Elder abuse and neglect is a critical health care issue that must be brought to the attention of health care providers and older adults' family members. Adults older than 65 who live at home or in long-term care facilities may be at risk for abuse. Nurses should be aware of the causes, screening questions, symptoms of abuse, and resources in the community. Armed with information and a better understanding about the issue, nurses can minimize the devastating effects of abuse on older adults and their families. 相似文献

This study aims to investigate the prevalence of elder abuse and neglect in an urban Chinese population. A cross-sectional study was performed in a major urban medical center in NanJing, China. A total of 412 participants completed the survey and 145 (35%) participants screened positive for elder abuse and neglect. The mean age of the victims was 69 years and 59% were male. Caregiver neglect was the most common form of abuse, followed by financial exploitation, psychological abuse, physical abuse, sexual abuse, and abandonment. Thirty-six percent of the victims suffered multiple forms of abuse and neglect. In the logistical regression analyses of the data, female gender, lower education and lower income were demographic risk factors associated with elder abuse and neglect. A better understanding of these and additional risk factors associated with elder abuse and neglect in older Chinese people is needed. 相似文献

Online training for care managers to identify, report, and prevent abuse, neglect, and exploitation was developed for a demonstration involving the dually eligible Medicare and Medicaid population. It was composed of three modules covering background, screening, and reporting abuse. Of 453 enrollees, 273 completed at least one module and 212 completed all three. Pre- and post-training surveys for each module were used to examine changes in the proportion of correct answers for each question, using the related-samples Cochran’s Q statistic. Improvements in knowledge from pre- to post-training were evident in modules covering background on abuse and reporting abuse, but not in the module about communication principles and screening for abuse. Its content may have already been familiar to the trainees, who were primarily social workers and nurses. Lessons learned are being used to adapt the training for a wider audience to increase public awareness of abuse. 相似文献

The legacy of the brutality of apartheid in South Africa is a violent social context characterized by high levels of unemployment, extremes of wealth and poverty, continuing racism, the easy availability of guns and patriarchal values and behaviours. Violent crime is widespread in South Africa and schools in disadvantaged areas suffer from serious problems of gang-related crime. This article discusses a pilot project with a cluster of three schools in Durban which investigated whether intervention and training coupled with mutual support between cooperating schools, and between schools and the police, can reduce incidents of crime and violence. The idea behind the project was to see whether a small-scale, simple and inexpensive intervention could help to improve security in a relatively short time span. The article describes the nature of the project, evaluates its outcomes and discusses emergent issues for South African education. The overall conclusion stemming from the project is that South African schools are not helpless in the face of an onslaught of crime and violence. While there is no complete and foolproof solution to the problem, schools can nevertheless be helped to find simple, relatively cheap and practical measures to reduce crime and violence. 相似文献

In a country in which human rights feature prominently in our discourse about who we are, as well as in the South African constitutional and legal framework, so many wrongs continue to be done to children. One category of wrongs is abuse, but it is not the only one. Poverty, patriarchy and gender violence, as well as the socialised obedience, dependency and silence of women and children, create conditions in which abuse can occur, often with few consequences. South Africa has extremely high rates of both physical and sexual abuse of children. Progressive, rights‐based legislation exists to protect children, but it is not adequately supported or resourced by services to fulfil their provisions. Child abuse and neglect will not be significantly reduced in South Africa, without simultaneous improvements in the social and economic conditions in which very large numbers of children live. Post-apartheid South Africa is characterized by growing feelings of pain, anger and frustration amongst black communities triggered by pervasive social inequalities. This has given birth to a new form of political and social activism shaped by crude violence, vandalism, destruction, brutal killings of women and children as well as thuggery in different black communities. It has also led to an upsurge in violence particularly on Africans from other parts of the continent. In this article, I attempt to examine how racial politics and resilient white privilege intersect to trigger afrophobic violence in South Africa. I draw on existing literature on broad conceptions of race and xenophobia to make a set of assertions about racial valuations, the resilience of white supremacy and black on black violence. In the article, I argue that black South Africans' pain, anger and the performance of violence on African migrants are on one level a consequence of resilient structural racism and racial practices, which continue to marginalize, emasculate and dispossess blacks. These racial practices force black South Africans to look elsewhere to express their anger, pains and frustrations. 相似文献

It is claimed that many incidences of children running away from home and residential care involve issues around violence, bullying and child abuse and neglect. Surveys show that between 14 and 20% of children run away from home for at least one night and that 2% run away 10 times or more from home or a care setting. The potential reasons for running away are discussed together with the increased possibility for further victimization, delinquency and criminal behaviour. Four ‘streetwork projects’ that provide information and support to young runaways in the UK are described and their limitations discussed. It is proposed that counselling should be a part of intervention with street children and that trained counsellors/psychotherapists should be available free to these young people with social and emotional difficulties outside the usual health/social service provision. © 1998 John Wiley & Sons, Ltd. 相似文献

