Cytochrome P450 testing for better psychiatric care   总被引:2,自引:0,他引:2  
Genotyping for CYP2D6 and CYP2C19 variations is emerging as a potentially useful clinical tool to help mental health professionals prescribe psychiatric medications for their patients. Cytochrome P450 testing uses a blood sample to determine an individual's required dosage of identified drugs that are metabolized by the two enzymes. To provide care for patients and families, nurses should be able to demonstrate identified essential nursing competencies related to genetics and genomics, which include an understanding of cytochrome P450 testing. As patient advocates, nurses are expected to understand how to identify patients most likely to benefit from CYP2D6 and CYP2C19 testing, how to ensure informed consent for such testing, and how to educate patients about testing and test results.  相似文献

The ideas of knowledge management, competence strategy and learning organisation have been dominating debates on organisational behaviour and management recently. This article argues that most contributions to this debate are based on a quite naïve conception of knowledge and competencies. One important problem is the neglect of tradition. Through an analysis of the conventional views of knowledge and competencies and through a rehabilitation of tradition the article shows how tradition can provide us with another conception of knowledge management and related ideas. Two case studies from small Danish manufacturing companies are presented in order to show how tradition functions even in the most modem organisations.  相似文献

Family narratives about the past are an important context for the socialization of emotion, but relations between expression of negative emotion and children's emerging competence are conflicting. In this study, 24 middle‐class two‐parent families narrated a shared negative experience together and we examined the process (initiations and collaborations) and function (the expression and explanation of emotions) of co‐constructed narratives in relation to preadolescents' perceived competencies and self‐esteem. Family narratives in which specific emotions were expressed and explained in a collaborative fashion, especially negative emotion, were positively related to preadolescents' reported competencies and self‐esteem, whereas family narratives that expressed general positive emotion were negatively related to preadolescents' perceived competencies. Implications of family narratives about emotional events, specifically the ways in which families discuss emotion, in relation to preadolescents' self‐development are discussed.  相似文献

Considerable advances have been made in recent years in medical genetics. These advances have led to a better understanding of the inheritance of disease. Social workers involved in schools, child welfare, medical, and other institutional settings often work with clients who have genetically inherited disorders. Consequently, there is a growing need for social workers to increase their knowledge of genetics and genetic counseling. Multiple issues that are unique to child welfare social work practice in rural areas markedly affect workforce recruitment and retention, yet little attention is given to the proficiencies needed to equip emerging social workers for this growing area of the field. Curriculum content is needed that provides students with the opportunity to master the skills needed to thrive as child welfare social workers in rural areas. Using an evidence-based practice critical thinking model as a guide, a systematic review of literature and documents addresses many of the competencies needed to prepare social work students for child welfare practice in rural areas. These competencies are identified. Suggestions for integration into the social work curriculum are offered.  相似文献

There is now a considerable body of knowledge conceptualizing the roles that public relations practitioners enact. A key element in the enactment of those roles is the competencies, or behaviors of those who occupy them, however there are no systematic studies of these competencies. Occupational psychologists who conducted in-depth interviews with 17 ‘top’ communicators in the UK, discovered 10 core competencies or behaviors specific to the senior communication practitioner role in publicly quoted companies, and 10 core competencies for those working in the public sector. These competencies are elucidated and compared and the implications discussed. Avenues for further research are also identified.  相似文献

This paper presents, illustrates and discusses a generic framework for studying knowledge co-elaboration in online epistemic communities ("OECs"). Our approach is characterised by: considering knowledge co-elaboration as a design activity; distinguishing discussion and production spaces in OECs; characterising participation via the notion of role; fine-grained analyses of meaning, content and communicative functions in interactions. On this basis, three key issues for ergonomics research on OECs are discussed and illustrated by results from our previous studies on OSS and Wikipedia. One issue concerns the interrelation between design (task) and regulation. Whereas design task-oriented activity is distributed among participants, we illustrate that OCEs function with specialised emerging roles of group regulation. However, the task-oriented activity also functions at an interpersonal level, as an interplay of knowledge-based discussion with negotiation of competencies. Another issue concerns the foci of activity on the (designed) knowledge object. Based on a generic task model, we illustrate asymmetry and distinctiveness in tasks' foci of participants. The last issue concerns how design-use mediation is ensured by specific forms of mediation roles in OECs. Finally we discuss the degree of generality of our framework and draw some perspectives for extending our framework to other OECs.  相似文献

A hierarchical needs assessment model used to identify the competencies required by communication management practitioners in the New Zealand workplace generated three key findings. First, there was evidence of a perspective of CM as a distinct discipline founded on an agreed body of knowledge, skills and values. Second, despite an identified lack of consensus about required CM competencies in the literature, we found high levels of agreement between practitioners and academics. Third, there was, however, a significant difference in views between these two groups about the importance of the competency managing client reputation. As this was rated as one of the two most important competencies by respondents, there are implications for changing the focus of tertiary curricula to ensure graduates are appropriately prepared for the reality of workplace practice.  相似文献

Currently, batterer intervention programs tend to focus on education and confrontation, with outcomes being less than promising. Limitations of current interventions have encouraged development of alternative treatment forms aimed at ending relationship violence. An emerging trend in the fields of social work and positive psychology is to build on strengths and competencies of offenders. The purpose of this qualitative study was to introduce an alternative position within the treatment field by identifying self-disclosed strengths of men who had been charged with a domestic violence offense and who were being evaluated for a voluntary batterer diversion program. A content analysis revealed competencies useful for helping men be accountable and end violence against their partners through redirecting negative behavior and identifying resources and strengths.  相似文献

