substantial evidence of the success of the agency. Patients had positive health attitudes, were knowledgeable, and practiced health-inducing behaviors. As compared to other community health centers, the quality of care was found to range from average to excellent. Notable gains in quality of care occurred from pre- to posttest, partially as a result of formative evaluation procedures. Despite the positive nature of the product evaluation, the Center was closed, raising questions as to the utility of evaluation in the decision-making process.  相似文献

This paper examines the relationship between various types of child care during the first year of a child's life and the child's language and social development measured at age three. A unique contribution of the paper is the estimation of a general selection-correction model that accounts for non-random selection of children into different types of child care. The analysis uses data from the Fragile Families and Child Wellbeing Study (FFCWS), a birth cohort of children born to predominantly low-income single mothers. The results indicate that compared with maternal care, relative care during infancy has more beneficial effects on a child's language development, while day care centers have more beneficial effects on a child's behavioral development.  相似文献

Eight years of data analysis on residents’ status from three small Alzheimer care centers suggest notable stability in function. Individuals’ functional ability was best in their first year, but their year-to-year decline was mostly not significant. Cognitive abilities also were best in the resident's first year, but first-, second-, and third-year cognitive scores were not significantly different. We compared the residents’ dementia progression rate to the true rate of natural disease progression. This study, like others on small homelike settings, suggests that appropriate homelike environments maintain persons with dementia at an optimum level longer.  相似文献

ABSTRACT

This research focused on the mental health issues at adult day centers and used a cluster analysis to profile older persons who received these services. The sample included 280 participants who attended adult day care. The data were collected from intake files using a biopsychosocial framework to guide the selection of variables from January 1, 1999 to December 31, 2000. A cluster analysis revealed two distinct profiles of participants. The first profile was characterized by women, who were single, African-American, had low incomes, relied more on public funding, and had no psychiatric diagnoses. The second profile was differentiated with more men, who were married, relied more on private funding, and had higher incomes and psychiatric diagnoses. Participants clustered in the second profile, including those diagnosed with psychiatric disorders, disenrolled at a faster rate than those in the first profile, including older adults without psychiatric disorders. Both profiles were similar in age, education, caregiving, religious affiliation, household size, and service intensity. They also resembled each other with respect to functional limitations, nutrition risk, and prior nursing home placement. Slight differences were found in frequency of previous hospitalizations and years of education. Recommendations for strengthening the mental health training of social workers employed at adult day centers were made.  相似文献

Using the first and second wave data of the Panel Study of Korean Children (PSKC), which is comprehensive longitudinal data with a nationally representative birth cohort, this study explores whether and when dual-working families place their infants and toddlers in child care centers and what factors influence these families' selection process. Among the 2078 families examined in the PSKC, 466 dual-working families which had any kind of nonparental care arrangement for their very young children at the time of the second survey were included in the analysis. Since our research questions address when the use of child care centers occurred, an event history analysis was conducted. The results show that 32.4% of the families had placed their infants in child care centers prior to the second interview. Family structure and income variables (number of children, nonparental adults in the household, and household income), mother's education level, and the mother's number of working hours turned out to be significantly related to the probability of child care center use.  相似文献

Grounded in critical feminisms, this study examines lesbian, gay, bisexual, transgender, and queer (LGBTQ) centers at public universities in the context of the regulating discourse of homonormative whiteness. Homonormative whiteness refers to dominant norms that constitute the queer body as White and male, positioning static ideas of whiteness and masculinity as normative. Interviews, including the use of mental maps, were conducted with 12 primary leaders at six campus LGBTQ centers. Findings signal three primary functions of LGBTQ centers: safety, legitimacy, and advocacy. Each function is situated in relation to the degree to which these centers spatially or discursively reproduce, resist, and/or transform homonormative whiteness. Taken together, these findings suggest a dynamic and context-specific understanding of the ways and reasons LGBTQ center leaders will act or otherwise shape the space and discourse of the center in ways that collude with or transform systems of power and dominance. Implications for research and practice, as well as specific recommendations for critically conscious organizational practice, are offered.  相似文献

Given large numbers of children attending center-based child care and considerable teacher and child mobility, it is important to study correlates and outcomes of children experiencing a change in their primary teacher/caregiver and/or a change to a different child care center. The present study investigated teacher and center stability in a group of 3238 urban, ethnically diverse, low-income, four-year-olds receiving subsidies to attend center-based child care. Children were individually assessed for cognitive and language development at the beginning and end of the pre-kindergarten year. Parents and teachers rated children on their socio-emotional skills and behavior at both time points. Children who experienced a change in their primary caregiver from the beginning to the end of the school year (41% of the sample) showed less growth in initiative for learning and attachment/closeness with adults over time, and scored lower on most indices of school readiness compared to those that had a stable caregiver. Children who moved to a different center during the year scored lower on teacher-reported initiative and attachment. African American children who switched centers were particularly at-risk for poorer outcomes, and boys who experienced a change in primary teacher, in particular, showed slower growth in cognitive development.  相似文献

Objectives: This study examined parental care of children with medical complexity (CMC) in terms of time spent providing care and impacts on employment and career.

Methods: We recruited caregivers of 153 CMC in a tertiary center complex care program to participate in a cross-sectional mail survey.

Results: Among 95 respondents (62% of eligible), 75% of primary caregivers and 53% of spouses experienced employment losses and one-third experienced negative career outcomes attributed to care for CMC. On weekdays during the day (6 a.m.–6 p.m.), 55% of caregivers provided care for 8 hours or more. On weekday evenings and nights, half of caregivers provided care for at least 6 hours, including 53% of caregivers who provided care from midnight to 6 a.m.; these proportions were higher on weekends. Compared to employed caregivers, non-employed caregivers reported spending more time providing care on weekdays (P?=?.001), but less time providing care on weekend evenings (P?=?.019). On weekend days and weekday evenings/nights, employment was not associated with duration of care.

Conclusions: Caring for CMC frequently impacts employment and careers of caregivers and spouses. Employed and non-employed caregivers invest substantial time in care. Research on CMC should include indirect costs of family care in terms of employment, income, and time.  相似文献
