Children and adolescents with functional somatic symptoms are challenging to understand and to treat. The challenges begin at the very outset of the intervention – with the neurological and psychiatric assessments. Patients presenting with functional somatic symptoms, as well as their families, frequently deny any emotional or family problem, and parents are often genuinely baffled as to why a child has suddenly become so ill and why no medical explanation is forthcoming. Families can be unwilling to engage in family assessment and therapy, and therapists may find that standard approaches to family therapy simply can end up alienating these families – the door to therapy is slammed shut. This article is the story of my struggle to understand somatising children and their families and to find a common language to enable us to co‐construct formulations, to agree to a treatment plan, and to work together towards a pathway to health. It is also about the role of research and how knowledge from different system levels – and most specifically about the body – may need to be integrated into the therapy to help bring about change.  相似文献

This article draws on ethnographic data from a field leading university licensing office to document and explain a key step in the process of institutionalization, the abstraction of standardized rules and procedures from idiosyncratic efforts to collectively resolve pressing problems. I present and analyze cases where solutions to complicated quandaries become abstract bits of professional knowledge and demonstrate that in some circumstances institutionalized practices can contribute to the flexibility of expert reasoning and decision-making. In this setting, expertise is rationalized in response to institutional tensions between academic and business approaches to deal making and professional tensions between relational and legal approaches to negotiation. Abstraction and formalization contribute both to the convergence and stability of routines and to their improvisational use in professional work. Close attention to these processes in a strategic research setting sheds new light on an interesting tension in sociological theories of the professions while contributing to the development of a micro-level, social constructivist institutional theory.  相似文献

Gerontologists have studied religion and spirituality for almost three decades. Much of this work focuses on determining how elders' religious and spiritual beliefs and practices affect various indicators of well-being. This paper critiques many of the assumptions underlying this research and suggests alternatives to individualistic formulations of well-being. Experiences in spiritual communities of various types may help aging persons to be mindfully present to others, to recognize that all are vulnerable to the afflictions of aging and the certainty of death, and to love and care for others while working to promote justice. In this paper, I describe how my marriage to a minister enabled me to develop relationships with frail elders and young adults seeking to live faithfully. These friends have influenced my work and have helped to shape my views on my own aging.  相似文献

Treatment of a borderline mother and her adolescent daughter is approached with a combination of individual and conjoint sessions, enabling the therapist to identify the distortions within their relationship and to interpret these to the individuals. Mother and daughter attempt to involve the therapist in a collusion in the rejecting-abandoning projection. A combination of interpretation and presentation of a real, nurturing object enables the adolescent to become more interested in peers and able to go to college, and the mother to become less accident prone and able to keep her job.  相似文献

SUMMARY. The past decade has shown increased awareness, concern and understanding about the extent of child abuse, particularly sexual abuse, and the caring agencies response to listen to and to support the young victims. This article looks at the response of the police service over that decade to meet such demands and to work jointly and responsively with social services to further the welfare and protection of children. Teachers learned knowledge, skills, values, and dispositions through spending significant amounts of time working with heritage sites as they learned more about social studies. Elementary social studies teachers engaged in teacher in-service to increase their abilities to meet geography standards. Teacher professional development that caused instructors to engage with cultural and historic sites benefited them and should be considered as well as other models of teacher in-service. Teachers used study travel opportunities to engage in geographic investigation and exploration to accomplish inquiry. Teachers determined that extended time was required to experience the place, to value it, and to harbor the disposition to teach about it; they learned in-depth content from spending time on-site, and they learned skills to extract knowledge from site through reading the landscape. Teachers encountered multiple perspectives, technology, and controversial issues through their studies of regions in the Ohio River Valley. Teachers of elementary social studies may wish to consider this type of professional development to understand a sense of place and how it relates to the curriculum they teach.  相似文献

Four hundred and fifty-one teenagers responded to a sexual attitude survey. Results focus on attitudes and values related to sexual activity and to parental and school involvement and responsibility for sex education. Findings are reported as they relate to six important sexual decisions: to have intercourse or not? to have children or not? to use contraceptives or not? to have an abortion or not? to give the baby up for adoption or not? and to marry or not? Lack of communication, feelings that parents were unaware of their teenagers' sexual behavior, and uncertainty and confusion about their sexual feelings also surfaced.  相似文献

Experimental vignettes were used to investigate attributions relating to child sexual abuse with a focus on the degree of blame allocated to the family and to society, factors thought to be particularly relevant in a collectivist society. One hundred and sixty-two undergraduates in Singapore evaluated media reports describing a case of child sexual abuse. A 2 x 2 x 2 between-subjects design manipulated victim sex, perpetrator sex, and victim–perpetrator relationship. Participants rated the vignettes on degree of blame and prevention potential and rated the abusiveness of the case. Individualism and collectivism attitudes of the participants were also measured. While the highest blame ratings were attributed to perpetrators, significantly more blame was attributed to the family and to society than to the victim. The demonstration of the present attributions of blame to family and to society is a timely finding given recent recommendations to broaden approaches to child abuse prevention by moving away from a reliance on school based child protection programs, which leave the onus on the child to prevent and report abuse, toward a public health approach, which is particularly inclusive of parent and community education approaches . Allocation of some blame to victims, in spite of their status as children, while not a unique finding in victimology research, emphasizes the challenges still to be faced in encouraging the reporting of child sexual abuse.  相似文献

