The main impetus to the development of information about major industrial hazards in the European Community comes from the so-called Seveso Directive, which defines an information network and requires the generation and transmission of information as the basis for accident prevention and risk management. This important policy development, which calls for the formal identification and analysis of major hazards and the communication of risk information to members of the public, presents new opportunities and challenges to risk analysis and research in Europe. This paper briefly reviews the accidents that gave rise to the Directive and shaped its content, and then summarizes its requirements. The status of its implementation in the EC Member States is discussed, with special emphasis given to the comparison of safety analysis practices, the Major Accident Reporting System (MARS), and risk communication. Some new research directions stimulated by the Directive are identified.  相似文献

Supply chain risk management (SCRM) is a nascent area emerging from a growing appreciation for supply chain risk by practitioners and by researchers. However, there is diverse perception of research in supply chain risk because these researchers have approached this area from different domains. This paper presents our study of this diversity from the perspectives of operations and supply chain management scholars: First, we reviewed the researchers' output, i.e., the recent research literature. Next, we surveyed two focus groups (members of Supply Chain Thought Leaders and International SCRM groups) with open‐ended questions. Finally, we surveyed operations and supply chain management researchers during the 2009 INFORMS meeting in San Diego. Our findings characterize the diversity in terms of three “gaps”: a definition gap in how researchers define SCRM, a process gap in terms of inadequate coverage of response to risk incidents, and a methodology gap in terms of inadequate use of empirical methods. We also list ways to close these gaps as suggested by the researchers.  相似文献

Risk-Based Ranking of Dominant Contributors to Maritime Pollution Events   总被引:2,自引:0,他引:2  
This report describes a conceptual approach for identifying dominant contributors to risk from maritime shipping of hazardous materials. Maritime transportation accidents are relatively common occurrences compared to more frequently analyzed contributors to public risk. Yet research on maritime safety and pollution incidents has not been guided by a systematic, risk-based approach. Maritime shipping accidents can be analyzed using event trees to group the accidents into "bins," or groups, of similar characteristics such as type of cargo, location of accident (e.g., harbor, inland waterway), type of accident (e.g., fire, collision, grounding), and size of release. The importance of specific types of events to each accident bin can be quantified. Then the overall importance of accident events to risk can be estimated by weighting the events' individual bin importance measures by the risk associated with each accident bin.  相似文献

In 1982 the Council of the European Communities issued a Directive on the Major Accident Hazards of Certain Industrial Activities, commonly known as the Seveso Directive. Besides other requirements, the Directive specifies that members of the public liable to be affected by a major accident from certain industrial activities must be informed of safety measures and how they should behave in the event of an accident. The Directive was subsequently amended in an extensive way. Upon the transposition of the Seveso Directive and its amendments into national legislations, the 12 European Member States are faced with the task of "appropriately and effectively" informing the public about major accident hazards. Delays are observed throughout the community, both in transposing the Directives and implementing public information. A pilot study on "Risk Information Needs" was commissioned and carried out at two Seveso sites in Italy in order to begin to resolve some of the problems involved in the provision of public information and to suggest appropriate guidelines. One hundred subjects answered an experimental protocol designed to assess their "information needs." This article presents the theoretical background, the pragmatic aims, and the design of the study. It also summarizes its most relevant findings and suggests some recommendations for the provision of information to the public.  相似文献

We take a novel approach to analyzing hazardous materials transportation risk in this research. Previous studies analyzed this risk from an operations research (OR) or quantitative risk assessment (QRA) perspective by minimizing or calculating risk along a transport route. Further, even though the majority of incidents occur when containers are unloaded, the research has not focused on transportation-related activities, including container loading and unloading. In this work, we developed a decision model of a hazardous materials release during unloading using actual data and an exploratory data modeling approach. Previous studies have had a theoretical perspective in terms of identifying and advancing the key variables related to this risk, and there has not been a focus on probability and statistics-based approaches for doing this. Our decision model empirically identifies the critical variables using an exploratory methodology for a large, highly categorical database involving latent class analysis (LCA), loglinear modeling, and Bayesian networking. Our model identified the most influential variables and countermeasures for two consequences of a hazmat incident, dollar loss and release quantity , and is one of the first models to do this. The most influential variables were found to be related to the failure of the container. In addition to analyzing hazmat risk, our methodology can be used to develop data-driven models for strategic decision making in other domains involving risk.  相似文献

