BackgroundWomen of refugee background may be particularly vulnerable to perinatal mental illness, possibly due to increased exposure to psychosocial stressors associated with their forced migration and post-resettlement adjustment.AimThis study aimed to compare psychosocial risk factors reported by women of refugee background receiving maternity services at a public hospital, to those reported by Australian-born women in the same hospital. It further aimed to examine the referrals offered, and accepted, by the women of refugee background reporting psychosocial risk factors for perinatal mental illness.MethodsA retrospective hospital record review was conducted to compare the antenatal and postnatal psychosocial risk factors of 100 women of refugee background and 100 Australian-born women who gave birth at a public hospital in Victoria between 1 July 2015 and 30 April 2016, and who had completed the Maternity Psychosocial Needs Assessment.FindingsWomen of refugee background were more likely than Australian-born women to report financial concerns and low social support at antenatal assessment, but were less likely to report prior mental health problems than Australian-born women at either assessment point. Both groups reported low rates of family violence compared to published prevalence rates. Of the women of refugee background assessed antenatally, 23% were offered referrals, with 52% take-up. Postnatally, 11.2% were offered referrals, with 93% take-up.Discussion/conclusionThis study showed elevated rates of psychosocial risk factors among women of refugee background, however, possible under-reporting of mental health problems and family violence raises questions regarding how to assess psychosocial risk factors with different cultural groups. Lower antenatal referral take-up suggests barriers to acceptance of referrals may exist during pregnancy.  相似文献

This paper examines the impact of unemployment on social problems in the post-war United States. Working within a conceptual framework derived from sociological and economic theory, dynamic macro social indicator models are constructed for four social problems-rates of suicide, homicide, divorce and alcoholism. In general, the results do not indicate a strong and consistent relationship between the unemployment rate and these social problems. High or increasing unemployment rates tend to raise the suicide rate, but lower the alcoholism rate and have no appreciable effect on the divorce rate. High levels of unemployment lower the homicide rate, but increases in unemployment tend to raise it. For all social problems except the divorce rate, the level of economic inequality has a consistently positive influence.  相似文献

In a national sample of the Italian population, surveyed four times between October 2002 and January 2007 (N = 2,008), we performed a multilevel longitudinal study aimed at predicting the increase in crime risk perception as a function of three families of independent variables, respectively lying at the within individual level (direct victimization and indirect victimization), at the between-individuals level (being a woman, being an older person, being a poorly educated person and size of area of residence) and at the ecological level (county’s crime rate, unemployment rate and immigration rate). Direct and indirect victimization, being a woman, being an older person, living in a large town and in a context characterized by high crime and unemployment rates positively influenced the change in crime risk perception, while the other individual and ecological predictors we used in our predictive model did not. Strengths, limitations, implications and future developments of this research are discussed.  相似文献

本文以6个省1586户农户的问卷调查结果为依据,运用序数Logistic模型,探讨农村公共事业、农民个人特征、家庭特征对农民生活满意度的影响。结果表明,农村公共事业发展对农民生活满意度影响显著,其中农村卫生事业发展对农民生活满意度的影响最大,其次是农村教育事业发展,农村文化事业发展对农民生活满意度的影响最小。根据分析结果,本文提出了以下政策建议:在今后的农村公共事业发展中,政府应渐进增加农村公共事业支出比重,不断调整农村公共事业支出结构,并随时更新文化娱乐活动内容,以适应广大农民的精神需要。  相似文献