In this era of rapidly urbanising human populations, urban practitioners are under increasing pressure to create resilient and sustainable cities and towns. Urban ecologists currently have a unique opportunity to apply solid, evidence-based research to help create biodiversity-rich and sustainable cities and towns for the future. Unfortunately, there is currently a mismatch between the questions planners, designers and decision-makers are asking urban ecologists that would allow them to improve the biodiversity outcomes in urban areas, and the questions urban ecologists must ask to contribute to the development and application of the science of urban ecology. For a number of reasons, urban ecologists over the past 25 years have primarily focused on describing the patterns of biodiversity in cities and towns using broad, aggregate predictor variables (e.g., distance to city center, land-use, percent cover of impermeable surfaces and vegetation, etc.). We refer to these results as ‘low-hanging fruit’. If the discipline of urban ecology is going to provide the necessary information to inform actions to preserve and enhance urban biodiversity, we need to move beyond place-based research, and work towards the development of confirmed generalizations regarding the relationship between the structure and function of urban ecosystems and biodiversity. We propose three essential strategies for achieving this refined understanding: 1) defining the study window to place the study into a broader global context, 2) collecting and using more explicit question-driven measures of the urban condition in order to improve our understanding of urban ecological drivers, as well as recording more detailed ecological responses to provide insights into the ecological mechanisms underlying an observed response, and 3) expanding studies to include multiple cities, regions and countries. These strategies will help to expedite the ability of urban ecology to contribute to the creation of biodiversity-rich, healthy, resilient cities and towns.  相似文献

Late-life stressors often require individuals to make substantial alterations in behavior and lifestyle and can affect their overall health and well-being. Relocation is a significant life stress, regardless of age. The primary aim of this study is to elucidate the push-pull factors associated with moving into congregate senior housing. The secondary aim is to investigate the decision-making processes and stresses associated with moving into a congregate living environment. Interviews were conducted with 26 women who were new residents in congregate senior housing. Relocation, as expected, was considered to be stressful, although individual differences were found among perceptions of relocation stresses. Women who had made the decision to relocate on their own showed evidence of better psychosocial well-being at the time of the move. One-quarter of the sample chose to move to provide care to another person. As the options for senior housing continue to evolve and the number of adults reaching advanced age continues to increase, it is important to understand the factors that contribute to successful adaptation. This knowledge will enable facility administrators to implement programs and procedures to assist incoming residents with acclimating to their new homes.  相似文献

Young people are exposed to gambling promotions while watching televised sports; however, little research has examined how this influences gambling attitudes and intentions. This paper developed and tested a research model underpinned by the Theory of Reasoned Action and specifically aimed to examine (1) adolescents' exposure and attitudes to, and recall and perceptions of, gambling promotions during televised sport; (2) associations between adolescents' exposure and attitude to these gambling promotions, and their intention to gamble on sports and other gambling activities once of legal gambling age. An online survey was conducted of 131 Australian adolescents. Greater intention to gamble both on sports and non-sports was associated with higher frequency of watching televised sports, and more positive attitudes to gambling operators, to gambling promotions during televised sport and to promotional techniques used. Regression analysis indicated that the strongest predictors of sports betting intention were male gender and a more positive attitude to gambling sponsors and their promotions during televised sport. Strongest predictors of gambling intention were male gender, subjective norms and a more positive attitude to promotion of gambling during televised sport. Findings can inform advertising restrictions for gambling during general television viewing times, and health promotion messages countering promotion of gambling to adolescents.  相似文献

The return of the Labour government to power in 1997 brought an increased focus upon inclusive education for children with special educational needs (SEN). Alongside this there has been a desire to enhance the opportunities young people have to access physical education (PE) and school sport. Previous research has shown that children with SEN often have fewer opportunities to access physical activity due to physical, social and emotional barriers to participation. With the advent of a new Con–Lib-Dem government in 2010 and a planned review of inclusion policies and practices, this paper examines the extent to which children with SEN have arrived at utopia in relation to their entitlement and access to PE. The paper identifies four themes central to successful inclusive PE, whilst suggesting there is still some way to go for children with SEN to have the same opportunities in physical activity as their non-disabled peers.  相似文献

The objectives of this pilot study were to explore the social network relationships of divorced persons prior to divorce and up to one year following divorce, and to delineate the network changes which take place and the ways in which these changes affect adjustment to divorce. Subjects were administered the Tennessee Self-Concept Scale and the General Well-Being Schedule and completed a social network questionnaire. In general the findings suggested that changes in the social networks of individuals from pre to post divorce tend to decrease the quaLily of the person's adjustment to this life crisis. Suggestions were provided for future research in this area and for clinical interventions designed to deal with divorced persons.  相似文献

The purpose of this study was to examine children's mathematical understandings related to participation in tithing (giving 10% of earnings to the church). Observations of church services and events, as well as interviews with parents, children, and church leaders, were analyzed in an effort to capture the ways in which mathematical problem solving was related to the social context of tithing. I introduce the direct influence model to describe components of the practice itself and to identify supports and constraints that exist related to children's opportunities to engage in problem solving that varied by participation frequency and mathematical complexity.  相似文献

Youth development does not take place only in institutions or even primarily in institutions. It takes place in the core economy-the economy of family, neighborhood, and community. Major challenges include rebuilding the kind of village it takes to raise a child and enabling a child to be part of that rebuilding. Another challenge is to make sure that any external incentives that are provided to youth are linked to activities that build self-esteem and convey a definition of value that is different from that established by money and market price. This chapter provides an introduction to time banking and to coproduction, approaches to youth development that enable youth to participate as major players, as opposed to recipients, in the reshaping of their lives and communities.  相似文献
