Treatment of the mother in the incest family: The beginning phase

This paper discusses significant counter-transference issues and presents a conceptual framework for ordering the strategies in treating the mother in the incest family. The development of empathy is suggested as the primary engagement tool, and obstacles to its development are analyzed.The major theoretical construct that is employed in formulating the treatment approach is that of the traumagenic dynamics model presented by Finkelhor and Browne (1985). The manner in which this framework can be employed in assessment and prioritizing treatment goals is outlined. Specific approaches for the beginning phase of work with the mother are then described.     

Feminist and queer criminology: A vital place for theorizing LGBTQ youth

Creating safe environments for lesbian, gay, bisexual, transgender, and queer and/or questioning (LGBTQ) youth has become a significant public health concern. Despite the disparities in the risk factors associated with identifying as a member of the LGBTQ community, the theoretical frameworks for understanding these issues, particularly within criminology, have been limited. The purpose of this work is to review the historical treatment of gender and sexual orientation within criminology. More specifically, this work reviews how gender was introduced into criminology through the work of feminist criminology, followed by the need for theorization that further analyzed sexual orientation. This then became the focus of queer criminology. After tracing the historical development of gender and sexual orientation, the author uses the theoretical foundations of both to show how it applies to recognizing the issues faced by LGBTQ youth. Finally, the author discusses why consideration of these frameworks is paramount in understanding the issues faced by LGBTQ youth, as well as how such consideration and understanding can help mend the existing gap in providing safer environments for LGBTQ youth.     

Sex makes a difference—How gender affects the therapeutic relationship

Because gender issues create obstacles or advance progress, this paper discusses how gender differences affect therapy. The psychoanalytic approach in the four treatment dyads of female therapist-male patient, male therapist-female patient, female therapist-female patient, and male therapist-male patient is presented to suggest propensities. A developmental focus on gender's effect on the therapeutic relationship is indicated by the theoretical rationale from psychoanalytic literature, and by clinical practice discoveries. Because therapists' perceptions about gender identity, gender roles and sexual partner orientation affect responses to patients, countertransference is considered. Acknowledgement of gender components within the transference is recommended if therapists are to provide the best treatment possible.     

Child and adolescent bereavement

Clinical social workers in non-medical settings often are presented with child and adolescent clinets who are bereaved, or with adults who experienced the loss of a significant other in childhood or adolescence. This article attempts to pull together the most relevant and significant issues relating to loss in childhood and adolescence. It provides an overview of the theoretical and treatment issues that inform practice with clients who have sustained a significant loss during the pre-adult years. Case material provides an illustration of some of the issues as they appear in clinical practice.     

The chronically traumatized child

This article discusses the effects of chronic traumatization on children, and its various manifestations in children. It further explains the typical symptom picture, and elaborates treatment issues unique to working with the effects of chronic trauma. Legal issues surrounding psychotherapy with minor clients

This article discusses several legal issues of concern to psychotherapists working with minor clients such as informed consent to treatment, confidentiality, and the minor's legal capacity to enter the psychotherapist-client relationship. Suggestions are offered to assist the clinician in dealing with these legal issues.     

Therapist pregnancy and maternity leave: Maintaining and furthering therapeutic gains in the interim

A therapist's pregnancy and maternity leave is a time of multiple transitions for both patient and therapist. This article reviews the literature on therapist pregnancy and its therapeutic implications. It discusses the treatment planning choices of: trial termination, therapeutic hiatus, and temporary transfer. The article further addresses the clinical and administrative issues of planning the maternity leave as well as the issues of time-limited treatment with the interim therapist. It included illustrative case examples based on the author's experience as an interim therapist during a six month maternity leave.This paper in its original form was presented as part of the requirement for The Hunter College School of Social Work: Post Masters Program in advanced Clinical Social Work.     

