Mechanisms producing positive change in troubled families were analysed. The study aimed to answer the questions: What kind of case and client outcomes does intensive family work (IFW) produce for child welfare clients? From the perspective of family workers, what kind of critical factors for positive child, parent and family outcomes can be identified? The study used child welfare case files of 35 families with 93 children from one social office and interviews with 20 family workers and IFW managers in 5 municipalities in Finland. The shades of worries scale was used in evaluating the change in each family's situation. The case outcome measures for children included changes in the child's status in open care, short-term placement or foster care. During IFW, positive changes took place in 23 families out of 35 (66%). Twenty-seven children from 16 families did not continue as child welfare clients. Eleven children were placed into foster care during IFW and two years after it. The critical processes for outcomes included time, trust building and engagement, practical help, facilitating communication and guaranteeing the best interest of the child. IFW was considered effective for positive outcomes for the majority of families with complex needs.  相似文献

The research in child protection and in work with other involuntary clients suggests that the use of certain skills by child protection workers is likely to be related to positive client outcomes. In particular, effective practice involves: helping clients and client families to understand the role of the child protection worker; working through a problem‐solving process which focuses on the client's rather than the worker's definitions of problems; reinforcing the client's pro‐social expression and actions; making appropriate use of confrontation; and using these skills within a collaborative client/worker relationship. This study attempts to identify the extent to which child protection workers make use of these skills and how these skills relate to client outcomes. Data were sought through 50 interviews with child protection workers and 282 interviews with family members. The child protection workers provided information about 247 client families. It was clear that when workers used the skills, their clients had better outcomes—the workers believed their clients showed better progress, the clients were more satisfied with the outcomes and the cases were more likely to have been closed 16 months later. The measurement of problems and needs is a major issue for policy and practice in social work and social service provision. There has, furthermore, in recent years, been an increasing emphasis on gaining an understanding of users or clients' views of needs and service provision. However, there have been few attempts to develop practitioner and ‘client friendly’ ways of gaining information on problems and needs, either on an individual basis, or through the collection of aggregated data which can be used by local authorities as a whole. Where these have taken place there has been little attempt to assess their reliability or validity. This paper reports on the design and development of the Parent Concerns Questionnaire, an instrument designed to obtain the views of mothers in general, and depressed mothers in particular, in families subject to child and family care intervention. The paper reports on the reliability and validity of this instrument, which is presented as appropriate for use, both with families containing depressed mothers and other families subject to child and family care intervention. It may be used to examine problems and needs both on an individual family‐by‐family basis, and through the aggregation of data, on an authority‐wide basis. In this research project a historian seeks to test whether a dichotomy in the historical analysis of working mothers also exists in recent mass-media portrayals of contemporary wage-earning mothers. The hypothesis is that only white, middle- and upper-class women who work outside the home are considered working mothers, with the emphasis placed on their role as mothers. When poor, minority, working women with children are discussed, they are considered primarily in their capacity as workers. Following from this dichotomy is a split in the assessment of day care for the children of working mothers. For the children of high socioeconomic status women, no day care facility can satisfactorily replace maternal care; for the children of low socioeconomic status women, any type of non-maternal care that enables a mother to keep functioning as a worker is considered superior to maternal care. This study is important because it clarifies some critical scholarly and popular assumptions about the importance of work, the nature of motherhood, and the needs of different classes of children. These assumptions in turn structure scholarly analysis, permeate public discourse, and ultimately shape work and child care policies that profoundly affect all of America's families. This work is intended as a brief analysis that can serve to highlight unconscious attitudes and to suggest how future studies and discussions might be restructured to avoid perpetuating a false separation of different classes of working mothers and their children.  相似文献

