Summary

This article specifies the knowledge and skills needed by human service professionals in order to utilize the potential of information technology for better care delivery. The conceptual foundation proposes an integrative approach that views management, case management and treatment as one integrated process in the human service agencies and which must be reflected in the information system design. A course for graduate students that incorporates this concept was developed in two schools of social work during a three-year period and is presented in details.  相似文献

Abstract

The health care model presented in this paper is a non-linear, systems approach to service delivery. The model was a by-product of three ethnic-specific conferences that were convened to discuss strategies for improving access to care and the quality of services provided to children with special health care needs (CSHCN) from diverse cultural backgrounds. The model evolved as insights regarding the common values and norms held by parents, health care providers, and cultural representatives were shared. Content analysis was used to identify core themes identified by participants. These themes were incorporated into a model that included the following elements: community-based, culturally competent, health-oriented, resiliency-focused, and family-centered care. At the fourth Consensus Conference, selected participants from the previous ethnic specific conferences reviewed the proposed model and deemed it a vehicle for facilitating health care service utilization by culturally and linguistically diverse CSHCN and their families.  相似文献

This article uses the evaluation of a school-aged therapy service for children with disabilities in Western Australia to investigate models of service delivery. The current literature on family-centered practice, multidisciplinary and transdisciplinary approaches, and 4 models of service are reviewed. The models include the life needs model, the relational goal-orientated model of optimal service delivery to children and families, the quality of life model, and the collaborative model of service delivery. Analysis of the data is presented together with a bespoke model of service delivery for children with disabilities, arguing that local contexts benefit from custom-made service design.  相似文献

ABSTRACT

The concept of public and private partnerships raises growing interest and hope from international development agencies, governments, and communities as an alternative strategy in social welfare delivery. Conceptualized as a strategy to build and capitalize on the tangible and intangible assets and resources endogenous to social organizations and communities, individually and collectively, this article embraces an asset-based approach to analyze the potency of such a framework in enhancing maternal health care delivery in developing countries such as Senegal and Mali. These countries face growing retrenchment of the state from social service delivery and scarcity of human and financial resources, which call for innovative approaches and sustainable solutions in addressing pervasive deficits in health service delivery. Drawing from empirical field research conducted in Senegal and Mali, this article examines the trend toward public–private partnerships and its gender impacts and critically analyzes the challenges and opportunities it offers conceptually and practically to address and improve equity in health care delivery, particularly safe motherhood and maternal health care delivery in developing countries.  相似文献

This article explores the dual themes of place and time as influences on health care service delivery to the rural elderly, through a case study of rural Appalachia. Traditional patterns of indigenous health care practice and values are contrasted with more “professional” formal models of health care service delivery that have entered the region. The result has been a clash of health care cultures. The continuing validity of this perspective is appraised in relation to apparent generational differences between the old-old and the young-old in the degree to which contemporary health care practices and values are adopted. It is concluded that traditional and contemporary health care cultures can be reconciled through an expanded vision of health care service delivery premised on: understanding health care within a total community context; redefining the role of the health practitioner; improving education of both the rural elderly and service providers; and enhancing communication in the rural health care environment.  相似文献

AGENCY is a computer simulation model developed to emulate service delivery in a mental health center, but broadly applicable to most human service delivery organizations. Definition of the service providing agency is input to the program from a separate data file, so the program is extremely flexible and general. Initial validation results are positive. The aim of this article is to demonstrate the diversity in delivery of long-term care at the provincial level, within a national legislative framework that provides universal health insurance and public administration. Not all provinces have legislated provision of long-term care, but mandates for provincial long-term care programs typically address the needs of those with chronic health needs and maintain them in the community for as long as possible. Eligibility is based on common criteria of residency, health need, facility, assessment, and consent. The three common components of the service delivery system are institutional care, community-based services, and home-based services; the kinds of services within each component and the mix among them vary from province to province. There are also five common features in provincial service delivery systems: single point of entry, assessment, client classification, case management, and single administration. Throughout the article, examples from different provinces show the varying ways in which these aspects of service delivery have been addressed, and recent innovations have furthered this diversity. A detailed account of quality management systems also shows that while all provinces have adopted a common set of principles, they use a range of methods to pursue quality of care and to promote good practice.  相似文献