Despite the rapid growth of the elderly African American population in the U.S., elder abuse and neglect in African American families continue to be underdeveloped areas of study. This article presents an ecological and culturally informed framework for the study of elder abuse in African American populations. The model was developed based on Bronfenbrenner's Human Ecological Theory. The model identifies risk factors associated with different systems that have an influence on the lives of African American families. Cultural protective factors also are identified in the model. The model is intended to provide an understanding of elder abuse and neglect in African American families by considering the influence of contextual factors such as the legacy of slavery, social exclusion, and structural segregation and racism. Specific suggestions for practice are proposed according to cultural strengths of African American communities as well as the ecological premises of the model. 相似文献

Exposure to violence is common in South Africa. Yet, few studies examine how violence exposure contributes to South African adolescents' participation in youth violence. The aims of this study were to examine effects of different violence exposures on violent attitudes and behavior, to test whether attitudes mediated effects of violence exposures on violent behavior, and to test whether adult involvement had protective or promotive effects. Questionnaires were administered to 424 Zulu adolescents in township high schools around Durban, South Africa. Structural equation modeling (SEM) was used to test associations among violence exposures and both violent attitudes and behavior. Victimization, witnessing violence, and friends' violent behavior contributed directly to violent behavior. Only family conflict and friends' violence influenced violent attitudes. Attitudes mediated effects of friends' violence on violent behavior. Multiple-group SEM indicated that adult involvement fit a protective model of resilience. These findings are discussed regarding their implications for prevention. 相似文献

The purpose of this research was to provide a systematic review of, and assign an evidence grade to, the research articles on elder abuse. Sixteen health care and criminal justice literature databases were searched. Publications were reviewed by at least two independent readers who graded each from A (evidence of well-designed meta-analysis) to D (evidence from expert opinion or multiple case reports) on the quality of the evidence gained from the research. Of 6,676 titles identified in the search, 1,700 publications met inclusion criteria; omitting duplicates, 590 publications were annotated and graded. 相似文献

ABSTRACT The problem of elder abuse and neglect in South Africa is widespread, but the definition of abuse remains problematic. An expanded typology is needed to classify certain types of abuse reported commonly, such as marginalization, disrespect, exploitation and violence. The history of elder abuse and responses is traced, relevant legislation and selected intervention programs are reviewed, and research and the status of elder abuse are evaluated. Interventions to address sociostructural factors that impact elders' settings, increase their vulnerability, and diminish their ability to realize human rights can contribute to an improvement in their condition and a reduction in abuse and rights violations in the long-term. 相似文献

This article draws on research conducted in Kenya, South Africa, and Zimbabwe that focused on violence in the context of political transition. The paper examines the relation between political transition and sexual and gender-based violence in the three countries. The paper argues that it is critical to recognise sexual and gender-based violence as bound to systemic gendered inequality if such forms of violence are to be addressed and mitigated when periods of violent conflict end. 相似文献

This study examined the prevalence of and risk factors for intimate partner physical violence against women. Interviews were conducted with a sample of 1,378 men working in Cape Town municipalities. An average of 42.3% (95% CI: 39.6, 44.8) reported physical violence against a partner of the last 10 years, and 8.8% (95% CI: 7.3, 10.3) reported physical violence in the past year. After adjustment for age, occupational group, and race, the factors associated with use of violence against partners of the last 10 years were having no post-school training (OR = 2.10), witnessing parental violence in childhood (OR = 1.87), involvement in fights at work (OR = 2.73) and in communities (OR = 1.54), drug use (OR = 1.99), problem alcohol use (OR = 1.98), perceiving hitting women to be acceptable (OR = 4.54), frequent conflict (OR = 2.40), women's alcohol use (OR = 2.25), conflict about sex (OR = 2.16), and conflict about his infidelity (OR = 2.81). The study shows that ideas supportive of gender inequality and normative use of violence in different settings are major underlying factors for men's violence against partners. 相似文献

Hundreds of thousands of children are being abused and neglected in the United States annually. The structures devised to respond to this abuse and neglect have been unsuccessful. The United States government continues to spend billions of dollars on programs to deal with the effects of these unsuccessful responses. Child abuse and neglect in the United States now represent a national emergency. In this article, we will provide a historical, cultural, and contemporary perspective on child abuse and neglect. A review of child welfare research literature reveals used to investigate and intervene in child abuse and neglect cases, the field does not know enough about their structural variations, implementation processes, or effectiveness. Moreover, although articles advocating multidisciplinary teams enumerate their apparent strengths, they lack attention to the teams' possible weaknesses. The paper presents a short overview of the demographic scene, reflecting population characteristics and needs that impact care giving as well as elder abuse and neglect. The developments of scientific knowledge and its accumulation, especially the empirical data from the first national survey on elder abuse and neglect are discussed. Further, legislative developments relating to four generational laws and the advancement of policies and innovative practice experiences are described and analyzed. The voices of elderly people from marginalized groups are rarely solicited, and the relationship between elder maltreatment and belonging to an oppressed group has not been adequately investigated. This article reviews the literature on oppression and elder abuse and describes findings from the secondary analysis of data from focus group discussions on elder abuse held with marginalized older adults and (quasi)professionals caring for them in two Canadian cities. Participants identified that increased vulnerability to elder abuse was related to oppression experienced as a consequence of ageism, sexism, ableism/disability, racism, heterosexism/homophobia, classism, and various intersecting types of oppression. 相似文献
