Pre‐birth risk assessment is a process by which circumstances affecting an unborn child can be identified and support for mother and infant embedded. This mixed methods study describes a community‐based pre‐birth assessment and care pathway that utilised the Parents Under Pressure (PuP) programme to assess parenting capacity and provide support pre‐ and post‐birth for ‘at risk’ women. Sixty‐eight pregnant women referred to children's social care services were allocated to the pre‐birth assessment and care pathway (n = 35) or to routine care (n = 33). Standardised measures of psychological distress, social support and alcohol measured change for the women in the assessment and care pathway. Twenty women who provided pre‐ and post‐data reported significant improvements on all measures except alcohol use. Safeguarding outcomes at 12 months were obtained for both groups using administrative data. Forty‐two per cent of the infants whose mothers received the pre‐birth assessment and care pathway showed an improvement in child protection status compared to 14 per cent of the routine care infants. Safeguarding status deteriorated or stayed the same in 52 per cent of the routine cases compared to 26 per cent of those receiving the pathway. Qualitative data revealed that the pathway was acceptable and helpful to service users and service providers. Copyright © 2017 John Wiley & Sons, Ltd.
‘Describes a community‐based pre‐birth assessment and care pathway that utilised the Parents Under Pressure (PuP) programme’

Key Practitioner Messages

  • The current project found that a community‐based pre‐birth assessment and care pathway with high‐risk pregnant women was feasible and acceptable for practitioners and service users.
  • The pathway began mid‐pregnancy and support was provided following the birth of the infant for up to 12 months.
  • Over 40 per cent of infants whose mothers were allocated to the pre‐birth risk assessment pathway showed improvements in child safeguarding status at 12 months.

A total of 208 therapists and therapists‐in‐training were surveyed to better understand if clinical assessments and recommendations regarding “sex addiction” changed based upon a client's gender, relationship status, or exclusivity status. Using clinical vignettes, this study examined the clinical assessment and recommendations of licensed mental health providers (n = 92) and mental health providers in‐training (n = 116). Results indicated that professional ratings differed based on relationship exclusivity and gender; specifically, female nonmonogamous behavior was rated more negatively than male nonmonogamous behavior, while male monogamous behavior was rated more negatively than female monogamous behavior. Recommendations for treatment varied, with higher addiction ratings leading to greater odds of recommending individual, group, and community support over relational therapy. Clinical and training implications are discussed. Video abstract accessible by clicking here .  相似文献

We compared offspring of problem gamblers (n = 42) to offspring of parents without gambling problems (n = 100) to see (1) whether the two groups differed with respect to depressive feelings and conduct/antisociality problems and (2) whether ineffective parenting or the offspring’s own gambling problems played a mediating role in this context. Participants were drawn from a relatively large community-based study (N = 1,872). Parents rated their own gambling and other mental health problems when their children were in mid-adolescence. The children’s self-reports on depressive feelings and conduct/antisociality problems were assessed at two points in time: by mid-adolescence and again by early adulthood. Results showed that children of parents with gambling problems reported more depressive feelings and more conduct problems by mid-adolescence than children of parents without gambling problems. Children of problem gamblers also experienced an increase in their depressive symptoms from mid-adolescence to early adulthood. Importantly, ineffective parenting, but not children’s gambling problems, mediated almost all the links between parental problem gambling and children’s adjustment problems. These results add to a very small data base showing that children of problem gamblers are at risk for a variety of adjustment problems.

The purpose of this study was to evaluate the effectiveness of the Respected Parents & Respected Children (RPRC) parenting education program for mothers of preschool-aged children in South Korea. A total of 99 mothers were assigned to an intervention group (n = 54) or a control group (n = 45). Mothers in the intervention group received 16 weekly sessions of the parenting education program and then were compared with mothers who did not take part in the program. Questionnaires were used to assess parenting characteristics (i.e., maternal depression and anger, parenting behavior, co-parenting, parenting efficacy, and parenting stress) at baseline (pre-test), immediately after the termination of the program (post-test), and 8 months after the end of the program (follow-up test). Maternal rejection/restrictions, dissatisfaction, and parental distress of the intervention group decreased and efficacy as parents improved after taking part in the program, and these effects were sustained at follow-up. Our findings support the effectiveness of the RPRC parenting education program in changing parenting strategies and satisfaction and emphasize the need for preventive interventions for parents with young children.  相似文献

