Despite increased access to insurance through the Patient Protection and Affordable Care Act of 2010, uninsurance rates are expected to remain relatively high. Having uninsured family members may expose children to financial hardships. Eligibility rules governing both private and public health insurance are based on outdated expectations about family structure. Using 2009–2011 data from the National Health Interview Survey (N = 65,038), the authors investigated family structure differences in family‐level insurance coverage of households with children. Children living with married biological parents were the least likely to have uninsured family members and most likely to have all family members covered by private insurance. Controlling for demographic characteristics and income, children in single‐mother families had the same risk of having an uninsured family member as children in married‐parent families. Children with cohabiting biological parents had higher rates of family uninsurance than children with married biological parents, even accounting for other characteristics.  相似文献

Children of parents with a mental illness are at significant risk of developing a mental illness. This risk may be reduced if appropriate interventions are provided that include information and knowledge about mental illness. While there are some interventions for children of parents with a mental illness, research is lacking about the type of mental health information children need and why they need that knowledge. This study presents the perspectives of a purposive sample of international research experts in the field of parental mental illness about the kind of mental health literacy information children with parents with a mental illness need. Twenty-three participants completed a self-constructed short answer questionnaire about the knowledge needs of children of parents with a mental illness. The qualitative data indicates that ‘identifying information’, ‘making sense of parents behaviour’, ‘coping better’ and ‘respecting safety’ are key knowledge needs of children. Given the views presented, these findings suggest that health care professionals should advocate for policies that support individual-, peer-, and family-focused programs driven by strong evaluation and rigorous research. If this is done, children of parents with mental illness may experience ‘myth busting’ of incorrect information about mental illness.  相似文献

This pilot study was conducted in one early childhood pre-school nursery setting in an area of high deprivation in England to explore the under-researched area of how practitioners promote the health of children. The research used an original tool, Child Health Promotion: A Toolkit for Early Childhood Education and Care Practitioners, containing a 5 Step Programme as a model to guide practitioners in identifying and implementing a health promotion activity. The findings will help to set the onward agenda for a larger scale study which will foreground the voices of practitioners and highlight the role that ECEC practitioners can play in promoting the health of children.  相似文献

The Special Supplemental Nutrition Program for Women, Infants, and Children (WIC) is a food assistance program designed to help pregnant (or postpartum) women and young children consume a nutritious diet. With WIC’s emphasis on providing healthy foods, and food being (generally) a communal commodity, age-ineligible children may benefit from living with a WIC participant. This paper used data from the third National Health and Nutrition Examination Survey to examine whether age-ineligible children who lived in WIC households were in better health than similar children who lived in households that did not participate in the program. Results suggested that older males received a health benefit as a result of living in a WIC household; however, no similar effect was found for younger males or for female children.  相似文献

In many countries, dual residence is increasingly common for children when parents separate. This works well for many children, but opinions differ on whether or not it should be the norm. In analysing interviews with 35 nine- to 19-year-olds with dual-residence experience in Norway, undertaken in 2018, we find clear traces of prevailing discourses in society. Claims of equality and fairness and claims of children's rights both emerge, the latter being particularly visible in the participants' recommendations to other children. Children value spending much time with both parents, but may still want more flexibility than their parents realise.  相似文献

Program designers and evaluators should make a point of testing the validity of a program's intervention theory before investing either in implementation or in any type of evaluation. In this context, logic analysis can be a particularly useful option, since it can be used to test the plausibility of a program's intervention theory using scientific knowledge. Professional development in public health is one field among several that would truly benefit from logic analysis, as it appears to be generally lacking in theorization and evaluation. This article presents the application of this analysis method to an innovative public health professional development program, the Health Promotion Laboratory. More specifically, this paper aims to (1) define the logic analysis approach and differentiate it from similar evaluative methods; (2) illustrate the application of this method by a concrete example (logic analysis of a professional development program); and (3) reflect on the requirements of each phase of logic analysis, as well as on the advantages and disadvantages of such an evaluation method. Using logic analysis to evaluate the Health Promotion Laboratory showed that, generally speaking, the program's intervention theory appeared to have been well designed. By testing and critically discussing logic analysis, this article also contributes to further improving and clarifying the method.  相似文献

