Personal care work in proprietary nursing homes was contrasted with similar work in non-profit long-term care facilities. One quarter of Manitoba long-stay beds are operated by proprietary (profit-making) institutions. In both types of home, the nursing assistants were younger women with children; essential wage earners for their families. Wages were low, but nursing assistants said they worked because they liked taking care of elderly people. Nursing home residents have cognitive and mobility impairments and need help with activities of daily living. The proportion of cognitively-impaired residents is higher in profit-making homes, and probably contributed to job pressure and workload, which were taken as markers of potential alienation experienced by these workers.  相似文献

We present a hypothetical case study using the Years of Potential Life Lost (YPLL) metric to compare cancer risks incurred by residents living near a Superfund site to occupational fatality risks incurred by workers employed in that site's remediation. Since cancer occurs late in life, and because we assume its mortality rate is 60%, each case results in 8.8 YPLL. Each occupational fatality, which typically occurs earlier in life, results in 38.1 YPLL. In our case study, the residential population of 5000 incurred 1.3 YPLL, compared to 5.7 YPLL incurred by the 500 workers. Several uncertain assumptions may influence our calculations; moreover, occupational risks may be viewed as more "voluntary" than risks incurred by residents. However, because the magnitude of the YPLL incurred by workers and residents may be comparable, risk managers should consider occupational risks when evaluating remedial alternatives.  相似文献

本文研究了风险偏好与风险成本,分析了知识型员工风险偏好的特征,利用委托代理模型讨论了确定与不确定环境下员工风险偏好与企业激励强度之间的关系,并对如何协调两者的冲突以及知识型员工与企业间协调作用的特征进行了剖析.研究表明随着员工风险规避度的增大,企业应减小激励强度与风险分担;一定条件下企业和员工可协调对方的行为,但企业的激励措施居于主导地位;与一般员工相比,知识型员工的风险规避度较小且更具可变性,影响企业激励行为的能力更强,与企业间基于可变风险偏好的激励协调行为更可能发生.  相似文献

Recent research suggests that the relationship between job insecurity and psychological outcomes is more negative among permanent compared with temporary workers. We investigate possible interaction effects between job insecurity and type of contract (temporary versus permanent) for various psychological outcomes (job satisfaction, organizational commitment, life satisfaction, and self-rated performance), some of which have received little attention. We aim to explain these interaction effects, while taking into account the heterogeneous nature of temporary workers in terms of tenure, employment prospects, and wish to do temporary employment. We argue that permanent workers expect higher levels of job security; job insecurity breaches permanent workers' but not temporary workers' expectations. This may relate to unfavourable outcomes. Similarly, the heterogeneous nature of temporary workers may relate to job security expectations and thus to reactions to job insecurity. This study was conducted on a sample of 477 temporary and permanent workers from various occupational sectors in Belgium. The results suggested that the interaction effect between job insecurity and contract type may be limited to job satisfaction and organizational commitment. Furthermore, permanent workers had higher expectations about job security. Breach of these expectations furthermore mediated the relationship between job insecurity and all outcomes, except for self-rated performance. However, the heterogeneity indicators were found to be unrelated to job security expectations.  相似文献

In this article, we present a methodology to assess the risk incurred by a participant in an activity involving danger of injury. The lack of high-quality historical data for the case considered prevented us from constructing a sufficiently detailed statistical model. It was therefore decided to generate a risk assessment model based on expert judgment. The methodology is illustrated in a real case context: the assessment of risk to participants in a San Fermin bull-run in Pamplona (Spain). The members of the panel of "experts on the bull-run" represented very different perspectives on the phenomenon: runners, surgeons and other health care personnel, journalists, civil defense workers, security staff, organizers, herdsmen, authors of books on the bull-run, etc. We consulted 55 experts. Our methodology includes the design of a survey instrument to elicit the experts' views and the statistical and mathematical procedures used to aggregate their subjective opinions.  相似文献

