We asked Jerry Rhodes, former top executive at CRC (now Acadia) and a leader in opioid treatment program management, what he thinks of methadone as a medication to be used in primary care to treat opioid use disorder (OUD), as some people — including former Office of National Drug Control Policy Director Michael Botticelli — recommended last year (see ADAW, July 16, 2018). “I take issue with that,” said Rhodes. “Methadone is a dangerous drug in an unregulated environment,” he told ADAW. Buprenorphine is prescribed this way, but “buprenorphine is a relatively safe drug, and methadone isn't,” he said. A veteran of many battles over methadone, including the near‐elimination of opioid treatment programs, Rhodes told ADAW that “you don't give unfettered access to methadone” to patients with OUD. “Be careful what you wish for” is his advice. This has the potential to cause harm, he said. “Only people who don't understand the history of its utilization would recommend this.”  相似文献

Although we often believe that nature stands apart from social life, our experience of nature is profoundly social. This paper unpacks this paradox in order to (1) explain sociology's neglect of the environment and (2) introduce the articles in this special issue on “the sociology of nature.” I argue that sociology's disinterest in the biophysical world is a legacy of its classical concern with tracing society's “Great Transformation” from gemeinschaft to gesellschaft: while early anthropologists studied “primitive” societies that allegedly had not yet completed “the passage from nature to culture” (Lévi‐Strauss 1963 : 99), pioneering sociologists presumed that industrialization and urbanization liberated “modern” society from nature and therefore focused their attention on “urbanism as a way of life” (Wirth 1938 ). As exemplified by the articles in this symposium, environmental sociology critiques the nature‐culture and town‐country dualisms. One of environmental sociology's core contributions has been demonstrating that nature is just as much a social construction as race or gender; however, its more profound challenge to the discipline lies in its refutation of the sociological axiom that social facts can be explained purely through reference to other social facts. “Environmental facts” are a constitutive feature of social life, not merely an effect of it.  相似文献

ABSTRACT

A brief intervention tailored for marijuana users in the contemplation stage of change is presented. This “no pressure to change” approach employs principles of motivational enhancement. Empirical support for brief interventions is reviewed. Rollnick and Miller's motivational interviewing principles are outlined, with particular emphasis on stage-specific tasks faced by individuals at early stages of readiness for change based on Prochaska and DiClemente' s Transtheoretical Model of Change. The history and utility of the “check-up” as a viable intervention for individuals in earlier stages of change is also described. The Marijuana Check-Up makes use of a personalized diagrammatic feedback report on the individual's personal use, providing a focus for the one session interview. Clinical challenges and therapeutic response are illustrated with typical case vignettes germane to the Marijuana Check-Up.  相似文献

A conference focusing on “value” — money well spent — in substance use disorder (SUD) treatment featured payment and reimbursement, and, in a refreshing twist, researchers instead of administrators and payers. Led by CHERISH (the Center for Health Economics of Treatment Interventions for Substance Use Disorder, HCV, and HIV, a collaboration of Cornell Weill Medicine, Boston Medical Center, the University of Miami Miller School of Medicine and the University of Pennsylvania, funded by the National Institute on Drug Abuse), the conference, held May 31 at the Leonard David Institute of Health at the University of Pennsylvania, featured speakers from the SUD treatment community who espouse medications. Topics and speakers included: ways to eliminate low‐value care (Joshua Sharfstein, M.D.), what value means (Colleen Barry, Ph.D., Brendan Saloner, Ph.D., and overcoming barriers to effective treatment (Michael Botticelli, Alexis Horan). It did give attendees the opportunity to network and interact directly, as researchers and policymakers, on how to overcome barriers to SUD treatment in communities. The bottom line: What works should be covered; what doesn't shouldn't.  相似文献

This paper explores the process of resettlement among recent refugees in Perth, Western Australia. We propose four refugee resettlement styles created through the interaction of a number of factors. These factors can be clustered as: (1) the social features of refugees (their human, social, and cultural capital), and (2) the host society's responses to refugee settlers (Australia's resettlement policy and services and the broader influence of the host society's responses to refugees). We propose that refugees approach their resettlement in predominantly active (“achievers” and “consumers”) or passive (“endurers” and “victims”) ways and that these are differentially successful strategies. Medicalization of the refugee experience in Australia is a factor that may influence refugees to adopt a passive “victim role”, so we propose that a greater emphasis during early resettlement should be placed on refugees' own culturally defined priorities such as employment and stable housing. The argument developed in this paper is supported by data from two qualitative research projects conducted in Western Australia. The fieldwork consisted of interviews, focus groups, and participant observation, and targeted refugees from the former Yugoslavia and the Horn of Africa who arrived in Australia during the 1990s and 2000s, as well as resettlement service providers.  相似文献

