Despite revising their reproductive health policies in line with the 1994 International Conference on Population and Development (ICPD) Programme of Action, a major challenge facing many developing countries is the inability to fully implement the policies owing to lack of funds, bureaucratic delays, and limited awareness among various stakeholders. In some countries, the policies fail to adequately address sexual and reproductive health (SRH) needs of vulnerable groups. This study examines the barriers to SRH programming for adolescents living with HIV from the perspectives of key stakeholders involved in SRH issues in Uganda. The data are from qualitative interviews conducted in 2007 with 23 key informants from bilateral institutions, government ministries, and civil society organizations. The study findings confirm that policy and programmatic gaps exist in addressing the SRH needs of HIV-positive adolescents. This is attributable to: (1) lack of clear guidelines on how to address the SRH of HIV-positive adolescents; (2) challenges of dealing with adolescent SRH in general; (3) HIV/AIDS treatment, care, and support services that are either pediatric- or adult-oriented; and (4) limited institutional and provider capacity to offer SRH services to HIV-positive adolescents despite recognizing that this is an emerging area that requires intervention. These results suggest the need for: (1) clear guidelines on dealing with SRH of HIV-positive adolescents; (2) establishing transition clinics or youth-friendly corners to cater for the needs of adolescents who cannot fit in either pediatric or adult clinics; and (3) providing training and reorientation on SRH of HIV-positive adolescents to service providers/counsellors.  相似文献

The goal of the United Nations International Conference on Population and Development (ICPD), held in Cairo in September 1994, was to agree on a Programme of Action in the field of population and development which would supersede the Plan of Action agreed to at Bucharest in 1974 and subsequently amended ten years later in Mexico City. The main purpose of the present paper is to characterize some of the principal intellectual and ideological developments of the last 20 years which have had an impact on the definition of this goal. I conclude with some brief comments on the Programme of Action adopted by consensus at the Conference. This is an amended version of the paper I presented at the Seventh National Conference of the Australian Population Association (in a Plenary Session entitled ‘Messages from Cairo’), held in Canberra in September 1994.  相似文献

A recent Population Council publication, Reproductive Health Approach to Family Planning, discusses integration of reproductive health into family planning programs in a series of edited presentations that Council staff and colleagues gave at a 1994 meeting of the US Agency for International Development (USAID) cooperating agencies. The presentations reflect the Council's view that family planning programs ought to help individuals achieve their own reproductive goals in a healthful manner. The report discusses four areas of reproductive health: reproductive tract infections (RTIs), including sexually transmitted diseases; prevention and treatment of unsafe abortion; pregnancy, labor, and delivery care; and postpartum care. Christopher Elias (Senior Associate, Programs Division) argued that family planning programs ought to provide services that target RTIs, given that these illnesses afflict a significant proportion of reproductive-age women. The family planning community has an ethical responsibility to provide services to women who experience an unwanted pregnancy. They must have access to high-quality postabortion care, including family planning services. Professional midwives are ideally suited to serve as integrated reproductive health workers trained to combat the five major maternal killers: hemorrhage, sepsis, pregnancy-induced hypertension, obstructed labor, and unsafe abortion. This was demonstrated in a highly successful Life-Saving Skills for Midwives program undertaken in Ghana, Nigeria, and Uganda, and soon to start in Vietnam in conjunction with the Council's Safe Motherhood research program. Family planning services should be viewed as part of a comprehensive set of health services needed by postpartum women, which include appropriate contraception, maternal health checks, well-baby care, and information about breastfeeding, infant care, and nutrition. Family planning programs should incorporate breastfeeding counseling into their services. When programs aim to help individuals meet their own reproductive goals in a healthful manner, this implies that services will not increase clients' risk of morbidity.  相似文献