Employers are more readily realizing that a nonsmoking policy in the workplace is a more effective way to run their businesses. They are recognizing costs such as productivity losses, increased health and life insurance costs, employer liability for diseases jointly linked to smoking and occupational exposures, absenteeism, passive smoking-induced health care costs among nonsmokers, workers' compensation, and fire losses. Concomitantly, employees are supporting policies that limit smoking to achieve a clean air environment. Former Surgeon General Koop's goal of "a smokefree society by the year 2000" is being recognized by more and more segments of society.  相似文献

Risk characterization in a study population relies on cases of disease or death that are causally related to the exposure under study. The number of such cases, so-called "excess" cases, is not just an indicator of the impact of the risk factor in the study population, but also an important determinant of statistical power for assessing aspects of risk such as age-time trends and susceptible subgroups. In determining how large a population to study and/or how long to follow a study population to accumulate sufficient excess cases, it is necessary to predict future risk. In this study, focusing on models involving excess risk with possible effect modification, we describe a method for predicting the expected magnitude of numbers of excess cases and assess the uncertainty in those predictions. We do this by extending Bayesian APC models for rate projection to include exposure-related excess risk with possible effect modification by, e.g., age at exposure and attained age. The method is illustrated using the follow-up study of Japanese Atomic-Bomb Survivors, one of the primary bases for determining long-term health effects of radiation exposure and assessment of risk for radiation protection purposes. Using models selected by a predictive-performance measure obtained on test data reserved for cross-validation, we project excess counts due to radiation exposure and lifetime risk measures (risk of exposure-induced deaths (REID) and loss of life expectancy (LLE)) associated with cancer and noncancer disease deaths in the A-Bomb survivor cohort.  相似文献

This study operationalizes and assesses the percentage of "good deaths" achieved among Chinese cancer patients in a palliative care program, the profile of these patients, the relationship between patients with a good death and psychosocial factors, and the differences in background factors, and physical and psychosocial conditions between patients who experienced a good death and those who did not. Clinical data mining was the research method used. Records of deceased cancer patients between 2003 and 2005 in a palliative care unit were the sole data source. Good death was operationally defined as the patient's record indicating no pain (physical) or anxiety (psychological), and having open and honest communication with family (social) in the final assessment by the Support Team Assessment Schedule (STAS) just before death. Using these criteria, about one-fifth of patients (21.5%; 137 out of 638) experienced a good death. Those with a good death were significantly older and were in palliative care longer. Their records also indicated lower levels of constipation, insomnia, oral discomfort, and family anxiety at their first and at their final STAS assessments. Good death was positively associated with recorded indicators of fullness in life, caregivers' acceptance and support, and negatively with reported feelings of upset about changes in the course of their illness. The results heighten awareness among social workers and other healthcare professionals about the value of good death in patients in palliative care. This empirically-based awareness can foster professionals' ability to set intervention objectives to help patients in palliative care achieve this universally accepted goal.  相似文献

The grief that certified nursing assistants (CNAs) experience following the deaths of nursing home residents has received scant attention in past research, particularly from an empirical standpoint. The purpose of this quantitative study was to investigate the grief experiences of CNAs in the nursing home setting and to identify and evaluate factors that may mediate or exacerbate grief. Participants (N=136) from 12 nursing homes completed self-administered surveys. Regression analyses revealed that CNAs with lower levels of perceived disenfranchised grief reported higher levels of personal growth, while CNAs with greater fear of death and those who experienced fewer deaths on the job reported higher levels of complicated grief. Practitioners and future researchers may benefit from these findings through the construction and implementation of interventions aimed at effectively enfranchising the grief experiences of this important group of healthcare workers.  相似文献

