Summary This article describes patterns of manpower utilization in Jerusalemmunicipal welfare offices before and after reorganization ofthe Welfare Department whereby the functions of eligibilitydetermination and provision of income maintenance grants wereseparated out from the tasks of family caseworkers and givento special non-professional administrative units.The time studycarried out after ‘separation’ showed a more rationaluse of professional staff time, where a worker's education wasbetter correlated with bis tasks and significantly more professionaltime was devoted to individual counselling and inter-agencybrokerage on behalf of welfare clients. Environmental psychology has been a topic of great interest for many researchers in the field of aging and dementia. Thus, several orientations can be identified in the contemporary scientific literature concerning environmental design for people with dementia. In this article, we discuss recent research on environmental design models for people with dementia and propose an approach referring to the concept of use of space and human rights.We use a methodology based on observation of special care units to determine an environmental design framework that should be taken into account when designing residential facilities for people with dementia. Content analysis identified 6 primary dimensions and 13 secondary dimensions relevant to the rights of people with dementia. We then discuss the results according to the related literature in environmental psychology and gerontology in terms of well-being and quality of life.  相似文献

Dental Manpower     
A recent report by the Department of Health and Society Security (DHSS) has argued that a 10% reduction in the intake to dental schools is needed if an oversupply of dentists over the next forty years is to be avoided. Whilst welcoming this recommendation it is likely that it may not go far enough. In this article we describe the approach used in the DHSS report to estimate the demand for and supply of dental care and examine the relevance and implications of the methods used and assumptions made in that report.
It is argued that the use of extrapolations of past trends and the absence of sensitivity analysis in much of the analysis implies that little confidence can be placed in the estimates and that on balance the conclusions of the report are likely to lead to an excess supply of dentists over the next forty years. Recommendations are made for improving the planning of the future supply of dentists.  相似文献

Abstract

This article reviews 24 empirical studies that examined the relationship between intraorganizational factors and effectiveness in human service organizations. Special attention has been paid to identify organizational variables critical to effectiveness, clarify levels of effectiveness, and demonstrate diverse criteria of effectiveness. The review suggests that nonmaterial factors such as goal congruence, participation in decision-making, culture, climate and management strategy predict effectiveness. However, generalization seems hasty due to different operationalization of organizational characteristics, different samples and diverse criteria of effectiveness. Replication from diverse samples will corroborate these findings. Several implications and future directions are also discussed.  相似文献

Using data from the China Health and Retirement Longitudinal Survey, we employed counterfactual propensity score matching to examine the association between income and health service utilization and expenditure of the elderly in China. The results demonstrated that income did not differentiate whether the poor elderly in China visited a doctor when suffering an illness, but their expenditure on medical treatment was significantly associated with income. Subgroup analysis further indicated that the medical demands of female, older, and rural elderly were more likely to be reduced by their income compared with their counterparts.  相似文献

ABSTRACT

This qualitative study used in-depth interviews to better understand the social service experiences and identified needs of 26 demographically diverse adolescent mothers and their mothers, who resided together in three-generational households. Most dyads accessed community services, particularly parenting and emotional supports, financial and material assistance, and medical care, and desired short-term assistance related to their circumstances and newly expanded families, rather than long-term support. The most commonly identified barrier to service utilization involved income eligibility guidelines. In light of welfare reform policies and the current economic climate, this research underscores the need for transitional supports for families, including assistance with basic needs. In terms of implications for social work practice and future research, adolescent mothers may need formal supports as they pursue independence after they leave their mothers’ care. Future research could further explore, within other communities and longitudinally, whether these preliminary trends hold true.  相似文献

Abstract

This study was conducted to determine the extent to which service providers in a community-based care program accurately identified and referred elders with symptoms of depression. Interviews were conducted with 79 frail, low-income elders, with the CES-D used to gauge depressive symptomatology. Reviews of their case files were then conducted to determine first, the extent to which case managers recognized depressive symptoms and second, the extent to which respondents who screened with significant symptoms were referred for mental health services. Results suggest an imperfect association between CES-D scores and case managers' perceptions and referrals. Less than 1/3 (31%) of those who screened for depression received counseling.  相似文献

In recent years a number of programmes have been developed which focus on the urban community rather than the individual as the beneficiary of welfare allocations. Working on the assumption that different views of social malaise will generate different programme types, this paper takes an analytical framework developed by Roland Warren and applies it to an Australian situation. The problem at stake is one of creating conditions in which a newly formed organization can enter an urban situation and begin to coordinate local activity. This paper discusses the difficulty a new organization has in breaking into the inter-organizational field, the difficulty of achieving legitimacy, and means by which its activities may be prevented, blunted and repelled. These are illustrated with examples in metropolitan Adelaide.  相似文献

Theoretical models or frameworks of change in human service organizations, both governmental and non-governmental, must adapt to the changing circumstances faced by such organizations in the later 1980s and 1990s. Too often change no longer means innovation initiated from within a service organization in order to improve service to its clients, but rather change engendered or enforced from outside the organization which may have uncertain or adverse implications for clients, target groups, or staff. Major changes may have to be implemented within tight time limits and in situations of high intra- and inter-organizational conflict. In such situations, sequential models of organizational change based upon a rational problem solving process, developed during the more favourable era of the 1970s, may be of limited use. A complementary framework for analysis and action planning can be built upon four dimensions of change identified here as rational-empirical, social-systems, power-politics and values-vision. These dimensions offer a comprehensive framework for understanding and intervening in various aspects of the change process, such as assessing the forces for and against change, preparing an organization for change, building up a change system, selecting change strategies, and evaluating a change process. Such a framework can be useful to those seeking to influence change in various alternative directions.  相似文献

Family caregivers face enormous challenges when attempting to oversee the medical, legal, financial, and daily affairs of loved ones with chronic or life-limiting illness. While formal services and agencies exist to assist caregivers with some of these tasks, caregivers in underserved communities do not utilize these services, or utilize them with unsatisfactory results. This study used focus groups (N = 5 groups) with underserved, minority caregivers (N = 22) to explore their experiences related to care provision, including barriers to support service use and challenges navigating systems related to the broad spectrum of caregiving demands. Thematic content analysis revealed three main themes: (1) prior difficulties with formal service use, (2) difficulties navigating financial, legal, and medical systems, and (3) caregivers’ personal strategies to address challenges associated with formal service use and systems navigation. Findings from this study identify underserved caregivers’ challenges in utilizing formal services, as well as caregiver-identified strategies for supporting their caregiving activities.  相似文献

In recent years a growing body of literature has examined the peculiar features of human service organizations and the difficulties which these pose for their administrators. This paper begins by reviewing recent writings on this subject, noting the differing emphases emerging from social welfare and public administration backgrounds respectively. The definition of human service organizations is then reconsidered, suggesting that it is useful to examine their characteristics in terms of two clusters of features: one related to their public source of mandate and resources, and the other related to the nature of the services they provide. A proposed definition based on these two clusters of features makes explicit two characteristics generally assumed in previous definitions of human service organizations; their purpose of meeting socially recognized needs, and the relatively powerless position of their clients. The definition proposed, rather than setting categorical boundaries, provides a model against which the features of a variety of organizations which wholly or partly fit it may be examined. The implications of the identified features of human service organizations for their administration are considered briefly, with particular reference to such problematic issues as the clarification of goals, the rights of consumers and the evaluation of services. It is suggested that the training and development of administrators for human service organizations must be based on an understanding of the particular difficulties and challenges arising from the nature of these organizations.  相似文献