Conflicting priorities between the recovery movement among consumers of mental health services and managed behavioral health care planners result in turbulence and ambiguity in the service delivery system. Based upon information from both published and unpublished written sources, areas of strain are described. Australian disability services have undergone a major review. Initiated by the government and now enshrined in new legislation, this review has significant implications for non-government social welfare organisations as major service providers in that it demands a significant service delivery reorientation. This paper describes one demonstration project involving four physically disabled persons, one female and three males, located in one domiciliary unit which signposts this new service direction and identifies it as a consumer-directed and empowering model. This is contrasted with previous institutional and care models of service. It identifies the staff role as important in this client-empowering model of service and raises a range of critical issues in the consumer-staff interface which enhances such empowerment. The paper suggests that where these staffing features and empowerment processes are lacking deinstitutionalisation may well have occurred without consumer empowerment being achieved.  相似文献

This article (1) describes the development of a strengths-based case management (SBCM) model for multiethnic sexual minority youths; (2) provides a profile of participant risks and strengths; and (3) discusses critical considerations for implementation with this population. Participants (N = 162) reported complex risks such as family rejection as well as personal strengths. SBCM is a promising intervention that provides opportunities to bridge the unique gaps in service for this vulnerable population. Strategies to incorporate strengths into program assessments and address issues such as family engagement and service delivery challenges are offered.  相似文献

This cross-sectional study utilized a stress-process model to examine the impact of caregivers' (N = 82) perceptions of their relationship quality with a female family member (i.e., care-recipient) with substance-use or co-occurring substance and mental disorders on caregivers' perceived burden. Regression findings indicate that relationship quality impacts both subjective and objective burden. Higher levels of emotional support given to the care recipient by the caregiver predicted lower levels of caregiver displeasure; whereas higher levels of undermining of the care recipient by the caregiver predicted higher levels of caregiver stigma. Higher levels of emotional support received by the caregiver from the care recipient were associated with lower levels of caregiver displeasure and lower objective burden; higher levels of undermining of the caregiver by the care recipient predicted higher objective burden. Implications for practice and service delivery are presented.  相似文献

ABSTRACT

There are several models for the delivery of etherapy on the Internet. This article presents a case study of one model, the LivePerson website. The model is based on a privately owned infrastructure through which etherapy is practiced by licensed counselors using chat or e-mail for service delivery. A content analysis of the website is presented, including policies, number of consumers, consumer ratings, and therapists’ degrees, licensure, age, sex, race, fees, and languages offered. In addition, the experiences of one LivePerson practitioner are presented to highlight methods, processes, and ethical concerns. This case study of the LivePerson model for service delivery highlights many of the benefits and concerns previously discussed in the literature related to etherapy. In addition, concerns are raised that there may be some conflict of interest when ethical standards of practice are delivered within the context of a profit-making company.  相似文献

Although foster care is often spoken of as a system with many interactive facets, foster care personnel often carry out their specific roles with little understanding or consideration of the ultimate impact of that system. A simulation of public foster care service delivery designed to sensitize participants to these interactions is described. Evaluation of the simulation's accuracy in portrayal of actual service delivery and a study of the kinds of learning demonstrated by participants at three points (pretest, posttest, and follow-up) are reported. Participants found the simulation an attractive training method, indicated a greater appreciation of system-wide foster care problems, and reported changes in foster care thinking and work, but other indicators of learning were inconsistent.  相似文献