The objective of this article is not to present a scientific or systematic study, but to provide an initial framework for designing a training workshop to enhance health practitioners’ (nurses, social workers, physicians, etc.) knowledge regarding substance abuse treatment and to decrease their bias toward substance-abusing women, particularly pregnant women in rural communities. We incorporated the 4 Transdisciplinary Foundations from the Substance Abuse and Mental Health Services Administration Competencies Model, with specific competencies targeted that related to provider bias. After the conference, 52 of the 70 participants completed a questionnaire to self-assess knowledge level and confidence in skill related to substance abuse management. Participant mean scores were statistically significantly higher following the conference than 1 week prior (p < .001) in the area of “gender difference with substance abuse,” moving from an average of 2.6 to 4.5 on a 5-point Likert scale. Our conference was successful in increasing attendees’ knowledge about gender difference and substance abuse among pregnant patients.  相似文献

This investigation yielded data on the perceived competencies of social workers in adolescent practice. On average, survey respondents related that their perceived knowledge/skill levels ranged from moderate to high when working with depressed, sexually abused youths, or those with behavioral issues. However, respondents also perceived their knowledge/skill levels as moderate to poor when adolescent clients presented with: chronic physical conditions including HIV+/AIDS, sexual orientation issues, gang involvement, or were youths of color. Survey respondents credited work experience and conferences as the primary sources of their perceived practice knowledge/skill. Greater knowledge/skill levels in adolescent practice were associated with greater years of practice.  相似文献

Social work education has long emphasized the need to provide competence-based education. However, little work has been done to identify, validate and evaluate competencies in specific fields such as addictions. The purpose of this study was to isolate a set of substance abuse intervention competencies using Motivational Interviewing that could be used to teach and evaluate students. The research proceeded in three stages. First, a modified Delphi approach involving social work practitioners was used to discern a set of positive and dysfunctional skills related to Motivational Interviewing. The primary researcher taught a graduate social work class aimed at teaching students theory and skills related to this model. Students demonstrated these skills with simulated clients through videotaped interviews. Finally, three observers rated the student interviews using this developed instrument. While inter-observer agreement was lower than anticipated, this study does provide early support for the development of competencies related to specific theoretical frameworks.  相似文献

The purpose of this study was twofold: first, to examine the sexual behavior of emerging adult women in relation to their sexual knowledge, sexual attitudes, and perceptions of their parents' sexual attitudes; and second, to discuss the implications of this research in working with young adult women. Three hundred and sixty-four college-age women were administered the Sexual Experiences Inventory, Miller-Fisk Sexual Knowledge Questionnaire, and the Hendrick Sexual Attitude Scale. Ninety-six percent of the sample reported primarily heterosexual behavior. Sexual knowledge of participants was not related to their reports of sexual behavior. The sexual attitudes of college-age women and their perceptions of their parents' attitudes toward sexual behavior were found to relate positively to participant sexual permissiveness and sexual practice. The implications of these findings were discussed in relation to working with families and providing effective interventions to reduce unsafe sexual practices among emerging adult women.  相似文献

In this paper we examine new genetics professionals’ accounts of the social context of their work. We analyse accounts given in interview by an ‘elite’ group of scientists and clinicians. Drawing on the work of Gilbert and Mulkay (1984), we consider interviewees’ discourse about knowledge, exploring the way in which they separate science from society through the use of what we have called the ‘micro/macro split’. We then go on to consider the reasons for such a discursive boundary, exploring the interviewees’ wider discourse about expertise and responsibility for the social implications of the new genetics. We argue that interviewees’ discursive boundaries allow them to appeal variously to their objectivity, to dismiss bad science and to characterize the public as ignorant. However, these discursive boundaries are permeable and flexible, and are employed to support the new genetics professionals’ role in guiding education and government policy, whilst at the same time deflecting ultimate responsibility for the use of knowledge on to an abstract and amorphous society. Responsibility is flexibly embraced and abrogated. These flexible discursive boundaries thus promote rather than challenge the cognitive authority of new genetics professionals as they engage in debates about the social implications of their work. We end by challenging the replication of these discursive boundaries, noting some of the implications of such a critique for evaluation of the new genetics.  相似文献

The ability to compare the genetics of aging in multiple model organisms is a powerful tool to dissect the biology of aging. Although the fruit fly has only about half again as many genes as the nematode worm, its tissue organization is much more complex. The emerging importance of tissue specificity and endocrine signaling in aging makes the fly a natural choice for many researchers interested in unraveling the next level of complexity of the aging process. This Perspective provides an overview of research related to aging that was described at the annual Drosophila conference held in Chicago in March 2003.  相似文献

The assumption that differences in Higher Education (HE) systems can be related to Varieties of Capitalism (VoC) is the underlying hypothesis of this article. Drawing on the example of skills and competencies of HE graduates in the UK and Germany, complementarities between the educational and economic system are analysed. First, hypotheses about differences in competencies are deduced on a theoretical basis from the VoC-approach. Second, the analysis of data from an international comparative survey (REFLEX) confirms that these differences indeed can be found empirically: German graduates put more emphasis on specific skills, while UK-graduates stress more general competencies. Additionally, the fit between study and later job is closer in Germany. These results show, first, that the complementarities postulated by the VoC-approach can also be found for the field of HE. Second, the recourse on the relationship between economic and educational system can stimulate research in comparative and international education. Third, these findings are also relevant for the current HE reform discourse.  相似文献