本研究借用了劳动力市场阶层化研究的思路和方法,利用专项调查获取的大量数据,实证分析了城市劳动力市场中的流动及其决定机制。本文的理论假说如下:第一,中国的城市劳动力市场包含了外劳和本地居民两大部分,但这两部分从业人员所属的市场阶层以及在市场阶层之间的流动状况是不相同的。第二,从非正规部门流向正规部门的上升移动会带来工资收入的增加,但本地居民和外劳,以及外劳中的民工与城镇居民实现流动的机会是不均等的。第三,在求职过程中选择什么性质的部门、以何种方式在不同部门之间流动,主要取决于以户籍为代表的制度因素,而不是个人的教育水平、工作经历等人力资本。本文的结论是:通过劳动力市场的竞争机制,人力资本的利用状况明显改善,但由于还存在制度歧视,劳动力市场依旧是二元性质的。消灭城市内部的新二元结构是下一步改革的重要目标。  相似文献

We estimated the number of transportation deaths that would be associated with water treatment in Albuquerque to meet the EPA's recently proposed revisions of the Maximum Contaminant Level (MCL) for arsenic. Vehicle mileage was estimated for ion exchange, activated alumina, and iron coagulation/microfiltration water treatment processes to meet an MCL of 0.020 mg/L, 0.010 mg/L, 0.005 mg/L, and 0.003 mg/L. Local crash, injury, and death rates per million vehicle miles were used to estimate the number of injuries and deaths. Depending on the water treatment options chosen, we estimate that meeting an arsenic MCL of 0.005 mg/L will result in 143 to 237 crashes, 58 to 98 injuries, and 0.6 to 2.6 deaths in Albuquerque over a 70-year period, resulting in 26 to 113 years of life lost. The anticipated health benefits for Albuquerque residents from a 0.005 mg/L arsenic MCL, estimated using either a multistage Weibull or Poisson model, ranged from 3 to 80 arsenic-related bladder and lung cancer deaths prevented over a 70-year period, adding between 43 and 1,123 years of life. Whether a revised arsenic MCL increases or reduces overall loss of life in Albuquerque depends on the accuracy of EPA's cancer risk assessment. If the multistage Weibull model accurately estimates the benefits, the years of life added is comparable or lower than the anticipated years lost due to transportation associated with the delivery of chemicals, disposal of treatment waste, and operation of the water treatment system. Coagulation/microfiltration treatment will result in substantially fewer transportation deaths than either ion exchange or activated alumina.  相似文献

Knowledge of trends in life expectancy is of major importance for policy planning. It is also a key indicator for assessing future development of life insurance products, substantiality of existing retirement schemes, and long-term care for the elderly. This article examines the feasibility of decomposing age-gender-specific accidental and natural mortality rates. We study this decomposition by using the Lee and Carter model. In particular, we fit the Poisson log-bilinear version of this model proposed by Wilmoth and Brouhns et al. to historical (1975-1998) Spanish mortality rates. In addition, by using the model introduced by Wilmoth and Valkonen we analyze mortality-gender differentials for accidental and natural rates. We present aggregated life expectancy forecasts compared with those constructed using nondecomposed mortality rates.  相似文献

The purpose of this article is to describe the end-of-life process in the nursing home for three groups of cognitively-impaired nursing home residents: those who died with a medical decision-making process prior to death; those who died without such a decision-making process; and those who had a status-change event and a medical decision-making process, and did not die prior to data collection. Residents had experienced a medical status-change event within the 24 hours prior to data collection, and were unable to make their own decisions due to cognitive impairment. Data on the decision-making process during the event, including the type of event, the considerations used in making the decisions, and who was involved in making these decisions were collected from the residents' charts and through interviews with their physicians or nurse practitioners. When there was no decision-making process immediately prior to death, a decision-making process was usually reported to have occurred previously, with most decisions calling either for comfort care or limitation of care. When comparing those events leading to death with other status-change events, those who died were more likely to have suffered from troubled breathing than those who remained alive. Hospitalization was used only among those who survived, whereas diagnostic tests and comfort care were used more often with those who died. Those who died had more treatments considered and chosen than did those who remained alive. For half of those who died, physicians felt that they would have preferred less treatment for themselves if they were in the place of the decedents. The results represent preliminary data concerning decision-making processes surrounding death of the cognitively-impaired in the nursing home. Additional research is needed to elucidate the trends uncovered in this study.  相似文献

