Little attention has been paid to the nature of service needs of elderly abuse victims, whether these needs are met, and the types of factors which are correlated with the service providers' decision as to what services to provide. This paper provides an analysis of services to elderly abuse victims using data from a study conducted by the Illinois Department on Aging. The report is based on data covering 204 cases which were collected during the calendar year 1986. Abuse types included: Physical Abuse, Confinement, Sexual Abuse, Deprivation, Neglect, Self Neglect, and Financial Exploitation. Case Management was the most frequent type of service the victims received, followed by Homemaking Assistance and Miscellaneous Legal Services. Self Neglect cases received a quite different pattern of services than other types of abuse victims.  相似文献

Elderly Navajo people and their families were surveyed to discover the extent and types of elder abuse among them. Neglect was the most prominent type of abuse. What appeared to be economic exploitation was also quite prevalent, but was in part defined by them as their cultural privilege and duty to share with their families. Abuse tended to be especially associated with suddenness of becoming dependent, having mental problems, family crises due to suddenly having caregiving responsibilities for which they were unprepared and personal problems of primary caregivers.  相似文献

In this paper the author recounts how one agency in a community of agencies sought to integrate services to multiproblem families. The paper describes how the integration was attempted, conceptualizing the community of agencies as a multiproblem family itself. The aging population of Canada and the associated caregiving demands highlight the need for elder abuse research. The objectives of this article are to provide a review of literature while identifying recommendations and directions for future research. Significant gaps in the knowledge base are identified that preclude the development of effective programming and policies. Future research directions include prevalence studies in community and institutional settings; studies on correlates, risk, and protective factors; research to clarify definitional issues and to develop psychometrically sound measurements; qualitative studies; and the need to support elder abuse research through capacity building and appropriate resource strategies.  相似文献

This article discusses findings from the First National Survey on Elder Abuse and Neglect in Israel, conducted during 2004–2005 under the sponsorship of The Association for Planning and Development of Services for the Aged in Israel (ESHEL) and the National Insurance Institute. The goals were to examine the prevalence and severity of various forms of abuse and neglect from the victims' perspective, to examine correlates and predictors, and to develop profiles of elders at risk. Data were collected through personal interviews from a national representative sample of community urban dwellers age 65 and older, using cluster sampling techniques and sampling proportionately both Arab and Jewish elders. The sample was composed of 392 males and 650 females, 89% were Jews and 11% were non-Jews. The instrument included sociodemographic data, health and activities of daily living (ADL), a measure of safety, and a battery to examine seven types of abuse (physical, emotional, verbal, limitation of freedom, financial exploitation, sexual, and neglect). Findings indicate that 18.4% of the respondents were exposed to at least one type of abuse during the 12 months preceding the interview, the highest form being verbal abuse followed by financial exploitation. The rates were quite similar between Jews and Arabs. Women were more exposed to physical violence and Arab women were the most vulnerable. Physical, emotional, verbal, limitation of freedom, and sexual abuse occurred mostly among partners. The rates of physical, sexual, and limitation of freedom abuse, however, were relatively low. Financial exploitation was mostly inflicted by adult children. Partners as perpetrators had more chronic health problems and physical and mental disabilities. Children as perpetrators were unemployed, had various mental health problems, and were substance abusers, often in a process of separation or divorce and tended to live with the victims. Neglect in answering primary needs was found among 20% of the sample, regarding the 3 months preceding the interview. This high rate might be related to elders tending to avoid seeking help or sharing their needs with their families. When victims sought help it was mostly from medical and health services. The findings thus corroborate that elder abuse and neglect is a social problem in Israeli society and has to be addressed in policy discourse and service developments.  相似文献

Population trends suggest that the next 20 years will witness a dramatic increase in the adult population aged 65 and older. Projected increases in the elderly population are expected to significantly increase the stress on family and professional caretakers. Stress, in the context of caregiving relationships, is a risk factor associated with increased prevalence of elder abuse in familial and institutional settings. As increasing numbers of older adults are moved from family caregiving to nursing home care settings, it becomes important to identify the pattern of elder abuse risk factors in nursing home facilities. An ecological model is proposed for better understanding the risk factors associated with elder abuse in nursing homes and the complex interaction of individual/person characteristics and contextual factors in institutional elder abuse. An ecological perspective to institutional elder abuse provides a framework for guiding and informing future research on the risk factors of nursing home abuse and, in turn, for the development of effective interventions and relevant social policies.  相似文献