IntroductionThe health effects for children with incarcerated parents, and methods to improve children's experience of the justice system, are under-researched areas. While some work has been done to illuminate these concerns, practical implementation of a “child-friendly prison” has been slow.AimsA Health Directorate-funded project examining children's interactions with the Australian Capital Territory (ACT) justice system was conducted in 2013, which made a number of recommendations. The current study sought to examine the ongoing impacts of parental incarceration for children in the ACT and follow up on the recommendations.MethodsSemi-structured interviews were conducted with seven key stakeholders with a relationship to the research area. The interviews were summarised, and a thematic analysis was carried out to identify relevant ideas. Results from recent Detainee Health and Welfare Surveys were used to estimate the number of children affected.ResultsThe findings from the interviews concluded that little action was taken in response to the original report, that children's rights and agency were compromised, that the prison lacked accessibility, that consistent and individualised information should be provided to affected children, and that a previously operational homework program should be reinstated. Model facilities were identified.ConclusionThree broadly-applicable recommendations were made in response to the data from the surveys: increasing accessibility of public transport, the establishment of a child liaison officer at prisons, and maintaining extended family visits.  相似文献

This paper studies Work Place Health Promotion at two international corporations as an example of an unobtrusive control that targets employees’ lifestyles. It uses Michel Foucault's concepts of neoliberal governmentality and post-disciplinary control to show how Work Place Health Promotion breaks with the disciplinary logic of control most commonly associated with studies of unobtrusive controls in organizations. While discipline is centripetal, correcting employees’ misconduct so that they freely keep within prescribed norms, Work Place Health Promotion is centrifugal, targeting employees’ lifestyles and promoting those existing faculties and inclinations that may increase their activity, performance and their health. It hereby emerges as less restrictive than organizational discipline, but also as more discriminating. For not only does it subject employees’ lifestyles to an economic logic of investment and disinvestment, it also contributes to an exclusion of employees that fail in this regard in the name of their lack of health.  相似文献

The distress of 52 parents with a technology-dependent child at home is measured using a standardized psychological inventory. Fifty-eight percent of these parents report enough symptoms to classify them as needing psychiatric intervention. Cost savings to third-party payers are accelerating the trend toward home care (rather than institutionalized care). Yet, these findings point to an underestimated cost to families in terms of parents' psychological well-being. One factor associated with less distress for mothers is having services reimbursed through the Medicaid Model Waiver Program, which provides a comprehensive plan for home care services. In contrast, services reimbursed by private third-party payers are more variable, placing greater financial strain and uncertainty on parents, which is associated with greater psychological distress. These results suggest the need for further study of the long-term impact on parents caring for technology-dependent children at home.Reprint requests to Barbara J. SUMMARY. Custodianship of children was introduced by the 1975 Children Act, but not implemented until 1985. It has now been overtaken by new concepts in the 1989 Children Act, but the experience thus far provides useful indications of how Residence Orders may be operated. Based on Department of Health funded research, this article shows how careers and birth families see such measures designed to give security to children while maintaining contact with their original parents. The article gives important information to all professionals who will have to work with the new principles of parental responsibility.  相似文献

This article draws on research with children who provide care for parents with serious mental health problems and signals ongoing research that uses photographic participation methods with these groups of vulnerable children. The intention of this article is to highlight the need to move away from popular and simplistic representations of children with caring responsibilities (young carers) as victims of their parents’ illnesses, as ‘little angels’ whose caring work is condoned through rewards or as (exploited) informal domestic workers whose childhoods are inevitably compromised by the caring activity they undertake. Recommendations are made for generating deeper understanding about the lives and needs of children who are affected by parental impairment that is congruent with the thrust of current UK policy, Every Child Matters and the 2004 Children Act.  相似文献

This article reviews the relatively new and emerging issue of children from the United States being placed in intercountry adoption. It specifically explores current practices of intercountry adoption involving the United States as a country of origin within the framework of the 1993 Hague Convention on the Protection of Children and Cooperation in Respect of Intercountry Adoption. Data from several receiving countries are explored to identify commonalities and concerns related to the practice of placing U.S. children with foreign families and the role of the birth parents in this process. Schoolbag use by children is a global common concern.. Children carry school books and other amenities in their school bags. Global evidence indicates that daily load carried by school children may have negative health implications. Backpack as a school bag model, is the healthiest way of load carriage for school children. Several initiatives have been launched world over to minimize unhealthy consequences resulting from schoolbags. Based on a situation analysis, Sri Lanka implemented a national healthy schoolbag campaign by joint efforts of Ministries of Health and Education. Actions were contemplated on; strategies for bag weight reduction, introduction of an ergonomically modeled schoolbag and bag behaviour change. New strategies were introduced with awareness campaigns to policy makers, bag manufacturers, parents, teachers and children. Four million schoolchildren benefitted. In 2000, the backpack strategy of "Pack it Light, Wear it Right" was started as a public health initiative in the United States by the American Occupational Therapy Association (AOTA). Over the last eleven years, thousands of occupational therapy practitioners and students participated in educational programs and outreach activities. In 2004, modeled after the success AOTA initiative, the Icelandic Occupational Therapy Association launched a national backpack awareness initiative. This article shares examples of practices that could be implemented in any context to the promote health of children.  相似文献

