A quantitative microbial risk assessment (QMRA) according to the Codex Alimentarius Principles is conducted to evaluate the risk of human salmonellosis through household consumption of fresh minced pork meat in Belgium. The quantitative exposure assessment is carried out by building a modular risk model, called the METZOON-model, which covers the pork production from farm to fork. In the METZOON-model, the food production pathway is split up in six consecutive modules: (1) primary production, (2) transport and lairage, (3) slaughterhouse, (4) postprocessing, (5) distribution and storage, and (6) preparation and consumption. All the modules are developed to resemble as closely as possible the Belgian situation, making use of the available national data. Several statistical refinements and improved modeling techniques are proposed. The model produces highly realistic results. The baseline predicted number of annual salmonellosis cases is 20,513 ( SD 9061.45). The risk is estimated higher for the susceptible population (estimate  4.713 × 10−5; SD 1.466 × 10−5  ) compared to the normal population  (estimate 7.704 × 10−6; SD 5.414 × 10−6)  and is mainly due to undercooking and to a smaller extent to cross-contamination in the kitchen via cook's hands.  相似文献

Lifetime cancer potency of alfatoxin was assessed based on the Yeh et al. study from China in which both aflatoxin exposure and hepatitis B prevalence were measured. This study provides the best available information for estimating the carcinogenic risk posed by aflatoxin to the U.S. population. Cancer potency of aflatoxin was estimated using a biologically motivated risk assessment model. The best estimate of aflatoxin potency was 9 (mg/kg/day)−1 for individuals negative for hepatitis B and 230 (mg/kg/day)−1 for individuals positive for hepatitis B.  相似文献

The total ban on use of meat and bone meal (MBM) in livestock feed has been very successful in reducing bovine spongiform encephalopathy (BSE) spread, but also implies a waste of high-quality proteins resulting in economic and ecological loss. Now that the BSE epidemic is fading out, a partial lifting of the MBM ban might be considered. The objective of this study was to assess the BSE risk for the Netherlands if MBM derived from animals fit for human consumption, i.e., category 3 MBM, would be used in nonruminant feed. A stochastic simulation model was constructed that calculates (1) the probability that infectivity of undetected BSE-infected cows ends up with calves and (2) the quantity of infectivity ( Qinf ) consumed by calves in case of such an incident. Three pathways were considered via which infectivity can reach cattle: (1) cross-contamination in the feed mill, (2) cross-contamination on the primary farm, and (3) pasture contamination. Model calculations indicate that the overall probability that infectivity ends up with calves is 3.2%. In most such incidents the Qinf is extremely small (median = 6.5 × 10−12 ID50; mean = 1.8 × 10−4 ID50), corresponding to an average probability of 1.3 × 10−4 that an incident results in ≥1 new BSE infections. Cross-contamination in the feed mill is the most risky pathway. Combining model results with Dutch BSE prevalence estimates for the coming years, it can be concluded that the BSE risk of using category 3 MBM derived from Dutch cattle in nonruminant feed is very low.  相似文献

Two-year chronic bioassays were conducted by using B6C3F1 female mice fed several concentrations of two different mixtures of coal tars from manufactured gas waste sites or benzo(a)pyrene (BaP). The purpose of the study was to obtain estimates of cancer potency of coal tar mixtures, by using conventional regulatory methods, for use in manufactured gas waste site remediation. A secondary purpose was to investigate the validity of using the concentration of a single potent carcinogen, in this case benzo(a)pyrene, to estimate the relative risk for a coal tar mixture. The study has shown that BaP dominates the cancer risk when its concentration is greater than 6,300 ppm in the coal tar mixture. In this case the most sensitive tissue site is the forestomach. Using low-dose linear extrapolation, the lifetime cancer risk for humans is estimated to be: Risk < 1.03 × 10−4 (ppm coal tar in total diet) + 240 × 10−4 (ppm BaP in total diet), based on forestomach tumors. If the BaP concentration in the coal tar mixture is less than 6,300 ppm, the more likely case, then lung tumors provide the largest estimated upper limit of risk, Risk < 2.55 × 10−4 (ppm coal tar in total diet), with no contribution of BaP to lung tumors. The upper limit of the cancer potency (slope factor) for lifetime oral exposure to benzo(a)pyrene is 1.2 × 10−3 per μg per kg body weight per day from this Good Laboratory Practice (GLP) study compared with the current value of 7.3 × 10−3 per μg per kg body weight per day listed in the U.S. EPA Integrated Risk Information System.  相似文献