Previous studies of risk perception have typically focused on the mean judgments of a group of people regarding the riskiness (or safety) of a diverse set of hazardous activities, substances, and technologies. This paper reports the results of two studies that take a different path. Study 1 investigated whether models within a single technological domain were similar to previous models based on group means and diverse hazards. Study 2 created a group taxonomy of perceived risk for only one technological domain, railroads, and examined whether the structure of that taxonomy corresponded with taxonomies derived from prior studies of diverse hazards. Results from Study 1 indicated that the importance of various risk characteristics in determining perceived risk differed across individuals and across hazards, but not so much as to invalidate the results of earlier studies based on group means and diverse hazards. In Study 2, the detailed analysis of railroad hazards produced a structure that had both important similarities to, and dissimilarities from, the structure obtained in prior research with diverse hazard domains. The data also indicated that railroad hazards are really quite diverse, with some approaching nuclear reactors in their perceived seriousness. These results suggest that information about the diversity of perceptions within a single domain of hazards could provide valuable input to risk-management decisions.  相似文献

Microbial food safety has been the focus of research across various disciplines within the risk analysis community. Natural scientists involved in food microbiology and related disciplines work on the identification of health hazards, and the detection of pathogenic microorganisms. To perform risk assessment, research activities are increasingly focused on the quantification of microbial contamination of food products at various stages in the food chain, and modeling the impact of this contamination on human health. Social scientists conduct research into how consumers perceive food risks, and how best to develop effective risk communication with consumers in order to improve public health through improved food handling practices. The two approaches converge at the end of the food chain, where the activities regarding food preparation and food consumption are considered. Both natural and social sciences may benefit from input and expertise from the perspective of the alternative discipline, although, to date, the integration of social and natural sciences has been somewhat limited. This article therefore explores the potential of a transdisciplinary approach to food risk analysis in terms of delivering additional improvements to public health. Developing knowledge arising from research in both the natural and social sciences, we present a novel framework involving the integration of the two approaches that might provide the most effective way to improve the consumer health associated with food-borne illness.  相似文献

The safe navigation of ships, especially in narrow shipping waterways, is of the utmost concern to researchers as well as maritime authorities. Many researchers and practitioners have conducted studies on risk assessment for maritime transportation and have proposed risk reduction/control measures accordingly. This article provides a detailed review and assessment of various quantitative risk assessment models for maritime waterways. Eighty-seven academic papers and/or project reports are summarized and discussed. The review then proceeds to analyze the frequency and consequence estimation models separately. It should be pointed out that we further summarize the advantages and disadvantages of frequency estimation models and provide recommendations for their application. From the overview, we find that the quantification of the impact of human error is of great importance and should be considered in future studies. Possible solutions are also proposed in the discussions.  相似文献

Engaging Expert Peers in the Development of Risk Assessments   总被引:1,自引:1,他引:0  
The participation of external technical experts in the development of risk assessment documents and methodologies has expanded and evolved in recent years. Many government agencies and authoritative organizations have experts peer review important works to evaluate the scientific and technical defensibility and judge the strength of the assumptions and conclusions (OMB, 2004; IPCS, 2005; IARC, 2006; Health Canada, 2007; U.S. EPA, 2006). Expert advice has been solicited in other forms of peer involvement, including peer consultation in, for example, the U.S. EPA's Voluntary Children's Chemical Evaluation Program (VCCEP). This article discusses how the principles and practices of peer review can be extended to other types of peer involvement activities (i.e., peer input and peer consultation) to develop high-quality risk assessment work products. A comprehensive process for incorporating peer input, peer consultation, and peer review into risk assessment science is outlined. Four key principles for peer involvement-independence, inclusion of appropriate experts, transparency, and a robust scientific process-are discussed. Recent examples of peer involvement in the development of Health Canada's Priority Substances and Domestic Substance List (DSL) programs under the Canadian Environmental Protection Act (CEPA) serve to highlight the concepts.  相似文献