Interactions with parents build the foundation for infants' social–emotional development. This study investigated coregulation of the interaction and quality of relationship between mothers and their 6‐month‐old full‐term (= 43) and very low‐birthweight/preterm (VLBW/preterm; = 44; ages corrected for prematurity) infants. The objectives were to examine (1) how coregulation changed following a perturbed interaction, (2) how coregulation differed between full‐term and VLBW/preterm infant–mother dyads, and (3) the association between coregulation and relationship quality. Coregulation was coded using the Revised Relational Coding System (Fogel et al., 2003). Quality of the relationship was measured using the Emotional Availability scales (Biringen et al., 2014; Carter et al., 1998). Dyads participated in the Still‐Face (SF) procedure (Tronick et al., 1978) consisting of two natural and one SF period where mothers assumed a neutral expression, refraining from interacting with their infants. Following the SF period, dyads engaged in more symmetrical and more disruptive patterns of coregulation. While full‐term dyads engaged in more sequential‐symmetrical, VLBW/preterm dyads engaged in more resonant‐symmetrical coregulation. These results suggest that VLBW/preterm dyads may show more emotional reactivity in their interactions than full‐term dyads; however, in both groups infant responsiveness and parenting stress influenced the types of coregulation exhibited.  相似文献

Emotional relationships in infant–mother dyads in families where mothers provided full‐time childcare were compared with those of families where mothers used in‐home childcare providers and family childcare providers (= 245). Infant relationships with childcare providers were also studied. Emotional relationships were adequate in all three childcare arrangements, but infant–mother dyads in in‐home childcare arrangements displayed healthier emotional relationships than infant–mother dyads in mother care arrangements; no differences in the health of emotional relationships with infants emerged among the three types of childcare providers (mother care, in‐home childcare, family childcare). Infant–mother dyads in in‐home childcare arrangements also displayed healthier emotional relationships than infant–in‐home childcare caregiver dyads, but infant–mother and infant–caregiver dyads were comparable in family childcare families. Emotional relationships in infant–mother and infant–caregiver dyads were not correlated, regardless of the type of childcare.  相似文献

In this two‐wave longitudinal, daily diary study that followed up with 421 Mexican American parent–adolescent dyads (adolescents: Mage = 15 years, 50% males) after 1 year, we investigated the contingency between parental stressors and adolescents' emotional support to family members. Adolescents provided support to their parents and other family members at similar rates, but adolescents were more likely to provide support to other family members than to their parents on days when parents experienced a family stressor. This pattern was especially pronounced in families with parents who reported physical symptoms and adolescents with a strong sense of family obligation. Adolescents' provision of emotional support was associated with same‐day feelings of role fulfillment, but not to their concurrent or long‐term psychological distress.  相似文献

This study examined the bidirectional relations between authoritative parenting and adolescents’ prosocial behavior over a 1‐year time period. Data were taken from Time 2 and 3 of the Flourishing Families Project, and included reports from 319 two‐parent families with an adolescent child (M age of child at Time 2 = 12.34, SD = 1.06, 52% girls). Cross‐lag analyses supported bidirectional relations between parenting and prosocial behavior with particular emphasis on the role of the adolescents’ prosocial behavior on subsequent parenting. Results also varied as a function of the reporter. Discussion focuses on the implications for understanding the multifaceted nature of prosocial development in adolescence.  相似文献

This study examined associations between parents’ relationship conflict and parent–adolescent triangulation, and changes in adolescents’ perceptions of sibling affection and hostility. The goal was to learn whether conflict in parents’ relationships spills over to siblings’ relationships, or whether siblings compensate by becoming less hostile and more affectionate. Using a subsample (N = 400) from the Flourishing Families Project (FFP), we found a trend for mother–adolescent triangulation predicting an increase in sibling hostility across 2 years. Fathers’ reports of relationship conflict were related to increased levels of initial sibling hostility, but predicted a marginal decrease in hostility over time. Findings support increased understanding of parenting dynamics associated with changes in sibling relationship quality, and have the potential to inform clinical practice.  相似文献