The relative contribution of four influenza virus exposure pathways—(1) virus-contaminated hand contact with facial membranes, (2) inhalation of respirable cough particles, (3) inhalation of inspirable cough particles, and (4) spray of cough droplets onto facial membranes—must be quantified to determine the potential efficacy of nonpharmaceutical interventions of transmission. We used a mathematical model to estimate the relative contributions of the four pathways to infection risk in the context of a person attending a bed-ridden family member ill with influenza. Considering the uncertainties in the sparse human subject influenza dose-response data, we assumed alternative ratios of 3,200:1 and 1:1 for the infectivity of inhaled respirable virus to intranasally instilled virus. For the 3,200:1 ratio, pathways (1), (2), and (4) contribute substantially to influenza risk: at a virus saliva concentration of 106 mL−1, pathways (1), (2), (3), and (4) contribute, respectively, 31%, 17%, 0.52%, and 52% of the infection risk. With increasing virus concentrations, pathway (2) increases in importance, while pathway (4) decreases in importance. In contrast, for the 1:1 infectivity ratio, pathway (1) is the most important overall: at a virus saliva concentration of 106 mL−1, pathways (1), (2), (3), and (4) contribute, respectively, 93%, 0.037%, 3.3%, and 3.7% of the infection risk. With increasing virus concentrations, pathway (3) increases in importance, while pathway (4) decreases in importance. Given the sparse knowledge concerning influenza dose and infectivity via different exposure pathways, nonpharmaceutical interventions for influenza should simultaneously address potential exposure via hand contact to the face, inhalation, and droplet spray.  相似文献

We estimated benzene risk using a novel framework of risk assessment that employed the measurement of radiation dose equivalents to benzene metabolites and a PBPK model. The highest risks for 1 μg/m3 and 3.2 mg/m3 life time exposure of benzene estimated with a linear regression were 5.4 × 10−7 and 1.3 × 10−3, respectively. Even though these estimates were based on in vitro chromosome aberration test data, they were about one-sixth to one-fourteenth that from other studies and represent a fairly good estimate by using radiation equivalent coefficient as an "internal standard."  相似文献

An adverse health impact is often treated as a binary variable (response vs. no response), in which case the risk of response is defined as a monotonically increasing function R of the dose received D. For a population of size N , specifying the forms of R(D) and of the probability density function (pdf) for D allows determination of the pdf for risk, and computation of the mean and variance of the distribution of incidence, where the latter parameters are denoted E[S N] and Var[ S N], respectively. The distribution of S N describes uncertainty in the future incidence value. Given variability in dose (and risk) among population members, the distribution of incidence is Poisson-binomial. However, depending on the value of E[S N], the distribution of incidence is adequately approximated by a Poisson distribution with parameter μ= E[S N], or by a normal distribution with mean and variance equal to E[S N] and Var[ S N]. The general analytical framework is applied to occupational infection by Mycobacterium tuberculosis (M. tb). Tuberculosis is transmitted by inhalation of 1–5 μm particles carrying viable M. tb bacilli. Infection risk has traditionally been modeled by the expression: R(D) = 1 – exp(– D ), where D is the expected number of bacilli that deposit in the pulmonary region. This model assumes that the infectious dose is one bacillus. The beta pdf and the gamma pdf are shown to be reasonable and especially convenient forms for modeling the distribution of the expected cumulative dose across a large healthcare worker cohort. Use of the the analytical framework is illustrated by estimating the efficacy of different respiratory protective devices in reducing healthcare worker infection risk.  相似文献

Nitrated polycyclic aromatic hydrocarbons (nitro-PAHs) constitute a group of substances formed during incomplete combustion. Sources include diesel vehicles, heating, smoking, certain types of food-processing, and incomplete combustion in general. 2-Nitrofluorene (NF) represents a model substance for the nitro-PAHs. An attempt has been made to estimate the lifetime human cancer risk due to background exposure to nitro-PAHs by two different models. The first model is based on genotoxic lesions produced by gamma-irradiation and the second model is based on an earlier published mega study (24,000 animals) on the carcinogenicity of the major metabolite of NF. Both models agreed well-representing a yearly human cancer risk in the range of 0.15–49 × 10−6 for an urban citizen. The weaknesses and strengths of the models are discussed.  相似文献

