Summary In this article a ‘socio-historical’ method of analysisand intervention in social work will be developed, illustratedby an example drawn from social work with families, the programknown in France as Action Educative en Milieu Ouvert. E-mail: d.tanner{at}coventry.ac.uk Summary At the heart of a number of New Labour health and social policyinitiatives are stated concerns with supporting independence,preventing deterioration and extending access to services. Thisarticle examines the potential impact of these initiatives,including Prevention Grants (Department of Health, 2000b), theNational Service Framework for Older People (Department of Health,2001a), the Fair Access to Care Services Guidance (Departmentof Health, 2002a), and the Single Assessment Process (Departmentof Health, 2002b), on older people with ‘low level’needs who have increasingly been excluded from services targetedat those in high risk categories. Key standpoints for this analysisare research findings indicating what older people themselveswant from preventive services. Central messages, consistentwith those from other service user ‘groups’, arethe need for support services that enable older people to continueto exercise choice and control over their lives. New preventivemeasures are evaluated in the light of these findings and foundwanting. It is argued that recent policy initiatives are likelyto do little to widen access to support services for those withlow intensity needs, and, moreover, that the processes involvedin identifying those at future high risk will detract from ratherthan enhance citizenship. A more diffuse approach to preventionis advocated, based on mutuality, community development, andcapacity-building.  相似文献

Summary Increasing calls for early diagnosis of dementia look set toimpact on social services. This article explores some of theissues, drawing on a series of multi-professional dementia workshopsheld in the UK. The workshops identified a series of questionsthat need to be addressed by social services in the areas ofthe drivers for early assessment, ensuing expectations, problemsand advantages in involving social services, and the benefitsof early diagnosis for individuals and their families. The articlediscusses the implications for social services in light of thedata and notes a possible move to specialist services, withinor without the social services sector. Tensions between currentresource shortages and anticipated demands for services areidentified. The difficulty of meeting low level needs whileservices are targeted on those with high levels of need presentsa further challenge to service delivery.  相似文献

Summary Theories within the social and psychological sciences continueto shape social work practice. However, recent debates withinthese sciences on the shortcomings of ‘social constructionism’,and the attempt to overcome them through a critical realistposition, have so far failed to capture social work's attention.This paper provides a critical overview of these theoreticaldebates, highlighting the shortcomings of social constructionismfor social work. The critical realist challenge to social constructionism,as articulated by the British philosopher Roy Bhaskar, is consideredand its relevance to social work formulated in terms of a returnto ‘depth’ in practice.  相似文献

Summary The Department of Health's Health of the Nation aims at a substantialreduction in suicide; this is a ‘vote of confidence’in the effectiveness of the community psychiatric and healthservices. However, it will be shown that a number of socioeconomicchanges have occurred which may lead to increased suicide; forexample British male suicide rose in the 1980s, significantlyassociated with unemployment. Though female suicide fell substantiallyover the period, across the regions suicide rates for both menand women were worse amongst the ‘work age’ bands,16–44 years, than the older groups. In respect to therelatively poorer rates amongst younger women, this may be linkedto unemployment and their unfulfilled job expectations. If thesuicide targets are not met, it is feared that the ‘failure’will laid at the door of the community and health services.  相似文献

This qualitativestudy, based in social services departments in London, usedthe focus group method to explore the provision of supervisionby team managers in a very pressurized work environment. Thestudy highlights the difficulty of protecting adequate supervisiontime but shows how supervisors can use their skills to conductcase related discussion concerned with developing professionalskills. ‘Inquisitorial’ and ‘empathic-containing’functions are identified and an approach is proposed for combiningthem in training courses for child protection supervisors. Thepaper recommends that supervision training needs to be expanded,to be more precisely targeted, and that outcomes need to berigorously and appropriately assessed.  相似文献