In November 2001, the Monterey Institute of International Studies convened a workshop on bioterrorism threat assessment and risk management. Risk assessment practitioners from various disciplines, but without specialized knowledge of terrorism, were brought together with security and intelligence threat analysts to stimulate an exchange that could be useful to both communities. This article, prepared by a subset of the participants, comments on the workshop's findings and their implications and makes three recommendations, two short term (use of threat assessment methodologies and vulnerability analysis) and one long term (application of quantitative risk assessment and modeling), regarding the practical application of risk assessment methods to bioterrorism issues.  相似文献

Seveso plants are complex sociotechnical systems, which makes it appropriate to support any risk assessment with a model of the system. However, more often than not, this step is only partially addressed, simplified, or avoided in safety reports. At the same time, investigations have shown that the complexity of industrial systems is frequently a factor in accidents, due to interactions between their technical, human, and organizational dimensions. In order to handle both this complexity and changes in the system over time, this article proposes an original and simplified qualitative risk evaluation method based on the system dynamics theory developed by Forrester in the early 1960s. The methodology supports the development of a dynamic risk assessment framework dedicated to industrial activities. It consists of 10 complementary steps grouped into two main activities: system dynamics modeling of the sociotechnical system and risk analysis. This system dynamics risk analysis is applied to a case study of a chemical plant and provides a way to assess the technological and organizational components of safety.  相似文献

Several major risk studies have been performed in recent years in the maritime transportation domain. These studies have had significant impact on management practices in the industry. The first, the Prince William Sound risk assessment, was reviewed by the National Research Council and found to be promising but incomplete, as the uncertainty in its results was not assessed. The difficulty in assessing this uncertainty is the different techniques that need to be used to model risk in this dynamic and data-scarce application area. In previous articles, we have developed the two pieces of methodology necessary to assess uncertainty in maritime risk assessment, a Bayesian simulation of the occurrence of situations with accident potential and a Bayesian multivariate regression analysis of the relationship between factors describing these situations and expert judgments of accident risk. In this article, we combine the methods to perform a full-scale assessment of risk and uncertainty for two case studies. The first is an assessment of the effects of proposed ferry service expansions in San Francisco Bay. The second is an assessment of risk for the Washington State Ferries, the largest ferry system in the United States.  相似文献

In the general framework of quantitative methods for natural‐technological (NaTech) risk analysis, a specific methodology was developed for assessing risks caused by hazardous substances released due to earthquakes. The contribution of accidental scenarios initiated by seismic events to the overall industrial risk was assessed in three case studies derived from the actual plant layout of existing oil refineries. Several specific vulnerability models for different equipment classes were compared and assessed. The effect of differing structural resistances for process equipment on the final risk results was also investigated. The main factors influencing the final risk values resulted from the models for equipment vulnerability and the assumptions for the reference damage states of the process equipment. The analysis of case studies showed that in seismic zones the additional risk deriving from damage caused by earthquakes may be up to more than one order of magnitude higher than that associated to internal failure causes. Critical equipment was determined to be mainly pressurized tanks, even though atmospheric tanks were more vulnerable to containment loss. Failure of minor process equipment having a limited hold‐up of hazardous substances (such as pumps) was shown to have limited influence on the final values of the risk increase caused by earthquakes.  相似文献

