The article presents social and health indicators of depression among custodial African American grandmothers. Using a cross-sectional design, a sample of 99 custodial African American grandmothers caring for one or more grandchildren was included in the analysis. The results indicated that approximately 20% of custodial African American grandmothers were depressed. Depressed grandmothers were more likely to report having all ten chronic physical health conditions listed in the study. One additional chronic condition increased the probability of depression by 68%. Old age, more social support, and caring for older children were associated with low levels of depression. The proliferation of grandchildren being raised by African American grandmothers suggests the need for more research, policy, and programmatic interventions.

African Americans are disproportionately represented in the number of grandparents raising their grandchildren. Using Role Strain Theory and Socioemotional Selectivity Theory, this study examines how older grandmothers fare relative to their younger counterparts. Eighty-five custodial African American grandmothers, aged 33–88 years old, completed demographic questionnaires and scales of Role Demand, Emotional Strain, Caregiving Strain Index, and Level of Care. Results showed older grandmothers experienced less emotional and caregiving strain relative to younger grandmothers. Furthermore, married grandmothers experienced less caregiving strain, and their age did not insulate them from the strain associated with the level of care. Implications are discussed.

ABSTRACT

This study explores the grandmother role as experienced by a nonrandom sample of nine lesbian women. It examines how they define the grandmother role, and the behaviors and actions through which they enact the role. During individual interviews each woman was asked to talk about what makes a woman a good grandmother, memories of her own grandmothers, and the relationship she has with one or more of her grandchildren. The effect of her sexual orientation on the relationship was not explored. These women define the grandmother role as providing emotional support to their grandchildren, providing varied experiences for their grandchildren, and providing support for the parents of their grandchildren.

BackgroundAfrican American women with low income face various challenges to having a healthy pregnancy. Unplanned pregnancies often amplify those challenges and negatively impact women’s ability to get the care that she needs. Research has shown that social networks can improve pregnancy outcomes by helping women cope with and address the problems that they face. However, there is little research on how unintended pregnancy impacts the medical and social support that African American women receive.AimThis research explored how unintended pregnancies impact women’s social networks and the type of support that they need throughout the pregnancy.Design and sampleThis ethnographic study included 20 African American women who used pregnancy services at a health department in the United States. Fourteen women were enrolled in the study during their first or second trimester and were interviewed upon enrollment, in their third trimester, and within one month of giving birth. An additional six women were enrolled and interviewed within one month of giving birth.ResultsAfrican American women’s needs and social support networks change throughout their pregnancy. When medical providers primarily offer pregnancy information and referrals to social service agencies during the first trimester, problems that women have later in the pregnancy are not properly addressed.ConclusionSocial support needs to be accessed at multiple times during the pregnancy. Pregnancy resources should be provided throughout the pregnancy, recognizing that social support and personal struggles evolve during the pregnancy.

This study compared the adjustment of two groups of grandmothers who were providing primary care for grandchildren in split-generation households: 25 grandmothers in the oldest generation of their families (G1), and 22 grandmothers who had living parents or in-laws (G2). It was hypothesized that because of potential demands on them as middle-generation women, G2 grandmothers would report greater burden and depression in their caregiving roles than G1 grandmothers. After controlling for numerous factors that are associated with generational position, the hypothesis was confirmed. In addition to being in the middle generation, having grandchildren with problems predicted elevated depression and burden. Grandmothers who were educated beyond high school also reported greater role burden.

ABSTRACT

This study examined changes, over 10 months, in self-assessed health, depression, anxiety, stress, coping and support in a convenience sample of 74 grandmothers living in the same home as grandchild(ren) and to compare them with grandmothers who had either primary (n = 49) or partial/supplemental (n = 25) responsibility for their grandchildren's care. Grandmothers showed high stability over ten months time, with many reporting elevated depression and parenting stress at both time points. Depression was associated with greater parenting stress, primary responsibility for caregiving, and with avoidant and minimizing coping. Better self-assessed health was linked with less parenting stress, and less formal and instrumental support. Study findings are discussed.

Caring for grandchildren provides grandparents an opportunity to sustain an active lifestyle and remain socially engaged in older age. Studies have examined the association between providing care to grandchildren and grandparents’ cognitive function. However, these studies had several limitations and yielded mixed findings. Using data from the China Health and Retirement Longitudinal Study, this study examined the longitudinal association between caregiving and cognitive functioning in grandparents aged 50–75 years. Grandparents’ caregiving status and cognitive functioning were assessed in 2011, 2013, and 2015. Random-effects and fixed-effects regression models were estimated and compared. The full sample (20,000?+?person-year observations) was divided into four subgroups to examine gender and rural–urban variations. Being a part-time noncoresident caregiver was associated with higher scores on episodic memory for grandfathers but not for grandmothers. Compared with their noncaregiving counterparts, rural part-time multigenerational caregiving grandmothers had significantly better mental intactness and global cognitive functioning, while rural full-time noncoresident caregiving grandmothers had significantly better episodic memory. Caregiving status was not associated with any cognitive measure among urban grandmothers. Taken together, these findings suggest that Chinese grandparents enjoy cognitive benefits from providing casual or intensive care to grandchildren, but these benefits vary substantially by gender and rural–urban status.

