The increasing number of MSW students engaged in private psychotherapy practice concurrent with their graduate studies has received little attention in literature on social work education. This article examines the educational, ethical, and legal dilemmas effected by this trend, and describes the policy developed at the University of Denver to address these problems. More broadly, it argues that schools of social work in general have a legitimate interest in adopting policies to prohibit or discourage MSW students from conducting private practice.  相似文献

This article presents a conceptual framework to link issues of health, services, physical activity, and environment to help older adults age in place in their own homes, which may reduce the demands on society to provide institutional long-term care. Using environments to promote physical activity has received attention by researchers, but its impact on aging-in-place at home has been largely ignored. This article interprets and synthesizes existing studies on aging-in-place in relation to health and services, contributions of physical activity to health and service accessibility, and environmental support of physical activity. The conceptual framework can serve as the core construct to address gaps in previous research and as a tool to refine practical targets for policies and interventions aimed at promoting healthy aging.  相似文献

Debates about the relationship between poverty and disability continue and are important in that they contribute to policies regarding how best to address the needs of disabled people living in conditions of poverty. Increasingly, researchers have begun to use Sen's capabilities approach in understanding disability. However, the approach has not been adequately applied to understand the nature of the poverty and disability nexus, particularly in developing contexts. This article seeks to address this gap by reporting on evidence from a study conducted in eight of the poorest wards in Johannesburg. Using the capabilities lens we demonstrate the ways in which both poverty and disability compound one another to limit the capabilities of people. The findings point to the need for broad-scale as well as targeted social development policies and programmes to address the consequences of poverty and disability.  相似文献

This paper i11ustrates the importance of the fiscal framework for monetary analysis by discussing three separate issues. I begin by examining how the fiscal framework changes the macroeconomic equilibrium associated with different steady state rates of money growth. This includes a summary of research that I have presented elsewhere and comments on several additional aspects of the way in which the fiscal structure destroys the neutrality of monetary policy.
The second section deals with the short-run impact of changes in monetary policy. Here again the fiscal structure complicates the economy's response to monetary policy.
The final section looks at the effect of the fiscal structure on the central banks choice of monetary policies. Fiscal structures are likely to influence the policies adopted because they affect the costs and benefits of monetary policies.  相似文献

The article concerns the extent to which elder abuse and neglect is known and recognized as a social, medical and legal problem in western European countries. Only in France and the Scandinavian countries have researchers and policy makers in the health and social service fields begun to address the problem. November 1987, the Norwegian Minister of Health and Social Affairs called for a Workshop on Violence within the Family in the Council of Europe, Strasbourg. The article is a review of the different national reports concerning elder abuse presented at the workshop. The concluding remarks are taken from the various group and plenary discussions as well as the author's own experience.  相似文献

Health maintenance organizations and similar pre-paid health plans are an alternative form of health care available to Medicare beneficiaries in some areas of the country. While proponents of HMOs have argued that these plans may be better suited than fee-for-service medicine to provide care to an older population, HMOs have played a relatively small role in health care of the elderly. This paper traces federal health policy relating to HMOs and Medicare and describes obstacles to HMO Medicare programs stemming from those policies. It then examines the Massachusetts experience in HMO program development as a case study of how state policy can play a critical role in implementation of policy objectives at a local level. The paper demonstrates how an active state policy involving the executive and legislative branches, elder advocacy groups, and the private sector created a public-private partnership to develop HMO programs for the Medicare population. When this paper was completed, however, Medicare HMO development in Massachusetts was at a standstill, waiting for several critical problems of federal policy to be resolved.  相似文献

This article proposes a theoretical approach to illicit-drug policy that emphasizes politics and institutions, as an alternative to dominant perspectives emphasizing social control. After revealing conceptual links between drug-control policies and those of social provision, I describe a typology of drug policies based on the division of labor among the institutions responsible for implementing the legislated agenda. I then develop a theoretical argument that approaches drug policy as social policy, employing concepts of institutional reach and policy linkages. In the concluding section I use this approach to compare drug policies in the United States and Canada after 1980.  相似文献

