Adoptions of children with disabilities are considered special needs, due to the higher level of support needed pre- and postadoption by families and children for adoptions to succeed. Despite this fact, very few studies examine the specific postadoption experiences of families with children with disabilities. Using secondary analysis of a national survey of adoptive parents, as well as interviews and a focus group with adoptive parents in one state, this study examines families’ with adopted children with disabilities challenges and unmet needs after adoption, as well as the postadoption needs and experiences of adoptive families with children with developmental and intellectual disabilities and adoptive families of children with emotional, behavioral, and mental health disabilities. Findings indicate that families with children with disabilities do report higher rates of challenges and barriers to service access, and that this is particularly true in families with adopted children with emotional, behavioral, and mental health disabilities. Implications for research and practice are discussed.  相似文献

Although children who have been adopted are referred for mental health services more often than their non-adopted peers and might be overrepresented within residential treatment centers (RTCs), little is known about adoptive families' experiences with RTCs. The present study sought to understand the experiences of families whose children were placed in residential treatment facilities after a finalized adoption. Qualitative interviews were conducted with a total of ten families who had adopted 21 children. Of these, 13 children had been placed in an RTC after the finalized adoption. Most of the children in the sample were between zero and five years of age at placement. Adoptive families reported the need for in-the-trenches training and community-based services that could provide ongoing support post-adoption. Implications from these findings include increasing in-home services and other options within the adoptive families' own communities. Future research could incorporate a larger sample to assess whether the experiences of families in this study are similar to other adoptive families.  相似文献

The mental health community has failed to recognize the specific knowledge and expertise that therapists must have if they are to help this population. A review of literature on outpatient treatment of emotionally disturbed adoptees is presented. Key issues in adoption therapy are discussed. A model featuring long-term support for adoptive children and their families is described with a menu of accompanying clinical strategies. Abstract

The objectives of this comparative study were to examine adoptive family functioning with a sample of gay, lesbian, and heterosexual adoptive parents and their children. The results suggested that parent sexual orientation is not a significant predictor of adoptive family functioning, adopted child's behavior, and parent's perceptions of helpfulness from family support networks. Furthermore, a regression analysis suggested the following variables were associated with higher levels of family functioning: adoptive parents who were previously foster parents and children who had more previous placements prior to adoption. Lower family functioning was associated with children adopted through CPS; with children who had mental health diagnoses, learning disorders, or other handicapping conditions; and with children who were in a higher grade in school. The results of this comparative study of adoptive families support the need for more methodologically rigorous research that includes gay and lesbian adoptive parents along with heterosexual parents.  相似文献

In one of the first longitudinal population-based studies examining adopted children's educational achievement, we analyze whether there is a test-score gap between children in adoptive families and children in biological families. Using data from the Early Childhood Longitudinal Study, we find in aggregate adopted children have lower reading and math scores than their counterparts living in biological families. Yet there is significant variation among adoptive families by their race and health status. On one hand adoptive parents tend to be White and have more economic capital than their non-adoptive counterparts potentially contributing to educational advantages. However adopted children are also more likely to have special educational needs, contributing to greater educational disadvantages. Untangling these variables through a multivariate regression analysis, we find that transracially adopted children have similar test scores to White children living with biological parents. We point to the interaction between race, family resources and children's health status and how these characteristics differentially shape achievement outcomes for adopted children.  相似文献

Children awaiting adoption through the child welfare system often have “special needs,” or characteristics that make securing an adoptive home for them challenging. A subset of these youth experiences significant psychiatric symptomatology, which may pose a problem for the stability of the adoptive placement. Clinicians and policymakers require information about precursors to mental health difficulties in order to plan effectively for the placement of these children.This study examines potential child and biological family risk factors for the presence of a DSM-IV mental health diagnosis among 368 children placed for adoption by a special needs adoption program between February 1997 and April 2005 using logistic regression within the Generalized Estimating Equations (GEE) framework. A significant proportion of the children and biological parents in our study experienced serious adversity prior to adoptive placement. Older age at adoptive placement, white race, male gender, having more than one placement, and a history of sexual abuse are significant predictors of the presence of a mental health diagnosis in the logistic regression model. Biological parent incarceration is significantly associated with the absence of a mental health diagnosis.Adoptive placement is a key transition for youth who have often experienced significant loss and trauma prior to placement. Support of the adoptive family and youth can be critical to ensuring placement stability and may be especially salient for youth with documented mental health diagnoses and corresponding behavioral problems, which put children at increased risk for placement disruption. More work is needed to understand the interplay of risk and protective factors for mental health difficulties among youth adopted from the child welfare system, including how these are affected by child welfare policies, informal procedures, and resources to produce varying outcomes for children in peril.  相似文献