ABSTRACT

Resilience theory has been suggested as a framework for research on HIV prevention among men who have sex with men. Among this population, literature indicates that African American/Black men who have sex with men experience additional health disparities including elevated HIV incidence rates, victimization, and poor physical health. Conceptualizing resilience as a part of one’s social environment, this qualitative study investigates resilience processes and HIV transmission risk among a sample of 21 substance-using African American/Black men who have sex with men. Data from in-depth interviews describe the social environmental context in which resilience is exhibited and document the influence of homophobia, expressions of agency, and access to social, economic, and cultural capital on resilience processes. Central to this are expressions of hidden resilience, in which African American/Black men who have sex with men mitigate risk and experience their lives as subjectively successful, whether or not outsiders see it as such.

ProblemPregnancy loss is a distressing experience for parents, however no research has addressed grandmothers’ experiences of grief and support following a child’s pregnancy loss.BackgroundNo research has specifically addressed grandmothers’ experiences of support and bereavement care following pregnancy loss.AimThis study seeks to understand three key areas: (1) the support grandmothers provide to their child; (2) the support they received themselves following pregnancy loss, and; (3) supports desired by grandmothers. The study aims to contribute insights into actions midwives could take to support grandmothers following pregnancy loss.MethodsSemi-structured interviews were conducted with 14 grandmothers to understand their support experiences. Interviews were analysed using Braun and Clarke’s approach to thematic analysis.FindingsThemes related directly to the three research questions: one: providing support, comprising two themes — being strong, protecting their family and the challenges of knowing how to help. Two, receiving support, also comprised two themes — lack of professional support offered to grandmothers and informal support and self-support strategies. Three, desired support, comprised three themes — I had no idea: increasing knowledge of pregnancy loss, peer support helps: they know how it feels, and honouring our grandchildren, making meaning.DiscussionGrandmothers may give extensive support to their child following pregnancy loss but lack confidence and face challenges in doing so. Few formal supports are available to grandmothers themselves, with grandmothers relying on their social networks for their own support.ConclusionsGrandmothers need early access to information and guidance from midwives and hospital bereavement services, as well as ongoing peer support with flexible delivery options.

This study explores the relationship between the life satisfaction of older adults and the social support from grandchildren in Hong Kong. Two hundred and fifteen older people (from the ages of 64 to 101, mean age 79.3), whose youngest grandchild was aged 12 or older, were recruited from elderly service agencies to participate in the study. Face-to-face interviews were conducted by trained interviewers using a standardized questionnaire, which included the Life Satisfaction Scale for Chinese, the Lubben Social Network Scale, the Chinese Tradition Scale, grandchildren social support measures, self-rated health, self-rated financial adequacy, and demographic variables. Hierarchical regression showed that the life satisfaction of older adults benefitted significantly from the social support from grandchildren (R 2 change = .05, F change = 7.15, p < .001); while controlling demographic characteristics, general social support, attitude towards Chinese tradition, self-rated health, and self-rated financial adequacy. The total explained variance was 51%. Emotional support and appraisal support from grandchildren were identified as significant contributing factors. The policy and practice implications for active aging policies are discussed in a Chinese context.

Although there is a growing body of knowledge about health among African American women in general, there is a dearth of information on African American lesbians. The primary purpose of this study was to investigate the correlates of health-related quality of life among African American lesbians using a cross-sectional anonymous survey with topics and measures developed by members of the African American lesbian community. Surveys were completed by 123 English speaking adult women who identified as African American lesbians and were attending the Zuna Institute's National Black Lesbian Conference. Overall, we found a very high health-related quality of life, in spite of a high frequency of health impairments. The average body mass index (BMI) for this sample was 32.2 (SD?=?8.0); 13% were morbidly obese, having a BMI of 40 or more and only 15% of the women were in a healthy weight range; advancing age was associated with poorer physical functioning, decreased physical role functioning, and more pain. Health-related quality of life was associated with depression and spirituality, but not religion. This study highlights the need for subjective measures of health-related quality of life as well as checklists of diseases and disorders.

ABSTRACT

Living with a chronic illness is an experience characterized by changes in self-management behavior. Few studies have addressed the role of spirituality in the self-management of a chronic illness among older African American women. The purpose of this exploratory study was to understand the role of spirituality in the self-management of chronic illness. Data from a sample of 10 African American women were collected from semi-structured interviews and analyzed for common themes through narrative analysis. Four themes emerge from the linkage of spirituality and self-management. Understanding this experience will assist public health providers in creating culturally appropriate health education interventions.