Infants and toddlers who experience physical abuse and/or neglect are at a severe risk for disruptions to emotion regulation. Recent prevention and treatment efforts have highlighted center-based child care as an important setting for providing support to the needs of these children, as child care centers are already an existing point of entry for reaching high-risk families. Guided by ecological theory, this review draws on the maltreatment and child care literatures to consider the opportunity for child care centers, specifically teacher-child interactions within the classroom, to support the unique regulatory needs of maltreated infants and toddlers. Existing research on the effects of child care for children facing other types of risk, as well as research with maltreated preschool children, provides a foundation for considering the role child care may play for infants and toddlers, whose emotion regulation skills are just emerging. More research is needed regarding teachers' roles in facilitating effective emotional experiences in the classroom that meet the unique needs of maltreated children. Additionally, early childhood teacher training that focuses on infant/toddler mental health and a trauma-informed perspective of care, as well as structuring child care centers as communities of support for high risk families, all may aid child care centers in better serving this vulnerable population.  相似文献

In the last years, researchers have been emphasizing the importance of promoting needs-led, context-specific, user-centered services in the context of child protection. However, policy-makers and service planners around the world largely depend on US-based research evidence of what is effective in the domain of family support. This work presents, in two studies, the process of design and evaluation of a targeted family support intervention that was developed and implemented in Portugal.Following the Common Language Approach to needs assessment (Dartington Social Research Unit, 2001), in study 1 we screened 100 children and their families attending a generalist child care service for risk and protective factors. Four different clusters of needs were identified. One cluster was selected as target-group for the design of a service to match their needs. Results indicated that families in this cluster had socioeconomical disadvantages, a challenging family environment and inadequate parenting practices. Children were showing signs of problematic social behaviors.In study 2 we describe the theoretical process model and the logic model for the intervention, and experimentally evaluate the program's efficacy. Results from pre–post assessments indicate improvements in the intervention group (N = 20) in several areas of parental empowerment and family relations, comparing with the control group (N = 20).This approach to need-service matching seems to be a viable pathway to design needs-led, context-specific, and user-centered services, and to assess their efficacy, thereby informing policy makers and service planners.  相似文献

A follow-up study of 83 families treated by family therapy at a child guidance clinic was conducted and the relation of client/therapist interaction to (i) drop out from therapy and (ii) improvement was examined. Early terminators were found to differ significantly from continuers as to their ratings of their relationship with the therapist, and trends were found for therapists' feelings to be associated with adherence to therapy. Early terminators were not distinguished by diagnosis, severity of case, or improvement rating. Improvement, on the other hand, was not associated with therapists' or clients' attitudes to each other. It is suggested that these factors are of greater significance in “joining” therapy than in its course.  相似文献

Welfare reform was examined for 39 Michigan families whose children have significant health or intellectual and behavioral disabilities. As a group, these families received little specialized assistance or services to address their unique needs. Family-cited barriers to self-sufficiency included poorly trained welfare caseworkers, limited public transportation, and inadequate child care. Having an older child was the only discriminating variable between working and non-working mothers. However, working mothers only had temporary positions with no benefits and low pay. All families, whether employed or not, lived below the poverty line.  相似文献

For many parents of disabled children, finding affordable and suitable childcare that meets their needs is very challenging. Research in the UK has shown that parents with disabled children experience barriers not just with cost, but also accessibility, the attitudes of childcare providers and in getting good information about sources of childcare. The Disabled Children's Access to Childcare (DCATCH) pilots was an initiative designed to improve access to childcare for disabled children in England. As part of an evaluation of DCATCH, qualitative interviews were carried out with 38 members of 22 families who had received support from the initiative. All of the families reported positive outcomes and highlighted: a beneficial impact on the parents' capacity to work; enjoyable experiences for the disabled child; increased confidence and independence for both parents and children; and the creation of time for parents to pursue other activities. The results of the DCATCH initiative support other research which argues that whilst cost is a significant factor around childcare choices for families with a disabled child, having confidence in the childcare provider's ability to meet specific needs (including complex health care needs) and providing positive experiences for the disabled child are also key, determining factors.  相似文献