The mental health of children seeking asylum and their families is a somewhat neglected area of research. Research on refugee children and children living with adversities suggests that environmental factors are crucial in preventing mental health problems. In this study, we aim to identify central environmental conditions that affect the mental health of children living with their families at governmental asylum processing centres in northern Norway. This study has a qualitative design, and is based on 11 focus group interviews with the staff at asylum processing centres. The interviews were transcribed verbatim and analysed focusing on important risk and protective factors for mental health problems presented by the informants. The results highlighted time spent at asylum centres and the parent's mental health as the most important risk factors. Schooling, activities, general living conditions and poor economy were also seen as crucial. The findings suggest that these children are indeed vulnerable, and at high risk of developing mental health problems. Their rights are, however, open to local interpretations, and they fall between two stools; their right to proper health care, and national and international immigration policies.  相似文献

Mental health disorders are prevalent among children, yet many do not receive treatment. Parents and caregivers play a vital role in recognizing mental health disorders in children and accessing treatment. But a substantial number of parents lack essential knowledge of children’s mental health disorders, including risk factors, symptom recognition, and treatment options. Little focus has been given to parents in the children’s mental health literacy literature. The purpose of this article is to begin filling that gap by using a survey to describe the perceptions of child and family mental health providers’ perceptions regarding the amount, accuracy, and origin of mental health literacy in the parents of the children they treat. The impact of those perceptions on the work of providers is also explored. Eighty-seven mental health providers completed a survey to assess their perceptions of parent mental health literacy in the area of children’s mental health. Providers perceived parent mental health literacy as low, inaccurate, and inconsistent. In addition, providers indicated that parents rely on informal sources of support, such as friends and family for information about children’s mental health. Implications for social work researchers, practitioners, and the children and families they serve are discussed.  相似文献

African American children are overrepresented in foster care by more than twice their proportion in the population (U.S. Government Accountability Office [USGAO], 2007). Building upon research relating faith (religiosity) to positive health and mental health, this study utilized cognitive and religious coping theories to examine the influence of faith on choosing to adopt, achieving positive adoption outcomes, and reducing disproportionality. From Louisiana and Texas, 113 families who adopted 226 children, 48% African American, participated in a survey measuring children's behavior and parent distress (PSI-SF Difficult Child and Parent Distress Subscales) and religiosity (Hoge Intrinsic Religiosity Index). Of the respondents, 93% of the respondents belonged to a religious congregation, 86% attended church weekly. Controlling for child's behavior, religiosity predicted lower stress in adoptive parenting; church attendance was related to improvement in parental health since adopting. Faith was rated most frequently as essential in parents' decisions to adopt. The study concludes that faith may be an asset in increasing adoptions and improving adoption outcomes resulting in increased numbers of African American children adopted.  相似文献

ABSTRACT

Analyzing the 2007 National Survey of Adoptive Parents, this study examines the impact of open adoption, demographics, and other factors on adopted children’s mental health, delinquent behavior, and family relationships. Specifically, we compare findings for youth in private and public (i.e., foster care) adoptions and identify key similarities and differences between predictors of children’s well-being across these two types of adoption. We find that youth in open foster care adoptions are more likely to receive an attachment disorder diagnosis than those in closed foster care adoptions but are also more likely to have family relationships characterized by trust and adoptive parents’ willingness to recommend adoption to others. Further, we find children in both public and private adoptions who are older at placement are more likely to have posttraumatic stress disorder diagnoses. For children in private adoptions, no statistically significant predictors affected youths’ delinquency outcomes or family relationships, with the exception of parents of private adoptees in households characterized by lower levels of poverty indicating they would be more likely to recommend adoption to others. The implications of the key findings are discussed with regard to service provision for multiple types of adoptive families.  相似文献