This article is Part 1 of a two‐part series, in which we report on our evaluation of a mindfulness‐based relationship enhancement program for couples who are expecting their first child. In this mixed‐methods randomized clinical trial, we assigned 33 couples to the 4‐week Mindful Transition to Parenthood Program treatment group (= 16 couples) or to a waitlist control condition (= 17 couples). Men in the treatment group significantly improved in relationship satisfaction, mindfulness, and negative affect; women had no significant treatment effects. Small to large effect sizes were present for treatment group men and women in multiple areas. Mixed‐methods analyses demonstrated that this intervention may be especially helpful for men because of differences in social support needs, effects of program enrollment, and relational processes in the prenatal period. This study explored parenting styles and values internalization. Perceived parenting styles were measured in Grade 7 (n = 749) and Grade 12 (n = 468), and values were measured in Grade 12 (n = 271) and one year postschool (n = 291). We measured three aspects of valuing: priority (extrinsic, intrinsic importance); regulation (controlled, autonomous); and successful enactment of values (success). Mothers’ authoritative parenting in Grade 7 predicted increased importance and autonomous regulation of values one year postschool. Fathers’ authoritative parenting in Grade 7 predicted decreased importance of extrinsic values. Fathers’ permissive parenting in Grade 7 predicted decreased importance of intrinsic values. Authoritarian parenting in Grade 12 predicted more controlled values regulation postschool, particularly for extrinsic values. Parenting in early and late adolescence predicts values internalization in emerging adulthood.  相似文献

The primary goal of this study was to test how mother and adolescent proficiency in a common language moderates the link from parenting to adolescent development. A sample of Mexican‐origin fifth‐grade adolescents (= 674, 50% female) was measured longitudinally on self‐control and aggression. Mothers were rated on observed positive discipline, warmth, and harsh discipline. Positive discipline and warm parenting predicted increases in self‐control and decreases in aggression, but only among mother–adolescent dyads who were proficient in a common language. Harsh parenting predicted decreases in self‐control and increases in aggression, but only among dyads who were not proficient in a common language. Similar results were found in a conceptual replication among a second sample of 167 Mexican‐origin adolescents.  相似文献

Despite the challenges of early parenting, many adolescents navigate motherhood successfully, underscoring an overlooked heterogeneity among adolescent mothers. The present study used Latent Class Analysis (LCA) to identify subgroups of adolescent mothers (n = 704) enrolled in a randomized controlled trial (RCT) evaluation of a home visiting program for young parents. The model incorporated demographic and background characteristics, as well as indicators of psychological vulnerability. Analyses revealed four distinct subgroups: (a) non-Hispanic high vulnerability (n = 209, 30%); (b) Hispanic high vulnerability (n = 98, 14%); (c) non-Hispanic moderate vulnerability (n = 241, 34%); and (d) Hispanic moderate vulnerability (n = 156, 22%). Mothers in the two high vulnerability subgroups exhibited the poorest personal and parenting functioning outcomes measured approximately two years postpartum, particularly in terms of child maltreatment (non-Hispanic high vulnerability) and depressive symptoms (Hispanic high vulnerability). Analyses revealed positive effects of the home visiting program within specific latent classes on such outcomes as healthy baby at birth, high school or GED attainment, and repeat birth.  相似文献

Children of substance abusing parents are at heightened risk to develop problem behaviors, yet little is known about the co‐occurring patterns of internalizing and externalizing behaviors among this population. With 183 children (M age = 11.54 years, SD = 2.55, range 8–16) whose mothers were diagnosed with a substance use disorder, the current study identified subgroups/classes of children that were clinically distinct in their co‐occurring patterns of internalizing and externalizing behaviors, and examined how children in different clinical subgroups responded to a family systems intervention. Latent class analyses identified four classes of internalizing and externalizing behaviors: internalizing only, externalizing only, comorbid, and normative. Latent transition analyses showed that participation in family systems therapy resulted in an increased likelihood of transitioning from the externalizing class at baseline to the normative class at 18 months post‐baseline, and from the comorbid class at baseline to the internalizing class at 18 months post‐baseline as well. The findings support the effectiveness of family systems therapy in interrupting the stability of children's problem behaviors and improving children's behavioral outcomes.  相似文献