The attempts to clarify (purify) the conceptual foundations of family therapy by means of “epistemology” have bred excitement, boredom, irritation and confusion. In the belief that at least the confusion can be alleviated, the present paper is offered as a study guide and something of a Rosetta Stone for translating the work of Gregory Bateson and Humberto R. Maturana. The paper demonstrates that Maturana's work is highly compatible with that of Bateson. In addition, several major points of contrast are argued: (1) Maturana's concept of structure determinism is an explicit ontological claim which directly implies an epistemology, whereas Bateson delineated an epistemology, but never clearly developed a corresponding ontology; (2) structure determinism is a more general concept than Bateson's concept of “mind” (i.e., cybernetic epistemology); (3) structure determinism deletes the remnants of objectivity from Bateson's theory (i.e., “the difference that makes a difference”); and (4) Maturana's concept of instructive interaction is a more general, nonsystemic version of what Bateson meant when he used the term “epistemological error.” Finally, it is claimed that the emphasis on epistemology has distracted proponents and detractors alike from the essential message of Bateson and Maturana: social systems and all human endeavor must be understood in light of our existence as biological entities that are coupled to a medium. The biological ontology implicit in Bateson's writings and explicitly delineated in Maturana's may (at long last) provide a sound foundation for the social and behavioral sciences.  相似文献

The paper analyses Sarajevo's music movement of New Primitives and its “poetics of the local” as a struggle against the cultural hypocrisy of Yugoslavia's “new socialist culture” and its privileging of “external‐cosmopolitan” as apotheosis of cultured refinement and sophistication while denigrating “local‐parochial” as epitome of uncultured primitiveness. I argue that the movement's praxis is best understood as a call to reject externally‐imposed frames of reference as the basis for self‐understanding, and to embrace a socio‐cultural awareness that the only way to be in the world is to be authentically “primitive”– i.e. to exist as a distinct and autochthon socio‐cultural self.  相似文献

When “History” is called to represent silence, its metaphysical position is symptomatically felt. Tracing what Fasolt calls “the historian's revolt”, this paper identifies the political impetus behind it as the symptom dictating Foucault's own silences/silencings (regarding Derrida's intervention in his History of Madness). In naming such a symptom/silence – in taking “Derrida's position” – this paper performs its own violence/decision by, both “justifying,” and betraying, this position; by installing itself in, instead of “above,” this curious “debate”. “The last refuge of the scoundrel” appears then as the reflective exteriority of a political antagonism that's based on a metaphysical difference with regards to the legitimate “seat” of authority (in Fasolt, an antagonism between the historian and the Catholic Church). Finally, this trajectory is installed within a wider – metaphysical and historical – context, where Hegel's famous saying, that the University is the Protestant's Church, might yet echo that distant metaphysical decision – still looming, like a “genealogical specter,” over Academia and its Social Sciences.  相似文献

The Russian-American novelist Gary Shteyngart has frequently been called a “Gogolian” writer, usually in an attempt to explain the pedigree of his grotesque humour. This article focuses on Shteyngart’s story “Shylock on the Neva” (2002), which is a modern-day rewriting of Gogol'’s tale “Portret” [“The Portrait”]. A close analysis of Shteyngart’s text and comparison to its Gogolian model reveals a complex relation that is not necessarily centered on Gogol'’s humour. In his rewriting of “The Portrait,” Shteyngart emphasizes the inherent venality and vulgarity of Gogol'’s characters, who turn into grotesque caricatures of their prototypes. In doing so, he seems to “Gogolize” Gogol'’s tale by adding some of the absurd humour that critics have found to be lacking in “The Portrait.” By making a painting the focus of their stories, both Gogol' and Shteyngart engage in a self-reflective comment about art and the role of the creative artist. Similar to the clichéd hack-paintings of Gogol'’s painter Chartkov, artistic creation has been reduced in “Shylock on the Neva” to the production of postmodern simulacra based on stereotypes and cultural myths.  相似文献

Hochschild described the “stalled revolution” in the late 1980s: women made great gains in labor force opportunities, particularly in stereotypically “masculine” fields, yet men did not move comparably into “feminine” roles. This article examines the current “stalls” in the gender equality movement regarding gendered experiences at work and home, including occupations, the gender wage gap, career trajectories, and the division of household labor. This article also discusses efforts to “unstall” the gender revolution. Pop culture solutions on the individual‐level and academic research on structural/cultural barriers often focus on women's access to historically “masculine” roles (e.g. representation in STEM fields). There is far less emphasis on men's involvement in historically “feminine” roles. Gender scholars examine hegemonic masculinity as the narrowly constrained expectations for men's “appropriate” behavior. While efforts to “unstall” the gender revolution focus largely on expanding women's opportunities, this article addresses why the gender revolution will remain incomplete and “stalled” without redefining hegemonic masculinity. Cross‐national research demonstrates that changing views of masculinity are critical for greater gender equality at work and home.  相似文献

AimsTo explore the unique aspects of the elder self-neglect phenomenon and to achieve phenomenological understanding of self-neglect through the eyes of self-neglecting elders.MethodA qualitative study based on a sample of 16 self-neglecting elders. Data collection was performed through in-depth semi-structured interviews, followed by content analysis.FindingsFour major themes emerged from the older participants: “I was unlucky:” a life course of suffering; “That's the way it is:” self-neglect as a routine of life; “They tell me that I'm disabled:” old age as exposing situations of self-neglect; “My empire:” how do I perceive my old age.ConclusionsSelf-neglect is not necessarily an issue of old age, but is related to the person's life history. Self-neglect as a way of life accompanied the participants into old age, but it was not originated or created there. The overall message of the self-neglecting elders was to see them as human beings and not as old neglected people; not to label them as an “age syndrome” but to perceive them in a holistic and humanistic manner.  相似文献