In December 2003, “acting on the encouragement of UN Secretary‐General Kofi Annan,” the Global Commission on International Migration was established as an independent body, consisting of 19 Commissioners co‐chaired by Jan O. Carlsson, former Minister for Migration and Development, Sweden, and Mamphela Ramphele, formerly the World Bank's Managing Director, from South Africa. The mandate of the Commission was to “provide the framework for the formulation of a coherent, comprehensive and global response to the issue of international migration.” The work of the Commission was assisted by a Geneva‐based Secretariat and a “Core Group of States,” eventually including 32 governments from all world regions, that acted as an informal consultative body to the Commission. (The United States, the most important host country to immigrants, was not among the 32.) In October 2005, in New York, the Commission presented its Report to Kofi Annan, the UN member states, and other interested bodies. The Report is also intended as an input to intergovernmental discussion of international migration issues at the UN General Assembly in the Fall of 2006. The Report, an 88‐page document, is accessible at « http://www.gcim.org ». That web site also provides access to extensive background materials on selected topics concerning international migration, regional studies of international migration prepared for the Commission, and reports of the regional hearings, consultations with “stakeholders,” and expert meetings held by the Commission. Reproduced below are three sections of the Report: its Introduction (titled “Dimensions and dynamics of international migration”) and two of its four Annexes: “Principles for Action and Recommendations,” and a compendium of data: “Migration at a glance.” Under the impact of globalization, international migration, long an important element of demographic change as experienced by individual states, has acquired increasing salience in international relations and in domestic politics. National sovereignty in deciding about immigration policy (probably the key determinant of contemporary international migration flows) remains an established principle in international law, subject only to treaty obligations to admit bonafide refugees. Increasingly in recent years, however, demands have surfaced to treat such policies as matters to be decided bilaterally between sending and receiving countries, or even to be regulated by an international or supranational body. (For earlier voices discussing this topic see the Archives section of this issue and the Archives section of the December 1983 issue of PDR:“On the international control of migration.”) Unexpectedly to some observers, the Report of the Global Commission fell short of recommending establishment of a new, WTO‐like, international organization within the UN system with responsibility for international migration. It recommends, instead, steps to be taken toward an Inter‐agency Global Migration Facility. Whether or not such arrangements will materialize and be influential, the Commission clearly sees international migration flows, primarily from less developed to more developed countries, as increasing in the future. While not quantified, this vision contrasts with the assumptions incorporated in the often‐cited projections of the UN Population Division, which envisage future net migratory flows as either constant in size or even decreasing. The Report's argument rests primarily on the perceived economic benefits of migration to both receiving and sending countries, fueled by persisting income differentials and by contrasting demographic configurations between migrants' places of origin and destination. It gives short shrift to arguments that question the economic gains of mass migration to receiving countries, or that see such gains at best as minor and likely to be counterbalanced by noneconomic considerations. Nor does the Report gauge the likelihood that heeding its strictures for a more welcoming treatment of migrants would increase the incentives to migrate.  相似文献

Using the 1994 International Conference on Population and Development (ICPD) as a case study, I examine how women's empowerment amassed public attention. I investigate feminist preparations for the conference and suggest that by playing an active role in preparatory meetings and the conference, women gained recognition from governments throughout the world and from the United Nations, and helped frame issues in a way that motivated the press to highlight women's empowerment. I argue empowering women became a catch phrase for media coverage and that abortion surfaced as a symbol of this empowerment. UN publications and reports of proceedings prior to, and during the 1994 ICPD, and articles from The New York Times illustrate use of this media package. The paper raises questions about types of policies governments will use to improve the position of women and the political power women will have in decisions.  相似文献

I propose that the primary goal of twenty‐first‐century population policies should be to strengthen the human resource base for national and global sustainable development. I discuss the shortcomings of the three dominant twentieth‐century population policy rationales: acceptance of replacement‐level fertility as a demographic goal; realizing a “demographic dividend” from the changing age structure; and filling the “unmet need” for family planning. I demonstrate that in all three cases the explicit incorporation of education into the model changes the picture and makes female education a key population policy priority. Population policies under this new rationale could be viewed as public human resource management. I argue that 20 years after the Cairo ICPD the international community needs a new rationale for population policies in the context of sustainable development and that a focus on human capital development, in particular education and health, is the most promising approach.  相似文献

By 1989 Asia's population will reach 3 billion. That Asia's countries can change the course of population development has been shown by China, whose population growth rate has decreased to 1.2%. 58% of the world's population in 1985 was Asian, and 53% of it was concentrated in 11 Asian countries, of which 37.6% was accounted for by India and China. Asia's population density is 3 times the world average, and the number of persons sustained by a square kilometer of land in Asia is 2.5 times the world average. Asia's population is young (median age 20.3), which means a high dependency burden, a large number of women of childbearing age, and low quality of life, as measured by infant mortality, life expectancy, and literacy. Rapid population growth ensures a low rate of development. Asia's goals are to achieve a 1% growth rate by year 2000, zero population growth and replacement level by 2015 for East Asia and 2020 for South Asia. The World Bank estimates that Asia's population will not stabilize until the end of the 21st century, by which time it will have reached 6 billion. Asia must find a way of achieving both population control and economic development. 5 recommendations are made to the Asian Forum of Parliamentarians on Population and Development (AFPPD): 1) that the AFPPD sponsor the activities of "the Day of 3 billion"; 2) that seminars and conferences on population be held among Asian nations; 3) that high-fertility countries adopt late marriages, few births, and programs for maternal and child health; 4) that organizations for family planning be strengthened and given the resources to upgrade the status of women; and 5) that international cooperation in the area of population be intensified.  相似文献