A Monte Carlo simulation is incorporated into a risk assessment for trichloroethylene (TCE) using physiologically-based pharmacokinetic (PBPK) modeling coupled with the linearized multistage model to derive human carcinogenic risk extrapolations. The Monte Carlo technique incorporates physiological parameter variability to produce a statistically derived range of risk estimates which quantifies specific uncertainties associated with PBPK risk assessment approaches. Both inhalation and ingestion exposure routes are addressed. Simulated exposure scenarios were consistent with those used by the Environmental Protection Agency (EPA) in their TCE risk assessment. Mean values of physiological parameters were gathered from the literature for both mice (carcinogenic bioassay subjects) and for humans. Realistic physiological value distributions were assumed using existing data on variability. Mouse cancer bioassay data were correlated to total TCE metabolized and area-under-the-curve (blood concentration) trichloroacetic acid (TCA) as determined by a mouse PBPK model. These internal dose metrics were used in a linearized multistage model analysis to determine dose metric values corresponding to 10-6 lifetime excess cancer risk. Using a human PBPK model, these metabolized doses were then extrapolated to equivalent human exposures (inhalation and ingestion). The Monte Carlo iterations with varying mouse and human physiological parameters produced a range of human exposure concentrations producing a 10-6 risk.  相似文献

Obvious spatial infection patterns are often observed in cases associated with airborne transmissible diseases. Existing quantitative infection risk assessment models analyze the observed cases by assuming a homogeneous infectious particle concentration and ignore the spatial infection pattern, which may cause errors. This study aims at developing an approach to analyze spatial infection patterns associated with infectious respiratory diseases or other airborne transmissible diseases using infection risk assessment and likelihood estimation. Mathematical likelihood, based on binomial probability, was used to formulate the retrospective component with some additional mathematical treatments. Together with an infection risk assessment model that can address spatial heterogeneity, the method can be used to analyze the spatial infection pattern and retrospectively estimate the influencing parameters causing the cases, such as the infectious source strength of the pathogen. A Varicella outbreak was selected to demonstrate the use of the new approach. The infectious source strength estimated by the Wells‐Riley concept using the likelihood estimation was compared with the estimation using the existing method. It was found that the maximum likelihood estimation matches the epidemiological observation of the outbreak case much better than the estimation under the assumption of homogeneous infectious particle concentration. Influencing parameters retrospectively estimated using the new approach can be used as input parameters in quantitative infection risk assessment of the disease under other scenarios. The approach developed in this study can also serve as an epidemiological tool in outbreak investigation. Limitations and further developments are also discussed.  相似文献

This article estimates the risk of tuberculosis (TB) transmission on a typical commercial airliner using a simple one box model (OBM) and a sequential box model (SBM). We used input data derived from an actual TB exposure on an airliner, and we assumed a hypothetical scenario that a highly infectious TB source case (i.e., 108 infectious quanta per hour) travels as a passenger on an 8.7-hour flight. We estimate an average risk of TB transmission on the order of 1 chance in 1,000 for all passengers using the OBM. Applying the more realistic SBM, we show that the risk and incidence decrease sharply in a stepwise fashion in cabins downstream from the cabin containing the source case assuming some potential for airflow from more contaminated to less contaminated cabins. We further characterized spatial variability in the risk within the cabin by modeling a previously reported TB outbreak in an airplane to demonstrate that the TB cases occur most likely within close proximity of the source TB patient.  相似文献

Some analysts suggest that discussing uncertainties in health risk assessments might reduce citizens'perceptions of risk and increase their respect for the risk-assessing agency. We tested this assumption with simulated news stories varying simple displays of uncertainty (e.g., a range of risk estimates, with and without graphics). Subjects from Eugene, Oregon, read one story each, and then answered a questionnaire. Three studies tested between 180 and 272 subjects each. Two focus groups obtained more detailed responses to these stories. The results suggested that (1) people are unfamiliar with uncertainty in risk assessments and in science; (2) people may recognize uncertainty when it is presented simply; (3) graphics may help people recognize uncertainty; (4) reactions to the environmental problems in the stories seemed affected less by presentation of uncertainty than by general risk attitudes and perceptions; (5) agency discussion of uncertainty in risk estimates may signal agency honesty and agency incompetence for some people; and (6) people seem to see lower risk estimates (10-6, as opposed to 10-3) as less credible. These findings, if confirmed, would have important implications for risk communication.  相似文献