Numerous studies and practical experiences with risk have demonstrated the importance of risk perceptions for people's behavior. In this narrative review, we describe and reflect upon some of the lines of research that we feel have been important in helping us understand the factors and processes that shape people's risk perceptions. In our review, we propose that much of the research on risk perceptions to date can be grouped according to three dominant perspectives and, thus, approaches to study design; they are: the characteristics of hazards, the characteristics of risk perceivers, and the application of heuristics to inform risk judgments. In making these distinctions, we also highlight what we see as outstanding challenges for researchers and practitioners. The current approach to health risk assessment of toxic waste sites in the U.S. may lead to considerable expenditure of resources without any meaningful reduction in population exposure. Risk assessment methods used generally ignore background exposures and consider only incremental risk estimates for maximally exposed individuals. Such risk estimates do not address true public health risks to which background exposures also contribute. The purpose of this paper is to recommend a new approach to risk assessment and risk management concerning toxic waste sites. Under this new approach, which we have called public health risk assessment, chemical substances would be classified into a level of concern based on the potential health risks associated with typical national and regional background exposures. Site assessment would then be based on the level of concern for the particular pollutants involved and the potential contribution of site contaminants to typical background human exposures. While various problems can be foreseen with this approach, the key advantage is that resources would be allocated to reduce the most important sources of human exposure, and site remediation decisions could be simplified by focussing on exposure assessment rather than questionable risk extrapolations.  相似文献

Although environmental equity research has focused primarily on chronic pollution sources, recent advances in environmental modeling and geographic information systems (GIS) provide a foundation for developing measures that can be used to evaluate differential exposure to acute pollution events. This article describes a methodology that uses facility-specific information to develop a risk surface representing the spatial distribution of accidental exposure to hazardous substances in a study area. Environmental pollution models recommended by the U.S. Environmental Protection Agency were used in conjunction with GIS software to achieve this objective. The methodology was implemented in a large metropolitan region (Hillsborough County, Florida) to examine disproportionate exposure to worst-case releases of extremely hazardous substances. The environmental inequity hypothesis was investigated by directly comparing the distribution of potential exposures within each racial (non-White versus White) and income (below poverty versus above poverty) subgroup. The results indicate that a significantly large proportion of both non-White and impoverished individuals resided in areas potentially exposed to multiple accidental releases.  相似文献

The recent occurrence of severe major accidents has brought to light flaws and limitations of hazard identification (HAZID) processes performed for safety reports, as in the accidents at Toulouse (France) and Buncefield (UK), where the accident scenarios that occurred were not captured by HAZID techniques. This study focuses on this type of atypical accident scenario deviating from normal expectations. The main purpose is to analyze the examples of atypical accidents mentioned and to attempt to identify them through the application of a well-known methodology such as the bow-tie analysis. To these aims, the concept of atypical event is accurately defined. Early warnings, causes, consequences, and occurrence mechanisms of the specific events are widely studied and general failures of risk assessment, management, and governance isolated. These activities contribute to outline a set of targeted recommendations, addressing transversal common deficiencies and also demonstrating how a better management of knowledge from the study of past events can support future risk assessment processes in the identification of atypical accident scenarios. Thus, a new methodology is not suggested; rather, a specific approach coordinating a more effective use of experience and available information is described, to suggest that lessons to be learned from past accidents can be effectively translated into actions of prevention.  相似文献

We systematically review the recent impactful leadership succession literature in three types of organizations/contexts, namely publicly-traded, privately-owned (mostly family businesses), and political organizations. We compare and contrast these literatures, and argue that business and political leadership succession researchers and practitioners can learn from each other. The purpose of the review is fourfold. First, to take stock of the existing leadership succession research in these three related literatures – that examine the same essential phenomenon – but that have evolved separately. Previous reviews have focused mostly on CEO succession (not the broader phenomenon of leadership succession) mainly in publicly-traded firms; and to our knowledge no (recent) comprehensive literature reviews on the important topics of privately-owned and political organization leadership succession exist yet. Second, to develop an overarching integrative conceptual framework (ICF) that structures the overall leadership succession literature and shows the potential areas of integration and difference among the three literatures. Third, to develop three organizational frameworks – one for each organization type – that review what we know and what we should know about leadership succession in each type. Fourth, to critically compare the ICF, the three organizational frameworks, and the three literatures to better understand the similarities and differences among these literatures. By doing so and using a multidisciplinary approach we aim to contribute to the field in the following ways. Firstly, we seek to synthesize the field of leadership succession to identify important research questions that are ripe for study in the near future in the business and political science disciplines. Secondly, we strive to uncover what succession researchers and practitioners across these disciplines may learn from each other.  相似文献