A MODEL FOR TRAINING PRACTICING PROFESSIONALS IN FAMILY THERAPY*

This paper offers a model for high quality training in family therapy. The unique aspects of the Family Consortium's model are that it: (a) is aimed at practicing professionals; (b) emphasizes structural family therapy principles with strategic and experiential elements; (c) is geared secondarily toward training members as effective peer consultants and trainers; and (d) uses an awareness of the isomorphic nature of training, therapy and family transactions. Part I describes the practical organization structure of the Consortium. Part II discusses the current theoretical philosophy and training approach of this evolving group. Examples are given of how the Consortium has dealt with specific training issues.     

Research note The study of sensitive subjects

This note examines some research issues raised by studies of sensitive topics and the disclosure of highly personal and confidential information. The first part focuses on the research method of in-depth interviewing and covers some theoretical and methodological problems associated with the method, together with a discussion of some strategies adopted by researchers and respondents in the interview setting. The second part discusses some of the hazards of this type of research and argues for the need to protect respondents and researchers alike. These issues are discussed with reference to the author's own fieldwork experiences of three research projects.     

To Integrate or Not to Integrate Dialectical Behaviour Therapy with Other Therapy Approaches?

With mounting evidence for its efficacy in the treatment of borderline personality disorder and related problems and increasing dissemination nationally and internationally, front line clinicians in practice settings are increasingly aware of dialectical behavior therapy (DBT). Indeed, it is likely that this treatment is reaching individuals who practice from a variety of theoretical frameworks, such as psychodynamic, humanistic, cognitive, or some combination thereof. Therefore, this paper considers the ways in which DBT is an integrative approach, as well as the issues that may arise in the integration of DBT with other theoretical frameworks or with practices spawned by these alternative frameworks. We recommend that therapists practicing DBT adhere to the behavioral and dialectical theoretical foundations of the approach, in order to provide therapy that is effective, coherent, and consistent.     

Racial Profiling/Biased Policing

Media and legal attention to the issue of racial profiling in the United States intensified in the 1990s, with numerous studies from a wide range of jurisdictions revealing evidence of biased policing. This article briefly examines the history of biased policing in the United States, discusses the results of some of the more recent studies as well as studies of biased policing in other countries, and also devotes considerable attention to important theoretical and methodological issues in conducting research on racial profiling. These issues include how race/ethnicity should be coded, how to establish an appropriate denominator/benchmark in racial profiling studies, and what factors in addition to race/ethnicity need to be considered in analyses of bias in citations/arrests/and searches. Although attention to racial profiling waned somewhat after the September 11, 2001, attacks, recent events suggest a renewed interest in this problem.     

The nature of AIDS and babies

The rapidly growing menace to young children, Pediatric AIDS, is generally unrecognized, under-reported and too often misunderstood. This paper discusses the nature of AIDS with particular emphasis on, the symptoms, modes of transmission and the various treatment issues.     

Treatment issues pertaining to pathological gamblers with a comorbid disorder

Evidence of an increased risk for various psychiatric disorders among pathological gamblers far exceeds our understanding of the impact that this psychiatric comorbidity has on the outcome of treatment for pathological gambling. One major source of the problem is that treatment efficacy and effectiveness studies for pathological gambling typically have not addressed comorbidity's impact on outcome. This paper discusses epidemiological, clinical, health service delivery, and research issues pertaining to the intersection of pathological gambling treatment outcome and comorbid psychiatric disorders. It is argued that this topic suffers from major knowledge gaps in terms of the nature of comorbidity of pathological gambling and other psychiatric disorders and the role of client characteristics on treatment outcome for pathological gambling. Research priorities are identified.     