PurposeAmong girls in foster care, 48% become pregnant at least once by age 19 (Dworsky & Courtney, 2010). Youth in or at-risk for foster care (YFC) report limited knowledge about, access to, and use of condoms; ambivalent attitudes towards teen parenting; and participation in other risky behaviors. For the current study, we adapted and supplemented an evidence-based sexual health program called SiHLE, using a systematic adaptation framework (ADAPT-ITT, Wingood & DiClemete, 2008), to address the unique and targeted needs of youth living in a temporary shelter due to lack of housing. Youth in this study were either in foster care and awaiting placement, or having serious family problems and were at-risk of entering the foster care system.MethodsThirty-six youth participated in SiHLE-YFC during their stay at a temporary shelter. Four 90-minute sessions focused on increasing sexual health knowledge, improving attitudes towards and self-efficacy of condom use, and developing core skills such as problem-solving and communication.ResultsAs hypothesized, youth showed high satisfaction with the intervention and significant improvement in sexual health knowledge from pre to post. At one-month follow-up, youth continued to show significant improvement in sexual health knowledge, along with a significant reduction in risky sexual behaviors. Though not significant, there were moderate effect sizes for changes in attitudes towards teen pregnancy and condoms. There were no changes in attitudes towards teen parenting.ConclusionTaken together, findings suggest that sexual health education directly targeting the unique needs of YFC may improve sexual health knowledge and behavior, and are discussed in the context of challenges associated with intervention and research with this population.  相似文献

Social work educators across the world are engaged in developing a curriculum that prepares students to be effective practitioners in a range of settings that involve working with children. This paper reflects on collaborative strategies and research that have influenced the child wellbeing content of social work courses in Australia. It presents a critical analysis of the tensions and challenges that can occur in (this) collaborative work. It then discusses an outcome of this advocacy to influence the child wellbeing content of social work education, an undergraduate module aimed at increasing students' knowledge and skills in communicating with children. This paper argues that Indigenous children (such as Aboriginal and Torres Strait Islander children) are over-represented in child protection systems and out-of-home care services, and that this needs to be acknowledged and given increased attention in the education of social work students internationally.  相似文献

Although some attempts are being made to increase children's participation in Norwegian child protection cases, much needs to be done in order to comply with the participation principle in the United Nations Convention on the Rights of the Child. This paper reports on a study of factors that are likely to predict if social workers will attempt to give children an effective voice in decision making processes. 53 child protection case managers and 33 social work students participated in a questionnaire survey in which they were asked to agree or disagree with 20 statements about child participation. Statistical factor analysis was used in order to identify underlying factors in the dataset. The results suggest three main reasons for children not being allowed to participate: communication difficulties (communication factor); because child participation was not deemed necessary (participation advocacy factor); or that participation was considered inappropriate because it might be harmful (protectionism factor). This research suggests that, if we are to improve participation within the child protection system, formal regulations and guidelines need to be accompanied by a greater attention to development of social work skills in working with children through participatory processes. The goal of the study was to explore professional challenges of child and youth care workers (CYCWs) in the South African context. The study was carried out within a qualitative approach which incorporated a collective case study design. Purposive sampling was used to include a sample of 93 participants representing child and youth care workers from government and non-government organisations based in rural, semi-urban and urban areas across the six provinces of South Africa. An interview schedule was designed by the researchers to collect qualitative data from 11 focus group interviews. With regard to data analysis, the researchers typically followed the following steps as suggested by Miles and Huberman (1994, p. 10): data reduction, data display and drawing and verifying conclusions. The most prominent challenges that the participants have cited are: dealing with client behaviours, lack of tangible and immediate results, poor stakeholder relations, lack of recognition, lack of clarity on the role and title of the CYCW, inconsistent job requirements, lack of professional growth and development opportunities and inadequate working conditions. The conclusion of this study was that CYCWs in South Africa experience a range of professional challenges. Given that the study was situated within an ecological systems theory, the impact of other systems such as poor stakeholder relations in working with individual children (microsystem), working within families (exosystems), working with communities (mesosystem) and working within the broader society (macrosystem) became evident. The researchers overall impression was that the majority of challenges raised emanated from the child and youth care field not yet being fully recognised as a profession at the time the study was conducted. Despite coaching being identified as an important implementation strategy, scant information is available on the core functions of coaching, and few empirical studies are specific to coaching in the child welfare setting. This study explored practitioners' perceptions of the core functions of coaching by using semi-structured focus groups with coaches (n = 13) and interviews with coachees (n = 11) who were delivering Parent Management Training, Oregon (PMTO) model to families of children in foster care. Four themes were identified as the core functions of coaching: (1) supporting practitioners via strengths-oriented feedback; (2) promoting skill-building via collaboration and active learning strategies; (3) problem-solving for appropriate use and adaptation of the EBI with real-world cases, and (4) providing an accountability mechanism for high fidelity implementation. Collectively, this study's findings build knowledge on the core functions of coaching, which may be a critical strategy for integrating evidence-based interventions (EBI) into usual practice in child welfare settings. The findings suggest that this implementation strategy is more than a simple extension of training. Coaching was viewed as vital for supporting practitioners full adoption of the intervention in their day-to-day practice, fitting an EBI to the complex needs of child welfare families, and ensuring high-quality implementation. Also identified were some unique aspects of PMTO coaching, such as a strict strengths-orientation and observation-based feedback via mandatory video recordings of client sessions. Further research is needed to explore different coaching techniques, protocols, and formats to examine whether certain features promote a more effective path to implementation and, ultimately, client outcomes.  相似文献