The effects of changes in mandated child maltreatment reporting laws have not been systematically evaluated. To better understand the effects of these changes, the objectives of the present study are: (1) to assess the relationships between report rates and state universal and clergy reporting laws in 2010; (2) to compare the changes in total, confirmed, and maltreatment type report rates and with changes in reporting laws from 2000 to 2010, and (3) to examine whether there is any relationship with report rates and the nature of the mandated reporting law change. We used county-level data from the U.S. National Child Abuse and Neglect Data System for the years 2000 and 2010 to evaluate changes in reporting rates for total reports, confirmed reports, and confirmed maltreatment types while controlling for concurrent changes in child and community variables. We found that trends in 2010 for increased total and confirmed reports were similar to 2000 for counties with universal and/or clergy reporting requirements, which significantly contributed to report rates even after controlling for child and community factors. Universal reporting was associated with higher report rates for physical and sexual abuse, and clergy reporting requirements were associated with more sexual abuse, medical neglect and psychological maltreatment confirmed reports. However, while counties in states that changed their clergy reporting laws had higher increases in total reports, they had fewer confirmed physical and sexual abuse, neglect, and psychological maltreatment reports. More pronounced changes were noted in a state that made more pronounced changes in its clergy reporting laws. Policymakers should consider whether changing requirements for mandated reporting meaningfully improves child maltreatment identification.  相似文献

Assessing for potential physical abuse is a fundamental task for those professionals undertaking assessments of parents involved in care proceedings. One tool developed to help assist in this endeavour is the Child Abuse Potential (CAP) Inventory (Milner, 1980 , 1986 ). The CAP Inventory provides an estimate of parental risk of child abuse and is one of the most widely used psychometric tools by psychologists in child protection settings. This review evaluates the psychometric properties of this measure and provides an overview of its potential uses, as well as its limitations. It concludes that there is evidence to support the validity and reliability of the CAP Inventory for use in care proceedings. Specifically, the review indicates that the CAP Inventory has good levels of reliability and validity (construct, content and concurrent). The only aspect of validity for which there is a lack of evidence is that of predictive validity and this is due to a dearth of prospective studies. Given the above, the practical, clinical and ethical implications for the use of the CAP Inventory in the assessment of parents involved in care proceedings are discussed. Rates of substantiated maltreatment documented by the 1993 and 1998 Ontario Incidence Studies of Reported Child Abuse and Neglect doubled between 1993 and 1998. Although increasing public awareness and changes in investigation procedures appear to account for part of this change, the increase also reflects a significant shift in the types of maltreatment agencies are investigating and substantiating. Exposure to spousal violence has increased eightfold, and the proportion of neglect cases has more than doubled, whereas cases of sexual abuse are decreasing. The field requires a differentiated response to address the maltreatment-specific challenges underlying child welfare caseload increases in Canada.  相似文献

This study of 267 child deaths associated with abuse or neglect in Texas during 1975 through 1977 suggests a number of indicators for identifying potential child fatalities. Families where abuse or neglect is implicated in a child fatality are characterized by small family size, young parents, and under-utilization of community support services. Over three-fourths of the families in the study had never come to the attention of the state's child protective services agency. In addition, when fathers were present in the home, they were as likely to be involved in the abuse or neglect as mothers. Neglect was implicated in the death of a child as often as abuse. The study suggests the need for further examination of child fatality profiles associated with abuse and neglect and increased community outreach efforts to provide support to high-risk families, The study also has implications for social policy decisions, particularly in relation to efforts to resist attempts to exclude neglect from child protective service responsibility.  相似文献

1. Older individuals in both community and institutional settings have traditionally underused mental health services. Providers of such care devote a minimum of their professional time to those over 65 years of age. 2. Ageism and difficulty with diagnosis are the most frequent explanations for inadequate mental health care to the elderly. This study found that lack of referrals and failure of the aged to seek services partially accounted for underuse. 3. Legislative changes mandating that aged persons be screened for and receive appropriate mental health services will increase the demand for qualified providers. 4. Nursing's approach to care of the older adult, which focuses on both physiological and psychological needs, can facilitate the delivery of comprehensive effective mental health interventions in and out of the institutional setting.  相似文献