Research (by a self-styled participant observer) into two fashions of persons—nurses and punks—lends unexpected significance to the ritual “frame” as this appears in Erving Goffman's thought. A new concept that was only implicit in Goffman's ritual frame is demanded by the research experiences. This is “ritual power.” Ritual power, especially when it is strong, is like “presence” or “possession”, and it may well exert a major claim on interactants' consciousnesses (whether the interactants are displaying it or appreciating it). Of course, it must follow that verbal forms which try to define ritual power will do so the more powerfully the more they arrest the reader's attention. So it may not be a good idea to use sociological rhetoric of any sort to suggest that punks and nurses are exemplary referents of “ritual power”, but this last possibility is only latent in what follows.  相似文献

This article explores the connections between Vygotsky's psychology and Marxism, arguing that his was a “Marxist psychology” in its “historical foundation”: a specific conception of history. This conception of history is evident in Vygotsky's analysis and diagnosis of the crisis in psychology. The creation of a Marxist, general psychology was the historical task that was defined by this crisis, and his developmental psychology was the historical project of such a psychology. In his practice of the methodology of this general psychology, Vygotsky recounted “child history”: the history of the genesis of mind. The conception of history evident in Crisis throws new light on Vygotsky's texts on child development: They tell a history of the objective tendencies of consciousness, of the dialectical processes of sublation, and of self-mastery. As Vygotsky interpreted the higher mental functions, they are manifestations of the child's ability to master himself or herself as a consequence of the “social moment” of consciousness. In fostering these functions, one shaped a human consciousness capable of free and deliberate choice.  相似文献

This article aims to explore how the role of education as an aide in the process of “European” identity formation is being articulated in the European Union's (EU) policy of “The European dimension of education”. After having located the EU's views on education in the context of the neo‐liberal discourse on economic globalization, the article goes on to trace EU discussions of the European dimension of education historically. Subsequently, it deliberates on the understanding of European culture and identity which the European dimension of education endeavours to advance. Here a critique is developed of the policy's ethno‐culturalism, thereby excluding delineation of a collective identity in the EU. Basing itself on a notion of cultural identity which, implicitly, includes only those who fit certain versions of European historical “roots” and cultural “heritage”, the policy, it is argued, impedes a discussion of how a trans‐ethnic identity formation could be created in today's EU. Towards the end of the article, a scrutiny of the European dimension's perception of the so‐called “language diversity” in the EU seeks to illustrate this issue further.  相似文献

Starting from the recently translated biography of Max Weber by Joachim Radkau, this essay re‐evaluates Weber's “science of reality” in relation to his personality, the cultural context of the early twentieth century, and the position of Weber's thought in the sociological canon. The argument progresses through sequentially enlarged analyses, which propose that Weber's general style of thinking is a type of dissonant composition that places emphasis on the many relationships between cultural reality and the concepts derived from it, and not as much on its content. The logic of such a compositional approach to reality is based on similar principles found in sound and music, which Weber in fact uses in a more latent as well as more active form, to pursue his aim of a style of thinking as “aesthetics of dissonance”. The latter is a sort of “methodological wedge” that pries open the many layers of reality. As such, Weber's “science of reality” is an early “classical” example of a recent and much needed call for a social science as the “art of listening”.  相似文献

Merton has made an important distinction between the “history” any “systematics” of sociological theory, and outlined the valuable functions of the former. Most histories of sociology, however, have been “presetist” or “Whiggish” in perspective; we propose an “historicist” alternative. Within this perspective, Durkheim's response to Spencer is analyzed in three areas: (1) the relation between “individual” and “society;” (2) evolution and social change; and (3) the scope and method of sociology. In these areas, Durkheim's critical style reveals a repetitive theme which is termed “inversion.” The essay concludes by re-affirming Merton's distinction and urging that the “historicist” perspective is the most valid and useful approach to the history of sociology.  相似文献

ABSTRACT

The authors' reply to Martindale and Gould's critique of Pickar's article (2007a, this issue), “Countertransference Bias in the Child Custody Evaluator.” Martindale and Gould's objections to considering an evaluator's countertransference reactions as a potential source of bias are addressed by focusing on these four areas: (1) Whether it is useful to introduce the term “countertransference bias” to identify a potential source of distortion in custody evaluations; (2) How the empirical literature on countertransference in the context of psychotherapy might apply to the child custody evaluation process; (3) Clarification of areas of agreement and disagreement with Martindale and Gould, and (4) The importance of evaluator attempts at “debiasing.” We conclude that gaining awareness of countertransference reactions can only enhance the evaluator's ability to provide the most objective custody evaluation possible.  相似文献