Environmental impact assessment (EIA) is a procedure that must be followed for certain types of development before they are granted development consent. The procedure requires the developer to compile an environmental impact report (EIR) describing the likely significant effects of the project on the environment. A regulatory requirement in Slovenia is that an accidental risk assessment for a new installation should be a part of an EIR. The article shows how risk assessment (RA) related to accidental release of methylene diphenyl diisocyanate (MDI) or a polyvalent alcohol mixture from a new planned unit of a chemical factory in the Alpine region of Slovenia was performed in the framework of an EIA for the purpose of obtaining a construction permit. Two accidental scenarios were considered: (a) a spill of 20 m3 of MDI or polyvalent alcohol mixture into the river Soča (the river runs close to the chemical factory) and (b) a fire in the warehouse storing the raw material, where emission of toxic gases HCN, NOx, and CO is expected during combustion of MDI. One of the most important results of this case is the agreement among the developer, the competent authority, and a consultant in the field of EIA and RA to positively conclude the licensing process despite the absence of formal (regulatory) limit values for risk. It has been approved that transparent, reasonably uncertain, and semi-quantitative environmental risk assessment is an inevitable component of an EIA, and an essential factor in informed, licensing-related decision making.  相似文献

The present study assessed cross-cultural differences in the perception of financial risks. Students at large universities in Hong Kong, Taiwan, the Netherlands, and the U.S., as well as a group of Taiwanese security analysts rated the riskiness of a set of monetary lotteries. Risk judgments differed with nationality, but not with occupation (students vs. security analysts) and were modeled by the Conjoint Expected Risk (CER) model.(1) Consistent with cultural differences in country uncertainty avoidance,(2) CER model parameters of respondents from the two Western countries differed from those of respondents from the two countries with Chinese cultural roots: The risk judgments of respondents from Hong Kong and Taiwan were more sensitive to the magnitude of potential losses and less mitigated by the probability of positive outcomes.  相似文献

A central part of probabilistic public health risk assessment is the selection of probability distributions for the uncertain input variables. In this paper, we apply the first-order reliability method (FORM)(1–3) as a probabilistic tool to assess the effect of probability distributions of the input random variables on the probability that risk exceeds a threshold level (termed the probability of failure) and on the relevant probabilistic sensitivities. The analysis was applied to a case study given by Thompson et al. (4) on cancer risk caused by the ingestion of benzene contaminated soil. Normal, lognormal, and uniform distributions were used in the analysis. The results show that the selection of a probability distribution function for the uncertain variables in this case study had a moderate impact on the probability that values would fall above a given threshold risk when the threshold risk is at the 50th percentile of the original distribution given by Thompson et al. (4) The impact was much greater when the threshold risk level was at the 95th percentile. The impact on uncertainty sensitivity, however, showed a reversed trend, where the impact was more appreciable for the 50th percentile of the original distribution of risk given by Thompson et al. 4 than for the 95th percentile. Nevertheless, the choice of distribution shape did not alter the order of probabilistic sensitivity of the basic uncertain variables.  相似文献

Concern about the degree of uncertainty and potential conservatism in deterministic point estimates of risk has prompted researchers to turn increasingly to probabilistic methods for risk assessment. With Monte Carlo simulation techniques, distributions of risk reflecting uncertainty and/or variability are generated as an alternative. In this paper the compounding of conservatism(1) between the level associated with point estimate inputs selected from probability distributions and the level associated with the deterministic value of risk calculated using these inputs is explored. Two measures of compounded conservatism are compared and contrasted. The first measure considered, F , is defined as the ratio of the risk value, R d, calculated deterministically as a function of n inputs each at the j th percentile of its probability distribution, and the risk value, R j that falls at the j th percentile of the simulated risk distribution (i.e., F=Rd/Rj). The percentile of the simulated risk distribution which corresponds to the deterministic value, Rd , serves as a second measure of compounded conservatism. Analytical results for simple products of lognormal distributions are presented. In addition, a numerical treatment of several complex cases is presented using five simulation analyses from the literature to illustrate. Overall, there are cases in which conservatism compounds dramatically for deterministic point estimates of risk constructed from upper percentiles of input parameters, as well as those for which the effect is less notable. The analytical and numerical techniques discussed are intended to help analysts explore the factors that influence the magnitude of compounding conservatism in specific cases.  相似文献