Data from the National Incidence Study of Child Abuse and Neglect are used to examine the maltreatment of adolescents. The discussion considers the type of maltreatment adolescents experience and the response of service agencies. Adolescents are compared with younger children to identify differences in the form of abuse and neglect they experience and the reporting of that mistreatment. ABSTRACT "Elder Abuse and Neglect" was constructed as a social problem by experts and has largely been a product of "expert" knowledge building and intervention. The idea of woman abuse as a social problem, on the other hand, originated with women themselves. The paper examines the changing social context in which some older people are currently seeking ownership of responses to mistreatment. It explores emerging criticisms of present constructions of "elder abuse and neglect" and the needs they imply, and links these to feminist frameworks. This study set out to measure rates of substantiation of initial abuse and neglect reports and to identify case related factors that are associated with substantiation. Toward this end, analysis was performed on 2,849 cases reported to the Wisconsin Elder Abuse Reporting System in 1988 and 1989. Over 57% of all initial reports were substantiated by the investigating agencies with higher rates for cases of self-neglect only. Among cases involving other abusers, physicial and emotional abuse had the highest substantiation rates. Results also indicate that substantiation rates for all types of abuse and neglect are higher in urban settings. Cases referred by professionals usually have higher substantiation rates than those referred by relatives and neighbors. Higher rates were found for elders living alone and those with several high risk characteristics such as mental illness and Alzheimer's disease. The abusers in substantiated cases were more likely to be spouses, sons, and other relatives. These abusers were also likely to be in one or more risk categories. The amount of services that were offered to and accepted by the victims was associated with substantiation only for alternative placements but not for community based services. Clients in substantiated cases were in fact more likely to refuse services than their peers in unsubstantiated ones.  相似文献

Multisystemic Therapy for Child Abuse and Neglect (MST-CAN) is an evidence-based program for families with children who experience maltreatment. This clinical trial is the first evaluation of MST-CAN in a German-speaking area. Parental psychological problems and parental stress have been shown to be risk factors for child abuse and neglect. By the end of treatment, parents reported significantly less psychological distress than before the start of MST-CAN. Six months after treatment, this reduction was still evident. However, parents did not report any significant reduction of parental stress at any of the three time points. MST-CAN is an effective intervention for child maltreatment that not only combats child abuse and neglect but also has the potential to alleviate parental psychological distress.  相似文献

Abstract

Deaf children are more susceptible than other children to abuse in home and institutional settings. Many helping professionals are unaware of the unique vulnerabilities and needs of the deaf; this lack of awareness limits the effectiveness of their services to that population. Laws and ethical standards mandate that social workers and others use communication methods that deaf clients can understand; however, many human service agencies rely on less than adequate means of communication in child abuse investigations and other procedures. Such ineffective intervention practices fail to protect a vulnerable population of children who literally cannot speak for themselves.  相似文献

With more families entering the child welfare system with problems of substance abuse, there is an increasing need for programs that can respond effectively to these families' needs. This article describes one community-based program designed to support and strengthen families with an identified problem of substance abuse. parents' progress in addressing their problems with substance abuse, reduction of risk in Project families. children's placement experiences, and client satisfaction are discussed. We present the victim rates and severity of child maltreatment in US Army families by the sex of the child and parent from 1990–2004. Neglect rates decreased from a high point in 1991 to a low in 2000, but by 2004 the rates had increased to about the 1991 level. During two large‐scale deployments of the US Army to the Middle East (1991 and 2002–2004), the rates of neglect increased. Neglect rates were highest for the youngest children and decreased as age increased. Physical abuse rates decreased from 1990–2004, but the decline was slowed during 2001–04. Physical abuse was more severe by male offenders. Emotional abuse showed wide fluctuations in rates. Emotional abuse rates were similar for boys and girls up to age 11, but the rates for older girls were higher. Sexual abuse had the lowest rates throughout the time of this report, decreasing from about 0.5/1000 to about 0.1/1000. The rates of physical abuse and neglect were generally higher for boys than for girls up to the teenage years when the rates reversed. Male offenders were more likely to maltreat children (even excluding sexual abuse) than were female offenders and offenders of both sexes were more likely to abuse male children.
