The present study advances our understanding of both physician adaptation and the physician-organization relationship in a managed care environment defined by structural diversity and constant change. It does so through a longitudinal examination of a single group of physician-employees experiencing their work lives within a nonprofit health maintenance organization (HMO) in the midst of major strategic developments. Using interview, observation, and archival data collected over a five-year period, the analysis reveals that the form and substance of individual physician adaptation to organizational life is dependent upon social exchanges over time with the HMO, making it an emergent, evolutionary process rather than a pre-determined, static phenomenon. However, the results also demonstrate that physician adaptive response to rapid, unpredictable organizational change is slow and delayed, in particular when this change makes physicians more dependent on their employing organization. This confers an advantage onto the organization vis-à-vis professionals in interpreting and responding to environmental change. These insights should encourage sociologists to employ research designs and contingency models of physician attitudes and behavior that capture the dynamic and particularistic nature of everyday physician work life in contemporary health care.  相似文献

Using data from a national survey of 501 Arab American women, this study examines the extent to which family behavior mediates the influence of religion on women's labor force activity. Prior research on families has largely overlooked the role of religion in influencing women's labor force decisions, particularly at different stages of the life cycle. The analysis begins to address this gap by examining whether religious affiliation and religiosity have direct relationships to women's work behaviors, or whether they primarily operate through family behaviors at different phases of the life course. The results show that religiosity exerts a negative influence on women's labor force participation, but only when children are present in the home. Among women with no children, religiosity has no effect on employment.  相似文献

Interviews with white, working professional and working class mothers suggest that these women share values about work, family and on-the-job sociality. Respondents draw from a common value system, and many of the statements voiced by one group are echoed in the other. Work and motherhood are important paths to the development of individual identity for both working class and professional women. Differences in jobs, however, lead to differences in the actualization of values in their lives. Professional women are pulled in two directions and face continuing dilemmas in balancing careers which extract strong commitments from them and the demands of motherhood. Working class women's jobs do not require the same intensity of commitment, and they find it easier to meet their families' demands on their time. But conceptualizing their work as meaningful becomes more difficult than it is for professional women. Sociality is the third area of concern. Women in this study emphasized sociality because it allowed them to complete their work more effectively. It also served expressive functions; some women made friends at work. Some de-emphasized sociality if it interfered with their work identities. Hindu nationalists made migration from Bangladesh an election campaign in West Bengal during the 1990s. Although there were various allegations against Bangladeshi migration, it never became a mainstream political issue in the state, like in neighbouring Assam. West Bengal shares the longest border with Bangladesh, compared to any other Indian state, and hosts a large number of Bangladeshi migrants, according to the Census reports in India. West Bengal and Bangladesh share a common ethnicity, both are predominantly Bengali. Can this shared Bengaliness explain why Bangladeshi migration did not become a divisive political issue in the state? If this sameness is a bonding factor, what about the Ghoti-Bangal differences? Drawn from in-depth interviews with the representatives of West Bengal’s key civil society organisations and political parties, I argue that a particular historical and cultural process, unique to West Bengal and Bangladesh, has shaped the current attitude towards Bangladeshi migrants in the state.  相似文献

This paper analyses the community values of residential neighborhoods in the southwestern region of Saudi Arabia as an approach to a new theory in urbanism. The indigenous masterbuilders and tribemen incorporated planning decisions pertinent to climatic, cultural, social, economic and religious factors when designing physical elements in their built environment. This is what makes every traditional settlement in the southwest region of Saudi Arabia unique in terms of urban form and social structure. As a step to examining and evaluating the process of residential neighborhood development, three stages of practiced urbanism are discussed. These stages are labelled “vernacular”, “transitional” and “new vernacularism”. These are planning concepts used worldwide in the development of residential neighborhoods throughout history until the present. “New Vernacularism” is envisioned in this investigation as a design/planning objective implemented in most recently planned neighborhoods in Al-Horaidhah, Southwest Saudi Arabia. The achievement of successful urbanism underlies the political goals of urban planning practice. The Al-Horaidhah planning concept is conceived in the light of preservation of community values in neighborhood design/planning as a critical issue.The paper aims to investigate and reconcile the conflicts in the planning of residential neighborhoods in a changing world. The conflicts are restricted between retaining traditions of architecture, urban design and planning with the necessary social, economic, and technological changes in urban formation, mainly, the vernacular and modern. The paper illustrates the concept “New Vernacularism” by presenting Al-Horaidhah scheme as a model for community development in three administrative regions along the Red Sea coastline. “New Vernacularism” as a planning concept looks at the community development in Al-Horaidhah in the light of Sharicah, the Islamic Law and Customary norms by emphasizing the importance of the involvement of local residents in the planning process and gives suggestions of how this might best be achieved and later implemented into new communities.  相似文献

Based on data from an eight year longitudinal study of Ohio's long-term care use patterns, this paper describes the changes now being experienced by this industry. Although Ohio has been a state with a heavy reliance on institutional services, the data suggest a change in how long-term care is provided in the state. Over the past eight years, despite an increasing disabled older population, nursing home occupancy rates have fallen from 92.5% to 83.5%. At the same time, in-home service and assisted living use has increased. The paper concludes by describing how such changes are likely to impact the system of the future.  相似文献

Mental disorders impose an enormous burden on society. In developing countries like India, there is a lack of adequate number of trained mental health professionals to provide specialized care and 75–85 % of affected individuals do not have access to appropriate mental health services. The National Mental Health Programme (NMHP) is being implemented by the Government of India to support state governments in providing mental health services in the country. The Urban Mental Health Programme (UMHP) is a pilot initiative that has attempted the integration of mental health services in primary health care settings in two municipal wards in Kolkata, West Bengal, India. The overarching aim of this paper is to describe the methodology used for the evaluation of the community based mental health programme and to understand the processes of the programme in terms of barriers and facilitators. The current evaluation is based on a concurrent nested design, where qualitative and quantitative data are both collected at the same time but analysed separately and priority was given to qualitative data. This experience will contribute in helping other researchers to make some evaluations more effective, useful and manageable. Ethics approval was obtained from an institutional ethics committee of an organization (Ekjut) based in Ranchi, Jharkhand, India. The evaluation was undertaken by the George Institute for Global Health, New Delhi from February- June 2016.  相似文献

The main aim of the article is discussion of the conceptual framework for defining religion from a sociological perspective. One of the possible orderings of its development is showing historical periods and conceptual streams. As far as history is concerned the author distinguishes three periods: classical, post-classical and contemporary, showing dominant themes and ways of approaching religion in each of them. An alternative way of ordering is proposed in the second part of the article based on the source of religious change identified as the crucial point by sociologists working on theories of religion. There are three perspectives in identifying religious change: giving priority to individuals, to social systems and to religion itself. Every perspective has some outcomes for understanding the place of religion in social and individual life. The last part of the article is devoted to presenting the impact of religious conceptions on interpretational disputes between sociologists of religion.  相似文献
