Individual data on menstrual cycles of noncontracepting women living in Western countries were used in order to verify whether the biological seasonality of conception persists after sexual behavior is controlled for. Episodes of intercourse were recorded daily, and the time of ovulation was detected by a marker We find that the seasonality of conception changes with woman's age and frequency of episodes of sexual intercourse. In particular; for women aged 27-31 having only one act of intercourse during the six most fertile days of the menstrual cycle, the seasonality of fecundability is stronger In this age group in the Northern Hemisphere, if seasonality of acts of sexual intercourse is controlled, the monthly distribution of probability of conception is bimodal, with two maxima (September and January) and two minima (December and March). When unobserved characteristics of the couples are considered, this seasonal pattern of conception persists.  相似文献

A new model of the behavioural and physiological causes of age-specific variation in marital fecundability is presented. Total fecundability is decomposed into a series of susceptibility factors (the length of ovarian cycles, the length of the fertile period within each cycle, the probability that a cycle is ovulatory, and the likelihood that an act of unprotected intercourse within the fertile period results in conception) and an exposure factor reflecting the effect of duration of marriage on coital frequency. The impact of intra-uterine mortality on effective fecundability is also modelled. Data on western women, from which standard age curves of fecundability are estimated, suggest that any decline in fecundity between ages 30 and 40 is attributable to changes, not in the ability to conceive, but in the capacity to carry a pregnancy to term. Sensitivity tests suggest that the most important potential sources of inter-population variation in fecundability are intra-uterine death and the incidence of anovulatory cycles.  相似文献

Abstract This paper describes a simple study of the chance of conception assuming a fixed-length menstrual cycle, a specific probability distribution for the day of ovulation, and a broad and a narrow fertile period. Eight different patterns of intercourse throughout the cycle are considered with frequency of intercourse ranging between 5 and 12 times per cycle. Results are given in terms of chance of conception and in terms of waiting times until conception.  相似文献

A companion paper in the last issue reviewed the literature on effects of frequency of intercourse on fecundability. At moderately high coital frequencies, predicted effects of further increases on fecundability based on previous work vary widely. New modelling reported here, designed to take into account the ageing of gametes, attempts to identify a set of limits within which the true relationship of coital frequency and fecundability at moderate to high frequencies may confidently be expected to lie.  相似文献

Abstract In this paper reliable estimates of mean fecundability at marriage for a sample of heterogeneous Taiwanese women are obtained by using a set of data collected retrospectively. The effects of 'truncation bias' and 'memory bias' are estimated by studying the relationships between mean fecundability and the duration of marriage. Then the variations in fecundabilities by age at marriage are studied. The data are taken from an intensive fertility survey of married women between the ages of 20 and 39 conducted in Taichung city of Taiwan towards the end of 1962 prior to a year-long family planning action programme. Fecundability, the probability of conception in the absence of contraception, is estimated by using a Type I geometric model and is estimated from the observed distribution of first pregnancy intervals-the period between the onset of marriage and the beginning of first conception. The estimated fecundability level relatively free from truncation and memory bias is 195±3 per 1.000 women. The fecundability level increases with wife's age at marriage (up to 25) independently of its association with the duration of marriage.  相似文献

This paper develops a multistate hazards model for estimating fecundability and sterility from data on waiting times to conception. Important features of the model include separate sterile and nonsterile states, a distinction between preexisting sterility and sterility that begins after initiation of exposure, and log-normally distributed fecundability among nonsterile couples. Application of the model to data on first birth intervals from Taiwan, Sri Lanka, and the Amish shows that heterogeneity in fecundability is statistically significant at most ages, but that preexisting sterility and new sterility are unimportant before age 40. These results suggest that sterility may not be an important determinant of natural fertility until later reproductive ages.  相似文献

A method for estimating conception rates, using vital statistics data, is developed and applied to data on five-year age groups of California women for 1971. The approach is deterministic and allocates total exposure time to the known pregnancy outcomes of live birth, spontaneous abortion, and induced abortion. The population at risk is defined to exclude women who are known to be sterile or sexually inactive. Early fetal loss, premarital conception, and contraceptive use are taken into account. Estimates are made of the fecundability which would obtain ifno contraception were used.  相似文献