The lessons from the 1994 World Population Conference in Cairo, Egypt, are summarized in this publication. The topics of discussion include the evolution of population policies, the changing policy environment, demographic trends, and solutions in the form of gender equity, provision of reproductive health services, and sustainable social and economic development. The program of action supported by 180 governments and targeted for 2015 articulated the goals of universal access to a full range of safe and reliable family planning methods and reproductive health services, a specified level of reduction in infant and child mortality, a specified level of reduction in maternal mortality, an increase in life expectancy to 70-75 years or more, and universal access to and completion of primary education. Other features include goals for improving women's status and equity in gender relations, expansion of educational and job opportunities for women and girls, and involvement of men in childrearing responsibilities and family planning. Steps should be taken to eliminate poverty and reduce or eliminate unsustainable patterns of production and consumption. Population policy must be integrated within social and economic development policies. About $22 billion will be needed for provision of family planning and reproductive health services by the year 2015. Costs will increase over the 10-year period due to the increased population to be served. Per person user costs for family planning alone are higher in countries without infrastructure and technical skills. Actual costs vary with the cost of contraceptive supplies, patterns of use, and efficiency of delivery systems. Although the plan offers 16 chapters worth of advice and recommends 243 specific actions, countries will have to be selective due to cost limitations. The 20/20 Initiative is proposed for sharing social service costs between international donors (20%) and host countries (20%). A separate UN projection of need is for 33% of support from international donors for family planning and related programs. The constraints to the implementation of the action plan are identified as the rate of demographic change, the extent of public support for population limitation and provision of family planning services, and potential conflicts of interests and funding between cooperating agencies. The World Bank has developed guidelines for policy development according to a country's identification as an emergent, transitional, or advanced country.  相似文献

The Population Council's issue paper entitled Reconsidering the Rationale, Scope, and Quality of Family Planning Programs calls on family planning programs to focus only on reducing unwanted fertility by helping people meet their own reproductive goals safely and ethically. Many family planning programs have been wrongly handed the extra responsibility of reducing wanted fertility. They have therefore used inappropriate means (e.g., incentives, quotas, and coercion) to boost contraceptive prevalence. If programs do focus on reducing unwanted fertility, they will foster reductions in overall fertility and population growth as well as improvements in clients' health and well-being. A new framework has emerged from this shift in rationale. It sets the stage for expanding the scope, improving the quality, and assessing the impact of family planning programs in terms of client choice, health, and well-being. A program needs to determine the range and quality of family planning services it provides at the local level. Local program managers, policymakers, and consumer interest groups should establish minimum or achievable standards of service based on the local health care capacity. Program items that should be assessed include choice of methods, information for clients, technical competence, interpersonal relations, mechanisms to encourage continuity of care, and appropriate constellation of services. The Population Council has conducted rapid appraisals of the quality of care of family planning services to help local program managers to evaluate the strengths and weaknesses of their programs. The HARI (Helping Individuals Achieve their Reproductive Intentions) Index measures a program's success in helping clients safely prevent unwanted or unplanned pregnancy. Program managers can conduct a self-assessment that revolves around answering four questions. Family planning services are an important social investment and are essential to development.  相似文献

A number of prominent demographers have recently reiterated the argument that a lasting mortality decline is a key determinant of the fertility transition. Of the main hypothesized pathways linking fertility to mortality, the one least studied is the insurance hypothesis: the notion that, in high‐mortality contexts, people decide to have more children in order to anticipate possible future child deaths and lessen the risks of having too few surviving offspring. In‐depth interviews and focus groups from Zimbabwe and Senegal are used to examine this hypothesis and to extend it into a broader theory of reproductive decision making under uncertainty. Whereas insurance strategies are frequent in Zimbabwe and occur in urban Senegal, in the higher‐mortality settings—the rural Senegalese site and the recent past described by respondents in Zimbabwe and urban Senegal—deliberate fertility‐limitation strategies are rare. The data depict fundamental changes in attitudes, strategies, and behaviors concerning family size over time and, in Senegal, over space. Important reproductive goals and risks extend far beyond numbers of children and mortality. Parents seek to have healthy, successful children for many reasons including companionship, descendants, and old‐age support. Diverse investments in child quality (their education, health, etc.) and quantity (numbers of births) are the main means to attain these goals and, less recognized by demographers, are also important ways for parents to manage uncertainty in family‐building outcomes; the “classic” insurance mechanism is only one, often minor, aspect of the quantity option.  相似文献