The risks of unfamiliar technologies are often evaluated by comparing them with the risks of more familiar ones. Such risk comparisons have been criticized for neglecting critical dimensions of risky decisions. In a guide written for the Chemical Manufacturers Association, Covello et al. (1) have summarized these critiques and developed a taxonomy that characterizes possible risk comparisons in terms of their acceptability (or objectionableness). We asked four diverse groups of subjects to judge the acceptability of 14 statements produced by Covello et al. as examples of their categories. We found no correlation between the judgments of acceptability produced by our subjects and those predicted by Covello et al. .  相似文献

Human H5N1 highly pathogenic avian influenza (HPAI) infection is associated with intimate exposure to live poultry. Perceptions of risk can modify behaviors, influencing actual exposure. However, greater hazard is not necessarily followed by perception of greater risk and more precautionary behavior because self-serving cognitive biases modulate precautionary and hazardous behaviors. We examined risk perception associated with avian influenza. A total of 1,550 face-to-face within-household interviews and 1,760 telephone interviews were derived to study avian influenza risk perception and live poultry use in Guangzhou and Hong Kong, respectively. Chi-square and Mann-Whitney tests assessed bivariate associations and risk distributions, respectively, and fully adjusted multivariate logistic models determined independent risk associations. Relative to Hong Kong, perceived "generalized" risk from buying live poultry (GZ, 58%, 95% confidence interval 55–60% vs. HK, 41%, 39–43%; χ2= 86.95, df  = 1, p < 0.001) and perceived self/family risk from buying ( z  =−2.092, p  = 0.036) were higher in Guangzhou. Higher perceived "generalized" risk was associated with not buying live poultry (OR = 0.65, 0.49–0.85), consistent with the pattern seen in Hong Kong, while perceived higher self/family risk was associated with buying ("likely/very likely/certain" OR = 1.74, 1.18–2.59); no such association was seen in Hong Kong. Multivariate adjustment indicated older age was associated with buying live poultry in Guangzhou (OR = 2.91, 1.36–6.25). Guangzhou respondents perceived greater risk relative to Hong Kong. Buying live poultry was associated with perceptions of less "generalized" risk but more self/family risk. Higher generalized risk was associated with fewer live poultry purchases, suggesting generalized risk may be a useful indicator of precautionary HPAI risk behavior.  相似文献

Exposure to methylene chloride induces lung and liver cancers in mice. The mouse bioassay data have been used as the basis for several cancer risk assessments. (1,2) The results from epidemiologic studies of workers exposed to methylene chloride have been mixed with respect to demonstrating an increased cancer risk. The results from a negative epidemiologic study of Kodak workers have been used by two groups of investigators to test the predictions from the EPA risk assessment models.(3,4) These two groups used very different approaches to this problem, which resulted in opposite conclusions regarding the consistency between the animal model predictions and the Kodak study results. The results from the Kodak study are used to test the predictions from OSHA's multistage models of liver and lung cancer risk. Confidence intervals for the standardized mortality ratios (SMRs) from the Kodak study are compared with the predicted confidence intervals derived from OSHA's risk assessment models. Adjustments for the "healthy worker effect," differences in length of follow-up, and dosimetry between animals and humans were incorporated into these comparisons. Based on these comparisons, we conclude that the negative results from the Kodak study are not inconsistent with the predictions from OSHA's risk assessment model.  相似文献

Point source pollution is one of the main threats to regional environmental health. Based on a water quality model, a methodology to assess the regional risk of point source pollution is proposed. The assessment procedure includes five parts: (1) identifying risk source units and estimating source emissions using Monte Carlo algorithms; (2) observing hydrological and water quality data of the assessed area, and evaluating the selected water quality model; (3) screening out the assessment endpoints and analyzing receptor vulnerability with the Choquet fuzzy integral algorithm; (4) using the water quality model introduced in the second step to predict pollutant concentrations for various source emission scenarios and analyzing hazards of risk sources; and finally, (5) using the source hazard values and receptor vulnerability scores to estimate overall regional risk. The proposed method, based on the Water Quality Analysis Simulation Program (WASP), was applied in the region of the Taipu River, which is in the Taihu Basin, China. Results of source hazard and receptor vulnerability analysis allowed us to describe aquatic ecological, human health, and socioeconomic risks individually, and also integrated risks in the Taipu region, from a series of risk curves. Risk contributions of sources to receptors were ranked, and the spatial distribution of risk levels was presented. By changing the input conditions, we were able to estimate risks for a range of scenarios. Thus, the proposed procedure may also be used by decisionmakers for long‐term dynamic risk prediction.  相似文献