BackgroundAlmost all babies are breastfed in Papua New Guinea (PNG); yet appropriate breastfeeding practices are not always followed.AimTo explore the perspectives of first-time mothers in rural PNG on how the language and discourse of grandmothers about infant feeding influence their breastfeeding practices.MethodsA critical discourse analysis (CDA) approach was used to theoretically frame the analysis of twenty first-time mothers’ narratives.FindingsAnalysis revealed three themes: (i) prescribed knowledge repository, (ii) social control and dominance, and (iii) disapproval and role conflict, which provides an understanding of grandmothers’ differing views and positions on infant feeding practices and their influence on breastfeeding.ConclusionThis study shows that grandmothers remain influential in infant feeding practices in rural PNG. There appears to be a societal expectation that empowers grandmothers in the maternal decision-making processes regarding breastfeeding practice. Grandmothers’ influence includes the early introduction of complementary foods to infants less than six months old. Interventions aimed at promoting, protecting, and supporting breastfeeding need to include grandmothers.

Custodial grandparenting can be especially challenging for older grandmothers facing age specific issues. Kinship navigator programs are social service delivery programs intended to inform grandparents and other relatives raising children about available resources and services, provide information specific to their individual needs, and help families navigate service systems. Our study utilizes self-report data from one kinship navigator federal demonstration project, which used a randomized control trial, to examine demographic characteristics for grandmothers under and over 55 years of age, whether grandmother caregivers (≥55 years) improve family resilience, social support, and caregiver self-efficacy, and which interventions improved outcomes for grandmothers (≥55 years). Each participant was randomly assigned to one of four groups: Usual Care (traditional child welfare services), Standard Care (family support and case management), Peer-to-Peer Care Only, and Full Kin Tech Care (peer navigators with computer access and interdisciplinary team). Thirty-nine percent of grandmothers (55-75 years) were mostly living in poverty, predominantly Caucasian, with 36% identifying as African American/Black, with at least one to two children at home. Repeated-measures ANOVAs for each subscale showed statistically significant within- and between-group differences for Family Functioning, Social Supports, Concrete Supports, Child Development, and Nurturing and Attachment, with the exception of Usual Care, which showed a decline in protective factors consistently across subscales. Future research with kinship families could qualitatively examine the experiences for older women in navigator programs and replication of kinship navigator programs could build capacity in data collection and maintenance systems to gain better perspective about how systems of care impact families.

BackgroundPerceived birth experiences of parents can have a lasting impact on children. We explored the birth and new parenting experiences of South African parents in 2020 during the Covid-19 lockdown.MethodsWe conducted a cross-sectional online survey with consenting parents of babies born in South Africa during 2020. Factors associated with negative birth emotions and probable depression were estimated using logistic regression.ResultsMost of the 520 respondents were females (n = 496, 95%) who gave birth at private hospitals (n = 426, 86%). Mothers reported having overall positive birth emotions (n = 399, 80%). Multivariable analysis showed that having a preterm baby (aOR 2.89; CI 1.51–5.53) and the mother self-reporting that Covid-19 affected her birth experience (aOR 4.25; CI 2.08–8.68) increased the odds of mothers reporting predominantly negative emotions about their birth. The mother having her preferred delivery method reduced the odds of having negative birth emotions (aOR 0.41; CI 0.25–0.66). Multivariable analysis showed that having predominantly negative emotions about the birth increased the odds of probable minor depression (aOR 3.60; CI 1.93–6.70). Being older reduced the odds of having probable minor depression (25?34 years aOR 0.36; CI 0.10–1.32; 35 years or older aOR 0.25; CI 0.06?0.91).ConclusionsLockdown exacerbated many birth and parenting challenges including mental health and health care access. However, overall experiences were positive and there was a strong sense of resilience amongst parents.

ABSTRACT

The purpose of this study is to describe health care needs and service utilization among institutionalized women of color. The sample was dichotomized by length of stay to determine how African American, Native American, Asian/Pacific Islander, and Hispanic/Latino women differed at two points in time. Data for this study came from the Current Resident Survey of the 1999 National Nursing Home Survey. The data were analyzed using GLM. The findings suggest that Native American women are the most impaired and Asian/Pacific Islander women are the least impaired. Additionally, very few women received mental health services despite their extensive need.

ABSTRACT

African American women (AAW) are particularly at risk for deleterious health outcomes that might be mitigated through increased preventive care use. A mixed methods study that examined relationships between knowledge of, beliefs about, and barriers to well-woman visits, flu vaccines, and mammograms was conducted with midlife AAW who participated in an online survey (n = 124) and in-depth interviews (n = 19). Findings showed that greater knowledge of preventive service recommendations and positive patient-provider relationships were associated with greater preventive service use. Flu vaccines were significantly underused. Study implications inform strategies to increase preventive care utilization among AAW and increase capacities to improve health disparities.