This paper argues that infant and child observation is not only valuable as part of social work training for work with children and families but that it has a wider value in equipping workers with assessment skills relevant in all areas of social work, including care management. It describes the introduction of infant and child observation as a required component of the BA/Diploma in Social Work at The University of Reading and the effect on students' learning and their practice skills. It shows how child observation festers the development of reflective practice, enabling workers to look beyond the presenting problem and to understand the situation as a whole. The resulting intervention is then more likely to be appropriate in meeting a client's needs.  相似文献

ABSTRACT

Commercial sexual exploitation of children has emerged as a critical issue within child welfare, but little is currently known about this population or effective treatment approaches to address their unique needs. Children in foster care and runaways are reported to be vulnerable to exploitation because they frequently have unmet needs for family relationships, and they have had inadequate supervision and histories of trauma of which traffickers take advantage. The current article presents data on the demographic characteristics, trauma history, mental and behavioral health needs, physical health needs, and strengths collected on a sample of 87 commercially sexually exploited youth. These youth were served in a specialized treatment program in Miami-Dade County, Florida, for exploited youth involved with the child welfare system. Findings revealed that the youth in this study have high rates of previous sexual abuse (86% of the youth) and other traumatic experiences prior to their exploitation. Youth also exhibited considerable mental and behavioral health needs. Given that few programs emphasize the unique needs of children who have been sexually exploited, recommendations are offered for providing a continuum of specialized housing and treatment services to meet the needs of sexually exploited youth, based on the authors’ experiences working with this population.  相似文献

The authors explored client psychological distress as a variable in career counseling. Forty‐two clients in a naturalistic setting were seen for a total of 290 sessions by 21 counselors‐in‐training. The results indicated that (a) 60% of the clients were psychologically distressed, (b) clients' scores decreased significantly from pretest to posttest on psychological distress variables, (c) significant linear growth occurred in the clients' perception of the working alliance across sessions, and (d) the psychological distress outcome variables were significantly related to both the first session level of the working alliance and its linear growth.  相似文献

Foster parents have increasingly assumed new and challenging roles during the past decade. Meeting the developmental, attachment, and grieving needs of children and youth in out of home care is challenging by itself, but can become even more difficult with the issues that arise when the child is lesbian, gay, bisexual, transgender, or questioning (LGBTQ). Preservice and in-service foster parent training programs can strengthen shared parenting skills by focusing on the universal critical issues of safety, well being, and permanence for children and youth in foster care. This article will focus on these skill areas: (1) sharing parenting to promote healthy growth and development of LGBTQ youth in foster care, (2) threats to safety of LGBTQ youth in foster care, and (3) general challenges and strategies for preparing foster parents of LGBTQ youth to build support systems.  相似文献

The point of departure in this article is a client study with children who had received services from child welfare, school counselling or child psychiatric clinics, compared to a cohort study targeting all children in the same community. It discusses whether parents' reservations towards letting their client children participate in the study indicates a fear of the researchers interfering in family life, and how the choice of research strategies influence the images constructed of client children and their families. A common finding in the two studies was that the children themselves provided the most positive information about their lives. It is argued that adult images of children as either vulnerable, and thus in need of protection, or competent, and thus with an ability to participate, influence access to, as well as construction of, client children's lives.  相似文献