There is evidence thatthe risk of contracting Hepatitis C (HCV) is greater than thatof HIV. Despite repeated warnings from a variety of sourcesand thousands of new infections among drug users each year andrising incidents of ‘'crack’ injecting, successivegovernments have failed to address a public health emergencyof immense proportions - the HCV epidemic. Summary For so long in search of a common base, there are early signsthat social work's intellectual outlook is fragmenting. Theunity that was once sought in both theory and practice as wellas policy and organziation is being abandoned. A child of modernity,social work now finds itself in a postmodern world, uncertainwhether or not there are any deep and unwavering principleswhich define the essence of its character and hold it togertheras a cohernt enterprise. The article outlines some of the characteristics of modernityand postmodernity, relating them to the rise of social work,its formation within modern society, and its current conditionin what many observers believe is a postmodern world. E-mail: W.J.Forsythe{at}exeter.ac.uk Summary In this article we argue that during the Victorian era certainethical foundations of best practice were identified and werecentral to the work of particular individuals. We also arguethat there was a strongly discriminatory moralistic basis tosocial policy and mainstream charitable intervention that militatedagainst these ethical foundations. We suggest that this contradictionis replicated in New Labour's ‘Third Way’ and thatwe need to heed the tradition of social inclusion espoused bysome of the Victorian practitioners discussed if we genuinelymean to put into practice the ideas of social worth and communityespoused by New Labour rather than return to the Victorian distinctionbetween the ‘respectable’ or ‘deserving’poor and the ‘pauper’.  相似文献

E-mail: paul.garrett{at}nottingham.ac.uk Summary Local authorities, in England, implemented the Framework forthe Assessment of Children in Need and their Families in April2001. The Framework is the first ‘official’ standardassessment model intended for use in the initial assessmentof all ‘children in need’ under Part 111, section17 of the 1989 Children Act. This new ‘conceptual map’needs to be understood in terms of previous policy documentsand earlier technologies of intervention related to child protectionand, more broadly, child welfare. However, it also needs tobe more expansively perceived, fixed and located as it relatesto other elements in New Labour's political ‘project’.The Framework's preoccupation with an ecological approach toassessments and with questionnaires and scales are likely tohave major implications for social work practice and for micro-engagementswith children and families.  相似文献

E-mail: spollack{at}wlu.ca Summary An anti-oppressive practice (AOP) framework emphasizes issuesof power and oppression within the provision of social workservices themselves as well as within the lives of clients whohave been marginalized and oppressed (Preston-Shoot, 1995; Garciaand Melendez, 1997). AOP seeks to deindividualize clients’problems in order to see them within the wider social contextof their lives. In addition, this framework attempts to moveaway from an ‘expert’ model of service deliverytowards one that is more inclusive of clients’ experiencesand that incorporates a recognition of coping and resistenceto oppression. In this paper I discuss how an AOP frameworkcan contribute to our understanding of and interventions withwomen in prison. In particular, I will focus upon how we canbetter understand and respond to women’s mental healthissues and the experience of imprisonment. Summary There are still a number of problems surrounding the relationshipbetween theories of social work practice and that practice itself.This paper examines the factors underlying those problems andemphasises their roots in the failure to examine the diversityof social work theories and practices. We refer to one attemptto resolve the problems—the distinction between ‘practicetheory’ and ‘theory of practice’—pointout some difficulties with it and suggest an alternative, three-folddistinction within theorising: between (a) materialist socialtheory, (b) strategic practice theory and (c) working concepts. As an example of how those distinctions can be used, we thenbriefly discuss the work of Oskar Negt and its introductioninto the Dutch welfare context, as it was the issues raisedby that which stimulated the ideas in this paper. We concludeby arguing that only this kind of perspective on theorisingcan produce ideas which are of real use to progressive socialwork practitioners.  相似文献