From an Agrarian to an Environmental,Food, and Natural Resource Base for Agricultural Policy: Some Reflections on the Case of the EC

Abstract The agriculture of the European Community (EC) has experienced significant changes in the last decade. From a situation of deficiency in agricultural and food production, the twelve-nation community has shifted to a situation of food overproduction. This change has also been characterized by a rapid decrease in the agricultural labor force and a decrease of its importance in the employment structure which has manifested itself in fewer, more efficient farms. This alteration of structural conditions has led to the emergence of a new set of agricultural issues. This article discusses these changes by underscoring the decline of agrarian-based issues and the emergence of environmental, food, and natural resource-based issues. It is maintained that EC agricultural policies have been instrumental in both the resolution of agrarian-based issues and in the creation of new problems embodied in the environmental, food, and natural resource-based policies. The combination of the demise of agrarian-based policies and the emergence of environmental, food, and natural resource-based policies is also employed as a theoretical perspective from which the evolution of the agricultural sector in the EC can be interpreted.     

Community and urban movements

Empirically and theoretically, this paper considers the link between the urban community, as a social base, and the emergence of urban movements. It examines whether different urban communities lead to different urban movements, specifically whether cohesive communities lead to powerful movements. Using an Australian case study, the first part of the paper tests this relationship empirically. The findings given raise doubts about whether cohesiveness is a necessary pre-condition for widespread mobilisation, as well as questioning the theoretical validity of ‘urban community’and ‘community’as sociological concepts. There is an apparent need to discard these concepts when referring to contemporary Western societies and a concomitant need to develop concepts which more appropriately identify the social organisation of urban households and residential areas. The second part of the paper discusses these theoretical issues. It presents an argument on the way household and residential organisations change as the city changes and how they differ if urbanisation differs between regions and between nations. This argument provides a more appropriate framework for examining the nature of the relationship between social bases and urban movements and it contributes a more useful explanation for the case study.

SOLUTION-FOCUSED THERAPY IN A PSYCHIATRIC HOSPITAL

This paper presents the authors' use of solution-focused brief therpay in an adult inpatient psychiatric hospital setting. we suggest that a theoretical inteeegration of the medical model and the solution-focused approach provides a useful treatment modality. We discuss implementation of the model, emphasizing assessment, treatment milieu, the role of staff, therapy, and discharge issues. We also describe the model's usefulness for training and supervision and offer recommendations for future research.     

Social Policy Practice

This paper identifies and discusses issues for a school of social work in the context of collective bargaining. Attention is given to the implications of collective bargaining on a professional social work program. The paper is based on a theoretical framework and an experience of approximately three years in the advent and development of collective bargaining in a large midwestern public university. Major substantive areas discussed have to do with intragroup and intergroup relations among students, community professionals, faculty and administration; the definition of “work” and “work load”; and the reward system for promotion, tenure, and recognition of merit. Major implications relate to the concerns of professionalism, the balancing of. democratic participation and responsible management, and the codification in contract of social work's historical role in participatory decision making.     

Out from under: Countertransference in the treatment of the masochistic youngster

The masochistic youngster tends to evoke in the treatment situation precisely the responses which impede progress. This article discusses countertransference issues and the dynamics of masochism, understood as a protection for a frightened ego rather than merely a willful manipulation. Toward an Alternative Analytic Approach to the Internal Politics of Unions

Abstract  This paper discusses theoretical issues in analyzing the internal politics of unions in Japan. The main argument is that the autonomy of unions' internal political processes from management should not be assumed and that the very development of internal politics of unions may be influenced by the actors with which the unions interact. The paper first reviews previous research on the internal politics of unions in Western countries and discusses a model of internal union politics based on these studies. Second, it reviews previous studies of enterprise unions in Japan and considers why these studies have been indifferent to the internal politics. Third, the paper proposes an alternative approach to internal union politics by synthesizing insights from previous studies of unions and union politics in Western countries and Japan. The approach is based on the concept of the "political space" of unions. Fourth, application of the alternative approach to the development of internal politics of the enterprise union of Yahata Steel (from 1970, Nippon Steel), the largest steel firm in Japan, shows how the union's political space has become more narrow over time. Finally, the paper concludes by noting the relevance of the proposed approach to studies of union-management relations in other industrialized countries.
