Sexual minority women face a plethora of structural, socioeconomic, and interpersonal disadvantages and stressors. Research has established negative associations between women’s sexual minority identities and both their own health and their infants’ birth outcomes. Yet a separate body of scholarship has documented similarities in the development and well-being of children living with same-sex couples relative to those living with similarly situated different-sex couples. This study sought to reconcile these literatures by examining the association between maternal sexual identity and child health at ages 5–18 using a US sample from the full population of children of sexual minority women, including those who identify as mostly heterosexual, bisexual, or lesbian, regardless of partner sex or gender. Analyses using data from the National Longitudinal Study of Adolescent to Adult Health (N?=?8978) followed women longitudinally and examined several measures of their children’s health, including general health and specific developmental and physical health conditions. Analyses found that children of mostly heterosexual and bisexual women experienced health disadvantages relative to children of heterosexual women, whereas the few children of lesbian women in our sample evidenced a mixture of advantages and disadvantages. These findings underscore that to understand sexual orientation disparities and the intergenerational transmission of health, it is important to incorporate broad measurement of sexual orientation that can capture variation in family forms and in sexual minority identities.

Looking beyond binary measurements of “male” or “female” can illuminate health inequality patterns that correspond to gender identity rather than biological sex. This study examines disparities in overall health among transgender men, transgender women, gender-nonconforming adults, and cisgender (nontransgender) men and women in the U.S. population. Behavioral Risk Factor Surveillance System (BRFSS) data from 32 U.S. states and territories between 2014 and 2016 yield an analytic sample that identifies 2,229 transgender and gender-nonconforming adults and 516,753 cisgender adults. Estimates from logistic regression models, using cisgender men as a reference group, show that gender-nonconforming respondents have significantly higher odds of reporting poor self-rated health than any other gender identity group. Transgender men also display higher odds of reporting poor health in some models, corresponding to their relative socioeconomic disadvantage. I find no apparent health disadvantage among transgender women and a persistent, if slight, disadvantage among cisgender women. Gender-nonconforming respondents’ predicted probabilities of reporting poor health remain nearly twice as high as those of cisgender men after adjustments for demographic, socioeconomic, and behavioral factors. Their persistent patterns of health-related disadvantage underscore the need for higher-quality data on gender-nonconforming respondents that account for sex assigned at birth.  相似文献

Accessible employment suited to residents’ needs is an important equity issue and social determinant of health and wellbeing, and a balanced jobs-housing ratio within a region has the potential to provide health benefits. This paper aims to provide evidence on the associations between employment and health, and the potential pathways, as well as identifying spatial indicators that can be used to measure urban employment, a construct of liveability. We used 2011 census data to create and test two area-level spatial measures of urban employment with geo-coded population health behaviour and outcome data (2011 VicHealth survey) in 5206 employed adults living in urban Victoria, Australia. Those living in areas with higher levels of local employment had reduced odds of a longer commute (OR 0.87). The odds of a longer commute time was also greater for those who lived in an area where more people commuted to work by private vehicle (OR 1.20), and less for those who lived in local areas where more people travelled to work by public transport (OR 0.85) or active travel (OR 0.80). The odds of reporting longer sitting times was less for those who lived in a local area where more people commuted to work by private vehicle (OR 0.65). Those who had a longer commute times, regardless of travel mode, had greater odds of reporting more sitting during a typical weekday (OR 1.67). In turn, those who spent more time sitting had significantly greater odds of reporting poorer self-rated health (OR 1.34). Such work provides evidence to policy-makers to help build the argument for which area-level attributes are needed to support urban employment across a region.  相似文献