Summary This article follows Gilbert Smith in questioning the usefulnessof some traditional ways of regarding need, which rely mainlyon surveying potential client populations in order to decidewhat provision a social services department ought to make. Itdescribes a small study which examined in detail what socialworkers meant when they said that certain families ‘needed’a Family Centre, and which also looked at the subsequent historyof the department's contact with those families in the absenceof such a resource. It is, therefore, based on looking at whathappened to families when the Centre was not provided, and onanalysing social workers' perceptions of the need they encountered. It is argued that, despite its limitations, such an approachcan give useful indications, not only of what services shouldbe provided but as to the specific way they should be offered.In view of the role played by social workers in assessment for,and allocation of, services, it is suggested that a clearerunderstanding of their perceptions is essential to achieve effectiveservice utilization.  相似文献

Summary A sample of 120 mental health service users in a Northern Townwas interviewed about satisfactions and expressed needs in arange of life domains. Most of the 120 users were male, in theolder age ranges, and not in current partnerships. Most hadservere and long-term conditions. Living situations includedhospital wards, hostels, a group home, living with family orfriends, and living alone. The domains of accommodation andemployment are discussed in this paper. The majority of peopleexpressed themselves as ‘satisfied’ with their accommodation,although a sizeable monority were not satisfied. Lack of choiceof alternatives emerged as an underlying feature of ‘expressedsatisfaction’. A sizeable group expressed the need forhelp in obtaining employment, particularly those living in thecommunity. It is argued that for consumer choice to have a realisticmeaning, people with severe mental illnesses need the experienceof choice from more than one alternative.  相似文献

E-mail: paul.garrett{at}nottingham.ac.uk Summary The heavily promoted Looking After Children: Good Parenting,Good Outcomes (LAC) project continues to exert a powerful influenceon social work in the UK and elsewhere. Despite the repeatedassertions that the documentation associated with LAC is embeddedin ‘objective’ research, this not sustainable onaccount of the failure of the researchers to interrogate theirown normative assumptions and judgements. Specific criticismscan also be levelled at the ‘community study’ connectedto the evolution of the Action and Assessment Records (AARs)which are the key part of the scheme. The system should alsobe viewed in the context of wider pre-occupations about ‘outcomes’.In addition, the project needs to be politically situated andthe researchers' relationship with the Department of Healthshould be more fully explored. In conclusion, it is suggestedthat the LAC experience contains important ‘messages’for the future of social work research.  相似文献

Summary Is there real sharing of decision-making responsibility withinsocial service agencies? What hope is there for participationby the consumer of services if the agency worker nearest andmost relevant to him has little voice or leverage within theorganization? This paper examines the notion of worker participationwithin the context of the social services department and theprobation service, noting the imperfections of the ‘publicmandate’ behind the authority structure of these organizations.Above all it looks for continuity between the values socialservice agencies publicly represent and the values in theirday-to-day dealings with their own staff and clients. Currenttrade union proposals for worker representation at committeelevel are considered, and Dutch experience in extending theworkers' council from the industrial to the social welfare sphereis presented as one alternative model.  相似文献

The paperseeks, therefore, to situate the LAC system in a social andpolitical context and highlights how it also relates to thedominant operational modalities of social work interventionin the late 1990s. In addition, the paper explores how ‘troublesome’children and ideas centred on ‘parenting’ featurein the LAC discourse. Summary This paper reports interim and largely positive results fromthe continuing evaluation of ‘Straight Thinking on Probation’,a substantial intensive probation programme in Mid-Glamorganbased on the work of Robert Ross et al. (1988) in Canada. Thepotential effectiveness of this type of programme is then discussedin relation to the influential model of the probation service'srole advanced by Bottoms and McWilliams in their 1979 ‘non-treatmentparadigm’. Are such programmes ‘treatment’,based on a model of offender pathology and therefore in conflictwith the paradigm, or are they ‘help’, based ona model of empowerment? An attempt to answer this question alsorequires consideration of how far the ‘non-treatment paradigm’was itself a response to perceived ineffectiveness requiringrevision in the light of new evidence. It is argued that suchrevision need not threaten the underlying moral philosophy ofthe paradigm.  相似文献