The social investment approach emerged as a new welfare paradigm, aimed at reconciling the traditional functions of the welfare supply with a productive social agenda, designed at preparing people to confront the ‘new social risks’, whether they be related to the problem of balancing paid work and family responsibilities, upgrading the skills, preventing inequalities and promoting the availability of in-kind services. In order to achieve these objectives, especially those related to care needs and work-life balance, the adoption of social investment-based strategies necessarily implies an expansion of the jobs related to health and social care services. In more recent years, many studies have analysed the limitations of the social investment policies because of their different redistributive impacts on social groups. Several studies have found a higher use of these policies for high-income families. Another source of criticism on social investment is that spending on these policies would seem to crowd out more traditional passive social expenditures. In this article, we examine another question related to the widespread of this approach: what are the effects of the social investment policies in terms of direct job creation? In fact, one of the more controversial issues, related to social investment policies, is their direct contribution to the labour market in terms of both quantity and quality of work within welfare services. The article analyses these issues focusing on Germany and Italy, two countries that represent not only two different care regimes but also two distinct models regarding job creation strategies in the care sector. In doing so, particular attention will be paid to long-term care policies, as they represent one of the pivotal areas of the social investment approach, both in terms of social services, to address new social risks, and new jobs related to welfare services  相似文献

A frustration often expressed by researchers and policy-makers in public health is an apparent mismatch between respective priorities and expectations for research. Academics bemoan an oversimplification of their work, a reticence for independent critique and the constant pressure to pursue evaluation funding. Meanwhile, policy-makers look for research reports written in plain language with clear application, which are attuned to current policy settings and produced quickly. In a context where there are calls in western nations for evidence based policy with stronger links to academic research, such a mismatch can present significant challenges to policy program evaluation. The purpose of this paper is to present one attempt to overcome these challenges. Specifically, the paper describes the development of a conceptual framework for a large-scale, multifaceted evaluation of an Australian Government health initiative to expand Nurse Practitioner models of practice in aged care service delivery. In doing so, the paper provides a brief review of key points for the facilitation of a strong research-policy nexus in public health evaluations, as well as describes how this particular evaluation embodies these key points. As such, the paper presents an evaluation approach which may be adopted and adapted by others undertaking public health policy program evaluations.  相似文献

This article analyses the content of 15 Poverty Reduction Strategy Papers from a growth and poverty reduction perspective. Contrary to new trends in developed and middle‐income countries, their policy frameworks lack the necessary flexibility to deal with external shocks and address macroeconomic volatility appropriately. Their fiscal and monetary policies are too narrowly focused on fiscal balance and price stability and consequently pay too little attention to economic fluctuations arising from external shocks, which have major effects on poverty and growth. To address these issues, the article proposes a set of policy measures, including avoidance of excessively tight inflation and fiscal targets (with provisions for fluctuations in commodity prices), more room for countercyclical policy and the adoption of permanent safety nets.  相似文献

The impact of globalisation upon the Korean economy is an important issue to Korean researchers. This paper is concerned with the current spatial policy and planning in Korea which is designed to facilitate the benefit from globalisation. As such, the paper discusses the meaning of globalisation in order to provide a framework for the review of policies and strategies. There follows a review of Korean policies within the area which have been adopted, as well as those which are expected to be followed in the future. A theoretical formulation is presented that represents an abstract and parsimonious conception of public policy adoption at the subnational level. The formulation consists of two components: the principle that social phenomena exist in contexts with which they are compatible, and several distinguishing attributes of subnational collectivities and public policies. Taken together, these statements lead to the conclusion: if there is a match between attributes of a policy and those of a collectivity, then the probability increases that the policy will be adopted by the collectivity. General hypotheses derived from the formulation appear generally consistent with findings of previous research. Specific hypotheses derived from the formulation regarding city adoption of War on Poverty and National Flood Insurance programs are tested with cross–sectional data for a sample of U.S. cities. Results of an ordinary least-squares regression analysis reveal support for derived hypotheses. It is concluded that the explanatory and predictive powers of the theoretical formulation are high.  相似文献