In Bangladesh twice as many births occur in December as in July. This paper examines the seasonal patterns of the risks of conception, fetal loss and return of menses post partum in a longitudinal study of 2,300 women in 14 villages of Matlab, Bangladesh. Life tables were estimated for each month of entry event and then ‘period’ life tables were constructed with the risks for a given calendar month. Confirming the results of earlier studies, risks of resumption of menses were higher in November and December, regardless of time elapsed since the last birth. Similarly, there are increased conception risks in the period from February to April for all fecundable women. The time of lowest fetal loss and stillbirth risks is in the cool season, though this variation makes only a minor contribution to the overall seasonality of births. The pattern of fecundability estimated from data on coital frequency did not match the pattern estimated from reported conceptions; these discrepancies imply possible seasonal changes in other parameters of fecundability besides intercourse.  相似文献

Abstract Data on 1898 menstrual cycles, for 241 married women, were analysed by means of a quantal regression programme. The locations of the day of ovulation was determined by the basal body temperature method. Estimates were obtained of the risk of conception from an act of coitus on each day, as measured from the day of ovulation. Abstract The analysis previouslyreported (Population Studies, 23, 1969) of the risk of conceptionon different days of the menstrual cycleis extended so as to take someaccount of the agesof wives. The risks are estimated from data (basal body temperatures and occurrences of coitus) recorded by 241 married couples not using any contraceptives, to whom at least one child had previously been born. Effective fecundability declines with age and parity. Furthermore, women differ in their effective fecundability: some women have persistently low or high monthly chances of live-birth conception. Estimates are presented concerning the magnitude of these effects in a natural-fertility population: 406 Hutterite women in North America who had 3,206 births, largely in the 1940s and 1950s. The estimates are based on models that incorporate the effects of persistent heterogeneity and that use the full information provided by multiple-spell duration data. In addition, hazards rather than probabilities are modeled, piecewise linear hazard functions are used, and age and parity effects are decomposed systematically. These methods permit the development of more elaborate models of changing fecundability and of heterogeneity in postpartum amenorrhea.  相似文献

Abstract A probability model, building on the work of Perrin and Sheps, is presented and applied. The model makes it possible to follow a cohort of women from a pregnancy outcome to next conception. Principal simplifying assumptions are homogeneity among women and constant fecundity. Cases treated include no contraception; a single contraceptive method practised; and a second contraceptive method practised following a first one. Expected durations to next conception may be calculated in relation to the following factors: outcome of previous pregnancy, length of anovulation, natural fecundability, time first contraceptive method is initiated, characteristics of first contraceptive (effectiveness and continuation rate), characteristics of second contraceptive, and gap between termination of first and commencing second contraceptive. By suitably pairing runs of the model, one can construct experiments in which the only differentiating factor is use and non-use of a specified contraceptive, and, by differencing the corresponding mean durations to next conception, compute the net delay of next conception produced by that contraceptive.  相似文献

A new method for the estimation of the mean and variance of fecundability is described. The data input required for this procedure is the distribution of the interval from marriage to first birth, or from the resumption of the conception risk after contraction to the subsequent birth. The estimates of the mean and variance of fecundability are obtained by fitting a model to the observed interval distribution. To test the method, it is applied to data from five historical populations. The fecundability means in these populations ranged from 0.18 to 0.31 while the co-efficients of variation all had values near 0.56. A short method for the estimation of the mean of fecundability based on the same model, but not requiring a computer, is also presented.  相似文献

Abstract A group of 209 married, fecund women in rural Bangladesh were studied prospectively for 24 months from 1969 to 1971 to define some of the biological and sociological factors relating to fertility performance. These women were selected from a larger study population of 112,000 that had been followed with a daily house-to-house vital registration programme since 1966. The selected women were interviewed bi-weekly and were asked questions about menstruation, pregnancy, lactation, husband's occupational absences, and monthly urine tests for pregnancy were taken. The results for 193 non-contracepting women revealed that the seasonal pattern of births previously observed in this population could be associated with a corresponding seasonal pattern of conceptions and that this was due to a seasonal trend in fecundability. The highest conception rates were in the coolest months of the year. Post-partum lactational amenorrhoea was very prolonged, averaging 17 months for women with a surviving child. The appearance of the first post-partum menstrual flow (onset of ovulation) also had a seasonal trend which could not be adequately explained. The median waiting time to conception, once menstruation had resumed was eight months. This interval was influenced by seasonal fluctuations, as well as by the age of women and by husbands' absences. The foetal wastage rate was 15·0 per 100 conceptions, with 62 per cent of the foetal losses occurring during the second month of gestation. Overall, the average birth interval was 33 months, with the prolonged lactational amenorrhoea accounting for almost 45 per cent of this interval. Estimates of fecundability (monthly probability of conception) in the absence of contraception are derived from the frequency distribution of conceptive delays immediately following marriage, reported by 2,443 married women aged 20 to 39 included in the Taichung (Taiwan) Intensive Fertility Survey of 1962. Average fecundability of women is positively associated with their socio-economic status. These differentials are not accounted for by differences among socio-economic groups with respect to memory and truncation biases (associated with the marriage duration), wife’s age at marriage, or unreported premarital conceptions. A Multiple Classification Analysis suggests that among the socio-economic characteristics, husband’s education, rural background, and modern family type are the more important predictors of fecundability. The importance of genetic factors as opposed to cultural factors in producing these socio-economic differences in fecundability can not be evaluated systematically. Moreover, the relation of a couple’s privacy, their attitude toward family building, and patterns of mate selection to their socioeconomic status would have to be taken into account before the differences in fecundability could be attributed to factors such as nutrition, health, or infections which might directly influence their physiological ability to conceive.  相似文献