Little research has examined the impact of having a parent with a disability on child mental health. This study used data from 7,116 families of children who participated in the 2006 National Health Interview Survey and propensity score matching (PSM) to investigate this question. Prior to PSM, differences in Child Mental Health Brief scores were significant with a moderate effect size. After PSM, differences in Child Mental Health Brief scores were still significant, but the effect size was small. Results indicate that much of the mental health risk for children of parents with disabilities is connected to related issues.  相似文献

Child welfare concerns have drifted to an inappropriate focus on crisis intervention and a punitive approach to child protection intervention at the expense of community-based preventive child welfare programs. Recent attempts to divert cases from the child protection system through differentiated response mechanisms have been criticised for failing to provide access to relevant services or preventing vulnerable families from re-entering the child protection process. A tension inherent in providing both child protection and family support within the one agency is also identified as a barrier to effective service delivery. This paper discusses the value of the UK Children in Need approach as a model for enhancing support to children and families outside the statutory child protection system. Information from an evaluation of a trial implementation of the UK Children in Need approach in Victoria is used to discuss the implications for policy and practice of placing responsibility for coordinating a response based on the needs of children and their families within family support services.  相似文献

This research examines 44 Chinese urban and suburban parents' priorities for quality early childhood education (ECE) program characteristics by applying a mixed methodology of Q-sort process and interviews in two Zhejiang cities in China. Four groups of parental views were identified, including Health and Well-rounded Development, Daily Routine and Care, Environment, and Child-centered Learning. Our findings showed that Chinese parents valued children's physical and emotional wellbeing most, as they believe laying this foundation is more important than acquiring academic knowledge and skills.The findings from this investigation contradict those of previous studies and literature in which ECE researchers described Chinese parents as unique and different from parents in the West by favoring teacher-centered teaching and learning to boost their children's initial academic achievements and prepare them well for formal schooling. We found Chinese parents preferred teachers keeping a balance between teacher-initiated and child-initiated learning. Additionally, they do not like providing children sand/water play, stuffed animals, and children-alone-time mainly because of health concerns. The paper discusses some possible reasons for the findings drawing on studies in Chinese social-cultural context and their contributions to improving ECE curriculum and practices.  相似文献

Fathers and men in families are one of the most important resources for children's well-being. UNICEF, the Ford Foundation, Save the Children, other nongovernmental organizations, and resolutions from the 1994 International Conference on Population and Development have all recommended efforts to involve men in reproductive health programs. However, despite this interest and recommendations, social service and health programs, as well as development interventions, continue to target mothers and children, ignoring the major role of men in the lives of children and families. Most development interventions focusing upon family well-being have stressed the importance of the mother/child relationship, even in societies in which the father controls decisions about the household and family welfare. The author surveys program initiatives, conferences, research, and publications concerned with the role of men in the family.  相似文献

Recent models relating to the affordance of children’s participation rights, based on articles 12 and 13 of the United Nations Convention on the Rights Of the Child (1989) , have focused on the role of and strategies used by the adults working with children (Children and Society 10 , 2001: 107–117; Children and Society 20 , 2006: 209–222). Whilst these adults play a pivotal role in affording participation rights, children, and their parents/family/whānau, play an equally important role. This paper explores the complex interplay of roles, relationships, and strategies used by adults and children in home‐based education settings in Aotearoa/New Zealand, that support the affordance of participation rights for children. It identifies key aspects of practice that underpin this, and presents an alternative model of participation that is in keeping with a sociocultural approach to children’s learning and development, and acknowledges them, in the right conditions, as having a voice and agency as citizens in their own right.  相似文献