Informal caregivers play an increasingly important role in caring for aging Americans. Yet existing social policies that could support informal caregiving have experienced “policy drift,” a failure to adapt to social risks that develop after policies are initially enacted. This article examines policy makers’ success at updating seven major policies to address caregiver needs. It draws on an original data set of legislation in this area introduced between 1991 and 2006 (n = 96). Findings indicate that drift is more likely when policy updates are costly, lack support from members of majority parties in the House and Senate, and fail to generate bipartisan support.  相似文献

Until recently, migrant health policies have been overlooked as a topic of policy analysis. Although interest in the issue has developed in parallel with the progressive acknowledgement of the presence of unhealthy migrants and the transformation of welfare states and policy dynamics in Europe, studies on migrant health policies have often focused on the state as a unique unit of observation while hindering the role played by other institutional and non‐institutional actors taking part in the migrant health multi‐level governance. This contribution will bridge this gap, deconstructing the various actors and levels involved in migrant health policymaking from a multi‐level perspective. By critically reviewing migrant health policy research and discussing it with the new MIPEX Health Strand, this contribution suggests a more encompassing perspective in the analysis of migrant health policies and processes, looking at the different players at stake in this multifaceted policy field.  相似文献

The global diffusion of digital technology, which occurred more rapidly than the global diffusion of any technology previously, has been mired by its uneven distribution across, and unequal effects on, societies worldwide. In addition, policy initiatives to close this global digital divide, which peaked with the two World Summit on Information Society conferences, still did not change the course of this differentiated globalization process. In this article, I attribute the cause of such stalling of policy on the issue of the global digital divide to the bifurcation of current international policy: attention is split between concern for the impeded access of the poor to this revolutionary technology, on the one hand, and the race to lead the world in creating the next “hot” technology, on the other. These two concerns, which have been given the pithy titles of the “global digital divide” and the “global innovation divide,” are leading to two separate policy tracks, targeting the world’s laggards and leaders as separate entities and operating under separate logics. This separation is problematic because the issues of access to technology and ownership of rights to technology are intertwined. This article describes the two global technology divides and analyzes the policies that are currently charted to address them.  相似文献

Although the occupation and associated living conditions of migrant and seasonal agricultural workers in the US pose exceptional health hazards to the workers and their dependents, relatively few occupational health professionals have been involved with this group. This articles examines the basis for this neglect and proposes a definition of the population that should be considered in farmworker health policy. It then reviews existing evidence regarding hazards of 4 major occupational exposures--pesticides, the sun, injuries, and poor field sanitation--and policies that have been developed to address these hazards. The extremely negative health consequences of farmworker living conditions, which are indirect occupational hazards, are also summarized. Numerous policy, planning, and research recommendations are made. Adequate solutions for this impoverished and powerless group, however, will require significant sociopolitical advances, such as are developing with unionization and other forms of political organization.  相似文献

Health outcomes are implicit in the government's major policies on reducing poverty, unemployment and social exclusion, and in environmental regeneration, as well as in explicitly targeted policies for modernising the National Health Service. The impact of policies in childhood are regarded as a key feature in determining socioeconomic outcomes in many domains, among which mental health plays a particularly important part. But although early intervention is recognised as crucial in the achievement of socioeconomic and health policy aims, outcomes in children and for children have only recently received full recognition. This article outlines the impact that government policies may have on health outcomes for children. However, the assessment of changes in health status is difficult and mostly relies on proxy measures. The evidence that any changes can be related to policy is extremely limited and mostly based on small‐scale and locally specific projects. What can be learnt from these and from collective analysis of linked projects, such as those funded through the CAMHS Innovation Grant, is discussed. Because of the attention paid by the government to evidence‐based policy, outcomes in all domains will become known before long for major national children's policy initiatives such as the Sure Start programme, highlighting the crucial and continuing need for evaluation of the ways in which policies are implemented. Communities with histories of oppression have shown great resilience, yet few health interventions focus on structural oppression as a contributor to health problems in these communities. This article describes the development and active ingredients of Community Wise, a unique behavioral health intervention designed to reduce substance use frequency, related health risk behaviors, and recidivism among individuals with a history of incarceration and substance abuse residing in distressed and predominantly African American communities. Community Wise, developed through the collaborative efforts of a board of service providers, researchers, consumers, and government officials, is a 12-week group intervention that aims to address behavioral health problems by raising critical consciousness in distressed communities.  相似文献