This study analyzes the stability of cohabiting and marital unions following a first birth. But unlike previous research, it compares the subsequent trajectories of unions that began with a pregnancy to those in which conceptions came after coresidence. The U.S. data from the 2006–2010 and 2011–2013 cross-sectional files of the National Survey of Family Growth indicate that roughly 1-in-5 first births were associated with rapid transitions from conception into either cohabitation or marriage. Moving in together following a pregnancy—especially an unintended one—is unlikely to lead to marital success or union stability. Compared with marital unions, dissolution rates following birth were particularly high for couples who entered a cohabiting union following conception. Only a small minority of these couples married (i.e., less than one-third), and these marriages experienced high dissolution rates. The results also suggest that the most committed cohabiting couples got married after finding themselves pregnant, leaving behind the most dissolution-prone cohabiting couples. The American family system is being transformed by newly emerging patterns of fertility among cohabiting couples.  相似文献

Abstract A computerized probability model of family-building, FERMOD, is described and then utilized in an investigation of relations between family planning and fecundity as applying to white couples of the contemporary United States. Models of this type that formulate reproductive performance as a stochastic process permit one to explore relations that are not directly observable and in this manner to secure at least partial answers to questions not subject to investigation by survey research alone. Two main questions are addressed concerning the dependence of family planning success upon fecundity: (1) How quickly does spacing control deteriorate when natural fecundability is taken at progressively lower values or when the risk of pregnancy wastage is set at progressively higher values? (2) What is the distribution of unsought births among couples of average fecundity when they practise contraception with specified effectiveness and have stipulated spacing and family size goals?  相似文献

ObjectiveStillbirth affects 1:200 pregnancies in high income countries. Most women are pregnant again within 12 months. Little is known about how couples negotiate a subsequent pregnancy. This paper presents findings from a study exploring the experiences of couples’ in pregnancy after stillbirth.MethodsQualitative, interpretive phenomenological analysis was used to conduct in-depth interviews with eight heterosexual couples in the immediate pregnancy after stillbirth. Couples were interviewed together to explore their dyadic, lived experiences of stillbirth and the pregnancy that follows.ResultsHoping for a born alive baby was one superordinate theme and Trying to conceive one of its subordinate themes, is presented here. Couples jointly negotiated their decision to get pregnant again, varying upon their individual circumstances, including their experiences of stillbirth. Gender differences were apparent in a couple’s agreement to pursue a pregnancy after stillbirth and may be explained by the desire of men to fully parent the baby who died before reaching a decision about a subsequent pregnancy. Sexual intercourse often became less about emotional connection and more about a means to achieve a pregnancy.ConclusionCouples spoke of the need for each partner to be in agreement with the decision for a pregnancy. The experiences of trying to conceive after stillbirth impacted the couple relationships. Couples who were able to discuss their feelings with one another appeared more cohesive than those who experienced communication challenges in the aftermath of loss. New insights into men’s thinking about the decision to get pregnant after stillbirth were revealed.  相似文献

The biological and demographic literature on the effects or frequency of intercourse on fecundability is reviewed in this paper. While empirical work and model-building results agree well on the effect of change from low to moderate coital frequencies, inconsistencies emerge when increase from moderate to high coital frequencies is considered. Of the models considered, it may be argued that the explicit provision for unfavourable cycles makes Schwartz's model an improvement over Barrett and Marshall's, Lachenbruch's and Trussell's. Moreover, even though he used a very different approach, Bongaarts obtained results that are much closer to those of Schwartz et al. than the others. Bongaarts's model is used as a starting point for new modelling, to be reported in the next issue, which deals with these inconsistencies by taking into account the ageing of gametes.  相似文献