It is estimated that, in the United States, one in ten children and adolescents suffer from illness severe enough to cause some level of psychosocial dysfunction. Urban children, and in particular low-income children of color, are at greater risk of developing mental health problems, and are less likely to receive effective child mental health services. Prompt and effective access to mental health services by urban children of color is a critical direct practice and social policy concern. This article provides an overview of significant barriers to mental health care experienced by poverty affected urban children of color and their families. It also addresses a broad range of practice and policy issues that need to be raised in order to ensure competent delivery of mental health services for children living in urban environments. The role of social work in assisting poor urban children of color to access and benefit from mental health services is stressed. In this paper we describe an intervention that focuses on the needs of children in families where a parent has a mental illness and attends a community adult mental health setting. After a brief outline of the literature, we present a family approach that includes children and young people, their parents and grandparents. Given growing public awareness about transgender and gender diverse identities, it is expected that greater numbers of children and families will seek mental health and social services, including transition-related services, from social workers and other mental health professionals. Transgender and gender diverse children have a range of transition-related needs which require the support of informed practitioners with transgender and gender diverse-specific knowledge and skills. Moreover, the needs and experiences of families and caregivers of transgender and gender diverse children will vary greatly. To date, research suggests a paucity of transgender and gender diverse-specific expertise among social workers and other mental health providers; this seems particularly evident with respect to the needs of transgender and gender diverse children. An affirmative practice framework to guide therapeutic work with transgender and gender diverse children and families is presented. In addition, key clinical practice considerations associated with engagement, assessment, psychoeducation, support and referral are provided. Finally, clinical examples illustrating use of the affirmative practice approach with transgender and gender diverse children are provided.  相似文献

Within the United Kingdom there is growing awareness of the need to identify and support the small number of children who are living in families experiencing multiple problems. Research indicates that adverse experiences in childhood can result in poor outcomes in adulthood in terms of lack of employment, poorer physical and mental health and increases in social problems experienced. It is acknowledged that most of these children are known to child welfare professionals and that some are referred to social services, subsequently entering the child protection system. This paper reports research conducted with 28 experienced child welfare professionals. It explores their views about families known to the child protection system with long‐term and complex needs in relation to the characteristics of children and their families; the process of intervention with families; and the effects of organisational arrangements on practice. The research indicates that these families are characterised by the range and depth of the problems experienced by the adults, such as domestic violence, mental health difficulties and substance misuse problems, and the need for professionals to have good inter‐personal skills and access to specialist therapeutic services if families are to be supported to address their problems. The Multiethnic Placement Act of 1994 and the Interethnic Adoption Provisions of 1996 (MEPA-IEP) require states to develop plans that "provide for the diligent recruitment of potential foster and adoptive families that reflect the ethnic and racial diversity of children in the state for whom foster and adoptive homes are needed." This paper explores the background of MEPA-IEP, describes the disparate outcomes for minority children in the child welfare system, and identifies agency challenges in finding permanent families for African American children. Tools are provided for successfully recruiting families while following MEPA-IEP and avoiding potentially discriminatory practices in placement decisionmaking.  相似文献

Adoption assistance entitlements support children whose birth parents cannot care for them. The entitlements are meant to offset the extra cost to adoptive families of raising children whose early adverse experiences leave them with special needs. Unlike other Social Security Act programs, adoption assistance is not federally administered but is administered by states or localities. State-to-state variation in administration of this federal entitlement leads to unequal treatment of similar children. Moreover, although adoption assistance is an entitlement for children, payments made by many states are systematically correlated with the characteristics of adoptive families.  相似文献

Building on a conceptual model of the transition to elementary school, this study explored the role of health in the early cognitive achievement of children from various racial/ethnic minority and immigrant families by applying multilevel modeling to data from a nationally representative sample of American kindergarteners. Whites tended to have the best physical health before transitioning to first grade. Children from immigrant Latino/a and Asian families had the worst physical health but the best mental health. Compared to white children from native families, these health differentials partially explained the lower math achievement and achievement growth of black children (whether from native or immigrant families) in first grade as well as the lower math achievement of children from Latino/a immigrant families and the lower achievement growth of children from Asian immigrant families during this period.  相似文献