This paper extends earlier research by Brenner and by Land and Felson on the specification and estimation of macrodynamic structural-equation models to explain changes in American mortality indexes as a function of exogenous changes in societal conditions (social, demographic, economic, and health care). After reviewing the record of annual changes in several general and cause — specific mortality indexes for the post-World War II United States, patterns of temporal covariation in the indexes are discussed and some tentative structural-equation models are described. Among other findings, these models indicate: (1) that changes in the age structure of the American population have substantial impacts on changes in mortality rates for diseases of the respiratory and circulatory systems as well as on deaths due to cirrhosis of the liver, accidents, and violence; (2) that the infective and parasitic diseases mortality rate is more closely related to per capita public health expenditures than to improvements in the general level of living in this post — war period; (3) that the business cycle, as indexed by the unemployment rate, has significant impacts on the cardiovascular, accident, and violence mortality rates; (4) that Brenner's finding of a positive association of an increase in the unemployment rate with an increase in cardiovascular diseases mortality two to three years later is partially mediated by an increase in per capita cigarette consumption during the economic recovery following a recession; (5) that an increase in the per capita level of cigarette consumption increases the respiratory systems mortality rate; (6) that both the general and the respiratory neoplasms mortality rates are more strongly affected by long-term moving averages of annual per capita levels of cigarette consumption than by single-years levels; (7) that the level of the degenerative diseases mortality rate is positively affected by an increase in per capita liquor consumption and negatively affected by an increase in health care utilization; (8) that the percentage of all vehicles traveling on highways at high speed is the exposure index most closely associated (of several that were studied) to the motor vehicle accident mortality rate; (9) that the levels of the maternal and infant mortality rates are positively related to an increase in the fertility rate (exposure) and negatively related to those advances in health care services associated with hospital births; (10) that the accuracy with which short-term changes in the crude mortality rate can be predicted from a knowledge of cause-specific mortality rates and how the latter are affected by societal conditions is effectively limited by the degree of accuracy of predictions of the respiratory diseases mortality rate because of its volatile influenza, pneumonia, and bronchitis component; and (11) that short-term changes in the life expectancy index can be somewhat more accurately predicted from such knowledge. Although most of these relationships have been noted before in mortality studies, only a small fraction have been studied in a macrodynamic structural-equation models context. These findings thus constitute a baseline of statistical evidence which can be explored in future research.  相似文献

Deadly violence has spread throughout Mexico, affecting the well-being of citizens. What is the impact of this violence on the daily lives of Mexican adults? Building upon the stress process model, we used a mixed-methods approach to examine relationships between multiple indicators of exposure to and fear of violence and four diagnosed mental health outcomes, as well as self-rated mental health, in a Mexican community using the Survey of Health and Mexican Migration (456 surveys; 49 interviews). The multivariate models provide evidence that perceptions of insecurity are associated with diagnosed depressive episode, agoraphobia, alcohol abuse, a total count of mental health conditions, and poor self-rated mental health. Past victimization is associated with anxiety. Stress and coping behaviors did not formally mediate these violence–mental illness associations. The qualitative results confirm that residents fear violence and cope by adjusting their personal behaviors. These results foreshadow the emergence of mental health conditions as a critical public health concern for Mexicans living under the threat of violence.  相似文献

In- and out-migration rates of 56 Israeli cities with a population of over 5000 were predicted for a five-year period (1977–1981), on the basis of four 1976 social indicators: crime rate, percent unemployment, population size and distance from a major metropolitan center. Rather than employ independent correlation coefficients, the four indicators were entered as predictors into regression equations with in- and out-migration rates serving as the dependent variable. The analytic methods are compared and the clear advantages of the regression method emerge. None of the indicators reliably predicted in-migration. Crime consistently predicted out-migration. Distance and unemployment each entered into three of the prediction models.  相似文献

A negative effect of unemployment on subjective well-being has been demonstrated in many studies casting substantial doubt about assumptions of decisions of individuals to choose unemployment voluntarily as the utility-maximising option. These studies have been extended to take into account national-level context factors which have been shown to moderate the relationship between unemployment and life-satisfaction. So far most studies focussed mainly on economic indicators, although demographic and cultural differences between countries also affect how unemployment is perceived. An important variable that is not included in the majority of proper multilevel studies is the extent of unemployment benefits. Traditional micro-economic approaches argue that more extensive provisions should reduce the cost of unemployment and therefore reduce the motivation to regain employment—reflected in a reduction of the negative impact of unemployment. This study investigates this claim by using European Values Study data from all European Union countries and Norway as well as harmonised macroeconomic statistics from Eurostat. It finds that the effect of unemployment on life-satisfaction is indeed moderated by economic and demographic national-level factors, but not by unemployment benefits. To what extent unemployment reduces life-satisfaction varies greatly between countries, but appears to not be influenced by the extent of state unemployment provisions.  相似文献

A social indicators model which included demographic variables (birth rate and aged child ratio), economic variable (unemployment) and marriage rates (control variable) was utilized in explaining changes in divorce rates over time. Analysis of time series data for two distinctive periods in the U.S.A. (1920–1940 and 1946–1969) revealed the following significant results:
  1. Demographic changes in the age structure of the population while controlling for marriage rates explains significantly larger amounts of the variance in marital instability than economic changes alone (unemployment).
  2. In the post-World War II era unemployment was not found to be a statistically significant predictor of marital instability when control for demographic changes were established.
  3. In the post-World War II era marriage rates appear to be a significant predictor of divorce rates when control for demographic changes were maintained.
  4. In the post-World War II era marriage rates were found to be more sensitive to changes in economic conditions than marital instability (divorce rate).