This article provides an overview of the implementation and evaluation of Family Support Centers (FSC) in the U.S. Air Force. After describing the FSC model, the quantitative aspect of the evaluation is discussed, including the evaluation design, implementation methods, survey response rates, and procedures for data analysis. A summary of evaluation results also are presented as well as a discussion of administrative and methodological issues encountered in performing the evaluation. The results of the evaluation suggest that the FSC has become an integral part of the base human service delivery system, especially for married members and civilian spouses.  相似文献

The past 50 years or so have witnessed significant changes in the way of conceiving and practising cooperation. Three major shifts are presented here: (1) from the single to the multi-stakeholder cooperative; (2) from internal to extended mutuality in cooperatives and (3) from the historical ‘double nature’ of cooperation to the social economy. It is argued that these shifts testify of the capability of the cooperative system to adapt to such global trends as privatization, deregulation and ‘tertiarization’ that came in the wake of globalization. Moving from the traditional model of the single stakeholder cooperative based on internal homogeneity and on the identification of the notion of member and user, to a new model of multi-stakeholder organization, broadened the scope of cooperatives yet, at the same time, sharpened the socio-economic tension at the inter-cooperative level. Hence, the need, for cooperatives, to strengthen their links with similar nonprofit organizations as partners in the social economy. Our approach is grounded on a Western European interpretation of cooperatives, although the social economy is recently making strides outside Europe, especially in Canada and Latin America.  相似文献

Engagement with globalisation is growing in the field of youth transitions from out of home care. This includes cross national exchange of research, policy and practise, regional advocacy networking and global policy development. Furthering this emerging international child welfare perspective requires extending it to countries in the developing world and building conceptual frameworks which encompass a social ecology of care leaving, including its global dimension, the latter needs to address not only the needs, expectations and rights of care leavers but also the theories of change underpinning service design and delivery. Such a model is presented combining resilience and social capital as personal assets situated within a social ecology of support. To illustrate how this provides a means to help engage with the experience of countries where there appears to be very little information available on care leaving, a small scale South African initiative is considered. SA-YES is a youth mentoring project for young people leaving a variety of out of home placements. Planned as a three-year pilot, initial results are encouraging but require more rigorous evaluation focusing on program process and outcomes, quality of interpersonal relationships and synchronisation with cultural expectations and policy environment.  相似文献

Almost all children in Sweden spend at least some part of their early childhood in organized childcare, primarily in the form of preschools. About 10% of all preschools in Sweden are parent cooperatives, which make the childcare area an anomaly in the Swedish welfare state; no other service area has such a significant proportion of cooperative service providers. The parent cooperatives are initiated by the parents themselves, they are social enterprises created from the bottom-up. Why do parents decide to start a parent cooperative preschool in a welfare state where childcare is considered a social right and a responsibility for the state? This article will explore several different motives for starting parent cooperatives in Sweden, with special attention to the role of the public sector and political decisions. The findings are based on an extensive empiric study performed between the years 2003 and 2007.  相似文献

ABSTRACT

With the recent sociopolitical changes in the Former Soviet Union, significant numbers of older Soviets have arrived in the U.S. with their families. Soviet immigrants who enter the U.S. are no longer automatically considered political refugees, which has limited their entitlement to services. Recent changes in public welfare and immigration policy place the responsibility for care of elderly members solely on the family. While caring for older adults is stressful for any family, new immigrants may be especially burdened since they have limited knowledge of services, few coping resources to provide adequate care, and little experience resolving cultural conflicts with their older parent. By using a practice model designed to evaluate late-life caregiving situations, two case scenarios are presented to examine practice and service delivery issues of older Soviet immigrants and their families.  相似文献

This paper develops a simple model of entrepreneurial enterprises. The analysis differs from traditional work on entrepreneurship by analyzing why entrepreneurial activities are typically conducted in small firms owned by the entrepreneur. I argue that ownership incentives are an advantage of small firms. When the probability of success of an economic activity becomes small, if becomes costly for large firms to commit to strong incentives, and small worker-owned firms emerge. The paper discusses application of the theory to innovation, wild-cat oil exploration, restaurants and retail trade, professional practices, salesmen, and franchising.  相似文献