To address the combined importance of fathers and neighborhoods for adolescent adjustment, we examined whether associations between fathers' parenting and adolescents' problem behaviors were qualified by neighborhood adversity. We captured both mainstream (e.g., authoritative) and alternative (e.g., no‐nonsense, reduced involvement) parenting styles and examined parenting and neighborhood effects on changes over time in problem behaviors among a sample of Mexican‐origin father–adolescent dyads (= 462). Compared to their counterparts in low‐adversity neighborhoods, adolescents in high‐adversity neighborhoods experienced greater initial benefits from authoritative fathering, greater long‐term benefits from no‐nonsense fathering, and fewer costs associated with reduced involvement fathering. The combined influences of alternative paternal parenting styles and neighborhood adversity may set ethnic and racial minority adolescents on different developmental pathways to competence.  相似文献

The primary goal of this study was to examine the relationship between adolescents’ psychological functioning (as indicated by depressive symptoms) and substance use (alcohol and drug use) and membership in adolescent–parent communication trajectory subgroups in a large, diverse community sample of adolescents from the United States (= 1,057; 53% female; 51% Caucasian; Age: M = 16.15, SD = .75). Adolescents completed questionnaires at three annual assessments. Fit indices from parallel process growth mixture models suggested three dual trajectory classes: (1) Average communication with both parents (Average‐Both); (2) Good adolescent–mother and poor adolescent–father communication (Good‐Mom/Poor‐Dad); and (3) Poor adolescent–mother and good adolescent–father communication (Poor‐Mom/Good‐Dad). The trajectory classes differed by gender. In addition, psychological functioning and substance use were differentially related to the trajectory classes.  相似文献

This research investigates how children from an affluent city in the UK exercise agency to construct their economic worlds. A survey (n = 484) showed that children (ages 10 to 14 years) conduct a range of monetary and non‐monetary activities. A second study (n = 83) with diaries, self‐documentation, and parental questionnaires, showed that children's economic activity involves individual and social motivations, occurs in formal and informal contexts, and is often self‐regulated. Parents acknowledge children's monetary activities and neglect non‐monetary activities. The results suggest that children's agency unfolds in diverse economic activities shaped by contextual factors and interactions with peers and parents, thereby supporting a relational view of children's agency.  相似文献

In this study, we identified unique clusters of parenting behaviors based on parents' school involvement, community involvement, rule‐setting, and cognitive stimulation with data from the Panel Study of Income Dynamics—Child Development Supplement. In early (n=668) and middle adolescence (n=634), parents who provided high cognitive stimulation (i.e., cognitive enrichment parents) or engaged in all parenting behaviors (i.e., engaged parents) had the highest family income, parent education, and percentage of European Americans. Adolescents of cognitive enrichment or engaged parents often evidenced the highest academic and social adjustment. Adolescents whose parents set a large number of rules (i.e., “Rule setters”) or were also heavily involved in the community (i.e., the “Managers” cluster) had the lowest adjustment.  相似文献

This study assessed bidirectional relationships among supportive parenting (knowledge), negative parenting (permissiveness), and deviance in a sample (= 5,325) of poor, inner‐city African American youth from the Mobile Youth Survey over 4 years. Cross‐lagged path analysis provided evidence of significant bidirectional paths among parenting processes (knowledge and permissiveness) and deviance over time. Follow‐up multigroup tests provided only modest evidence of dissimilar relationships by sex and by developmental periods. The findings improve our understanding of developmental changes between parenting behaviors and deviance during adolescence and extend current research of the bidirectionality of parent and child relationships among inner‐city African American youth.  相似文献