The goal of the study is to investigate whether positive mental health complements mental illness within a theoretically informed (the dual-continua model) and psychometrically tested (the Mental Health Continuum-Short Form) framework. National-level, population-based data from the 2012 Canadian Community Health Survey on Mental Health (CCHS-MH) was used, with comparisons between sexual minority and heterosexual adults. Results show that gay, lesbian, and bisexual Canadians have substantially lower rates of positive mental health and are more likely to have been diagnosed with a mental illness, with the disparities between health and illness being the most pronounced among lesbians and bisexual females. Results show considerable support for the dual-continua model, which posits that the absence of health does not automatically translate into the presence of illness, and vice versa. Suggestions are made for practitioners and researchers toward the use of the dual-continua model as a surveillance tool, especially among sexual minority individuals.  相似文献

While U.S. unemployment rates remain low, rates of job loss are high and rising. Job loss is also becoming increasingly common in more advantaged, white-collar occupations. This article is concerned with how these patterns impact the health of U.S. workers. Drawing on recent data from the U.S. Panel Study of Income Dynamics, I find that job loss harms health, beyond sicker people being more likely to lose their jobs. Respondents who lost jobs but were reemployed at the survey faced an increased risk of developing new health conditions; they were not, however, more likely to describe their health in negative terms. This suggests that recent job “churning” within the United States (i.e., high rates of job loss but low unemployment) may impact certain health outcomes but not others. I find no evidence that the health consequences of job loss differ across white- and blue-collar occupations, although health-related selection out of jobs appears stronger within the blue-collar category.  相似文献

The inter- and intra-state migration of American families with work-disabled members is a neglected area of empirical study. Longitudinal migration and health status data from the 1996 Panel of the Survey of Income and Program Participation (SIPP) are merged with state-level welfare policy indicators to investigate migration behavior under welfare reform’s emphasis on requiring work and encouraging reliance on social support networks. We use a nested discrete-choice event history model that incorporates the departure decision and interstate destination choice in a single model that tests the effects of state-level welfare policy and economic opportunity characteristics, with state fixed effects, plus family sociodemographic characteristics and social networks, as the basis for comparing migration of families with and without work disabilities. The results show that although families with disabilities and illnesses are less likely to migrate than other families generally, they are “pushed” to migrate if they live in states that do not exempt them from TANF activities requirements. Furthermore, in-migration is inhibited by stringent state welfare illness exemption rules and high state unemployment rates. Intrastate migration is more likely among families who received family and community social support, regardless of work-disability status.  相似文献

Hundreds of thousands of Louisiana citizens were displaced from their homes as a result of Hurricanes Katrina and Rita. Of those displaced within Louisiana some relocated to other parishes, some to other residences within the same parish, and others were able to return to their pre-storm residence. This article draws upon data gathered by the 2006 Louisiana Health and Population Survey to examine the social costs of displacement across 18 Louisiana parishes approximately 1 year after the hurricanes. Specifically, we examine how displacement affected housing, economic, and health outcomes for individuals and families. Further, we compare the implications of two types of displacement (1) internal displacement—within-parish relocation versus (2) external displacement—relocation across parish lines. We found that the displaced had lower odds of owning their homes, living in detached housing, and retaining access to primary health care facilities. The displaced were also more likely to be unemployed and exhibit symptoms consistent with severe mental illness. The externally displaced suffered income declines. These trends are critically important for understanding both the short- and long-term ramifications of displacement after disaster. Our findings have implications for theories, policy makers, and planners considering the larger social costs of disaster and large-scale displacement.  相似文献

Human history observed significant changes in life expectancy over the last three and half decades (1960–1995). This study attempts to explore the determinant factors behind improvements in life expectancy in most of the countries in the Post World War II period. In this context the study estimates a double logarithmic convergence type model where the dependent variable is relative change of life over the period and the explanatory variables are initial per capita real gross domestic product, initial per capita calorie intake as percentage of requirement, adult literacy rate at the initial point of time, and initial physician per thousand of population. The study estimates different variants of the basic model. Female population has been treated separately. All estimates have been offered for three broad income groups low, middle and high. An attempt is also made to take into account the effects of some public health measures such as access to safe drinking water and access to sanitation.  相似文献