Recent research in information systems and operations management has considered the positive impacts of information technology (IT). However, an undesirable side effect of firms’ increasing reliance on IT to support the distribution and delivery of goods and services to customers is a greater exposure to a diverse set of IT security risks. One such risk is intentional employee misuse of technology resources. In this article, we draw upon modern deterrence frameworks to develop a predictive model of technology misuse intention that incorporates formal and informal sanctions as well as employment context factors. The model specifies previously untested relationships between formal and informal sanctions, thereby providing fresh insight into the role of sanctions in deterring technology misuse in organizations. Our results suggest that a predisposition toward the need for social approval and moral beliefs regarding the behavior are key determinants of technology misuse. Contrary to criminological research that has questioned the relative importance of formal sanctions in the deterrence process, we also found that the threat of formal sanctions has both direct and indirect influences on technology misuse intention. Further, from an employment context standpoint, employees who spend more working days away from the office (i.e., “virtual” mode) appear more inclined to misuse their organization's technology resources. The findings have implications for the research and practice of technology management.  相似文献

Renewed interest in increasing flexibility in working time arrangements derives from both social and economic changes. The dominant reasons for greater flexibility in shiftwork arrangements relate to diverse business demands resulting from competitive pressures and rapid technological change, modern workers' personal preferences about the timing of work, and the reduction in hours of work. Many new patterns are applied to meet these demands. A review of innovative patterns reveals their close link with shorter working hours and diversifying career needs. Typical patterns include (a) hours averaging schemes, (b) flexible daily hours, (c) individualized free time schemes. (d) flexitime plus shift systems, (e) a combination of full-time and part-time systems, (f) job sharing, (g) temporary transfer to day work, and (h) career break schemes. Attention is drawn to multi-skilling of workers associated with more independent responsibility and closer communication. Changes to a flexible pattern are usually a result of consultations that respond to the conflicting interests of employers and workers. Support is necessary for a process of informed joint planning. Particularly useful are: (1) identification of operational demands and individual preferences about increasing flexibility in work schedules; (2) presentation of available options with information on implications for workers' health and social life; and (3) facilitating training and group consultation. The adjustments required depend greatly on how the tasks of individual workers are reorganized. In increasing the flexibility of shift schedules, it is important to learn from positive experiences of joint optimization of work content and working time arrangements.  相似文献

The article considers point processes most commonly used in reliability and risk analysis. Short-term and long-term behavior for the point processes used as models for repairable systems(1) are introduced. As opposed to the long term, the term short term implies that a process is observed during an interval limited by a time close to the mean (or the median) of the respective underlying distribution. A new simple upper bound is proposed on the cumulative intensity function of the renewal process and G-renewal process with an increasing failure rate underlying distribution. The new bound is compared with some known bounds for the renewal process. Finally, a formal definition of "a boundary point" between the short-term repairable system behavior and long-term behavior is introduced. This point can also be used as a lower time limit beyond which the "long-term" Barlow and Proschan bound for the renewal process with NBUE underlying distribution could be effectively applied.  相似文献

The past 15 years have seen an increasing governmental and corporate concern for the environment worldwide. For governments, information about the environmental performance of the industrial sector is required to inform macro-level decisions about environmental targets such as those required to meet UN directives. However, in many African, Asian, and Latin American countries, researching and reporting company environmental performance is limited. This article serves as a contribution to filling the gap by presenting evidence of physical and chemical risk in Nigerian factories. One hundred and three factories with a total of 5,021 workers were studied. One hundred and twenty physical and chemical hazards were identified and the result shows a high number of workers exposed to such hazards. The study also reveals that workers' awareness level of chemical hazards was high. Yet the danger was perceived in behavioral terms, especially by manufacturing firms, which tend to see environmental investment in an increasingly global economy as detrimental to profitability.  相似文献