The new forms of atypical contracts increasingly diffused beside standard permanent full-time employment has been argued being detrimental for workers’ mental health. Despite a growing body of studies is now appearing on the topic, they generally fail to recognize that atypical workers represent a heterogeneous group. This study addresses such oversight for Italy by scrutinizing the association between four major domains of mental health—vitality, social functioning, role emotional, and general mental health—and six types of atypical contract—temporary, casual, part-time by choice permanent, part-time by choice temporary, not chosen part-time permanent, and not chosen part-time temporary. First, we find that mental health is compromised by atypical working arrangements depending on the specific atypical contract considered. Second, we verify that the choice of the atypical experience is relevant in shaping the relationship with mental health (a novelty for Italy). Third, we prove that, regardless the type of contract, variations across mental health outcomes exist. We conclude that more reflection is needed when designing studies on atypical works and their consequences on workers’ well-being.  相似文献

The 2009 economic crisis in Greece had harsh implications on the Greek society. The crisis led to high and increasing unemployment rates, which previously found to relate to increases in suicide rates, health risking behaviors and increased risk of mental health problems. The public’s perceptions regarding the consequences of the crisis are crucial to their recovery. In order to get a comprehensive and thorough view of the public’s perceptions, which will allow to draw conclusions and recommendations for a possible effective intervention program, the current study assessed the perceptions of a representative sample of the Greek adult population (N = 3002) in three sampling waves: December 2011, March 2011 and November 2012. These assessments included questionnaires designed to evaluate perceptions of current quality of life, economic decline, coping with economic and occupational commitments, physical and psychological health, wellbeing, health related behaviors, attitudes towards emigration, sources of support and political and social trust. Findings showed significantly higher perceptions of financial uncertainty and difficulties coping with economic commitments. Participants generally rated their psychological health lower than their physical health. Trust ratings were also found to be generally low, especially for younger populations. Younger populations also showed a more willing attitude towards emigrating out of Greece, compared to older populations. Coping measurements revealed a strong possible tendency to go back to the traditional ties of confidence and trust in family and friends as sources of support. In light of the results, we recommend to build programs that see the family and the local community as a resources of resilience and coping.  相似文献

The way public policies are managed in Brazil has changed since the 1988 Federal Constitution. This study aimed to identify how changes in the structure of public expenditure composition at Brazilian federal states influenced local human development in these states. The states’ public expenditures were categorized according to their nature as spending indices whereas human development was measured through a human development index (HD). To verify the relationships between these variables, an accounting-social theoretical model was created and estimated through latent growth modeling (LGM). The LGM measurement period comprised five administration cycles of the Brazilian states (1988–2011). Variables were measured on data of the second year of state government term; their mean initial values and growth rates were recorded. Results show that, influenced by policies of centralized regulation promoted by the federal government, only the social spending growth rates had statistically significant effect on the human development growth rate, although not considered of great magnitude. Among mean initial values, the most significant was that of minimum spending (SIm), which denotes the direct impact of the Fiscal Accountability Law [Lei de Responsabilidade Fiscal, in Portuguese] on human development improvements. The mean initial value of economic spending also showed a positive and significant effect on HD growth rate.  相似文献

Researchers have provided clear evidence that the fear of crime can lead to various mental health-related issues including anxiety and psychological distress. However, studies on the effects of fear of crime on physical health are limited. Adding to and extending this literature, we evaluated the association between fear of crime and mental and physical health outcomes in a new setting (New Zealand) and at a national scale. As an added contribution to the literature, we examined whether the fear of crime is independently associated with mental and physical wellbeing, regardless of neighbourhood crime rates. Using data from the New Zealand General Social Survey, the 2006 census and the New Zealand Police, we fitted linear and two-level hierarchical linear models regression models to assess the impact of fear of crime on mental and physical health, at varying stages of individual and area-level confounder adjustment. Even after adjusting for a number of individual- and area-level factors that are related to social inequalities in health in the country, a significant effect of increased fear of crime on lower mental and physical wellbeing was detected. We did not, however, detect significant independent effects for neighbourhood crime rates for either outcome. Our findings indicate that fear of crime, rather than recorded crime rates, was associated with detrimental mental and physical health outcomes. As such, efforts to not only reduce crime but perceived risk of crime could yield public health and social wellbeing benefits.  相似文献

This paper performs an assessment and imputation of the benefits produced to the individuals and households well-being by the public services provision. This analysis is very relevant in the current framework of economic crisis of the European economies. Although most of the papers in the literature compare countries, decentralization processes are included in this paper by accounting the effect of public expenditure on regional health care and education upon the levels of inequality and rates of poverty in each Spanish region. This case is relevant because of the high decentralization degree of the Spanish political structure and the persistent regional disparities. The results show a clear downward tendency in both phenomena following imputation, although education shows a greater impact. Furthermore, a decomposition, used in this context for the first time, of the effects shows the very relevant redistributive effect of this public expenditure.  相似文献
